Cognitive load during speech perception in noise: the influence of age, hearing loss, and cognition on the pupil response.

OBJECTIVES: The aim of the present study was to evaluate the influence of age, hearing loss, and cognitive ability on the cognitive processing load during listening to speech presented in noise. Cognitive load was assessed by means of pupillometry (i.e., examination of pupil dilation), supplemented with subjective ratings. DESIGN: Two groups of subjects participated: 38 middle-aged participants (mean age = 55 yrs) with normal hearing and 36 middle-aged participants (mean age = 61 yrs) with hearing loss. Using three Speech Reception Threshold (SRT) in stationary noise tests, we estimated the s

Increasing Motivation Increases Intelligibility Benefits of Perceptual Training in Dysarthria.

PURPOSE: Perceptual training offers a promising, listener-targeted option for improving intelligibility of dysarthric speech. Cognitive resources are required for learning, and theoretical models of listening effort and engagement account for a role of listener motivation in allocation of such resources. Here, we manipulate training instructions to enhance motivation to test the hypothesis that increased motivation increases the intelligibility benefits of perceptual training. METHOD: Across two data collection sites, which differed with respect to many elements of study design including age

Noise and a Speaker's Impaired Voice Quality Disrupt Spoken Language Processing in School-Aged Children: Evidence From Performance and Response Time Measures.

Purpose Our aim was to investigate isolated and combined effects of speech-shaped noise (SSN) and a speaker's impaired voice quality on spoken language processing in first-grade children. Method In individual examinations, 53 typically developing children aged 5-6 years performed a speech perception task (phoneme discrimination) and a listening comprehension task (sentence-picture matching). Speech stimuli were randomly presented in a 2 × 2 factorial design with the factors noise (no added noise vs. SSN at 0- dB SNR) and voice quality (normal voice vs. impaired voice). Outcome measures were ta

Nontraditional 'aids' to hearing: assistive listening devices.

Unresolved hearing loss and its associated sense of isolation burden the hearing-impaired elderly. The plight of both listeners and talkers justifies every effort to encourage and enhance communication. Assistive listening devices can benefit many family practice patients. The following service welcomes questions or comments about implementing these devices: Audiology-Speech Pathology Service, VA Medical Center, 700 South 19th Street, Birmingham, AL 35203; (205)933-8101, ext. 6701 and 6702. Cognitive Function Prediction of Performance During Dual-Tasks Across Adults With and Without Hearing Loss.

BACKGROUND: Dual-task procedures are commonly implemented to examine cognitive load and listening effort as individual differences in cognition often determine successful listening. However, which methods are most efficacious is unclear. Specifically, standardized, targeted assessment procedures for establishing cognitive function, and age-related changes that might account for changes in dual-task performance have yet to be established. Additional data are needed across aging populations, including middle-aged adults and older adults to establish the trend of performance changes throughout th

Parent-Reported Ease of Listening in Preschool-Aged Children With Bilateral and Unilateral Hearing Loss.

OBJECTIVES: Evidence from school-aged children suggests that the ease with which children listen varies with the presence of hearing loss and the acoustic environment despite the use of devices like hearing aids. However, little is known about the ease of listening in preschool-aged children with hearing loss-an age at which rapid learning occurs and increased listening difficulty or effort may diminish the required capacity to learn new skills. To this end, the objectives of the present study were to (i) assess parent-reported aided ease of listening as a function of hearing loss configuratio

Effect of stimulus bandwidth on auditory skills in normal-hearing and hearing-impaired children.

OBJECTIVE: Previous studies from our laboratory have shown that a restricted stimulus bandwidth can have a negative effect upon the perception of the phonemes /s/ and /z/, which serve multiple linguistic functions in the English language. These findings may have important implications for the development of speech and language in young children with hearing loss because the bandwidth of current hearing aids generally is restricted to 6 to 7 kHz. The primary goal of the current study was to expand our previous work to examine the effects of stimulus bandwidth on a wide range of speech materials

Low-gain hearing aids for individuals with normal hearing thresholds: a scoping review of listening-related outcomes.

OBJECTIVES: Listening difficulties (LiD) can occur despite normal hearing thresholds, affecting speech understanding in complex environments and increasing listening effort. Audiologists have increasingly considered hearing aids for this population despite supporting evidence remaining unclear. This scoping review examined whether amplification should be used in individuals with normal hearing thresholds with or without LiD. STUDY SAMPLE: Following PRISMA-ScR guidelines, 67 studies were reviewed: 14 involving adults and/or children with normal hearing thresholds with LiD and 54 involving adul

Relationship Between Daily Device Usage and Listening-Related Fatigue in Adults and Children With Cochlear Implants.

OBJECTIVES: Cochlear implants (CIs) enable people with moderate-to-profound sensorineural hearing loss to perceive sounds, but perceptual outcomes with a CI remain highly variable from person to person. Individuals who wear their CI consistently tend to exhibit better auditory outcomes than those who do not wear their CI as often. However, even the best-performing CI users typically exert more effort to understand speech than their peers with normal hearing, which can lead to listening-related fatigue. In this study, we aimed to understand whether the amount of time an individual wears a CI ea

Listening under difficult conditions: An activation likelihood estimation meta-analysis.

The brain networks supporting speech identification and comprehension under difficult listening conditions are not well specified. The networks hypothesized to underlie effortful listening include regions responsible for executive control. We conducted meta-analyses of auditory neuroimaging studies to determine whether a common activation pattern of the frontal lobe supports effortful listening under different speech manipulations. Fifty-three functional neuroimaging studies investigating speech perception were divided into three independent Activation Likelihood Estimate analyses based on the

Voice quality changes following phonatory-respiratory effort treatment (LSVT) versus respiratory effort treatment for individuals with Parkinson disease.

Perceptual ratings of hoarseness and breathiness were used to assess the efficacy of two intensive methods for treating dysarthrophonia in individuals with idiopathic Parkinson disease. One method emphasized phonatory-respiratory effort (the Lee Silverman Voice Treatment, LSVT) and the other emphasized respiratory effort alone (RET). Perceptual ratings were performed by two expert listeners based on random order presentation of the patients' pretreatment and posttreatment recordings of the "Rainbow Passage." The listeners were blinded to the patients and their treatment group. Statistically si

Language proficiency drives listening-related fatigue in those who communicate in English as their second language.

Individuals who listen to speech in their second language ('L2 listeners') are disproportionately impacted by adverse acoustic conditions; a phenomenon coined the 'non-native speech-in-noise disadvantage'. Evidence also suggests that L2 listening taxes cognitive resources to a greater extent than in L1 listeners. Very little, however, is known about the longer-term consequences of this sustained cognitive engagement. Listening-related fatigue refers to the daily life subjective experience of extreme tiredness from effortful listening. The current study recruited a large sample of L2-English li

The role of binaural input in music perception: Evidence from adults with single-sided deafness.

Single-sided deafness (SSD) is characterized by unilateral auditory deprivation, which disrupts binaural hearing. This study aimed to evaluate music perception in adults with SSD using behavioral measures and subjective ratings. Thirty adults with SSD were compared with 54 normal-hearing (NH) listeners tested under binaural (NH-B, n = 27) and simulated acute monaural (NH-M, n = 27) conditions in a free-field setup. Behavioral music perception was assessed using the Clinical Assessment of Music Perception (CAMP) battery, including Pitch Direction Discrimination (PDD), Timbre Recognition (TR), a

A Voice Ergonomic Study in Icelandic Preschools: What Causes the Activity Noise and How to Attenuate It.

INTRODUCTION: Preschool sound environments are predominantly shaped by activity noise generated by voices, object impacts, furniture movement, and daily transitions. Although Icelandic preschools typically comply with reverberation standards, staff frequently report excessive noise, high listening effort, and vocal strain. METHODS: This mixed-method pre-post study examined the sources and reducibility of activity noise in three Icelandic preschools using a voice-ergonomic framework. Data included objective noise measurements at child ear height, structured observations, and teacher questionna

Measurement of speech quality as a tool to optimize the fitting of a hearing aid.

The purpose of the present research was to develop a theoretical basis for the adjustment of hearing aid frequency response based on speech quality measurements. Speech quality measurements were made using continuous discourse and a category rating procedure for the following dimensions: intelligibility, pleasantness, loudness, effort, noisiness, and total impression. Speech quality ratings were obtained from a group of listeners with hearing loss who wore hearing aids. The stimulus conditions simulated hearing aid frequency response alterations within a frequency response range where intellig

A Longitudinal Framework to Describe the Relation Between Age-Related Hearing Loss and Social Isolation.

Many older adults live with some form of hearing loss and have difficulty understanding speech in the presence of background sound. Experiences resulting from such difficulties include increased listening effort and fatigue. Social interactions may become less appealing in the context of such experiences, and age-related hearing loss is associated with an increased risk of social isolation and associated negative psychosocial health outcomes. However, the precise relationship between age-related hearing loss and social isolation is not well described. Here, we review the literature and synthes

Acoustic correlate of vocal effort in spasmodic dysphonia.

OBJECTIVES: This study characterized the relationship between relative fundamental frequency (RFF) and listeners' perceptions of vocal effort and overall spasmodic dysphonia severity in the voices of 19 individuals with adductor spasmodic dysphonia. METHODS: Twenty inexperienced listeners evaluated the vocal effort and overall severity of voices using visual analog scales. The squared correlation coefficients (R2) between average vocal effort and overall severity and RFF measures were calculated as a function of the number of acoustic instances used for the RFF estimate (from 1 to 9, of a tot
