Effects of embryo transfer quality on pregnancy and live birth delivery rates.

BACKGROUND: To analyze the effects of embryo transfer (ET) quality on clinical pregnancy (CPR) and live birth delivery rates (LBDR). METHODS: In a retrospective study at a single, private infertility center between November 2005 and December 2009 one thousand fifty-five day-3 and day-5 ETs following IVF/ICSI/IMSI were evaluated. We analyzed the impact of an atraumatic ET with a soft catheter (ET 1), after external guidance (ET 2), after probing of the cervix with a stylet (ET 3), or after grasping the portio vaginalis with a tenaculum (ET 4) on CPR and LBDR. RESULTS: The use of external guid

Pulmonary interstitial emphysema in live birth determination: radiographic and gross pathologic features.

Pulmonary interstitial emphysema (PIE) has previously been reported as a useful finding in the determination of live birth in cases where the birth and death were not attended. Herein, we present the radiographic and gross pathologic features of PIE in such a case. In this instance, in addition to parenchymal interstitial emphysema, there were subpleural air collections and a pneumothorax.

Antenatal diagnosis of intrauterine infection with coxsackievirus B3 associated with live birth.

BACKGROUND: Prior reported cases of stillbirth and neonates infected with enteroviruses suggest transplacental infection. We present a case of fetal infection with coxsackievirus B3, diagnosed antenatally and resulting in live birth. CASE: A pregnant woman presented at 26 weeks with fetal tachycardia and non-immune hydrops fetalis. Coxsackievirus B3 was cultured from amniotic fluid. Maternal antibody to coxsackievirus B3 was positive at 1:512. At 32 weeks, the fetus deteriorated and was delivered. Cord blood antibody to coxsackievirus B3 was positive at a higher titer. Following neonatal deat

Exploring the live birth rates of women living with HIV in British Columbia, Canada.

OBJECTIVE: To evaluate the birth rates of women living with HIV (WLWH) compared to the general population in British Columbia (BC), Canada. METHODS: We retrospectively reviewed clinical and population level surveillance data from 1997 to 2015. Live birth rates from 1997 to 2015 among WLWH aged 15-49 years were compared with those of all BC women. Next, the number of live births among WLWH with a live birth between 1997-2012 and HIV-negative controls matched 1:3 by geocode were compared. RESULTS: WLWH had a lower birth rate compared to all BC women [31.4 (95%CI, 28.6-34.3) vs. 40.0 (39.3-40.1

Environmental hazards, social inequality, and fetal loss: Implications of live-birth bias for estimation of disparities in birth outcomes.

UNLABELLED: Restricting to live births can induce bias in studies of pregnancy and developmental outcomes, but whether this live-birth bias results in underestimating disparities is unknown. Bias may arise from collider stratification due to an unmeasured common cause of fetal loss and the outcome of interest, or depletion of susceptibles, where exposure differentially causes fetal loss among those with underlying susceptibility. METHODS: We conducted a simulation study to examine the magnitude of live-birth bias in a population parameterized to resemble one year of conceptions in California

Recurrence of preterm births: A population-based linkage with 3.5 million live births from the CIDACS Birth Cohort.

OBJECTIVE: To investigate the recurrence of preterm birth (PTB) among the poorest half of the Brazilian population. METHODS: A population-based retrospective study was conducted in Brazil with the live births of multiparous women extracted from the CIDACS Birth Cohort between 2001 and 2015. We used multivariate logistic regression to estimate the odds of recurrent PTB in second and third births. RESULTS: A total of 3 528 050 live births from 1 764 025 multiparous women were analyzed. The adjusted odds for the occurrence of a PTB given a previous PTB was 2.58 (95% confidence interval [CI] 2.5

Advanced paternal age affects miscarriages and live birth outcomes in oocyte donation cycles.

STUDY QUESTION: Does advanced paternal age affect miscarriage and/or live birth outcomes following the first transfer attempt in oocyte donation cycles? SUMMARY ANSWER: Paternal age has a gradual but cumulative effect that becomes apparent above 45 years of age, where it evidently increases miscarriage rates and decreases live birth rates following the first transfer in oocyte donation cycles. WHAT IS ALREADY KNOWN: Donor oocyte cycles are becoming a common strategy for addressing female age-related infertility, offering high success rates (∼60% ongoing pregnancies) attributed to superior oo

Cumulative live birth rates following testicular sperm extraction in non-obstructive male infertility.

This retrospective observational cohort study was designed to evaluate the cumulative live birth rates achieved through testicular sperm extraction (TESE) followed by intracytoplasmic sperm injection (ICSI) in couples affected by isolated non-obstructive male infertility, to better estimate their chances of achieving biological fatherhood. We also aimed to estimate their time to live birth, and identify predictive factors of successful TESE. We included all couples undergoing a first TESE procedure for isolated non-obstructive male infertility at a tertiary university hospital between January

Embryo-maternal cross-talk: key players in successful implantation and live birth rates.

A successful pregnancy requires that the embryo and the endometrium undergo regulated alterations during the preimplantation phase, which can have positive effects on each other due to these adaptations. The focus of this review is to study the alterations in both the immune system and gene expression within the endometrium, along with exploring the embryonic surface and secretory markers crucial for successful implantation. Innate and adaptive immune cells in the endometrium phenotypically differ from immune cells in other human tissues. Research has demonstrated the significance of the commu

Metabolic imaging of human cumulus cells reveals associations with pregnancy and live birth.

STUDY QUESTION: Can fluorescence lifetime imaging microscopy (FLIM) detect associations between the metabolic state of cumulus cell (CC) samples and the clinical outcome of the corresponding embryos? SUMMARY ANSWER: FLIM can detect significant variations in the metabolism of CC associated with the corresponding embryos that resulted in a clinical pregnancy versus those that did not. WHAT IS KNOWN ALREADY: CC and oocyte metabolic cooperativity are known to be necessary for the acquisition of developmental competence. However, reliable CC biomarkers that reflect oocyte viability and embryo dev

Pregnancy with mixed connective tissue disease: Exploration of factors influencing live birth outcomes.

Mixed connective tissue disease (MCTD) predominantly affects women in their reproductive age (30-40 years). This study is aimed to analyze a case series of MCTD-complicated pregnancies. The study design utilized a combined case-series and case-control approach. Pregnant women with MCTD were included and categorized into two groups: the live-birth and non-live birth (encompassing miscarriages at <12 weeks and stillbirths at ≥12 weeks) groups. Primary outcomes included delivery outcomes and factors associated with live births. A total of 57 pregnancies from 34 mothers (median age: 33.0 years) we

Predictions of live birth in IVF programs of patients with recurrent implantation failure.

BACKGROUND: When a high-quality embryo is implanted into the uterus, but the pregnancy is not established as shown by the ultrasound visualization of an intrauterine gestational sac, this is known as "implantation failure." Cases when more than two times implantation failure occurred was defined as recurrent implantation failure (RIF). Additional testing is done at this stage of infertility treatment to avoid a repeat of the same result with a future in vitro fertilization (IVF) effort. AIM OF THE STUDY: The study aimed to evaluate predictive value of using embryo transfer personalization bec
