Defining candidate drug characteristics for Long-QT (LQT3) syndrome.

Mutations of the SCN5A gene can significantly alter the function of cardiac myocyte sodium channels leading to increased risk of ventricular arrhythmia. Over the past decade, detailed Markov models of the action potential of cardiac cells have been developed. In such models, the effects of a drug can be treated as alterations in on- and off rates between open and inactivated states on one hand, and blocked states on the other hand. Our aim is to compute the rates specifying a drug in order to: (a) restore the steady-state open probability of the mutant channel to that of normal wild type chann

[Prevention of sudden death in congenital long-QT syndrome].

Congenital long QT syndrome (LQTS) is associated to an increased risk of ventricular arrhythmia, syncope and sudden cardiac death (SD). Four disease genes have been identified and different mutations described in each gene. This locus heterogenicity appears to have important functional and prognostic implications. Sympathetic imbalance has been invoked to explain an arrhythmogenic substrate. Prolonged repolarization is associated to increased dispersion of repolarization enhancing the propensity to develop early afterdepolarizations that may initiate polymorphic ventricular tachycardia (torsad

Long QT syndrome causing an apparently typical first seizure.

Long QT syndrome is an uncommon cardiac condition characterised by QT prolongation with a risk of syncope and sudden death from torsades de pointes ventricular arrhythmias. Seizures and seizure-like episodes can occur, and there may be a delay in recognising them as the underlying cause. A 37-year-old man who presented with a seemingly uncomplicated single seizure was later found to have long QT syndrome. His initial ECG showed a prolonged QTc (QT interval corrected for heart rate) of 507 ms (normal 350-450), while subsequent 'interictal' ECGs were reportedly normal. We highlight the importanc

Prenatal presentation of fetal bradycardia and long QT syndrome.

OBJECTIVES: Fetal bradycardia may be defined as a ventricular rate more than 2 SD below the gestational-age-specific mean. Sinus bradycardia, functional 2:1 atrioventricular block (AVB) and/or polymorphic ventricular tachycardia (VT) are recognized as potential presentations of fetal long QT syndrome (LQTS). The objectives of this study were to describe the associations with LQTS and outcomes of fetuses presenting with isolated sinus bradycardia, 2:1 AVB or VT. METHODS: This was a retrospective review of all cases presenting with sinus bradycardia, non-immune mediated 2:1 AVB or VT on referra

Cardiac Channelopathies in the Pediatric Patient: Long QT Syndrome.

Congenital long QT syndrome is the most common inherited arrhythmia syndrome and is characterized by prolongation of the QT interval in the absence of structural heart disease or external factors. Disease will often manifest during childhood resulting in life-threatening arrhythmias or sudden cardiac death. Diagnosis involves detailed personal and family history in combination with clinical testing. Genetic testing has a high diagnostic yield, though a negative result does not rule out the presence of disease. The incidence of arrhythmia events is significantly decreased with trigger avoidance

Genetic variant annotation scores in congenital long QT syndrome.

BACKGROUND: Congenital Long QT Syndrome (LQTS) is a hereditary arrhythmic disorder. We aimed to assess the performance of current genetic variant annotation scores among LQTS patients and their predictive impact. METHODS: We evaluated 2025 patients with unique mutations for LQT1-LQT3. A patient-specific score was calculated for each of four established genetic variant annotation algorithms: CADD, SIFT, REVEL, and PolyPhen-2. The scores were tested for the identification of LQTS and their predictive performance for cardiac events (CE) and life-threatening events (LTE) and then compared with th

Dose response to nadolol in congenital long QT syndrome.

BACKGROUND: Beta-blocker therapy is the cornerstone of treatment for patients with long QT syndrome (LQTS). Few details on the dose to be used are available. As the response is variable between patients, we systematically evaluated the effect of treatment by performing an exercise test. OBJECTIVE: The purpose of this study was to explore dose response to nadolol on exercise test in LQTS patients in order to propose a more personalized therapeutic approach. METHODS: LQTS patients followed at the Reference Centre for Hereditary Arrhythmic Diseases of Nantes with at least 1 exercise test under

Long QT syndrome-associated mutations in intrauterine fetal death.

IMPORTANCE: Intrauterine fetal death or stillbirth occurs in approximately 1 out of every 160 pregnancies and accounts for 50% of all perinatal deaths. Postmortem evaluation fails to elucidate an underlying cause in many cases. Long QT syndrome (LQTS) may contribute to this problem. OBJECTIVE: To determine the spectrum and prevalence of mutations in the 3 most common LQTS susceptible genes (KCNQ1, KCNH2, and SCN5A) for a cohort of unexplained cases. DESIGN, SETTING, AND PATIENTS: In this case series, retrospective postmortem genetic testing was conducted on a convenience sample of 91 unexpla

Subclinical cardiomyopathy and long QT syndrome: an echocardiographic observation.

OBJECTIVE: Long QT syndrome (LQTS) is a cardiac channelopathy predisposing to syncope and sudden death secondary to LQT-triggered ventricular arrhythmias. Long QT syndrome has been regarded as a purely electrical disease. Recent reports have shown by echocardiography that LQTS patients have contraction abnormalities that are associated with cardiac arrhythmias. The purpose of this study was to determine the spectrum and prevalence of echocardiographic anomalies in a large cohort of patients diagnosed genetically and/or clinically with LQTS. OUTCOME MEASURES: Two-dimensional and Doppler echoca

Selective acquired long QT syndrome (saLQTS) upon risperidone treatment.

BACKGROUND: Numerous structurally unrelated drugs, including antipsychotics, can prolong QT interval and trigger the acquired long QT syndrome (aLQTS). All of them are thought to act at the level of KCNH2, a subunit of the potassium channel. Although the QT-prolonging drugs are proscribed in the subjects with aLQTS, the individual response to diverse QT-prolonging drugs may vary substantially. CASE PRESENTATION: We report here a case of aLQTS in response to small doses of risperidone that was confirmed at three independent drug challenges in the absence of other QT-prolonging drugs. Neonatal seizures and long QT syndrome: a cardiocerebral channelopathy?

We identified a patient with electrophysiologically verified neonatal long QT syndrome (LQTS) and neonatal seizures in the presence of a controlled cardiac rhythm. To find a cause for this unusual combination of phenotypes, we tested the patient for mutations in seven ion channel genes associated with either LQTS or benign familial neonatal seizures (BFNS). Comparative genome hybridization (CGH) was done to exclude the possibility of a contiguous gene syndrome. No mutations were found in the genes (KCNQ2, KCNQ3) associated with BFNS, and CGH was negative. A previously described mutation and a

Acquired long QT syndrome secondary to cesium chloride supplement.

The use of complementary medication is on the rise worldwide. More often than not, the treating physicians are unaware of this and also unfamiliar with the potential benefit or toxicity of the agents. Here, we present the case of a 39-year-old woman who presented with new onset of syncope as a result of acquired long QT syndrome secondary to taking a cesium chloride supplement. A brief discussion of the pathophysiology of this agent follows the case presentation.

Tissue Doppler echocardiography in patients with long QT syndrome.

BACKGROUND: Congenital long QT syndrome (LQTS) is a well-defined clinical entity associated with a high mortality among untreated patients. Tissue Doppler (TD) echocardiography that has been recently introduced, facilitates wall motion analysis. Therefore, to further characterize myocardial velocity abnormalities associated with LQTS, using TD and conventional echocardiography, we compared control subjects and LQTS patients. METHODS AND RESULTS: Ten patients with mild LQTS and 14 control subjects were examined with standard and TD echocardiography. We studied myocardial velocities in basal an

Human congenital long QT syndrome: more than previously thought?

Mutations in KCNQ1 and KCNE1, the alpha- and beta-subunits of the I(KS) K+ channel, produce the cardiac long QT (LQT) syndrome. These subunits are expressed in heart and inner ear, but also in epithelial tissues such as kidney or intestine where their functional roles have remained elusive. Recent work has shown that KCNE1-deficient mice display chronic hypokalemia and hyperaldosteronism. These results have significant implications for human congenital LQT syndromes because hypokalemia increases the risk of ventricular arrhythmia and cardiac sudden death.

[Long QT syndrome under cisapride in neonates and infants].

BACKGROUND: Cisapride is frequently used in the newborn and infant for treatment of gastroesophageal reflux. Twisting-spikes have been reported in adults due to overdosage or therapeutic interaction. We report seven cases of QT prolongation in infants treated with cisapride. PATIENTS AND METHODS: Seven children (one full-term, two mature preterms, four preterm babies), aged (mean, range) 41.8 +/- 21 days (14-79) weighing 2.1 +/- 1.1 kg (1.2-4), free from any cardiac abnormality, except one patent ductus arteriosus, have been studied by ECG and Holter monitoring. They received cisapride at a m

Long QT syndrome associated with syndactyly identified in females.

The identification of female children with this syndrome is evidence that this disorder is not X-linked in inheritance. Possible inheritance modes still include autosomal recessive or, more likely, a de novo mutation, given the absence of family history in any of the patients. Children of both sexes with syndactyly should be screened with an electrocardiogram for this syndrome. Female children with this syndrome may have an increased risk of sudden death similar to male children.

[Atypical form of long-QT syndrome--a case report].

An unusual case of familial Long QT syndrome is reported. After a longstanding symptom free interval this eight year old girl experienced multiple episodes of malignant ventricular arrhythmias including ventricular tachycardias of the torsades-de-pointes type and ventricular flutter. The treatment with beta- and alpha-blocking agents, phenytoin, calcium antagonists, clonidine and magnesium in increasing doses and varying combinations was unsuccessful. A left sided cardiac sympathectomy failed also to control the life threatening ventricular arrhythmias and to decrease the relative high mean he

[Fetal torsades de pointe and the long QT syndrome].

The authors report a case of pregnancy interrupted by caesarian section at 38 weeks' gestation for attacks of fetal ventricular tachycardia. The recording of a long QT interval at birth indicated a diagnosis of fetal torsades de pointe. These attacks recurred 2 hours after birth leading to cardiac arrest. The arrhythmia was immediately controlled by betablockers therapy. The diagnostic, epidemiological and physiopathological features of the long QT syndrome are reviewed. The links with maturation of the sympathetic nervous system are recalled. Finally, a diagnostic and therapeutic strategy is
