The associations between staffing hours and quality of care indicators in long-term care.

BACKGROUND: Long-term care (LTC) staffing practices are poorly understood as is their influence on quality of care. We examined the relationship between staffing characteristics and residents' quality of care indicators at the unit level in LTC homes. METHODS: This cross-sectional study collected data from administrative records and resident assessments from July 2014 to June 2015 at 11 LTC homes in Ontario, Canada comprising of 55 units and 32 residents in each unit. The sample included 69 registered nurses, 183 licensed/registered practical nurses, 858 nursing assistants, and 2173 residents

Quality of in-home care, long-term care placement, and the survival of persons with dementia.

PURPOSE OF THE STUDY: Caregivers of persons with dementia living at home adopt a variety of caregiving styles that vary in quality. Three styles of high-quality care and poor-quality care have been identified. The outcomes, however, of varying styles of caregiving are unknown. Our purpose was to investigate the linkage of quality of care to long-term care placement and survival. DESIGN AND METHODS: We used a sample of 148 primary caregivers of a relative living at home and needing assistance due to memory or thinking problems. We used items from four existing scales and five new items to cons

Dimensions of care for dementia sufferers in long-term care institutions: are they related to outcomes?

OBJECTIVES: This study empirically examined whether dimensions of care cluster in special care units (SCUs) compared with non-SCUs. The relationship between SCU status plus separate measures of the dimensions of care and outcomes for dementia sufferers was then investigated. METHODS: Data were drawn from the Intermediate Care Facility Project. The sample (N = 510) included residents with dementia, aged 65 and older, in intermediate care facilities throughout the province of British Columbia. Canada. Longitudinal data included 6 outcomes: cognitive function, behavioral problems of agitation an

Navigating (Long-Term) Care in Crisis: A Critical Ethnography of Care Partners in a Pandemic.

The COVID-19 pandemic highlighted significant vulnerabilities in long-term care (LTC) homes, severely impacting residents and care partners. This study investigates how care partners of older adults living in Ontario LTC homes perceived residents' experiences during the COVID-19 pandemic, and how those perceptions shaped their own caregiving experiences. Using critical ethnography, we identified four key themes: (a) masks and miscommunication, (b) loneliness and loss, (c) from interaction to isolation, and (d) loss of the advocacy role. Supportive actions included transparent masks, increased

Moral sensitivity of long-term care givers: A qualitative study.

BackgroundChina's rapidly aging population and the implementation of long-term care insurance have increased the demand for long-term care givers (LTCGs), a certified non-nursing workforce with both practical and ethical responsibilities. Research on moral sensitivity has predominantly focused on nurses, leaving the experiences of LTCGs underexplored, particularly through qualitative inquiry.Research aimThis study characterizes how moral sensitivity is experienced and enacted by certified Chinese LTCGs and examines how the provisional dimensions of moral duty, moral burden, and moral strength

Monitoring of wheelchair use in long-term memory care units.

Residents in long term care (LTC) with Alzheimer's disease (AD) or related dementias (ADRD) who require wheelchairs to accommodate impaired mobility experience increased risk of falls and sitting-related pressure injuries (PrIs). Addressing that risk requires a multifactorial approach and strategic prioritization of care needs. Objective data about patterns of wheelchair use, including timing of transfers and overall time in chair could inform decisions about strategies to reduce fall or pressure injury risk. This pilot feasibility study used passive data collection from force sensors placed u

Social connectedness in long-term care settings: A concept analysis.

BACKGROUND: Social connectedness is associated with improved health outcomes, yet long-term care residents are separated from loved ones and vulnerable to social isolation. This paper presents a concept analysis of social connectedness in long-term care. METHODS: Utilizing the Walker and Avant method, PsycINFO and CINAHL databases were searched. In total, 22 studies were examined that emphasized the critical nature of social connectedness to health in long-term care. FINDINGS: The attributes of social connectedness identified were social support, social identity, and reciprocity. A new defin

Rationing Rights: Administrative Burden in Medicaid Long-Term Care Programs.

CONTEXT: States use Medicaid waivers to provide supports for disabled people in communities rather than in institutions. Because waivers are not entitlements, those deemed eligible are not guaranteed these supports. How do states, in practice, use bureaucratic procedures to ration this "conditional" right? METHODS: Drawing on primary and secondary data, the authors analyze waiver programs and document state administrative procedures that indirectly and directly ration access. FINDINGS: Burdens indirectly limit disabled people's access to Medicaid home- and community-based services via a comp

Substance Use Disorders in Postacute and Long-Term Care Settings.

Substance use disorders (SUDs) have not been rigorously studied in postacute and long-term care (PALTC) populations. SUDs are among the fastest growing disorders in the community dwelling older population. Untreated SUDs often lead to overdose deaths, emergency department visits, and hospitalizations due to SUD-related adverse effects, especially exacerbation of comorbid physical and mental health conditions. Primary care providers (PCPs) working in PALTC settings can and should play a key role in its prevention and treatment. This clinical review identifies several practical strategies that P

Raynaud's Phenomenon: A Vascular Acrosyndrome That Requires Long-Term Care.

BACKGROUND: Some 5-10% of the German population are affected by Raynaud's phenomenon (RP). In around 10-20% of cases RP arises from an underlying disease, most commonly a connective tissue disease. This review encompasses the diagnosis and differential diagnosis of RP and examines the efficacy of the currently available pharmaceutical and non-pharmaceutical treatment options. METHODS: We conducted a selective literature search in PubMed using the search terms "Raynaud's phenomenon", "Raynaud's syndrome," "vasospasm," "vascular acrosyndrome," and "systemic sclerosis," together with a search of

Transforming Direct Care Jobs, Reimagining Long-Term Services and Supports.

The diverse array of individuals who receive long-term services and supports share one common experience, which is the need for assistance with personal care and/or other daily activities. The direct care workers (including nursing assistants, home health aides, and personal care aides) who provide this assistance play a critical role in keeping individuals safe, supporting their health and well-being, and helping prevent adverse outcomes. Yet despite decades of research, advocacy, and incremental policy and practice reform, direct care workers remain inadequately compensated, supported, and r

Effect of body mass on future long-term care use.

BACKGROUND: Obesity is a known predictor of disability and functional limitations, and, in turn, of health care use. In this study, we aim to explore whether obesity is also a significant risk factor for future long-term care use, overall and by type of care. METHODS: We use multinomial logistic regression analysis on data from the English Longitudinal Study of Ageing (ELSA) for individuals aged 65 and older between 2002 and 2011. Selection issues are tackled using the rich set of control variables, exploiting the data's longitudinal structure and accounting for loss to follow-up (including d

[Bladder exstrophy: Quality of primary care and long-term prognosis].

BACKGROUND: It is universally agreed that successful and gentle initial bladder closure is decisive for favorable long-term outcome. Due to a number of reasons, including a lack of comparable multicenter studies, there are numerous concepts for initial exstrophy closure. DISCUSSION: Therefore, we describe our concept of delayed, staged reconstruction without osteotomy in classical bladder exstrophy, while taking into considerion the available literature on long-term follow-up as well as on own clinical and research data. CONCLUSION: Most notably there are multiple medical but also psychologi

Alzheimer's disease and mortality in traditional long-term care facilities.

Although there is evidence that different types of dementia share similar pathophysiologic mechanisms, research studies support the concept that dementia of the Alzheimer type (AD) is a distinct clinical entity, which may differ in terms of disease progression and outcome. We assessed whether a diagnosis of probable AD in elderly patients admitted to traditional long-term care facilities results in different mortality rates. We analyzed data belonging to a prospective, multi-center (n=4) cohort study involving 378 long-term care facility residents. In our population the prevalence of dementia
