Modulating altered sensory prediction using bilateral auditory cortex high-definition transcranial random noise stimulation and acoustic masking.

OBJECTIVE: To assess the combined effect of high-definition transcranial random noise stimulation (HD-tRNS) of bilateral auditory cortex and acoustic stimulation (AS) on auditory evoked potentials and tinnitus perception. METHODS: A double-blind, randomized, controlled, within-subject crossover trial was conducted with thirteen adults with chronic tinnitus. Each participant completed four sessions, at least 48 h apart, consisting of active HD-tRNS + AS, active HD-tRNS alone, sham HD-tRNS + sham AS and sham HD-tRNS alone. Auditory evoked potentials (including mismatch negativity) and tinnitus

Bilateral Dorsomedial Prefrontal Cortex rTMS for Tinnitus Treatment: A Successful Case.

We present a successful tinnitus treatment case with intensity and distress reduction in a patient subjected to bilateral 10 Hz repetitive transcranial magnetic stimulation (rTMS) to the dorsomedial prefrontal cortex (DMPFC). Subjective tinnitus is the perception of sound in the ears or head when no corresponding external stimulus exists. Approximately 1%-2% of the population report severe tinnitus with daily life impairment. Sham-controlled studies have revealed benefits using rTMS in tinnitus, although the improvement is moderate or temporary, indicating the need for new strategies. Evidence

Relationships between fear of flying, loudness dependence of auditory evoked potentials and frontal alpha asymmetry.

Previous research has suggested that fear of flying, which is defined as a situational, specific phobia, could overlap with depressive and anxiety disorders. Whether the neuronal dysfunctions including altered serotonergic activity in the brain and altered neural oscillations observed for depressive and anxiety disorders also overlap with alterations in fear of flying is unclear. Here, thirty-six participants with self-reported fear of flying (FF) and forty-one unaffected participants (NFF) were recruited. The participants completed the Beck Depression Inventory (BDI-II), the State-trait Anxie

Emotion dynamics and tinnitus: Daily life data from the "TrackYourTinnitus" application.

It is well established that emotions influence tinnitus, but the role of emotion dynamics remains unclear. The present study investigated emotion dynamics in N = 306 users of the "TrackYourTinnitus" application who completed the Mini-Tinnitus Questionnaire (Mini-TQ) at one assessment point and provided complete data on at least five assessment points for the following state variables: tinnitus loudness, tinnitus distress, arousal, valence. The repeated arousal and valence ratings were used for two operationalizations of emotion dynamics: intra-individual variability of affect intensity (pulse)

Loudness adaptation: real or illusory.

There is general agreement that, except at low sensation levels, the loudness of a continuous pure tone does not appear to change with time. By contrast, using a simultaneous dichotic loudness balancing procedure (SDLB) a substantial fall in loudness can be revealed. As a result it has been held that the latter does not represent a loudness loss in real terms but is a manifestation of some form of central binaural interaction possibly involving localization mechanisms. In two studies reported here, one involving a loudness scaling procedure using as a standard a reference tone of 65 dB (HL) an

A comparison of gain for adults from generic hearing aid prescriptive methods: impacts on predicted loudness, frequency bandwidth, and speech intelligibility.

BACKGROUND: Prescriptive methods have been at the core of modern hearing aid fittings for the past several decades. Every decade or so, there have been revisions to existing methods and/or the emergence of new methods that become widely used. In 2001 Byrne et al provided a comparison of insertion gain for generic prescriptive methods available at that time. PURPOSE: The purpose of this article was to compare National Acoustic Laboratories-Non-linear 1 (NAL-NL1), National Acoustic Laboratories-Non-linear 2 (NAL-NL2), Desired Sensation Level Multistage Input/Output (DSL m[i/o]), and Cambridge M

Suggested deafness during hypnosis and simulation of hypnosis compared to a distraction and control condition: A study on subjective experience and cortical brain responses.

Hypnosis is a powerful tool to affect the processing and perception of stimuli. Here, we investigated the effects of hypnosis on the processing of auditory stimuli, the time course of event-related-potentials (ERP; N1 and P3b amplitudes) and the activity of cortical sources of the P3b component. Forty-eight participants completed an auditory oddball paradigm composed of standard, distractor, and target stimuli during a hypnosis (HYP), a simulation of hypnosis (SIM), a distraction (DIS), and a control (CON) condition. During HYP, participants were suggested that an earplug would obstruct the pe

Lidocaine injections to the otic ganglion for the treatment of tinnitus-A pilot study.

Tinnitus is defined as the perception of noise without an external acoustic stimulus. Due to the heterogeneity of tinnitus, no treatment has proven equally beneficial to every single of these patients. Previous studies have shown that trigeminal input can interfere with the perception of tinnitus. Therefore, we aimed to explore the therapeutic potential of lidocaine injections in trigeminal structures. We conducted a pilot study with 19 participants to explore feasibility and tolerability of this approach. The intervention consisted of three injections of lidocaine in the anatomical area of th

The BDNF Val66Met polymorphism regulates vulnerability to chronic stress and phantom perception.

Auditory phantom percepts, such as tinnitus, are a heterogeneous condition with great interindividual variations regarding both the percept itself and its concomitants. Tinnitus causes a considerable amount of distress, with as many as 25% of affected people reporting that it interferes with their daily lives. Although previous research gives an idea about the neural correlates of tinnitus-related distress, it cannot explain why some tinnitus patients develop distress and while others are not bothered by their tinnitus. BDNF Val66Met polymorphism (rs6265) is a known risk factor for affective d

A behavioral method to estimate charge integration efficiency in cochlear implant users.

BACKGROUND: In cochlear implants, pulse amplitude (PA) or pulse phase duration (PPD) can be used to increase loudness. Loudness grows more slowly with increasing PPD, resulting in a larger dynamic range (DR), possibly reflecting "leaky" charge integration associated with neural degeneration due to hearing loss. Here, we propose a method to estimate charge integration efficiency for CI users. NEW METHOD: The DR was measured with increasing PA or PPD, relative to a common threshold anchor with a short PPD (25μs/ph); DRs were converted to the common unit of charge (nC). Charge integration effici

Contextual modulation of olivocochlear pathway effects on loud sound-induced cochlear hearing desensitization.

This study shows that the cochlear hearing losses [temporary threshold shifts (TTSs)] induced by traumatic sound and the effect of olivocochlear (OC) pathways to the cochlea on these hearing losses depend on the context of the sound. Background atraumatic white noise (WN) has been shown to 1) exacerbate loud-pure-tone-induced TTSs, and 2) promote the modulation of TTSs by the uncrossed OC (UOC) pathways additional to the action on TTSs, elicited by binaural loud tones themselves, by the crossed OC (COC) pathway. Here the same atraumatic WN reduced TTSs caused by loud narrow band sound. It also

A computer program for fitting a master hearing aid to the residual hearing characteristics of individual patients.

The electroacoustic characteristics of an experimental master hearing aid are determined by the choice of transducers and settings of 26 controls which include a preamplifier, the gains and limiting levels in each of 12 channels as well as the gain of a final power amplifier. Here, a computer program which aids in the fitting of the electroacoustic characteristics of this aid to the characteristics of a patient's residual hearing is described. The patient's audiologic data and the hearing-aid parameters are entered into the computer and the gain and limiting level of each channel of the hearin

A Dynamically Focusing Cochlear Implant Strategy Can Improve Vowel Identification in Noise.

OBJECTIVES: The standard, monopolar (MP) electrode configuration used in commercially available cochlear implants (CI) creates a broad electrical field, which can lead to unwanted channel interactions. Use of more focused configurations, such as tripolar and phased array, has led to mixed results for improving speech understanding. The purpose of the present study was to assess the efficacy of a physiologically inspired configuration called dynamic focusing, using focused tripolar stimulation at low levels and less focused stimulation at high levels. Dynamic focusing may better mimic cochlear

Touch Helps Hearing: Evidence From Continuous Audio-Tactile Stimulation.

OBJECTIVES: Identifying target sounds in challenging environments is crucial for daily experiences. It is important to note that it can be enhanced by nonauditory stimuli, for example, through lip-reading in an ongoing conversation. However, how tactile stimuli affect auditory processing is still relatively unclear. Recent studies have shown that brief tactile stimuli can reliably facilitate auditory perception, while studies using longer-lasting audio-tactile stimulation yielded conflicting results. This study aimed to investigate the impact of ongoing pulsating tactile stimulation on basic a

Investigation of hearing aid fitting according to the national acoustic laboratories' prescription for non-linear hearing aids and the desired sensation level methods in Japanese speakers: a crossover-controlled trial.

OBJECTIVE: Some studies have directly compared the National Acoustic Laboratories' prescription for non-linear hearing aids (HAs) version 2 (NAL-NL2) and Desired Sensation Level for non-linear HAs version 5 (DSLv5), although none were performed in Japan. As the Japanese language is a tonal language that has different linguistic characteristics than those of the studied languages, we compared the outcomes of the NAL-NL2 and DSLv5 in hearing-impaired Japanese participants. METHODS: A crossover-controlled trial was conducted on 18 first-time HA users with bilateral moderate sensorineural hearing

Audiological considerations in Ménière's disease.

The audiological findings in Ménière's disease are reviewed particularly in respect of loudness recruitment and speech discrimination loss. Although recruitment is pathognomonic of all cochlear lesions, certain distinguishing features are apparent in Ménière's disease which differ from those in other non-metabolic disorders of the cochlea. With mild degrees of hearing loss, recruitment appears to aid speech discrimination supporting the suggestion that it represents the ears adjustment to failing hearing. With more profound degrees of hearing loss, there is a linear inverse relationship betwee

Conceptual framework for tinnitus: a cognitive model in practice.

Tinnitus is a conscious attended awareness perception of sourceless sound. Widespread theoretical and evidence-based neurofunctional and psychological models have tried to explain tinnitus-related distress considering the influence of psychological and cognitive factors. However, tinnitus models seem to be less focused on causality, thereby easily misleading interpretations. Also, they may be incapable of individualization. This study proposes a Conceptual Cognitive Framework (CCF) providing insight into cognitive mechanisms involved in the predisposition, precipitation, and perpetuation of ti

Noise induced hearing loss and other hearing complaints among musicians of symphony orchestras.

OBJECTIVES: An investigation of the hearing status of musicians of professional symphony orchestras. Main questions are: (1) Should musicians be treated as a special group with regard to hearing, noise, and noise related hearing problems (2) Do patterns of hearing damage differ for different instrument types (3) Do OAE have an added value in the diagnosis of noise induced hearing loss (NIHL) in musicians. METHODS: 241 professional musicians, aged between 23-64 participated. A brief medical history and the subjective judgment of their hearing and hearing problems were assessed. Musicians were

Stress-Related Functional Connectivity Changes Between Auditory Cortex and Cingulate in Tinnitus.

The question arises whether functional connectivity (FC) changes between the distress and tinnitus loudness network during resting state depends on the amount of distress tinnitus patients' experience. Fifty-five patients with constant chronic tinnitus were included in this study. Electroencephalography (EEG) recordings were performed and seed-based (at the auditory cortex) source localized FC (lagged phase synchronization) was computed for the different EEG frequency bands. Results initially demonstrate that the correlation between loudness and distress is nonlinear. Loudness correlates with
