BACKGROUND: Immunosuppressed patients are at an increased risk for developing odontogenic and orofacial infections, which can present with atypical processes and features that may mimic rare but life-threatening infections such as Ludwig's angina. Differentiating cellulitis from a deep neck space infection is often challenging in this population in acute settings due to a broad differential diagnosis and blunted inflammatory responses. This diagnostic uncertainty complicates acute risk stratification and may delay recognition of conditions requiring early airway evaluation and intervention.

OBJECTIVES: Ludwig's angina is a potentially lethal submandibular space infection. We aim to describe the epidemiological characteristics of Ludwig's angina patients presenting to the emergency department (ED) and to examine outcomes and resource utilization to determine their burden on ED and hospitals.
METHODS: Using the Nationwide Emergency Department Sample database, a nationally representative all-payer database, we retrospectively reviewed all ED visits between 2006 and 2014 for patients admitted with a primary diagnosis of Ludwig's angina (International Classification of Diseases, Nint

OBJECTIVE: To investigate the effectiveness of negative pressure wound therapy (NPWT) for Ludwig's angina (LA).
METHODS: We retrospectively reviewed 18 patients with LA admitted to the 82nd Group Army Hospital of PLA between October 2014 and October 2021. All patients underwent surgical drainage and debridement within 6 h after admission. A minimally invasive approach involving bilateral small incisions in the submandibular area was used to perform the procedure. An NPWT device was applied for positive drainage of the involved spaces after debridement. Postoperatively, the patients received t

Necrotising descending mediastinitis may rarely originate from Ludwig's angina, which is an infection of the submandibular space. The use of the bilateral pectoralis major muscle flap for the treatment of sternal wound dehiscence is common, but reports of the unilateral application of this flap are scarce. This study aims to report the use of the unilateral pectoralis major muscle flap for the treatment of sternotomy dehiscence in a patient with mediastinitis due to Ludwig's angina. A 21-year-old male patient underwent an exploratory cervicotomy and median sternotomy for drainage of a submandi

BACKGROUND: Ludwig's angina is a potentially life-threatening condition characterized by bilateral cellulitis of the submandibular, submental, and sublingual spaces. Intravenous (I.V) penicillin G or amoxicillin-clavulanate (Augmentin) has been recommended for use as empirical management before obtaining culture and sensitivity results.
AIM: The aim of this study was to compare the therapeutic efficacies and clinical outcomes of I.V benzylpenicillin with I.V Augmentin in the empirical management of Ludwig's angina.
METHODS: This was a prospective randomized clinical study carried out to meas

Ludwig angina is a life-threatening type of soft tissue cellulitis involving 3 compartments on the floor of the mouth including the submental, sublingual, and submandibular spaces bilaterally. Prevention, early recognition, and treatment of Ludwig angina are critical because this is a clinical diagnosis with unpredictable progression. This article describes a rare case of Ludwig angina that evolved from an odontogenic infection and the specific microbiology and clinical course and discusses possible etiologies and prevention.

Ludwig angina is a rare but potentially lethal infection of the submandibular space that can cause significant upper airway obstruction. We report a case of undiagnosed Ludwig angina that progressed rapidly to death. Ludwig angina was suspected after post-mortem computed tomography (PMCT) found swollen mylohyoid muscle with stranding in subcutaneous fat, thickening of deep fascia, and local lymphadenopathy. Subsequently, an autopsy revealed woody induration of the submental region and liquefactive necrosis of the mylohyoid muscle, confirming the diagnosis. It is likely that the dental abscess

Two cases of Ludwig's angina treated successfully led us to review the main English language literature from 1945 to January 1979. The most common cause of Ludwig's angina is dental, and the organism cultured most often is streptococcus. However, other avenues of infection and numerous pathogens have been implicated in this disease. Penicillin remains the preferred primary antibiotic, with combinations of this drug and other antibiotics being used by various authors. In both the pre-antibiotic and antibiotic era, the airway remains the prime concern of the surgeon in the management of this pot

INTRODUCTION: Ludwig angina is a potentially fatal condition characterized by soft tissue infection of the submandibular, sublingual, and submental compartments. Pseudo-Ludwig angina is a rare condition characterized by sublingual swelling of non-infectious etiology, typically in the setting of supratherapeutic anticoagulation. However, other etiologies, such as angioedema and trauma, have been described.
CASE REPORT: We present the case of a 59-year-old female with pseudo-Ludwig angina that developed in the setting of warfarin therapy and supratherapeutic international normalized ratio. She

BACKGROUND AND OBJECTIVES: Ludwig's angina is a rapidly progressive cellulitis involving the submandibular, sublingual, and submental spaces bilaterally that can swiftly compromise the airway. Data on deep neck space infections in Somalia remain scarce. This study aimed to describe the demographic profile, etiology, clinical presentation, management, complications, length of hospital stay (LOS), and outcomes of patients with Ludwig's angina treated at Mogadishu Somali-Türkiye Recep Tayyip Erdoğan Training and Research Hospital over a one-year period.
MATERIALS AND METHODS: We conducted a retr

Ludwig's angina can seldom be complicated by necrotizing fasciitis. Due to the rapidly progressing nature of this infection and the potential for airway compromise and death, it is important to be aware of different ways in which this disease process can present in order to recognize and treat it emergently. We report here an unusual presentation of a case of Ludwig's angina complicated by necrotizing fasciitis in an elderly patient. The clinical features, diagnosis, and treatment are discussed in detail as well as a brief literature review on craniocervical necrotizing fasciitis.

Background: Ludwig's angina (LA) is a diffuse cellulitis of the submandibular space and adjacent tissues. During the coronavirus disease 2019 (COVID-19) pandemic, odontogenic treatments were often delayed because of the implementation of safety measures to avoid the spread of the virus. We hypothesized that delayed odontogenic treatments associated with the onset of the COVID-19 pandemic would be associated with an increase in the incidence of LA and worse outcomes related to these infections. Patients and Methods: Patients from June 2018 to June 2022 with computed tomography images suggestive

We present an unusual case of Ludwig's angina secondary to a left tonsillar phlegmon in a previously fit and well 50-year-old woman. This tonsillar phlegmon spread along the peritonsillar/parapharyngeal plane to cause a diffuse cellulitis and collection in the submental, sublingual, and submandibular spaces despite empirical intravenous antibiotic therapy.

Odontogenic infections are commonly considered a cause of Ludwig's angina; their progression to necrotising fasciitis (NF) is rare and often associated with significant morbidity and mortality. We report a case of a previously healthy patient in his 20s, a heavy smoker, who developed extensive cervicothoracic NF after wisdom tooth extraction. He initially presented with neck swelling and pain, with stable vital signs and no airway compromise. Within 24 hours, he deteriorated abruptly with respiratory distress and septic shock. CT showed widespread fascial involvement with emphysema tracking in

Pediatric deep neck space infections (DNSIs)-peritonsillar, retropharyngeal, and parapharyngeal abscesses and Ludwig's angina-are uncommon but potentially life-threatening due to rapid progression to airway obstruction, sepsis, and mediastinal extension. This narrative review summarizes evidence for emergency clinicians. A literature search of PubMed/Scopus (2000-2025) identified studies on pediatric DNSI epidemiology, imaging, microbiology, and management. DNSIs peak at ages 2-5 years because of prominent retropharyngeal lymph nodes. Nonspecific symptoms (fever, irritability, neck stiffness)

Ludwig’s angina, although uncommon, remains a potentially life-threatening condition because of the risk of impending airway obstruction. Effective treatment is based on early recognition of the clinical process, with the appropriate use of parenteral antibiotics, securing the airway, and formal surgical drainage of the infection. Awake fiberoptic intubation under topical anesthesia may be the preferred method to secure the airway. Flexible nasotracheal intubation requires skill and experience. When fiberoptic bronchoscopy is not feasible, not available, or has failed, an elective awake cricot

Sublingual hematoma secondary to excessive anticoagulation is a rare, life-threatening condition. Reports in the literature have emphasized the importance of a prompt reversal of the causative coagulopathy by intravenous administration of vitamin K and fresh frozen plasma. In the event of an unstable airway, surgical intervention via tracheostomy or cricothyroidectomy is advocated. We report a case of sublingual hematoma that was treated conservatively, and we discuss the presentation and management of this entity.
