Post-traumatic osteonecrosis of the lunate after fracture of the distal radius.

We present a case of post-traumatic osteonecrosis of the lunate after fracture of the distal radius. Post-traumatic osteonecrosis of the carpal lunate after a fracture of the distal radius has, to our knowledge, not been reported previously. We treated the patient with vascularised bone graft from the distal radius, with a satisfactory result.

The anatomy and vascularity of the lunate: considerations applied to Kienböck's disease.

PURPOSE: The purpose of this study was to assess the anatomy and vascularity of the lunate. The genesis of lunatomalacia requires some combination of vascular risk and mechanical predisposition. The findings will be correlated with the major existing theories of the cause of Kienböck's disease. METHODS: We studied 27 cadaver upper limbs using latex injection and the Spalteholz technique. We investigated the blood supply to the lunate. In 24 wrists we evaluated the incidence and distribution of anatomic features, arthrosis, and soft tissue lesions. We correlated the lunate morphology and ligam

[Significance of intraosseous pressure for pathogenesis of Kienböck disease].

Femoral head necrosis is thought to be the consequence of decreased venous drainage. To confirm this pathogenetic model in lunatomalacia, the intraosseous pressure in 16 necrotic, 16 normal lunates and 16 normal capitates was measured. Intraosseous pressure was recorded in different functional positions: neutral position, extension, flexion, venous stasis, and exsanguinated conditions. Both in extension and in flexion, the intraosseous pressures of all bones exceeded those in neutral position; this corresponded to the increase in pressure during venous stasis. The increase in pressure in norma

3D printing lunate prosthesis for stage IIIc Kienböck's disease: a case report.

Stage IIIc Kienböck's disease is a clinical challenge to treat collapse of the lunate bone. A new reconstructive surgery was described in one patient using 3D printing lunate prosthesis. The prosthesis shape was designed by tomographic image processing and segmentation using technology compared with the intact side matched by mirror symmetry and 3D post-processing technologies. The patient recovered nearly full range of motion of the wrist after 12 months. The visual analog scale scores and Cooney scores were 2 points and 91 points. We demonstrated that an anatomical reconstruction to Kienböck

Improvement in lunate perfusion after radial closing-wedge osteotomy in patients with Kienböck's disease.

Radial closing-wedge osteotomy is a widely accepted treatment for Kienböck's disease. However, despite favorable long-term clinical outcomes, its impact on lunate perfusion has not been documented. The purpose of this study was to determine whether radial closing wedge osteotomy improved lunate perfusion on gadolinium (Gd)-enhanced magnetic resonance imaging (MRI). We conducted a retrospective review of 12 patients with Kienböck's disease who received radial closing wedge osteotomy. Mean age at surgery was 25 years. Preoperative Gd-enhanced MRI was performed in 7 patients. After bone union, im

The Optimal Donor Site From the Foot as a Nonvascularized Osteochondral Graft for the Reconstruction of Lunate Facet Defects in Distal Radius Intra-Articular Fracture: A Computed Tomography and Cadaveric Study.

PURPOSE: This study aimed to identify the best donor sites for a nonvascularized osteochondral graft from the foot to reconstruct lunate facet defects in malunited distal radius fractures. METHODS: Fifty-six wrist computed tomography (CT) scans, obtained from scaphoid fracture patients, assessed the lunate facet's articular surface for dorsal and volar width, dorsovolar length, and concavity depth. Additionally, 60 foot CT scans, obtained from calcaneus fracture patients, assessed the second and third metatarsals and cuneiforms for dorsal and plantar width, dorsoplantar length, concavity/conv

Outcome of treatment of complex, perilunate fracture-dislocation of the wrist: a case report.

Complex, perilunate fracture-dislocations of the wrist comprise severe and challenging injuries, characterised by a total loss of contact between the lunate bone and the head of the capitate as well as fracture of one, or more bones surrounding lunate bone. The principle of treatment of such injuries involves stable fixation of fractures, anatomical reduction of the displaced carpal bones with temporary arthrodesis wit K-wires to maintain their position. We present a patient who sustained a complex, perilunate fracture-dislocation of the wrist with fractures of the scaphoid, hamate and base of

Kienbock's Disease: Treatment Options - A Search for the Apt!

Kienbock's disease remains a condition of uncertain etiology and our understanding about the pathogenesis is still evolving. However, a lack of consensus on the treatment is the most striking as there are wide array of treatment options described ranging from 'doing nothing' to a free vascularized bone graft reconstruction of the lunate. Furthermore, most treatment modalities report equivalent success rate but cumulative evidence is lacking. This 'black hole' in the literature presents difficulties to surgeons treating the condition and even patient's confidence with the treatment. All the tre

Osteoarthritis of the wrist secondary to non-union of the scaphoid.

The surgical treatment of osteoarthritis of the wrist secondary to pseudarthrosis of the scaphoid is presented in the light of twenty years experience, and with improved knowledge of the pathological anatomy of this problem. The condition is difficult to treat owing to the varied pathology, the different needs of the patients and the alternative treatments available. Where a pseudarthrosis exists with involvement of the scaphoid-radius joint only, we recommend radial styloidectomy and repair of the pseudarthrosis. Where the pseudarthrosis is associated with carpal collapse which can be correct

[Alloplastic partial replacement of the scaphoid bone].

Operative treatment for non-union of the carpal scaphoid bone with a small proximal fragment often presents some difficulties. A complete Silastic carpal scaphoid replacement does not always fit well because the shape and dimensions of the implant differ from the natural carpal scaphoid bone. The proximal fragment looks more like the lunate bone and so a Silastic carpal lunate implant may be used to replace the proximal fragment of the scaphoid while the well preserved distal fragment remains in place. This prevents disturbance of the intercarpal joint composition and improves stability of the

Unusual combined scaphoid and lunate fracture of the wrist: a case report.

All perilunate fracture-dislocations combine ligament ruptures, bone avulsions, and fractures in a variety of clinical forms. The most frequent is the dorsal trans-scaphoid perilunate dislocation. In rare cases, however, these dislocations also have been associated with capitate fractures, triquetral fractures, or lunate fracture. We report a combined scaphoid and lunate fracture of the wrist that was not associated with perilunate dislocation.

Anterior wedge-shaped bone graft for old scaphoid fractures or non-unions. An analysis of relevant carpal alignment.

For appraisal of anterior wedge-shaped grafts for the humpback deformity of the scaphoid, a retrospective study of 27 cases with old scaphoid fractures or non-unions was carried out. 11 cases were treated with Herbert screw fixation and anterior wedge-shaped graft and the other cases with other methods. For the assessment of carpal alignment, radio-lunate and scapho-lunate angles were measured with peri-operative radiographs. For the clinical assessment, the scoring system of Cooney was applied. In 25 cases, primary bone union was obtained with one attempt and in two cases, with the second ope

Computed tomography osteoabsorptiometry alterations in stress distribution patterns through the wrist after radial shortening osteotomy for Kienböck disease.

PURPOSE: The distribution pattern of subchondral bone density is considered to closely reflect the stress distribution across a joint under physiological loading conditions. Our purpose was to determine alterations in the distribution pattern of subchondral bone density across the distal articular surfaces of the radius and the ulna in patients with Kienböck disease after radial shortening. METHODS: We collected preoperative and postoperative computed tomography (CT) image data from 7 wrists of 7 patients who had undergone radial shortening for Kienböck disease. We measured the distribution o

More than 40 years' follow-up of an unconstrained metal lunate replacement for the treatment of Kienböck's disease.

Kienböck's disease is characterised by avascular necrosis of the lunate bone, and over the years it has been a challenging disease to manage, with differing opinions on the best intervention. We present an interesting case of a metallic unconstrained lunate replacement that is still functioning well in a patient 40 years after surgery. This case report represents the longest follow-up of any such prosthesis.

Postoperative magnetic resonance imaging following lunate resection and vascularized os pisiform transfer in Kienböck's disease.

Although good clinical results have been reported following lunate resection and vascularized os pisiform transfer for advanced stages of Kienböck's disease, the occurrence of wrist osteoarthritis has been highlighted. We aimed to investigate the postoperative condition of the pisiform and the surrounding bones in patients who underwent surgery for advanced stages of Kienböck's disease using magnetic resonance imaging. We retrospectively reviewed the data of six patients (mean age, 45 years; Lichtman stage, IIIb) who underwent lunate resection and vascularized os pisiform transfer for advanced

Increased perfusion in dynamic gadolinium-enhanced MRI correlates with areas of bone repair and of bone necrosis in patients with Kienböck's disease.

BACKGROUND: Osteonecrosis of the lunate, Kienböck's disease, can lead to fragmentation of the lunate, carpal collapse, and severe osteoarthritis. Since the etiology of Kienböck's disease is impaired circulation, a diagnostic method capable of assessing perfusion would be valuable. Recent studies have suggested that dynamic contrast-enhanced (DCE) MR examinations at 3 T can assess perfusion in healthy carpal bones. PURPOSE: To evaluate the use of DCE-MR for assessing perfusion in the lunate bone in patients with Kienböck's disease. Furthermore, to compare perfusion with histopathology with a f

Three-Dimensional Analysis of Acute Scaphoid Fracture Displacement: Proximal Extension Deformity of the Scaphoid.

BACKGROUND: Our goal was to analyze the movement of acute scaphoid waist fracture fragments and adjacent bones in a common coordinate system. Our hypothesis was that the distal scaphoid fragment flexes and pronates and the proximal fragment extends. METHODS: Computed tomography (CT) scans of patients diagnosed with an acute scaphoid waist fracture were evaluated using a 3-dimensional (3D) model. The scans of 57 nondisplaced and 23 displaced fractures were compared with a control group of 27 scans showing no pathological involvement of the wrist. Three anatomical landmarks were labeled on the

[Luxations and subluxations of the scaphoid bone].

The rare occurrence and the difficulties in diagnosis make partial and total luxations of the scaphoid to a problematic chapter in hand surgery. The main part in diagnosis lies in the correct interpretation of the X-ray pictures from which the different formes of displacement may be recognized. These include isolated luxations of scaphoid in palmar, radial or dorsal directions, varying degrees of scapho-lunate subluxations and luxations and the special type of isolated subluxation of the scaphoid bone. In the first part of the paper the diagnostic criteria and in particular the radiologic sign
