Reproducibility of transpulmonary thermodilution measurements in patients with burn shock and hypothermia.

Thermal noise, including hypothermia and short-term variations in body temperature, has been reported to influence the accuracy and reproducibility of thermodilution measurements. This variation might theoretically limit the usefulness of this technique in patients with acute burns. We therefore sought to determine the reproducibility of hemodynamic parameters derived from arterial thermal dilution in patients with acute burns and hypothermia. This was a prospective, clinical study of 50 mechanically ventilated patients with burns involving more than 25% of the body surface area that were trea

Physiologic diagnosis and function in asthma.

Asthma is a condition in which there is airway hyperresponsiveness, with the propensity for widespread, reversible airways narrowing on exposure to diverse inciting factors (triggers). Inhalation of nonspecific agents such as methacholine or histamine leads to bronchoconstriction in most cases, and in some, the bronchoconstriction follows exposure to specific agents such as antigen or occupational irritants. Chest tightness and cough, which are the most common symptoms of asthma, are probably the result of inflammation mucus plugs, edema, or smooth muscle constriction in the small peripheral a

Determination of Lung Volume and Hemodynamic Changes During High-Frequency Ventilation Recruitment in Preterm Neonates With Respiratory Distress Syndrome.

OBJECTIVES: To evaluate the changes in end-expiratory lung volume during an oxygenation-guided stepwise recruitment procedure in elective high-frequency ventilation. We hypothesized that high continuous distending pressure impedes pulmonary blood flow as evidenced by reduced lung volume measurements using respiratory inductive plethysmography. Changes in oxygenation, ventilation, and peripheral perfusion were evaluated as secondary outcomes. DESIGN: A prospective, single center, observational, nonrandomized study. SETTING: The study was conducted in a neonatal ICU in Italy. PATIENTS: High-f

Pathophysiology of dialysis related hypoxaemia.

In order to elucidate the pathogenesis of haemodialysis related hypoxaemia, lung function studies, measurements of intra-thoracic fluid changes, leucocytes and complement activity were made in 12 patients during a routine six hours dialysis using cellulose membranes. Coincidently with the fall in arterial oxygen tension, lung function was significantly impaired, paralleled by a decrease in transthoracic impedance, leucopenia and a decrease in plasma complement activity. It therefore is suggested that blood membrane interaction leads to complement-mediated pulmonary leucostasis evoking mild pul

Effect of previous volume history on airway closure.

We studied the effect of volume history on airway closure in six healthy males ranging from 32 to 67 yr of age. The method used was to compare the regional distribution of 133Xe boluses distributed according to N2O uptake during open-glottis breath-hold maneuvers with the regional distribution of boluses of intravenously injected 133Xe. Measurements were made at two lung volumes, one close to residual volume (RV) and the other just below closing volume. The required volume was reached either by expiring from total lung capacity or by inspiring from RV. Although there was considerable airway cl

[A 49 year old male with a giant pulmonary bulla--a case report and review of the literature].

A 49 year old previously healthy smoker was diagnosed with a giant bulla in his right lung, following a history of dry cough, repeated upper airway infections and increasing dyspnea for several years. Computed tomography (CT) confirmed the presence of a giant bulla in the right inferior lobe and several smaller bullae in the right superior lobe. The giant bulla was 17 cm in diameter, occupying more than half of the right hemithorax. On spirometry a moderate restrictive and a mild obstructive pattern was observed. Lung volume was measured with two different techniques, nitrogen washout and plet

It's not all about small airways disease: key limitations of oscillometry interpretation.

An exciting aspect of oscillometry is the ability to detect small airways disease (SAD), particularly through frequency dependence of resistance, commonly measured as R5-R20. While SAD influences oscillometry measurements, it is important to realize that oscillometry is not specific for SAD. In addition, oscillometry measurements do not take into account lung volume, which can influence the measurements and make interpretation less clear. Finally, while oscillometry is very sensitive at detecting abnormalities of lung function, it is not specific for distinct lung diseases. These limitations s

Changes in functional residual capacity and lung mechanics during surgical repair of congenital heart diseases: effects of preoperative pulmonary hemodynamics.

BACKGROUND: To characterize the impact of lung volume changes in the lung function impairment after the surgical repair of congenital heart diseases, combined measurements of functional residual capacity, lung clearance index, and respiratory mechanics were performed in children with hypoperfused lungs (tetralogy of Fallot [TOF]) or with pulmonary hyperperfusion (ventricular septal defect [VSD]). METHODS: Lung volume and clearance were assessed by using a sulfur hexafluoride washout technique, and the mechanical properties of the respiratory system were assessed using a low-frequency oscillat

Impact of bronchoscopic lung volume reduction with endobronchial valves on dynamic hyperinflation: Results from the PIERCE study.

BACKGROUND AND OBJECTIVE: Dynamic hyperinflation (DH) is a major marker of exertional dyspnoea in severe emphysema. We hypothesized that bronchoscopic lung volume reduction (BLVR) using endobronchial valves (EBVs) decreases DH. METHODS: In this prospective bi-centre study from both Toulouse and Limoges Hospitals, we assessed DH during an incremental cycle ergometry before and 3 months after EBVs treatment. The primary objective was to observe the change in inspiratory capacity (IC) at isotime. Target lobe volume reduction (TLVR) and changes in residual volume (RV), forced expiratory volume in

Special article: evaluation of a novel noninvasive respiration monitor providing continuous measurement of minute ventilation in ambulatory subjects in a variety of clinical scenarios.

BACKGROUND: Currently there is no technology that noninvasively measures the adequacy of ventilation in nonintubated patients. A novel, noninvasive Respiratory Volume Monitor (RVM) has been developed to continuously measure and display minute ventilation (MV), tidal volume (TV), and respiratory rate (RR) in a variety of clinical settings. We demonstrate the RVM's accuracy and precision as compared with a standard spirometer under a variety of clinically relevant breathing patterns in nonintubated subjects. METHODS: Thirty-one voluntary subjects completed the primary study. MV, TV, and RR meas

MRI Lung Volume Prediction of ECMO and Mortality in Congenital Diaphragmatic Hernia.

INTRODUCTION: This study evaluated fetal MRI lung measurements with ECMO use and survival of prenatally diagnosed CDH. METHODS: This was a retrospective chart review of 113 patients from January 2012 to December 2023 with prenatally diagnosed congenital diaphragmatic hernias. Lung measurements from prenatal MRIs, infant ECMO use, and survival were analyzed to determine optimal cut-off value for outcome prediction. RESULTS: Patients were categorized as mild (10.1%, n = 11), moderate (34.3%, n = 37), and severe (55.6%, n = 60). Mortality was 29.2% (n = 33), and 52.2% (n = 59) required ECMO. Pa

Lung function tracking in children with perinatally acquired HIV following early antiretroviral therapy initiation.

INTRODUCTION: Lung disease remains a frequent complication in children with perinatal HIV infection (CHIV) and exposure without infection (CHEU), resulting in diminished lung function. In CHIV, early antiretroviral therapy (ART) initiation improves survival and extrapulmonary outcomes. However, it is unknown if there is benefit to lung function. METHODS: Cohorts of CHIV (ART initiated at median 4.0 months), CHEU and HIV-unexposed children (CHU) prospectively performed pulmonary function testing (PFT) consisting of spirometry, plethysmography and diffusing capacity from 2013 to 2020. Individualized positive end-expiratory pressure guided by end-expiratory lung volume in early acute respiratory distress syndrome: study protocol for the multicenter, randomized IPERPEEP trial.

BACKGROUND: In acute respiratory distress syndrome (ARDS), response to positive end-expiratory pressure (PEEP) is variable according to different degrees of lung recruitability. The search for a tool to individualize PEEP based on patients' individual response is warranted. End-expiratory lung volume (EELV) assessment by nitrogen washing-washout aids bedside estimation of PEEP-induced alveolar recruitment and may therefore help titrate PEEP on patient's individual recruitability. We designed a randomized trial to test whether an individualized PEEP setting protocol driven by EELV measurement m

Transpleural ventilation of explanted human lungs.

BACKGROUND: The hypothesis that ventilation of emphysematous lungs would be enhanced by communication with the parenchyma through holes in the pleural surface was tested. METHODS: Fresh human lungs were obtained from patients with emphysema undergoing lung transplantation. Control human lungs were obtained from organ donors whose lungs, for technical reasons, were not considered suitable for implantation. Lungs were ventilated through the bronchial tree or transpleurally via a small hole communicating with the underlying parenchyma over which a flanged silicone tube had been cemented to the s

[Lung function measurements with portable spirometers in children].

In the majority of patients with respiratory disease, the most important diagnostic information can be obtained just by taking a thorough medical history and appraising the general clinical status. Lung function is a valuable tool in routine practice for assessing the general status at the beginning of treatment, for evaluating the clinical course and for documenting the response to therapy. The parameters measured by spirometry are read as volume (liter) or volume per unit of time (flow). If a spirometer is used in pediatrics, its measurements should be linear, within the range of the flow ra

Volume-limited and volume-targeted ventilation.

This article shows that volume-targeted ventilation is physiologically more logical than pressure-limited ventilation, and is associated with a reduced risk of pneumothorax, hypocarbia, duration of ventilation, death or bronchopulmonary dysplasia, and severe intraventricular hemorrhage. Therefore, it should now be adopted as the main mode for mechanical ventilation of preterm neonates.

Pulmonary function in Morquio's disease: A study of two siblings.

Two siblings with characteristic clinical and radiologic features of Morquio's disease are presented. Detailed pulmonary function tests, including vital capacity, flow rates, lung volumes, airway resistance, and lung compliance, were performed. The ventilatory studies suggested a restricted pattern due to chest cage dysfunction. Hypoxemia was noted in one patient and was shown to be due to right-to-left shunting, probably related to the microatelectasis as a result of the restricted chest cage.
