Evaluation of Artificial Fixation of the Incus and Malleus With Minimally Invasive Intraoperative Laser Vibrometry (MIVIB) in a Temporal Bone Model.

BACKGROUND: A significant number of adults suffer from conductive hearing loss due to chronic otitis media, otosclerosis, or other pathologies. An objective measurement of ossicular mobility is needed to avoid unnecessarily invasive middle ear surgery and to improve hearing outcomes. METHODS: Minimally invasive intraoperative laser vibrometry provides a method that is compatible with middle ear surgery, where the tympanic membrane is elevated. The ossicles were driven by a floating mass transducer and their mobility was measured using a laser Doppler vibrometer. Utilising this method, we asse

Ossicular chain mobilisation versus reconstruction in surgery for isolated malleus and/or incus fixation: systematic review and meta-analysis.

OBJECTIVE: To compare post-operative audiometric outcomes for the two prevailing surgical approaches for isolated malleus and/or incus fixation: ossicular mobilisation with preservation of the ossicular chain, and disruption and reconstruction of the ossicular chain. METHODS: A search was conducted, in December 2016, of PubMed, Scopus, and Cumulative Index to Nursing and Allied Health Literature articles written in English. Papers presenting original data regarding post-operative audiometric outcomes in patients who underwent surgical treatment for malleus and/or incus fixation with a mobile

[Stapes surgery performed for congenital stapes foot plate fixation--comparison of prosthesis fixation to the malleus vs. the incus].

We report the surgical results of stapes surgery using the Schukneht-type wire piston prosthesis performed on 30 ears with fixation of the stapes footplate and absence of the long process of the incus. The prosthesis was reformed to avoid dislocation and fixed to the handle of the malleus. The surgical results in another 49 ears, the comparator group, in which the prosthesis was fixed to the long process of the incus because the anomaly was restricted to fixation of the footplate without other associated anomalies, were also analyzed for comparison. The mean postoperative air conduction hearin

Ossicular reconstruction in cases of absent or inadequate incus, congenital malformation of the middle ear and epitympanic fixation of the incus and malleus.

This paper reports on the reconstruction of the sound-conducting system in the middle ear in those ears in which the incus and the stapes suprastructure are either absent or inadequate as after prior otitis media or tympanoplasty. When the incus is inadequate after prior stapedectomy or when the stapes suprastructure and incus are absent in congenital malformations of the middle ear, hearing restoration by ossicular reconstruction can cause problems. In cases in which the malleus head and incus are fixed in the epitympanum with the absence of the stapes suprastructure, the ossicular reconstruc

[Scanning electron microscopy studies in arthritic changes in the malleus incus joint and reflections on the function of the middle ear muscles].

The surfaces of immobilised incus and malleus joints were studied with the scanning electron microscope. The hyaline cartilage showed arthritic changes similar to osteoarthritic big joints of the body. A direct radiologic magnification technique could likewise demonstrate typical arthritic signs. Comparing findings from histologic studies in the literature, we could demonstrate that the intact hyaline cartilage degenerates if the ossicles are immobilised. The minimal static friction of the intact smooth joint surface is a prerequisite for its assumed role namely protection of the inner ear aga

Diagnostic accuracy of intra-operative assessment of de-epithelisation of the malleus by the surgeon in type 1 tympanoplasty compared to histopathological examination.

OBJECTIVE: This study was undertaken to determine the accuracy of the surgeon's assessment in detecting epithelial remnants over the malleus after de-epithelisation in tympanoplasty. METHODS: Intra-operatively, the umbo was assessed for epithelial remnants with the microscope. The umbo was then resected and sent for histopathological examination to detect epithelial remnants. RESULTS: Out of 42 cases, microscopic examination findings for epithelium were positive in 16 cases and negative in 26 cases. Histopathology findings were positive in 13 cases. The surgeons' assessment was accurate only

Morphologic variations of clinically normal mallei and incudes.

OBJECTIVES: Various features and shapes of malleus and incus are reported. Partially or unaddressed are their bilateral symmetry, correlation with otitis media, and concordance of features and shapes. Such information may contribute to the understanding of malleus and incus ontogeny and the installation and function of implantable middle ear devices. We sought to address the following hypotheses: (1) a cranium's malleus and incus have bilateral symmetry, with respect to their features and shapes; (2) features and shapes of malleus and incus are unrelated to the mastoid size indicator of childh

Developmental anomaly of the process of Folius.

We present four cases of an ossicular anomaly considered to consist of an abnormality of the anterior process and manubrium mallei (malleus handle). In one case, a thick bony bar was found extending from the neck of the malleus which fused with the posterior bony wall or the tympanic bone. No trace of the short process and umbo of the malleus was recognized. In two other cases, a similar bony bar was seen as well as a cartilaginous malleus handle that apparently was attached to the anterior part of the bony bar. On the basis of these findings, the bony bar was assumed to represent abnormally d

Anomalies of the middle ear.

Malformations of the ossicles in 52 ears of 48 patients were analyzed on the basis of surgical findings. The external meatus of each of these ears was patent. The long process of the incus and the stapes are frequently involved in this series of observations. There are distinct groups of ossicular anomalies, in each of which patients show similar degrees of hearing loss and ossicular findings. These include spindle handle, malleus bar, and dislocated annulus. Spindle handle gets its name from the shape of the malleus handle. There is disconnection between the long process of the incus and the

Morphometric evaluation of middle ear anatomical structures using full volume (12 × 9) CBCT scans: A retrospective study in Navi Mumbai.

INTRODUCTION: The tympanic cavity contains three tiny bones, the malleus, incus, and stapes, which have a fundamental role in the transmission of sound. Recent research emphasizes the use of CBCT for the anatomic study of the temporal bone. The information about middle ear anatomy on CBCT scans is meager; hence, this retrospective study was conducted to identify and determine the various morphometrical parameters of the malleus using CBCT which can be helpful during reconstructive procedures for the otologic surgeon. MATERIALS AND METHODS: The retrospective study was performed on 200 subjects

[Fixed mallear head syndrome].

Fixed head malleus syndrome is a rare anatomoclinical entity first described by Goodhill in 1966. We present a series of 9 patients who underwent surgery between 1991 and 1997 and discuss the technical procedures used and functional outcome. Ossicular mobility can be re-established with two surgical methods. The more simple method consists in a classical incus transposition with malleus neck section. The more physiological method consists in drilling the synostosis fixing the malleus without disrupting the ossicular chain; stapedotomy is associated in certain cases (Type III).
