Preoperative and postoperative nipple-areola sensibility in patients undergoing reduction mammaplasty.

Objective data on nipple and areola sensibility are scarce. For women with macromastia, there is little published information available indicating the incidence and intensity of postoperative nipple and areola sensibility. This prospective study was undertaken to evaluate nipple and areola sensibility in "small-breasted" control subjects as well as in patients with macromastia before and after reduction mammaplasty. Preoperative and postoperative Semmes-Weinstein pressure threshold testing was performed on 84 breasts in 43 patients and on 12 breasts of A or B cup size in the control group. The

Resident participation in reduction mammaplasty: Safe, efficient, and equivalent in patient satisfaction.

BACKGROUND: Breast reduction surgery is one of the most common procedures in plastic surgery and a cornerstone of resident training. While its safety and benefits are well established, the impact of resident participation on surgical outcomes and patient satisfaction remains underexplored. METHODS: A retrospective single-center study was conducted between January 2019 and January 2023, including 208 patients (416 breasts) who underwent bilateral reduction mammaplasty using an inverted-T technique. Procedures were performed either by a senior surgeon alone (S group, n = 56) or by a senior surg

Dual perfusion superomedial pedicle in reduction mammaplasty - A prospective intraoperative radiographic study.

This study uses intraoperative ultrasound to confirm dual perfusion to the nipple-areolar complex from internal mammary artery and intercostal artery perforators using a superomedial pedicle technique. Findings presented include color-Doppler ultrasound images, anatomical illustration, operative information, and postoperative outcomes. Dual blood supply may improve SMP safety in patients with extremely large and/or ptotic breasts.

Vertical Scar Reduction Mammaplasty Using the Superomedial-Based Pedicle Technique in Gigantomastia.

BACKGROUND: Breast reduction surgery has witnessed significant advancements in recent years; however, it continues to pose challenges for both surgeons and patients when dealing with cases involving excessive breast volume and severe breast ptosis. This study aimed to assess the aesthetic outcomes and the impact on the quality of life, as measured by the BREAST-Q questionnaire, in patients with gigantomastia and severe breast ptosis who underwent reduction mammaplasty using the superomedial-based pedicle technique. METHODS: We present a retrospective series comprising 84 patients who underwen

No-Scar Breast Reduction Utilizing Power-Assisted Liposuction Mammaplasty, Loops, and Lipofilling.

BACKGROUND: In continued interest to develop and refine minimally invasive procedures, recent publications reported a scarless approach in breast lifting. OBJECTIVES: The authors sought to describe a power-assisted lipomodeling technique combined with surgical loops to achieve breast reduction and reshaping with minimal scars. METHODS: Between 2014 and 2018, 94 patients underwent breast reduction by combined liposuction and loops. Following infiltration of the breasts, liposuction of the outer quadrants and the lower pole was achieved to reduce the breast footprint and the lateral and inferi

Post Reduction Mammaplasty Pyoderma Gangrenosum: An Unusual Presentation of a Misdiagnosed Entity.

Pyoderma gangrenosum is a rare skin disorder that could be triggered after surgical trauma. However, more uncommon, this condition could follow breast surgery. An opportune diagnosis and adequate treatment are essential in order to avoid wound chronicity and aesthetic sequels. We report a 51-year-old woman who presented multiple wound complications after a reduction mammaplasty. This particular case is unique, because bilateral and unusual presentation after another uncomplicated previous surgical procedure, and illustrates how a patient could be taken through different rational surgical thera

Superomedial pedicle reduction mammaplasty: increased resection weight does not increase nipple necrosis.

BACKGROUND: The superomedial pedicle (SMP) reduction mammaplasty offers several advantages over more traditional operative techniques, such as retained sensation to the nipple areola complex (NAC) and improved preservation of long-term breast shape. However, many surgeons believe that using the SMP can cause an increase in NAC necrosis rates up to 13.1%, especially in large volume reductions. The aims of this paper are to determine the rate of nipple necrosis when performing an SMP reduction mammaplasty at our institution and establish if there is a difference in those rates based on the weigh

Plasma volume expansion with Haemaccel does not impair haemostasis during reduction mammaplasty.

OBJECTIVES: An in vivo study under well-controlled conditions was undertaken to determine the effect of Haemaccel, a colloidal plasma volume expander, on normal haemostasis. METHODOLOGY: Twenty patients, who were admitted for reduction mammaplasty, were included in this study. A standardised anaesthesia protocol was followed with all patients. Ten patients received 500 ml Haemaccel and 10 controls received 1,500 ml Ringer's lactate, a crystalloid solution. The solutions were administered intravenously during surgery over a period of 30-40 minutes. Standardised clinical observations and haemat

The Pedicled Robertson Mammaplasty: Minimization of Complications in Obese Patients With Extreme Macromastia.

BACKGROUND: Breast reduction for extreme macromastia in obese patients is a potentially high-risk endeavor. Free nipple grafting as well as a variety of pedicled techniques have been advocated for large reductions in obese patients, but the number of different approaches suggests that no single method is ideal. This paper suggests the Robertson Mammaplasty, an inferior pedicle technique characterized by a curvilinear skin extension onto the pedicle, as a potentially favorable approach to this clinical situation. OBJECTIVES: The author sought to determine the safety of the Pedicled Robertson M

Reduction Mammaplasty in a Patient with Kniest Dysplasia: Case Report and Literature Review.

Collagen plays a fundamental role in wound healing and consequently defective collagen can impair normal wound healing processes. Kniest dysplasia (KD) is a collagenopathy that results from a pathogenic mutation in a gene that codes for type II collagen. Clinical manifestations of the dysplasia include short-trunk dwarfism, kyphoscoliosis, hand arthropathy, cleft palate, hearing loss, and ocular abnormalities. We present the case of a 21-year-old woman who desired reduction mammaplasty. A review of the literature was performed, and there were no published reports of any plastic surgery procedu

A comparison of complication rates in large and small inferior pedicle reduction mammaplasty.

The main objective of this retrospective study was to determine whether the rates of complications are higher in large reductions (> or =1000 g per breast) as compared with smaller reductions (< or =999 g per breast) using the inferior pedicle technique. A retrospective chart review of 133 consecutive patients operated on between October of 2000 and March of 2002 was undertaken. Complication data were recorded and analyzed on a per-breast basis. Two hundred sixteen breasts had reductions of 999 g or less, whereas 50 breasts had reductions of 1000 g or more. The overall mean follow-up period wa

Exceptional presenting conditions and outcome of augmentation mammaplasty in male-to-female transsexuals.

Driven by a persistent and unchangeable need to undo the discrepancy between reality of the body and gender of the mind, most male-to-female transsexuals seek physical feminization through hormonal and surgical treatment. The authors report some rare presenting conditions and exceptional results of augmentation mammaplasty in 11 male-to-female transsexuals treated between January 1979 and January 1998, as well as describe how to treat these conditions. In patients in whom gynecomastia was treated previously, the remaining subcutaneous fatty tissue may be insufficient to cover the implants safe

Reduction mammaplasty. A comparative study of the Orlando and Robbins methods in 292 patients.

Between January 1984 and November 1990 a total of 292 patients underwent reduction mammaplasty for hypertrophic breasts. Of these 233 had a superomedial pedicle (Orlando's method) and 36 an inferior pedicle (Robbins' method). Twenty-three had various other operations. Those operated on by Orlando's and Robbins' methods were compared retrospectively, and it was found that Robbins' method was superior in younger women because of increased sensitivity, particularly of the nipple-areola complex, and better lactation. In the physician's opinion ugly scars developed in 32% of the patients (n = 287).

Reduction mammaplasty followed by development of a bilateral non-synchronous carcinoma of the breast.

Case report of a woman who at the age of 26 underwent mammary reduction (strmbeck) for macromastia. A primary bilateral non-synchronous carcinoma of the breast was discovered 20 months and 5 years after the plastic surgery. On pathological examination lymph node metastases were not found in both instances. There was a positive family history of the occurrence of carcinoma. Mammography prior to mammary reduction is recommended.

Managing Necrosis of the Nipple Areolar Complex Following Reduction Mammaplasty and Mastopexy.

The objectives of this article are to explain the mechanisms of injury that result in ischemia of the nipple areolar complex (NAC) after reduction mammaplasty or mastopexy, to offer recommendations about the management of this complication, and to illustrate reconstructive techniques that can be used to correct deformities arising from necrosis of the NAC. With these goals in mind, the article is divided into 3 sections: prevention of ischemia of the NAC, management of the ischemic nipple, and reconstruction after ischemic necrosis of the nipple and areola. Necrosis of all or part of the NAC i

Sensitivity of the Nipple-Areola Complex and Sexual Function Following Reduction Mammaplasty.

BACKGROUND: The sensitivity of the nipple-areola complex (NAC) is very relevant to female sexuality. OBJECTIVE: To evaluate NAC sensitivity and sexual function after breast reduction, and to assess whether altered NAC sensitivity is related to sexual dysfunction. METHODS: The study included 80 patients, who were allocated to a control group with eutrophic breasts (CG, n = 20), a hypertrophy group without surgery (HG, n = 20), or a mammaplasty group (MG, n = 40). The MG was assessed preoperatively and 6 months postoperatively. The HG and CG were evaluated once. NAC sensitivity was assessed fo

Breast lesions in reduction mammaplasty specimens: a histopathological pattern in 534 patients.

BACKGROUND: The prevalence of breast lesions (benign, precancerous and cancer lesions) in reduction mammaplasty (RM) specimens has rarely been reported in Europe and never in the Swiss population. METHODS: Personal and histopathological data from 534 female patients who underwent RM were reviewed. RESULTS: Benign and/or malignant lesions were detected in 76.2% of all patients. Benign breast lesions associated with an increased risk of developing breast cancer represented 2.8% of all lesions. Breast cancer in situ was identified in 5 (0.9%) patients. Patient age and previous history of breast

[B-technique after augmentation mammaplasty to correct dislocation of nipple and areola].

"B-technique" was first developed for breast reduction. In the past there were some new information characterizing the "B-technique" as being a good method for some breast cancer patients undergoing conversation surgery as well. In this paper we report on a patient suffering from anisomastia; after augmentation mammaplasty the "B-technique" was used to put the nipple and areola in the right position. If the dislocation of the nipple is not extreme we considered the "B-technique" a favourable method to obtain a good aesthetic result.
