[Combined genioglossus advancement (ACMG): inferior sagittal mandibular osteotomy with genioglossus advancement and stabilization with suture in patients with OSAS. Preliminary clinical results].

Surgical treatment of obstructive sleep apnea syndrome (OSAS) centres on the identification of the level of obstruction of the upper airway and the choice of the most suitable procedure to correct it. Shaping of the retrolingual hypopharyngeal space is among the most difficult to achieve because it stems from an alteration of the soft tissue of the tongue, from the hypopharynx, and is correlated to the contraction pattern of the genioglossus and the pharyngeal constrictors. We propose a surgical technique of combined genioglossus advancement (CGA) in cases of anteroposterior collapse of the re

Mandibular advancement devices and treatment-emergent central sleep apnea (TECSA): Coincidental or critical finding?

Treatment-emergent central sleep apnea (TECSA) can occasionally be observed when obstructive sleep apnea (OSA) is managed with most therapeutic approaches, including the mandibular advancement device (MAD). Although TECSA appears to be a low-incidence condition, evidence is lacking on its mechanism and possible health risks. The primary objective of this critical review is to provide guidance to clinicians managing OSA with MAD. Prompt return to a sleep medicine specialist is recommended, however, when a patient continues to present complaints or reports potentially associated with central sle

Modified Mandibular Advancement Device Is Effective in Patients With Insufficient Teeth: A Systematic Review.

BACKGROUND: Mandibular advancement devices (MADs) are established alternatives to continuous positive airway pressure (CPAP) for obstructive sleep apnea syndrome (OSAS), particularly valued for better patient compliance. Traditional contraindications include insufficient healthy teeth for retention, affecting a significant portion of patients due to global edentulism rates of up to 37.8% in ageing populations. Despite emerging reports challenging this, no synthesis exists on MAD efficacy in edentulous or partially edentulous patients. METHODS: This PRISMA-compliant systematic review (PROSPERO

Wearing the mandibular advancement orthosis and dental movements: Contribution of a digital monitoring protocol.

OBJECTIVES: Oral appliance (OA) has been increasingly used for the treatment of obstructive sleep apnoea hypopnea syndrome (OSAHS). OAs work by propelling the mandible, increasing the upper airway calibre and reducing collapsibility. While they have shown efficacy in reducing OSAHS, long-term use can lead to adverse effects, such as dental displacement. The present study focuses on the impact on dental displacements of the NARVAL® computer-aided design and manufacturing OA from the ResMed laboratory. MATERIALS AND METHODS: The study included 39 patients aged 18 or older who were treated for O

DLL4/Notch blockade disrupts mandibular advancement-induced condylar osteogenesis by inhibiting H-type angiogenesis.

BACKGROUND: Blocking Delta-like 4 (DLL4)/Notch has emerged as a promising therapeutic target for the treatment of tumours by deregulating angiogenesis. However, DLL4/Notch serves as a negative regulator of angiogenesis in multiple organs while acting as a positive regulator of H-type angiogenesis in postnatal long bones. Therefore, the effect of DLL4/Notch signalling blockade on mandibular condylar osteogenesis attracted our attention. OBJECTIVE: To explore the effect of blocking DLL4/Notch on mandibular advancement (MA)-induced condylar osteogenesis. METHODS: Six-week-old young male C57BL/6

Mandibular advancement reduces pharyngeal collapsibility by enlarging the airway rather than affecting velopharyngeal compliance.

Mandibular advancement devices (MADs) are frequently prescribed for obstructive sleep apnea (OSA) patients, but approximately one third of patients experience no therapeutic benefit. Understanding the mechanisms by which MADs prevent pharyngeal collapse may help optimize MAD therapy. This study quantified the relative contributions of changes in airspace cross-sectional area (CSA) versus changes in velopharyngeal compliance in determining MAD efficacy. Sixteen patients with moderate to severe OSA (mean apnea-hypopnea index of 32 ± 15 events/h) underwent measurements of the velopharyngeal closi

Peripharyngeal tissue deformation, stress distributions, and hyoid bone movement in response to mandibular advancement.

Mandibular advancement (MA) increases upper airway (UA) patency and decreases collapsibility. Furthermore, MA displaces the hyoid bone in a cranial-anterior direction, which may contribute to MA-associated UA improvements via redistribution of peripharyngeal tissue stresses (extraluminal tissue pressure, ETP). In the present study, we examined effects of MA on ETP distributions, deformation of the peripharyngeal tissue surface (UA geometry), and hyoid bone position. We studied 13 supine, anesthetized, tracheostomized, spontaneously breathing adult male New Zealand White rabbits. Graded MA was
