Access to the superior mediastinum.

Surgical access for diseases that involve the superior mediastinum can be achieved without thoracotomy by utilizing the suprasternal approach with extension of the head and neck and transection of the interclavicular ligament, median sternotomy, resection of the medial third to one half of the clavicle, or resection of the manubrium sterni. There has been minimal morbidity and no operative or postoperative mortality in a review of 53 consecutive patients. There has been no significant disability. Combined with exposure at the base of the neck, the access to the superior mediastinum is good to

A Case of Primary Sternomanubrial Osteomyelitis With Oxacillin-Resistant Staphylococcus aureus (ORSA) Bacteremia.

We herein report the case of a previously healthy 26-year-old male patient who presented to our hospital with chest pain and fevers. Investigations revealed oxacillin-resistant Staphylococcus aureus (ORSA) osteomyelitis of the manubrium, for which no inciting event or background was identified, classifying it as primary sternomanubrial osteomyelitis (PSO). The patient was appropriately treated with intravenous antibiotics, resulting in clinical improvement. The sternomanubrial site without trauma has rarely been described in the literature.

[Ultrasound in prevention of postoperative immunodeficiency].

Phonoprophylaxis of postoperative immunodeficiency was developed and tested in 34 patients. From the first day of hospitalization to the day of elective surgery the patients were daily exposed to ultrasound on the area of the manubrium sterni according to the labile technique through petrolatum or glycerol medium (0.05 W/cm2, 120-150 s, each other day, 4-6 procedures per course). Immune status studies after surgery prove a prophylactic effect of the above ultrasonic procedures.

Radiographic vertical tracheal diameter assessment at different levels along the trachea as an alternative method for the evaluation of the tracheal diameter in non-brachycephalic small breed dogs.

BACKGROUND: Tracheal narrowing due to congenital tracheal hypoplasia, acquired tracheal stenosis and tracheal collapse can lead to life-threatening respiratory distress. Tracheal hypoplasia has been identified in brachycephalic dog breeds, predominantly English Bulldogs, by measuring the tracheal diameter compared to the diameter of the thoracic inlet and creating a ratio. However, reference ranges for tracheal diameter have not been established for non-brachycephalic small breed dogs. It would be advantageous to have established tracheal diameters for non-brachycephalic small breed dogs, as t

Estrogen Receptor-Low Breast Cancer With Sternal Metastasis Presenting as "Stiff Neck" in a Young Female.

A previously healthy 20-year-old Japanese woman was referred to the outpatient clinic for evaluation of right neck stiffness, which had persisted for two months, along with weight loss, malaise, and elevated inflammatory markers. On physical examination, spontaneous pain was noted in the right upper trapezius, along with incident pain around both clavicles and shoulders. Swelling was found around the sternal manubrium, and two palpable masses were present in the right breast. A computed tomography scan of the thorax revealed a low-density, lobulated area in the right breast, a soft-tissue mass

"Tuberculosis on the spot" - Discussion of a probable sternal tuberculosis from a late medieval cemetery from Hungary.

Investigations of non-adult remains are particularly suitable for finding epidemic periods in past populations. This study presents a probable unique example of osseous manifestation of tuberculosis on a child's skeletal remains from medieval Hungary. Between 2009 and 2011 the Field Service for Cultural Heritage excavated the exceptional cemetery of Perkáta - Nyúli-dűlő in Hungary, with around 5000+ graves. The analysed skeleton (SNR 948) was located in the medieval (10-16th century) part of the cemetery. Besides the standard macroscopic pathological observation, we also performed radiographic

Morphometric Comparison of Tympanic Membrane in Unilateral Mucosal Chronic Otitis Media.

UNLABELLED: Previous studies of the middle ear in Chronic Otitis Media have focussed on radiological assessment of temporal bone; endoscopic studies were focussed on perforation size and location. The malleus handle (manubrium) is a constant ossicular feature visible on otoendoscopy. It's relative position may indicate the previously documented differences in ossicular chain and middle ear cleft development relative to the tympanic ring and inner ear capsule between affected ear and contralateral ear. DESIGN: descriptive. SETTING: hospital based. SUBJECTS: 84 patients with unilateral mucosa

Optical coherence tomographic measurements of the sound-induced motion of the ossicular chain in chinchillas: Additional modes of ossicular motion enhance the mechanical response of the chinchilla middle ear at higher frequencies.

Wavelength-swept optical coherence tomography (OCT) was used to scan the structure of cadaveric chinchilla ears in three dimensions with high spatial resolution and measure the sound-induced displacements of the entire OCT-visible lateral surfaces of the ossicles in the lateral-to-medial direction. The simultaneous measurement of structure and displacement allowed a precise match between the observed motion and its structural origin. The structure and measured displacements are consistent with previously published data. The coincident detailed structural and motion measurements demonstrate the

[Suprasternal bone (author's transl)].

Human skeletons have many variations which may occasionally necessitate distinction from pathologic changes. Suprasternal bone is an unfamiliar normal variation in the vicinity of the sternoclavicular joint. It was first described by Béclard in 1820 and thereafter many reports have appeared, mostly in anatomy and embryology. An incidence less than 5% is reported abroad. This report describes the incidence of suprasternal bone in Japanese and a few clinical reference cases. Suprasternal bone is now considered to have derived from the persistent rudiment of epicoracoid which should have normally

[Laryngeal reconstruction using endoprostheses for benign stenosis].

A clinical trial was made of two types of laryngeal endoprostheses in surgery of stenosis caused by benign diseases. A total of 15 patients were operated on. In 11 patients endoprosthesis of biocompatible polymers was inserted, in 4 patients the stenosis was corrected with endoprosthesis of silicon rubber. Most of the patients had stenosis as a result of complication of thyroid surgery. The operative technique is described. In extensive defects of the anterior wall of the larynx and trachea, advanced scar stenoses a new operation is proposed-autoalloplasty using body and manubrium of the stern

A technique for concurrent repair of pectus excavatum and intracardiac defects.

Pectus excavatum deformity may present problems when median sternotomy is required for repair of intracardiac defects. We have used a partial median sternotomy extending from the manubrium to the level of the second intercostal space with extension into that space. The perichondral sheaths are disconnected from the sternum on the right side after removing the deformed costal cartilages. Exposure is thus provided for the cardiac procedure. Following this, the upper sternal fragments are sutured together and the repair is completed in a manner similar to the Ravitch technique.

Minor sternum and vertebral column congenital defects in Lisbon Identified Skeletal Collection.

This study has mainly a descriptive aim, in which crude prevalence of minor skeletal congenital defects is calculated and sex differences are tested. Prevalence is compared with other studies to recognize regional patterns. Association with age-at-death and year-of-birth is tested to identify impact of environmental stress on minor congenital defects presence. Testing association between defects will identify defects with a probable identical etiology. Chi-square was used to identify sex differences, between studies differences, and to test relationships between defects and Spearman correlatio

Can standard anterior Smith-Robinson supramanubrial approach be utilized for approach down to T2 or T3?

PURPOSE: The aim of this study was to determine a plain radiographic criterion for determining the feasibility of using the standard anterior Smith-Robinson supramanubrial approach for anterior surgery down to T2 or T3. METHODS: The surgical database (2002-2014) was searched to identify patients with anterior cervical surgery to T2 or T3. A method to determine whether a standard anterior Smith-Robinson approach can be used to operate on the upper thoracic levels was evaluated. The surgeon chose the surgical approach preoperatively using a lateral radiograph by determining if a line from the i

Curative resection of esophageal cancer with a double aortic arch.

Thoracic esophageal cancer with a double aortic arch is extremely rare. We herein report the case of a 63-year-old man with a double aortic arch who underwent an esophagectomy with a three-field lymphadenectomy for cancer in the lower thoracic esophagus. In such cases, it is important to recognize the relationship between the right and left aortic arches and the bilateral recurrent laryngeal nerves (RLNs). We were able to accurately understand the anatomical position of the RLNs using a precedent cervical procedure with partial resection of the manubrium to remove the nodes along the bilateral

Mediastinal impalement with a fibreglass sheet.

Mediastinal impalement injuries are uncommon and often fatal. There have been very few reported cases of survival following mediastinal impalement. Patients who present with these injuries always undergo operative intervention regardless of their underlying haemodynamic status or associated injuries. We herein present a case of mediastinal impalement injury, where a sheet of fibreglass had fractured the manubrium and entered the anterior mediastinum with no associated great vessel injury. The fibreglass sheet was removed via a partial sternotomy and the patient made an uneventful postoperative

Sternocostoclavicular hyperostosis: a review and report of 11 cases.

Sternocostoclavicular hyperostosis is a benign ossifying diathesis of unknown etiology characterized by hyperostosis and soft-tissue ossification between the clavicles, anterior portion of the upper ribs, and manubrium, with variable hyperostosis or ankylosis in the spine and sacroiliac joints. Our cumulative experience with 11 cases is reported, with emphasis on radiographic features of the condition. Scintigraphic results in five patients and computed tomographic findings in one patient are presented. A review of the literature and our own material indicates that sternocostoclavicular hypero

Posterior Sternoclavicular Joint Dislocation and Reconstruction.

This case presentation describes a technique for reconstruction of an acute posterior sternoclavicular joint dislocation. The patient was a 37 year-old female who sustained a left posterior sternoclavicular dislocation after a fall. A curvilinear incision was made directly over the sternoclavicular joint. After reduction, a semitendinosus allograft was used to reconstruct the sternoclavicular joint in a figure-of-8 fashion through drill holes in the manubrium and the proximal clavicle and secured with suture tape. Sternoclavicular reconstruction with semitendinosis allograft provides a reliabl
