BACKGROUND: Marathon running, while offering health benefits, is associated with a high incidence of knee injuries. The infrapatellar fat pad (IFP) plays a critical role in knee joint homeostasis and injury mitigation. This study investigated IFP adaptations to the acute stress of marathon running using quantitative magnetic resonance imaging (MRI).
METHODS: Fourteen amateur marathon runners (12 male, two female) were prospectively enrolled and underwent 3.0T MRI (GE SIGNA Architect) one week before and after the marathon. Left knee imaging included MAGiC and IDEAL-IQ sequences. MAGiC sequenc

BACKGROUND: Despite the increasing popularity of endurance running competitions among adolescent runners, there is currently limited information regarding expected biomechanical changes across the duration of a long-distance running event, and the relationship between young runners' biomechanics and running performance. Wearable technology offers an ecological means to continuously assess runners' biomechanical data during outdoor running competitions.
RESEARCH QUESTION: Do adolescent athletes adopt changes in sensor-derived biomechanics throughout a marathon race, and are there relationships

The purpose of this study was to determine the type and frequency of lower extremity running injuries incurred by athletes participating in the New York City Marathon. A survey was conducted of 265 athletes presenting to medical stations for podiatric care during the 1994 New York City Marathon. The results of the survey indicated that the most common injuries occurring in marathon runners were corns, calluses, blisters, muscle cramps, acute knee and ankle injuries, plantar fasciitis, and metatarsalgia. An inverse relationship was observed between the number of miles trained per week and the n

Runners commonly utilize cryotherapy as part of their recovery strategy. Cryotherapy has been ineffective in mitigating signs and symptoms of muscle damage following marathon running and is limited by its duration of application. Phase change material (PCM) packs can prolong the duration of cooling. This study aimed to test the efficacy of prolonging the duration of cooling using PCM on perceptual recovery, neuromuscular function, and blood markers following a marathon run. Thirty participants completed a marathon run and were randomized to receive three hours of 15°C PCM treatment covering th

The question of cardiac strain arises when considering the emerging class of recreational runners whose running strategy could be a non-optimal running pace. Heart rate (HR) monitoring, which reflects exercise intensity and environmental factors, is often used for running strategies in marathons. However, it is difficult to obtain appropriate feedback for only the HR value since the cardiovascular drift (CV drift) occurs during prolonged exercise. The cardiac cost (CC: HR divided by running velocity) has been shown to be a potential index for evaluation of CV drift during the marathon race. We

BACKGROUND: Several single race events (5 km, 10 km, half-marathon, marathon, ultra-marathon) in different countries and different years have been analyzed in multiple studies, representing the rising interest in endurance-based activity and thus physical health. With focus on participation numbers, performance or sex difference, many single study results were obtained. The running trends in a whole country covering a longer period of time and several race distances are missing so far.
OBJECTIVES: The aim of this study is therefore to examine 5 km, 10 km, half-marathon, marathon and ultra-mar

The first official sub-2-hour marathon provides a unique real-world case for re-examining the determinants of elite marathon performance. This Perspective argues that the key scientific issue is not whether human physiology can theoretically support sub-2-hour running, but how physiological capacity can be preserved and expressed under late-race fatigue. Sabastian Sawe's 1:59:30 performance was marked by a 59:01 second half and, based on available split data, a 27:36 segment from 30 to 40 km, indicating late-race acceleration rather than simple pace maintenance. The wider front-group pattern,

BACKGROUND: Providing runners with injury prevention suggestions from a biomechanical perspective is crucial in light of the increased incidence of running-related injuries forecasted with the rising popularity of the half-marathon. Previous research has demonstrated associations between running injuries and patterns of lower extremity biomechanics, as well as indicating that inadequate rest between training sessions can result in injuries and underperformance. However, whether half-marathon running elicits lower extremity biomechanical changes and the recovery time remains largely unclear. Th

The increasing participation in running events, particularly half-marathons, has been noteworthy in recent decades. However, the time course of recovery of muscle performance after a half-marathon running remains largely unclear. This study aimed to investigate changes in concentric and eccentric quadriceps and hamstring peak torques and mean powers, as well as hamstring to quadriceps torque and power ratios, after a half-marathon. Thirty-eight recreational runners participated in this study. Isokinetic dynamometry was used to measure peak torque and mean power at four time points: pre (baseli

The aim of this study is to identify risk factors for lower extremity injuries in male marathon runners. A random sample of 1500 recreational male marathon runners was drawn. Possible risk factors were obtained from a baseline questionnaire 1 month before the start of the marathon. Information on injuries sustained shortly before or during the marathon was obtained using a post-race questionnaire. Of the 694 male runners who responded to the baseline and post-race questionnaire, 28% suffered a self-reported running injury on the lower extremities in the month before or during the marathon run.

BACKGROUND: Running performance depends not only on aerobic capacity but also on the functional integration of the respiratory system and core musculature. However, morphological and functional adaptations of the diaphragm and abdominal muscles across different running disciplines remain insufficiently characterized. The aim of this study is to analyze diaphragm and abdominal muscle morphology using ultrasound, as well as respiratory muscle strength and pulmonary function, in amateur runners classified according to their primary competition distance.
METHODS: A cross-sectional observational s
