Prevalence and socioeconomic factors associated with smoking in people living with HIV by sex, in Recife, Brazil.

INTRODUCTION: Smoking is the leading cause of preventable death in the world. The prevalence of smoking is higher in people infected with HIV than in the general population. Although it is biologically plausible that smoking increases the morbidity and mortality of people living with HIV/AIDS, few studies in developing countries have analyzed the determinants and consequences of smoking in HIV infected people. OBJECTIVE: To estimate the prevalence of smoking and identify the socioeconomic factors associated with smoking and smoking cessation in patients with HIV by sex. METHODS: A cross-sect

Coronary no-flow and ventricular tachycardia associated with habitual marijuana use.

A 34-year-old man reported heart fluttering and near syncope a few hours after marijuana smoking. In the emergency department, he was found to have a right bundle-branch-type ventricular tachycardia. The patient underwent a successful electric cardioversion. Coronary angiography showed no pericardial artery stenosis yet very slow coronary blood flow. Clinical tachycardia was also inducible in the electrophysiologic laboratory. After verapamil therapy and cessation of marijuana, his coronary flow normalized and ventricular tachycardia was no longer inducible in the electrophysiologic laboratory

Microbiomic differences at cancer-prone oral mucosa sites with marijuana usage.

Marijuana smoke contains cannabinoids, immunosuppressants, and a mixture of potentially-mutagenic chemicals. In addition to systemic disease, it is thought to contribute to oral disease, such as tooth loss, tissue changes in the gums and throat, and possibly oral pharyngeal cancer. We used a cross-sectional study of 20 marijuana users and 19 control non-users, to determine if chronic inhalation-based exposure to marijuana was associated with a distinct oral microbiota at the two most common sites of head and neck squamous cell carcinoma (HNSCC), the lateral border of the tongue and the oral ph

Have recreational marijuana laws undermined public health progress on adult tobacco use?

Public health experts caution that legalization of recreational marijuana may normalize smoking and undermine the decades-long achievements of tobacco control policy. However, very little is known about the impact of recreational marijuana laws (RMLs) on adult tobacco use. Using newly available data from the Population Assessment of Tobacco and Health (PATH) and dynamic difference-in-differences and discrete-time hazard approaches, we find that RML adoption increases prior-month marijuana use among adults ages 18-and-older by 2-percentage-points, driven by an increase in marijuana initiation a

Pot smokers puffing away lung health.

BACKGROUND: Marijuana is the second most commonly used inhalational agent after tobacco. It has been used for therapeutic benefits in cancer, epilepsy, inflammation and pain. Inhalation of marijuana causes reversible and irreversible lung injury. CASE: We present a 26-year-old female with cough, chest pain, epistaxis, hemoptysis, night sweats and breathlessness few hours after smoking marijuana. Physical exam was positive for tachycardia, tachypnea, and diminished coarse breath sounds. Further investigation revealed elevated white blood cell count, chest X-ray, computed tomography of the ches

Complete Occlusion of Bilateral Internal Carotid Artery in a Marijuana Smoker: A Case Report.

Here we described a case of a young male who developed stroke attributable to heavy marijuana smoking. Although, instances of stroke due to cannabis smoking have been reported earlier, this case is distinctive due to the fact that the patient had bilateral complete occlusion of internal carotid artery (ICA) at a very early age. In view of studies that have correlated marijuana smoking to intracranial stenosis and atherogenesis, this case of bilateral ICA occlusion illustrates the severity and burden of the disease.

Marijuana use and pneumonia risk in a cohort of HIV-infected and HIV-uninfected men.

BACKGROUND: The prevalence of marijuana use is increasing in the United States. Marijuana smoking has been shown to impair the microbicidal activity of alveolar macrophages and decrease the number of ciliated epithelial cells in the bronchi with a parallel increase in the number of mucus-secreting surface epithelial cells, which may increase the risk of pneumonia. However, it remains unclear whether there is an association between smoking marijuana and pneumonia. METHODS: Using data from the Multicenter AIDS Cohort Study (MACS), a long-term observational cohort study of men who have sex with

Association between marijuana exposure and pulmonary function over 20 years.

CONTEXT: Marijuana smoke contains many of the same constituents as tobacco smoke, but whether it has similar adverse effects on pulmonary function is unclear. OBJECTIVE: To analyze associations between marijuana (both current and lifetime exposure) and pulmonary function. DESIGN, SETTING, AND PARTICIPANTS: The Coronary Artery Risk Development in Young Adults (CARDIA) study, a longitudinal study collecting repeated measurements of pulmonary function and smoking over 20 years (March 26, 1985-August 19, 2006) in a cohort of 5115 men and women in 4 US cities. Effects of marijuana on the task-elicited physiological response.

Ten healthy male marijuana users smoked either a placebo or active (1.8% delta 9-THC) marijuana cigarette during five daily testing sessions. Ten minutes after drug administration, six subjects (Group I) performed a 10-min serial acquisition task and four subjects (Group II) rested quietly. Blood pressure was sampled at 2-min intervals and heart rate (HR) was recorded continuously. Marijuana alone increased HR by 18 beats/min and mean arterial pressure (MAP) by 3 mmHg, while task performance alone increased HR by 4 beats/min and MAP by 5 mmHg. The combination of drug and task performance had g

[Passive exposure in detection of low blood and urine cannabinoid concentrations].

Whenever small amounts of drugs are present in blood or urine samples, especially of substances that are preferentially smoked such as cannabinoids, the discrimination between active and passive inhalation may cause severe problems. The statement of a passive exposure by marijuana smoke has been scrutinized reviewing the literature. The pharmacokinetics of smoked marijuana as well as experimental data on cannabinoid concentrations in plasma and urine samples following passive exposure are summarized. As a conclusion it seems urgent to enlarge the existing data base.

Bilateral Optic Neuropathy Associated with Acute Inhaled Marijuana Use: Case Report and Review of the Literature.

Marijuana is the most commonly used federally illegal drug in the United States. Acute marijuana use is associated with several cardiovascular and neuropsychological adverse effects. Ocular complications of marijuana abuse are very rare. Herein, we present the first report of bilateral optic neuropathy following smoking marijuana. A 28-year-old man presented to the emergency room with sudden onset of bilateral blurring of the inferior visual field 8 h after smoking marijuana. His best-corrected visual acuity was 20/30 in the right eye and 20/20 in the left eye. Fundus examination revealed blur

[A 38 YEARS-OLD YOUNG MALE WITH CHEST PAIN AND ECG CHANGES].

Coronary vasospasm is a well-known condition causing acute chest syndrome and can lead to myocardial infarction, ventricular arrhythmias, and even sudden cardiac death. While there are extensive case series showing the association of coronary vasospasm with drugs like cocaine, the association of marijuana with coronary vasospasm has not been reported frequently. When smoked, marijuana results in a rapid, dose-dependent tachycardia, an increase in blood pressure, and an increase in cardiac output. This in turn leads to increased oxygen demand, which is augmented by the vasoconstriction endothel

Recreational Marijuana Use and Coronary Artery Dissection: A Case Series.

The cardiovascular effects of marijuana have been shown to be a result of the activation of the CB1 and CB2 receptors located in the myocardium and coronary vasculatures. Adverse cardiovascular consequences of recreational use of marijuana and synthetic cannabinoids include stroke, artery dissection, vasospasm, vasculitis, coronary artery thrombosis, myocarditis/pericarditis, postural hypotension, arrhythmias, and acute heart failure. With marijuana being legalized for medicinal purposes and recreational use in more and more states in the United States, physicians should have a low threshold f

Marijuana use, diet, body mass index, and cardiovascular risk factors (from the CARDIA study).

Marijuana use has been associated with increased appetite, high caloric diet, acute increase in blood pressure, and decreases in high-density lipoprotein cholesterol and triglycerides. Marijuana is the most commonly used illicit drug in the United States, but its long-term effects on body mass index (BMI) and cardiovascular risk factors are unknown. Using 15 years of longitudinal data from 3,617 black and white young adults participating in the Coronary Artery Risk Development in Young Adults (CARDIA) study, we assessed whether marijuana use was associated with caloric intake, BMI, and cardiov

Marijuana Use and the Risk of Early Ischemic Stroke: The Stroke Prevention in Young Adults Study.

BACKGROUND AND PURPOSE: Few studies have examined the dose-response and temporal relationships between marijuana use and ischemic stroke while controlling for important confounders, including the amount of tobacco smoking. The purpose of our study was to address these knowledge gaps. METHODS: A population-based case-control study with 1090 cases and 1152 controls was used to investigate the relationship of marijuana use and early-onset ischemic stroke. Cases were first-ever ischemic stroke between the ages of 15 and 49 identified from 59 hospitals in the Baltimore-Washington region. Controls

Chest CT Findings in Marijuana Smokers.

Background Global consumption of marijuana is increasing, but there is a paucity of evidence concerning associated lung imaging findings. Purpose To use chest CT to investigate the effects of marijuana smoking in the lung. Materials and Methods This retrospective case-control study evaluated results of chest CT examinations (from October 2005 to July 2020) in marijuana smokers, nonsmoker control patients, and tobacco-only smokers. We compared rates of emphysema, airway changes, gynecomastia, and coronary artery calcification. Age- and sex-matched subgroups were created for comparison with toba

Effects of marijuana on performance of a computerized cognitive-neuromotor test battery.

To evaluate the sensitivity of a computerized cognitive-neuromotor test battery in assessing the effects of marijuana (delta-9-tetrahydrocannabinol [THC]), we conducted a study of 10 healthy male volunteers who were experienced marijuana smokers. After extensive training on the performance tasks, each subject was tested on three separate days after smoking a cigarette containing either 1.75% THC, 3.55% THC, or placebo according to a randomized double-blind repeated measures design. Testing was carried out before smoking and 30, 90, and 150 minutes after smoking the cigarette. Of the five tasks

Marijuana and head and neck cancer: an epidemiological review.

BACKGROUND: Marijuana is the most widely used illicit substance in Canada. To date, no conclusive study has looked at the epidemiologic basis of marijuana use and head and neck cancer (HNC). Due to the imminent recreational legalization of marijuana in Canada, the epidemiologic relationship between marijuana use and HNC is becoming increasingly important. OBJECTIVE: To examine the epidemiologic characteristics of HNC patients who are recreational marijuana users. METHODS: This study was conducted at a single tertiary care centre from 2011 to 2014. Patients were enrolled consecutively at time
