Traumatic aneurysms of the internal maxillary artery and their treatment by embolisation: a report of two cases.

Traumatic aneurysms of the head and neck are rare lesions and in the past their treatment has been by a variety of surgical approaches. Two civilian cases of traumatic aneurysm of the internal maxillary artery are reported. These were treated by embolisation, which offered for their management an effective alternative and less invasive approach than surgery.

[Treatment of posterior epistaxis by ligation of the internal maxillary artery in the pterygo-maxillany fossa].

The microsurgical, transantral ligature of the internal maxillary artery is the treatment of choice for recurring posterior epistaxis. An immediate success was obtained by this method in 5 out of 7 patients. In one case the concomitant coagulation of the ethmoidal arteries was necessary and in another instance a revascularization of the spheno-palatine artery could be visualized by arteriography.

Blindness and total ophthalmoplegia. A complication of transantral ligation of the internal maxillary artery for epistaxis.

Unilateral blindness and total ophthalmoplegia developed on the same side on which transantral ligation of the internal maxillary artery was performed for persistent posterior epistaxis. Evaluation with arteriograms and phlebograms showed no compromise of retinal vessels or cavernous sinus thrombosis. At reexploration of the maxillary antrum, marked swelling of previously placed oxidized cellulose gauze was found. An experimental model, using the rabbit, was designed to determine if implantation of oxidized cellulose gauze adjacent to the optic nerve would produce blindness and ophthalmoplegia

Endsocopic Internal Maxillary Artery Cauterization in a Patient with Severe Posterior Epistaxis: A Case Report.

Epistaxis from the posterior superior region of the nasal cavity might be fatal in some cases. It is particularly severe in an individual with hypertension, arterial aneurysm of traumatic origin, and bleeding from posterior lateral nasal artery, which is frequently difficult to reach and ligate directly on a bleeding area. Certain cases have been reported in which the bleeding could be stopped only by ligating/embolization of the external carotid artery or the internal maxillary artery at its branching off from the external carotid artery. Despite the multiple anastomoses, the effect of such l

Anatomical relationship between the lateral pterygoid muscle and maxillary artery: Morphological classification and clinical implications.

BACKGROUND: The lateral pterygoid muscle is important for temporomandibular joint function and is closely related to the maxillary artery, which is clinically significant during procedures like botulinum toxin injections and infratemporal fossa surgery. METHODS: Thirty hemifaces from human cadavers were dissected. The study documented the number of muscle heads, insertion sites, and the course of the maxillary artery in relation to the muscle (superficial, deep, or transitional). Morphometric measurements included the distance between the maxillary artery and surface landmarks. RESULTS: A tw

Maxillary artery utilization in subcranial-intracranial bypass procedures: a comprehensive systematic review and pooled analysis.

The utilization of the internal maxillary artery (IMAX) in subcranial-intracranial bypass for revascularization in complex aneurysms, tumors, or refractory ischemia shows promise. However, robust evidence concerning its outcomes is lacking. Hence, the authors embarked on a systematic review with pooled analysis to elucidate the efficacy of this approach. We systematically searched PubMed, Embase, and Web of Science databases following PRISMA guidelines. Included articles used the IMAX as a donor vessel for revascularizing an intracranial area and reported at least one of the following outcomes

[Embolization of Arteriovenous Fistulae of the Maxillary Artery after Le Fort I Osteotomy:A Case Report].

BACKGROUND: Le Fort I osteotomy, one of the most common techniques applied to correct jaw deformities, is generally considered operatively safe. However, a few articles reported that this procedure can lead to formation of arteriovenous fistulae(AVF)involving the maxillary artery infrequently. CASE REPORT: A man in his 40s with a jaw deformity underwent Le Fort I osteotomy. Two days later, he noticed pulsating tinnitus in his right ear. Angiography revealed AVF between the proximal part of the maxillary artery(first segment)and the pterygoid plexus. On further evaluation, AVF were embolized w

Maxillary Sinus Dimensions With Respect to the Posterior Superior Alveolar Artery Decrease With Tooth Loss.

BACKGROUND: The posterior superior alveolar (PSA) artery is frequently encountered in the area where the lateral osteotomy is performed during direct sinus augmentation procedures. OBJECTIVE: To investigate the correlation between patient-dependent variables and measurements related to PSA using cone beam computed tomography (CBCT) data. METHODS: Three hundred ninety-four CBCT scans were evaluated to assess the PSA artery diameter and distances to the sinus floor and to alveolar crest. Patient's age, gender, and edentulism status were recorded. RESULTS: The PSA artery tends to be wider in o

Alveolar Antral Artery: Does its Diameter Correlate with Maxillary lateral wall Thickness in Dentate Patients?

INTRODUCTION: Knowledge of the presence of the alveolar antral artery in the lateral maxillary sinus wall is essential for surgeons who operate in this region. The purpose of this study was to investigate the correlation between alveolar antral artery diameter and lateral maxillary bony wall thickness in dentate patients. MATERIALS AND METHODS: Thirty five Cone-Beam Computed Tomography (CBCT) scans from 35 dentate patients were selected in coronal sections in three locations; second premolar (P2), first molar (M1), and second molar (M2). The presence of the alveolar antral artery in each situ

Reverse facial-submental artery island flap for the reconstruction of maxillary defects after cancer ablation.

This study assessed the reliability of the reverse facial-submental artery island flap for reconstructing maxillary defects. Twelve patients with cancer underwent surgical resection and sequential maxillary reconstruction using a reverse facial-submental artery island flap. There were 9 men and 3 women. The patients ranged in age from 42 to 73 years. Palate squamous cell carcinoma was present in 7 cases; and maxillary gingival squamous cell carcinoma, in 5 cases. The remaining defects were classified as class 1 in one case, class 2a in 8 cases, and class 3b in 3 cases. The sizes of the skin pa

Traumatic dissection of the internal maxillary artery associated with isolated glossopharyngeal nerve palsy: case report.

OBJECTIVE AND IMPORTANCE: Spontaneous or traumatic dissection of the internal carotid artery with resultant lower cranial nerve palsies is well documented. However, dissection of the external carotid artery with lower cranial palsies has not been reported previously. CLINICAL PRESENTATION: A 42-year-old man experienced an epidural hematoma as the result of a fall and underwent a craniotomy and hematoma removal. Subsequently, he developed dysgeusia and difficulty in swallowing. Brain magnetic resonance imaging showed a dilated linear structure, with isosignal intensity on T1-weighted images an

Running pattern of the internal maxillary artery assessed using arterial-phase CT images in 434 sides in Japanese individuals.

OBJECTIVE: The internal maxillary artery (IMA) follows two primary running patterns: lateral, where it traverses the superficial layer of the lateral pterygoid muscle, and medial, where it passes through the deep layer. The frequency of these patterns varies among populations, with the medial type reported in 6-18 % of Asians and 37-46 % of Caucasians. Ligation of the IMA, commonly performed during total or partial maxillectomy to reduce bleeding, is more challenging in the medial type due to its deeper anatomical position. Therefore, preoperative identification of its pattern is crucial. This

Direct Graftless Internal Maxillary Artery-Middle Cerebral Artery Bypass via a Combined Sublabial Transmaxillary-Pterional Corridor: Cadaveric Study With 3-Dimensional Models.

BACKGROUND AND OBJECTIVES: Bypass surgery has become one of the essential techniques in neurosurgery. The internal maxillary artery (IMAX) has emerged as an increasingly used donor vessel for large-caliber extracranial-intracranial bypass. However, the limited donor length reported in previously described techniques often necessitates the use of interposition grafts. This study describes a novel graftless direct IMAX-middle cerebral artery (MCA) bypass technique using a combined endoscopic sublabial transmaxillary and pterional approach. METHODS: Six silicone-injected, embalmed cadaveric head

Distribution of the intraosseous branch of the posterior superior alveolar artery relative to the posterior maxillary teeth.

PURPOSE: Preoperative identification of the intraosseous posterior superior alveolar artery (PSAA) is critical when planning sinus surgery. This study was conducted to determine the distance between the cementoenamel junction and the PSAA, as well as to identify factors influencing the detection of the PSAA on cone-beam computed tomography (CBCT). MATERIALS AND METHODS: In total, 254 CBCT scans of maxillary sinuses, acquired with 2 different scanners, were examined to identify the PSAA. The distance from the cementoenamel junction (CEJ) to the PSAA was recorded at each maxillary posterior too

Penetrating foreign body in the masticator space with injury to the internal maxillary artery: a surgical challenge.

INTRODUCTION Foreign bodies (FBs) in the masticator space (MS) are a unique problem because of the difficulty of accessing this deep compartment within the head and neck. In addition, MS contents include critical structures such as the internal maxillary artery (IMA) and mandibular nerve. CASE HISTORY A 39-year-old tradesman was involved in a construction accident whereby a metallic projectile from a machinery drill penetrated his left cheek. Computed tomography revealed a metallic object of dimension 1.9 ×1.2 cm within the MS, with concomitant fracture of left maxillary anterior and lateral w

[A study of intra-arterial infusion with carboplatin (CBDCA) for 2 patients with maxillary gingival cancer].

In 2 cases of squamous cell carcinoma of maxillary gingiva, carboplatin (CBDCA) was administered by continuous infusion for 5 days, and the clinical outcome and platinum concentrations in the peripheral blood, urine and tissue were investigated. Clinical effects were PR in one of the 2 cases and CR in the other. Platinum concentrations were as high as 3.17 and 13.90 micrograms/g in the tissue, but low in peripheral blood, not exceeding 0.61 and 0.62 micrograms/ml, respectively. These findings suggested that the present method secures the transport of the agent to a local site, with possible re
