Caffeine in Parkinson's disease: a pilot open-label, dose-escalation study.

INTRODUCTION: Epidemiologic studies consistently find an inverse association between caffeine use and PD. Numerous explanations exist, but are difficult to evaluate as caffeine's symptomatic effect and tolerability in PD are unknown. PATIENTS AND METHODS: We designed an open-label, 6-week dose-escalation study of caffeine to establish dose tolerability and evaluate potential motor/nonmotor benefits. Caffeine was started at 200 mg daily and was increased to a maximum of 1,000 mg. RESULTS: Of 25 subjects, 20 tolerated 200 mg, 17 tolerated 400 mg, 7 tolerated 800 mg, and 3 tolerated 1,000 mg. T

Optimizing dose selection in oncology: the rationale for the clinical dose selection of giredestrant, an oral selective estrogen receptor degrader.

INTRODUCTION: 'Maximum tolerated dose' (MTD) was historically used to maximize clinical efficacy. However, there is a drive to optimize dose selection to improve safety/tolerability, while maintaining efficacy. This paradigm is increasingly important in oncology clinical development, leading to a focus on careful early-phase trial design to ensure that appropriate dose - or exposure-response data are obtained to guide dose selection. AREAS COVERED: We describe the development pathway and risk-benefit considerations for clinical dose selection for giredestrant, a next-generation, highly potent

Opportunities for model-based precision dosing in the treatment of sickle cell anemia.

Hydroxyurea is the primary pharmacotherapy to prevent complications of sickle cell anemia (SCA). Accumulated clinical experience across multiple age ranges has suggested that the use of an individualized maximum tolerated dose (MTD) will achieve optimal benefit of hydroxyurea treatment. However, the current empirical and trial-and-error approach for dose escalation often results in a lengthy titration process and is not strictly implemented in many clinics. Opportunities exist for pharmacokinetics model-based precision dosing of hydroxyurea to quickly achieve individual MTD. This review intend

Initial evaluation of human lymphoblastoid interferon in patients with advanced malignant disease.

A study was carried out to establish the maximum tolerated dose of human lymphoblastoid interferon and to define its side effects when given by intramuscular injection. Pyrexia limited the initial dose to a maximum of 3 megaunits/m2 body surface area, but tolerance to this effect developed over 4 to 5 days and the dose was then increased to 5 to 7.5 megaunits/m2; subjective disturbance prevented further increase in dose, but 2.5 to 5.0 megaunits/m2 daily was well tolerated and appears suitable for long-term administration. Other side effects were hypertension, hypotension, myelosuppression, an

Thermal Modeling for the Next Generation of Radiofrequency Exposure Limits: Commentary.

This commentary evaluates two sets of guidelines for human exposure to radiofrequency (RF) energy, focusing on the frequency range above the "transition" frequency at 3-10 GHz where the guidelines change their basic restrictions from specific absorption rate to incident power density, through the end of the RF band at 300 GHz. The analysis is based on a simple thermal model based on Pennes' bioheat equation (BHTE) (Pennes 1948) assuming purely surface heating; an Appendix provides more details about the model and its range of applicability. This analysis suggests that present limits are highly

The value of target-site physiologically-based pharmacokinetics for high-affinity small molecules: a mechanistic foundation for precision dosing in oncology and hematology.

For decades, oncology dose selection has been guided by the maximum tolerated dose (MTD) and plasma pharmacokinetics (PK), reflecting assumptions appropriate for classical cytotoxic chemotherapies. However, the advent of high-affinity, targeted therapies, including kinase inhibitors, epigenetic modulators, and radioligands challenges this paradigm. These agents achieve robust target engagement at doses far below the MTD, and systemic plasma concentrations often fail to reflect pharmacologically relevant exposure at tumor or hematologic sites. Physiologically-based pharmacokinetic (PBPK) modeli
