The primary goal of the maze procedure is to prevent thromboembolism by restoring atrial function. We used Doppler echocardiography to evaluate the atrial function of patients who had undergone the maze procedure for the treatment of chronic atrial fibrillation. Thirty-five patients who converted to sinus rhythm after the maze procedure were enrolled in this study. Doppler echocardiography was performed in all patients in the early (mean, 19 days) and late (mean, 245 days) phases of the postoperative period. Left and right atrial active contraction fractions and left and right atrial storage f

To determine whether cut and sew Cox maze III procedure is still associated with adequate safety endpoints when performed in conjunction with other open-heart procedures. Between January 2008 and January 2015, 113 consecutive adult patients were submitted to cut and sew Cox maze III procedure in association with other operations for structural heart disease. Mean age was 49 years and 80 (70.8%) were females. Longstanding or persistent atrial fibrillation has occurred in 87.6% and rheumatic heart disease in 80.7%. Valve surgery was performed in 98.2%. The number of associated procedures was cor

OBJECTIVE: In an effort to avoid postoperative sick sinus syndrome( SSS), we omit the ablation line to the superior vena cava( SVC) in the Cox-mazeⅢ lesion set. We report the long-term outcomes, including the freedom from SSS.
METHODS: We studied 102 patients who underwent bi-atrial maze procedure for persistent atrial fibrillation (Af) from 2009 through 2023. Bipolar radio frequency ablation or cryoablation was used except for right-side atriotomy and right atriotomy. Cryoablation was used for atrioventricular annulus. The patient age was 68±9.4. Duration of Af was 3.4±6.5 years (unknown 9 c

BACKGROUND: Atrial functional mitral regurgitation (AFMR) is a newly discovered condition associated with longstanding atrial fibrillation. This retrospective study aimed to analyze the outcomes of the maze procedure and mitral regurgitation (MR) surgery in AFMR and atrial fibrillation in comparison with those in degenerative MR (DMR).
METHODS: Patients who underwent mitral valve repair/replacement with a maze procedure at a hospital (July 2012-August 2021) were included. We excluded patients aged below 18 years undergoing concomitant coronary artery bypass grafting or atrial septal defect re

PURPOSE: Left atrial calcification (LAC) is found in long-lasting rheumatic valvular disease and is almost always accompanied by atrial fibrillation (AF). In the presence of LAC, endoatriectomy is required when performing the maze procedure. However, the technical feasibility of endoatriectomy and the long-term outcomes of the maze procedure in patients with LAC are uncertain.
METHODS: The medical records of 18 consecutive patients who underwent combined endoatriectomy and maze procedure were analyzed retrospectively.
RESULTS: Accompanying operations were mitral valve replacement (n = 16) an

Atrial fibrillation is one of the most common cardiac arrhythmias, classically presenting with an "irregularly irregular" rhythm with or without chest pain, palpitations, shortness of breath, lightheadedness, or fatigue. The maze procedure is an open-heart operation that creates a carefully designed maze of incisions and ablations in the atrial myocardium. Although it is a common procedure, serious complications may happen. Herein, we report on a 76-year-old man who presented with chest pain and atrial fibrillation and was found to have multi-vessel disease on a coronary angiogram. He underwen

INTRODUCTION: Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, and results in significant morbidity and mortality. The Cox-Maze IV procedure (CMP-IV) has been shown to have excellent efficacy in returning patients to sinus rhythm, but there have been few reports of late follow-up in sizable cohorts of patients with longstanding persistent AF, the most difficult type of AF to treat.
METHODS AND RESULTS: Between May 2003 and March 2020, 174 consecutive patients underwent a stand-alone CMP-IV for longstanding persistent AF. Rhythm outcome was assessed postoperatively for

BACKGROUND: Tricuspid regurgitation (TR) is common in patients undergoing mitral valve surgery, and atrial fibrillation (AF) can cause progression of TR. This study examined the hypothesis that correction of AF with the Maze procedure can prevent the late progression of TR after mitral valve surgery.
METHODS: We compared severity of TR in 33 patients (January 1, 1993, to January 1, 2003) who underwent concomitant Maze procedure plus mitral valve surgery and converted to normal sinus rhythm postoperatively with case-matched control patients who underwent mitral valve surgery alone and remained

Between February 2003 and January 2004 a microwave-modified Maze III procedure was performed as an associated procedure in nine patients in chronic atrial fibrillation undergoing surgery for heart valve disease. Clinical follow-up was carried out in all survivors, and an echocardiographic assessment done in all those in sinus rhythm, during the first week of February 2004. There were six women and three men with a mean age of 60 +/- 9.4 years. Their rhythm at the end of surgery was sinus in 2 patients, nodal in 4, and complete AV block in 3. One patient died in hospital and there patients had

OBJECTIVE: To evaluate the results of the maze procedure combined with mitral valve (MV) surgery in patients with chronic atrial fibrillation (AF).
METHODS: From 1994--1999, 47 patients with chronic AF underwent the maze procedure combined with MV surgery (maze group). They were compared to 47 patients matched for age, sex, left ventricular function and type of MV surgery (non-maze group). The maze group had less severe symptoms but larger left atrium, and AF of longer duration than the non-maze group. One surgeon performed all operations in both groups of patients.
RESULTS: There were two e

OBJECTIVE: The objective was to determine whether the Cox maze procedure provides adjunctive benefit in patients with atrial fibrillation undergoing mitral valve repair.
METHODS: We compared the outcome of 39 patients who had the Cox maze procedure plus mitral valve repair between January 1993 and December 1996 (maze group) with that of 58 patients with preoperative atrial fibrillation who had mitral valve repair during the same interval by the same surgeons (control group). Patients in the 2 cohorts were similar for age, gender, preoperative New York Heart Association class III or IV, and du

We report a case of constrictive pericarditis with atrial fibrillation after mitral valve repair and the Maze III procedure. A 66-year-old male underwent mitral valve repair and the Maze procedure for mitral valve regurgitation and chronic atrial fibrillation. About 4 months after discharge, he suffered from shortness of breath. Physical examination revealed a heart rate of 80 beats/min with irregular rhythm, external jugular venous dilatation and abdominal ascites. Electrocardiography revealed atrial fibrillation, and chest X-ray revealed moderate left pleural effusion. Computed tomographic i

OBJECTIVES: One of the earliest recognized postoperative complications of the maze procedure was the fluid retention in the immediate postoperative period. Routine postoperative administration of diuretics markedly reduces the frequency and severity of the fluid retention. However, the cause of the abnormal fluid balance is still uncertain.
METHODS: We evaluated 24 patients: 15 patients underwent the maze procedure (maze group) and 9 patients did not (nonmaze group). Blood samples were obtained before and in the time course after operation for atrial natriuretic peptide measurement. To evalua

BACKGROUND: Different types of recurrent atrial tachycardia (AT) after Cox-maze procedures have been reported, whereas biatrial tachycardia (BiAT) has not been systematically analyzed.
OBJECTIVE: In this study, we retrospectively investigated the electrophysiologic characteristics of BiAT after Cox-maze procedures by use of an ultrahigh-density mapping system.
METHODS: Of a consecutive 76 patients who underwent catheter ablation of AT after Cox-maze procedures, 12 BiATs were identified. High-density activation mapping was performed in both the left atrium and right atrium in combination with

BACKGROUND: The absence of atrial contraction (AC) after the maze procedure has been reported to cause subsequent annular dilatation and to increase the risk of embolic stroke. We hypothesized that the lack of AC could increase the risk of permanent pacemaker (PPM) implantation in patients undergoing the maze procedure.
METHODS: In 376 consecutive patients who had undergone a cryo-maze procedure and combined valve operation, recovery of AC was assessed at baseline and at immediate (≤2 weeks), early (≤1 year, 4.6±3.8 months), and late (>1 year, 3.5±1.1 years) postoperative stages.

BACKGROUND: Atrial fibrillation (AF) duration is one of the most consistent predictors of Cox maze (CM) procedure failure. We examined the impact of AF duration on CM success in patients having first-time concomitant surgery.
METHODS: First-time concomitant CM was performed in 505 patients. Freedom from atrial arrhythmia (AA) and class I/III antiarrhythmic drug (AAD) data were collected prospectively. Patients with longer AF duration (≥ 5 years; n = 113) were compared with shorter duration (<5 years; n = 392) in primary analyses. The AF duration was examined as a continuous variable in regres

OBJECTIVES: Atrial fibrillation (AF) is associated with less favorable outcomes in patients undergoing mitral valve and tricuspid valve surgery. Despite growing evidence on the potential benefits of surgical ablation for AF there is significant variability among surgeons in treatment of AF. The purpose of our study was to assess the effect of the Cox-maze procedure on operative and follow-up outcomes.
METHODS: In our prospective study, patients who underwent isolated mitral valve or mitral valve+tricuspid valve surgery without history of AF (n = 506), with untreated AF (n = 75), or with Cox-m

To examine the surgical indications and the results of the maze procedure, we reviewed 30 cases who underwent the maze procedure concomitantly with other open heart surgery from October 1995 to October 1997.: the average age was 60.9 years (37 to 75 years) and mean follow up period was 12.3 months (1 to 25 months). The modified maze procedure described by Kosakai and associates was applied in all patients. Twenty one patients (72.4%) regained atrial rhythm and eight patients (27.6%) sustained atrial fibrillation in the follow up periods. The left atrial diameter measured by echocardiography an
