Mediastinal ectopic parathyroid adenoma: report of two cases successfully treated by VATS approach.

Ectopic parathyroid adenomas are the cause of approximately one in four cases of primary hyperparathyroidism (pHPT). Most ectopic parathyroid adenomas are located adjacent to the upper thymus gland and can be resected with a collar incision. In rare cases, however, adenomas located in the chest require a transsternalor transthoracic approach. Due to the high rate of morbidity with a sternotomy or thoracotomy, minimally invasive methods such as video-assisted thoracoscopic surgery (VATS) or video-assisted mediastinoscopic surgery (VAMS) are becoming increasingly popular. We present two cases wi

Does endoesophageal ultrasound-guided fine-needle aspiration replace mediastinoscopy in mediastinal staging of thoracic malignancies?

OBJECTIVE: To determine the impact of endoesophageal ultrasound-guided fine-needle aspiration (EUS-FNA) on management of thoracic malignancies. METHODS: One hundred and twenty patients referred for invasive diagnostic and resection of thoracic malignancies were studied prospectively. Negative and inconclusive EUS-FNA findings were assessed by video-assisted mediastinoscopic lymphadenectomy (VAMLA) or open lymphadenectomy. RESULTS: One hundred and twenty patients, aged 64.1 years (range 38-85) underwent 120 EUS-FNA, 53 video-assisted mediastinoscopic and 48 open lymphadenectomies for diagnosi

Video-axillaroscopy: a new mini-invasive technique for surgical exploration of the axillary region.

Subclavicular lymph nodes are one of the deeper lymph node groups of the lymphatic network located in the axillary region. As its location is surrounded by vessels and nerves, biopsy of subclavicular lymph nodes is not possible without eye control. We describe a new, mini-invasive technique for accessing the thoracic outlet region. Video-axillaroscopy is a reliable mini-invasive technique for targeting biopsies of lymph nodes in the axillary region. It is safer and more accurate than radio-guided techniques. Exploration of this region might benefit treatment of lymphoma and breast cancer and a

Biopsy of anterior mediastinal masses under local anesthesia.

Treatment of most primary mediastinal tumors is based on positive histological diagnosis; we describe a variation of anterior mediastinoscopy under local anesthesia. After infiltration with local anesthetic, a transverse skin incision is made in the second, third, or fourth interspace. The endothoracic fascia is incised vertically adjacent to the periosteum and a mediastinoscope is inserted in the mediastinum. Between 1995 and 2001, we have employed this technique in 46 patients with anterior mediastinal tumors. Histological diagnosis was obtained in all patients. Pneumothorax (2 patients) was

Sclerosing thymoma with extensive ossification and myasthenia gravis: an extremely rare case in the anterior mediastinum.

Sclerosing thymoma is a rare variant of thymomas, recently recognized in pathology. Since its first description in 1994, only 15 cases have been reported. Furthermore, osseous metaplasia in sclerosing thymoma is an even unusual characteristic. To date, only 7 such cases have been described, while half of them show the symptom of myasthenia gravis. Herein we present a 57-year-old female with ossifying sclerosing thymoma and myasthenia gravis, which is the first case in literature of type B3 sclerosing thymoma with extensive ossification and myasthenia gravis. In consideration of its rarity, awa

Coexistence of Ankylosing Spondylitis and Löfgren's Syndrome.

A 46-year-old male patient diagnosed with ankylosing spondylitis presented to our polyclinic with complaints of pain, swelling, and limitation in joint mobility in both ankles and erythema nodosum skin lesions in both pretibial sites. The sacroiliac joint graphy and the MRI taken revealed active and chronic sacroiliitis. On the thorax CT, multiple mediastinal and hilar lymphadenopathies were reported. Mediastinoscopic excisional lymph node biopsy was taken and noncalcified granulomatous structures, lymphocytes, and histiocytes were determined on histopathological examination. The patients were

[A case of sarcoidosis who showed rapid swelling of mediastinal and abdominal lymph nodes].

A 49-year-old woman noticed hoarseness and facial palsey three months prior to her visit to our hospital. Chest radiograph and CT scanning revealed bilateral mediastinal and hilar lymphadenopathy. Bronchofiberoptic biopsy showed sarcoidosis. Her symptoms improved under no treatment. However, she showed rapid increase of mediastinal and abdominal lymph nodes swelling and elevation of serum level of sIL-2R during observation. Therefore, we must discriminate sarcoidosis-lymphoma syndrome from exacerbation of sarcoidosis. Mediastinoscopic biopsy was conducted for diagnosis, and it revealed exacerb

Esophageal Atresia Caused by Corrosive Esophagitis for over 50 Years: A Case Report.

INTRODUCTION: Corrosive esophagitis, often caused by the ingestion of alkalis, acids, or heavy metals, can result in severe esophageal damage and complications, such as stenosis or closure. Although initial treatment is conservative, surgical intervention is necessary when a chronic stricture occurs. A case of esophageal atresia persisting for 50 years due to corrosive esophagitis has not yet been reported. Here, we describe such a case. CASE PRESENTATION: The patient was a 72-year-old woman. At 20 years of age, she ingested an alkali substance in a suicide attempt, leading to the development

Novel use of the internal thoracic artery retractor in transcervical thymectomy.

Transcervical thymectomy is a standard approach to the thymus and anterior mediastinum, but it is limited by poor exposure of the lower recesses of the anterior mediastinum. A novel technique using the widely available internal thoracic artery sternal retractor and a 30°-video camera allows enhanced exposure of the anterior mediastinum.

Technique of endoscopic mediastinal dissection of the oesophagus.

Conventional transhiatal dissection of the oesophagus is usually performed without visual control. The attendant danger of this is that complications such as bleeding and tracheal lesions may be overlooked. This problem can be avoided by the use of an endoscopic operation system. This report describes a new technique of endoscopic microsurgical dissection of the oesophagus (EMDOE) and the results of the first 35 cases. Under visual control with a specially designed mediastinoscope the soft tissue surrounding the oesophagus is carefully dissected, and the oesophageal blood vessels can be safely

Sarcoidosis Presenting as Löfgren's Syndrome with Myopathy.

A 34-year-old female patient, who had proximal muscle weakness for 8 months, presented with erythema nodosum lesions on the pretibial region in addition to pain, swelling, and movement restriction in both ankles for the last one month. Thoracic CT demonstrated hilar and mediastinal lymphadenopathy. She underwent mediastinoscopic lymph node biopsy; biopsy result was consistent with noncaseating granuloma. Serum angiotensin converting enzyme level and muscle enzymes have been elevated. Muscular MRI and EMG findings were consistent with myositis. Muscle biopsy was done, and myopathy was found. Th

[Squamous cell carcinoma of unknown origin affecting mediastinal lymph nodes].

A 65-year-old woman was admitted to the hospital because of an abnormal shadow on a chest X-ray film. Chest CT scans showed enlargement of the right mediastinal lymph nodes. However, no primary lesion was detected despite thorough examination. Mediastinoscopic biopsy revealed that the tumor was a lymph-node metastatic lesion of a squamous cell carcinoma of unknown origin. A thoracotomy was done and the mass was completely resected. Radiotherapy was given after the operation. We have followed the patient for 1 year since the operation, but no primary lesion has yet been detected. Metastasis of

[Clinical usefulness and cost-effectiveness of transbronchial needle aspiration for the diagnosis of mediastinal adenopathy].

Transbronchial needle aspiration (TBNA) of mediastinal or hilar enlarged lymph nodes is a useful and safe technique. Nevertheless, its use has not become widespread, and the necessity of a specific training to obtain good results has been reported. In order to evaluate the clinical usefulness and cost-effectiveness of TBNA in inexperienced brochoscopists in this technique, a prospective study was conducted of this technique used in all patients with a chest CT with paratracheobronchial lymph nodes larger than 10 mm who had a fiberbronchoscopy ordered. TBNA was performed in 66 lymph node stagin

[Laparoscopically-assisted trans-mediastinal esophagectomy].

In esophageal cancer blunt dissection of the esophagus is reasonable in high-risk patients. Manual dissection can compromise the heart and reduce blood pressure. This can be avoided by laparoscopically assisted transmediastinal dissection of the esophagus. We describe the operative procedure and report on a patient with coronary heart disease who suffered from cancer of the cardia spreading into the esophagus as well as into the stomach. Esophagogastrectomy was performed with jejunal replacement.

Simultaneous double video mediastinoscopy and video mediastinotomy--a step forward.

Mediastinal lymph node biopsy plays a fundamental role in diagnosis, staging and management of lung cancer. We describe a novel method of using the video-mediastinoscope for concurrent cervical mediastinotomy and anterior mediastinoscopy. We have performed five concurrent procedures using this system in the last 14 months. In four cases, we assessed the aorto-pulmonary (A-P) window lymph nodes. In the fifth case, we performed a cervical mediastinoscopy for para-tracheal and sub-carinal lymphadenopathy followed by an anterior videomediastinotomy, video assisted intrapericardial assessment, dire

Mediastinal myxopapillary ependymoma primary or late metastases of paracoccygeal ependymoma: a case report.

A 25-year-old woman presented in 2002 with progressive shortness of breath and weight loss. A computed tomographic scan of the chest showed a huge anterior mediastinal mass, and pathological examination of a mediastinoscopic needle biopsy revealed typical myxopapillary ependymoma, an extremely unusual diagnosis at this site. Further workup and questioning of the patient revealed that she had opted not to disclose a history of surgery for right gluteal fold mass in 1993, which was primarily treated with surgery followed by radiotherapy for relapse. Review of the histology of the excised mass sh

[Clinical examination of thymic abnormalities and thymectomy in patients with ulcerative colitis].

Results and significance of thymectomy in 66 patients with ulcerative colitis were studied. The thymus delineated by means of pneumomediastinography was almost always successfully removed by our procedure of thymectomy via the transverse incision in the suprasternal notch, frequently combined with dissection by the mediastinoscope through the parasternal incision. Sixty-six thymectomized patients with ulcerative colitis consisted of 29 females and 37 males, ranging in age from 13 to 62 years. In the resected thymuses, lymphoid follicle formations, which are often encountered in high incidence

Demonstration of feasibility and technique of transesophageal endoscopic epicardial access in a porcine model.

BACKGROUND AND AIMS: Peroral endoscopic myotomy uses a submucosal tunnel that staggers the esophageal mucosotomy and myotomy, allowing minimally invasive interventions in the esophagus to treat achalasia. Our goal was to assess the feasibility of this method to access the mediastinum to perform endoscopic epicardial access and pericardial window in a porcine model. METHODS: Experiments were conducted to assess the feasibility of this procedure. Commonly available ERCP and EUS instruments were used to show epicardial access under both direct transesophageal mediastinoscopic visualization and E

A novel technique of mediastinoscopy-assisted esophagectomy with a flexible laparoscope and endoscopic overtube.

In mediastinoscopy-assisted esophagectomy, mobilizing the thoracic esophagus with visualization through the mediastinoscope is often difficult. We report a novel technique of transmediastinal esophagectomy with a flexible laparoscope and endoscopic overtube for a patient with early thoracic esophageal cancer. An endoscopic overtube was carefully inserted through the hiatus into the posterior mediastinum, and a laparoscope was inserted into the overtube. Surgical devices were inserted into the mediastinum through another overtube through the hiatus, and the middle and lower thoracic esophagus w

Advancing Gastroscope From Intraluminal to Extraluminal Dissection: Primary Experience of Laparo-gastroscopic Esophagectomy.

Transhiatal esophagectomy facilitates esophageal resection without the need for thoracotomy. However, this procedure carries the risks of blind and blunt dissection within the mediastinum. More recently, video-assisted or mediastinoscopic transhiatal esophagectomy was introduced to mobilize the esophagus under direct visualization. Even though, the procedure is technically demanding and animal studies have shown that the CO2 pneumomediastinum may be associated with hemodynamic instability. By further developing already established techniques, we pioneered the transhiatal esophageal mobilizatio
