Analysis of night-time and weekend otolaryngology service.

The night-time and weekend otolaryngology service provided to an urban population slightly in excess of half a million people in analysed over a six month period. During this time, 147 calls were received, of which 116 (79%) required a hospital visit by the Registrar-on-call. Children accounted for 39% of patients seen and adults 61%. The problems encountered were in keeping with traditional perceptions of otolaryngology. The most common diagnosis necessitating a hospital visit was a suspected foreign body in the upper aerodigestive tract (39 cases), followed by epistaxis (28 cases) and some f

Piloting online self-audit of methadone treatment in Irish general practice: results, reflections and educational outcomes.

BACKGROUND: Work based learning underpins the training and CPD of medical practitioners. Medical audit operates on two levels; individual self-assessment and professional/practice development. In Ireland, annual practice improvement audit is an essential requirement for the successful completion of continuous professional development (CPD) as determined by the regulatory body, the Irish Medical Council. All general practice (GP) doctors providing methadone maintenance treatment (MMT) in Ireland have a contractual obligation to partake in a yearly methadone practice audit. The Irish College of

Studies of avoidable factors influencing death: a call for explicit criteria.

OBJECTIVE: To analyse studies evaluating cases of potentially "avoidable" death. DESIGN: The definitions, sources of information, and methods were reviewed with a structured protocol. The different types of avoidable factors,--that is, deficiencies in medical care that may have contributed to death--were categorised. The presence of explicit classifications and standards was examined. basic criteria for quality of the studies were defined and the numbers of studies fulfilling these criteria were assessed. SETTING AND PARTICIPANTS: 65 studies, published during 1988-93 in peer reviewed medical

Lessons learned from nationwide perinatal audit of severe postpartum haemorrhage in the Netherlands.

BACKGROUND: Severe postpartum haemorrhage (PPH) remains a leading and largely preventable cause of maternal morbidity and mortality worldwide. Delayed recognition, inappropriate fluid replacement, and suboptimal clinical management contribute to adverse outcomes. Clinical audit can identify deficiencies in care and stimulate quality improvement. In the Netherlands, a nationwide perinatal audit system enables multidisciplinary review of maternity care to enhance collaboration and optimize clinical practice. This study reports the first nationwide audit of severe PPH management in the Netherland

Preparing for inspection: a tool to maximize quality and minimize risk.

Self-assessment of a dialysis unit can provide an effective means of improving or assuring the level of quality the unit provides in delivering safe and effective care. A tool for use by facility personnel was developed and is being used in 12 dialysis units within one dialysis company. This tool has been useful in identifying areas of risk and in assisting units in developing and maintaining survey readiness. This article presents the facility tool and describes how it may be used to assist other dialysis facilities in risk management and quality assurance activities.

The class certification of medical monitoring claims.

The tort claim of medical monitoring has produced a disarrayed set of state and federal court opinions. The procedural dimensions of this claim are as vexing as the related substantive issues with which courts and commentators have long been grappling. Ordinarily, mass tort actions, typically involving claims for money damages, are certified under Rule 23(b)(3), which class category requires the right to notice and to opt out of a proceeding, and the fulfillment of "predominance" and "superiority" requirements. Such features are absent in Rule 23's mandatory classes. Nevertheless, this Note ar

Alcohol-history taking by nurses and doctors--how accurate are they really?

The aim of this study was to look at how nurses and doctors record alcohol histories on the patients under their care and the frequency and appropriateness of the way they use an alcohol withdrawal scale in the acute hospital setting. To achieve these aims patient records were audited and compared at two points in time, in 1992 and 1994. Results of the study showed that nurses and doctors took alcohol histories from patients on approximately three-quarters of occasions and that this level of recording has not changed over time. The adequacy of alcohol-history taking has, however, increased for

Decision support to avoid medication errors - how far have we come in Denmark and what are the present challenges.

The number of medication errors reported to The Danish National Board of Health in Denmark exceeds 5000 per year. It is well known that computerized physician order entry (CPOE) with addition of decision support tools may reduce the frequency of medication errors. The primary scope of the work in Denmark has been to help health care professionals avoiding harmful errors. Using data primarily from The Danish National Board of Health, based on the reports of errors from Danish hospitals, and with our previous foundation in the international literature, we analyzed the errors which led to harmful

An audit of completeness of HIV clinical histories: before and after introduction of an HIV proforma.

The completeness of a 'first consultation' human immunodeficiency virus (HIV) clinical history before and after the introduction of an HIV proforma was audited by a retrospective case notes review. Twenty key variables considered essential to every history were assessed. There was a significant improvement in the documentation of 14 of the 18 items for men and 14 of the 20 items for women post-proforma with no deterioration in documentation of any of the variables. Our study supports the introduction of a structured 'first consultation' notes proforma for use during consultations with both new

A retrospective single-site data-linkage study comparing manual to electronic data abstraction for routine post-operative nausea and vomiting audit.

BACKGROUND: Post-operative nausea and vomiting (PONV) is a common cause of patient dissatisfaction following anaesthesia. Audit of adherence to PONV prevention guidelines is resource intensive when performed by manual chart extraction. Electronic audit can require costly anaesthetic and medical records. OBJECTIVE: In our single-site study we sought to compare manual and electronic PONV audits by utilizing existing non-anaesthetic electronic medical records to avoid expensive additional software. METHODS: The audits were performed from 13 January 2020 to 1 February 2020 for surgical inpatient

Skin disorders at sea.

The purpose of this study is to characterize the types of skin disorders occurring at sea requiring acute treatment. The case logs of a tele-medicine service for US flagged ships at sea were reviewed from March 1, 2006 until March 1, 2009. Of 1844 total cases, 10% (n = 183) were for skin disorders. Sixty-eight percent (n = 125) were infections, 14% (n = 25) were inflammatory, 7% (n = 13) were environmental, and 11% (n = 20) were non-specific rashes. Cutaneous abscesses and cellulitis (n = 84) were the most common acute skin disorders encountered. In some cases (n = 81), still digital photograp

An Audit of Adherence to the Agitation and Arousal Chart on an Inpatient Child and Adolescent Psychiatric Ward.

BACKGROUND: Evidence has shown that pro re nata (PRN) medications are often prescribed to manage agitation and disruptive behaviors. They require meticulous monitoring to prevent adverse effects and medication overuse. This article describes an audit that evaluated staff adherence to the use of an "agitation and arousal" chart that monitored PRN medication use on a child and adolescent psychiatric ward. METHODS: The audit was conducted using the Plan-Do-Study-Act cycle design. Results from the first cycle were analyzed and presented in multiple formats. The second cycle was undertaken 4 month
