Promotion of Preventive Sleep Research from the Perspective of Health and Public Safety in Japan - Secondary Publication.

Ensuring good sleep quality and adequate sleep duration is crucial for health. Sleep apnea syndrome (SAS) impairs sleep quality and increases the risk of diabetes, cardiovascular diseases, and accidents. The author has significantly advanced the understanding of SAS in Japan through over 20 years of epidemiological studies. Findings have revealed that individuals with a 3% oxygen desaturation index (ODI) ≥15 events/h have a 1.69-fold higher risk of developing diabetes. Those with mild to severe sleep-disordered breathing (SDB) with a 3% ODI ≥5 events/h face high risks of heart disease and lacu

COVID-19 pandemic and transgender migrant women in India: Socio-economic vulnerability and vaccine hesitancy.

In India, transgender women, often referred to as hijra or kinnar, remain a marginalized group and encounter interpersonal and structural barriers that subject them to social exclusion, discrimination, lack of access to education and health care, and fewer job opportunities compared with the general population. During the COVID-19 pandemic these disparities were heightened and the livelihood of transgender migrant women were severely hit and disrupted, causing further financial and physical hardship. The present study aims to explore the socio-economic vulnerability faced by these women during

Quality of life perceptions amongst patients co-infected with Visceral Leishmaniasis and HIV: A qualitative study from Bihar, India.

BACKGROUND: Co-infection with Visceral Leishmaniasis (commonly known as Kala Azar, KA) and Human Immunodeficiency Virus (HIV) is increasingly being diagnosed among patients in Bihar. This qualitative study is the first assessment of self-reported quality of life among patients co-infected with KA-HIV in the Asian context. METHODS: We conducted semi-structured, in-depth interviews and adopted an inductive thematic analysis to generate evidence on the quality of life of patients co-infected with KA and HIV. Patients were purposively sampled until saturation was attained. RESULTS: We found that

Substance use and symptoms of mental health disorders: a prospective cohort of patients with severe substance use disorders in Norway.

BACKGROUND: There is high co-occurrence of substance use disorders (SUD) and mental health disorders. We aimed to assess impact of substance use patterns and sociodemographic factors on mental health distress using the ten-item Hopkins Symptom Checklist (SCL-10) over time. METHODS: Nested prospective cohort study of 707 participants with severe SUD across nine opioid-agonist-therapy outpatient clinics and low-threshold municipality clinics in Norway, during 2017-2020. Descriptive statistics were derived at baseline and reported by means and standard deviation (SD). A linear mixed model analys

Do sleep deprivation and time of day interact with mild obstructive sleep apnea to worsen performance and neurobehavioral function?

STUDY OBJECTIVES: Sleep deprivation, time of day (circadian influences), and obstructive sleep apnea (OSA) all reduce performance and neurobehavioral function. We assessed the interactive effect of sleep deprivation and time of day on performance and neurobehavioral function in subjects with and without mild OSA. METHODS: This was a cross-over study in which 13 subjects with mild OSA and 16 subjects without OSA had performance and neurobehavioral testing after a normal night's sleep and after a night of supervised sleep deprivation. All subjects were studied in the sleep laboratory of a unive

Lethal forethought: delayed reward discounting differentiates high- and low-lethality suicide attempts in old age.

BACKGROUND: The decision to commit suicide may be impulsive, but lethal suicidal acts often involve planning and forethought. People who attempt suicide make disadvantageous decisions in other contexts, but nothing is known about the way they decide about the future. Can the willingness to postpone future gratification differentiate between individuals prone to serious, premeditated and less serious, unplanned suicidal acts? METHODS: Four groups of depressed participants aged 60 and older made choices between smaller immediate and larger delayed monetary rewards: 15 who had made high-lethalit

Special considerations in managing pain and psychosocial distress in patients with opioid use disorder and cancer: the role of the supportive care and psycho-oncology interdisciplinary team.

People with a substance use disorder (SUD) have shortened lifespans due to complications from their substance use and challenges engaging with traditional health care settings and institutions. This impact on life expectancy is especially prominent in patients with co-occurring SUDs and cancer, and often has a much worse prognosis from the cancer than a similar patient without a SUD. Palliative care teams are experts in serious illness communication and symptom management and have become increasingly embedded in the routine care of patients with cancer. We argue that the skill set of palliativ

Gender, Marital, and Parental Barriers to Pursuing Gastroenterology Fellowship: A Multicenter Analysis of a National Survey and Actionable Solutions.

INTRODUCTION: Female gastroenterologists comprise 19% of the gastroenterology (GI) workforce, despite females making up 30% of GI fellows and over 50% of medical students in the USA. Barriers to pursuing GI fellowship have not been studied at the resident level. We aimed to determine multiple barriers that may prevent internal medicine (IM) residents from pursuing GI fellowship. METHODS: Our multi-center team distributed an IRB-approved electronic survey to residents in IM residency programs nationwide. Multiple rounds of emails with survey description and link were sent to program directors

Acceptance of advance care planning and influencing factors from the perspective of the life cycle: a cross-sectional study.

BACKGROUND: Advance care planning is an important part of palliative care. Public acceptance is a prerequisite for the widespread development and implementation of advance care planning. However, little is known about the level of public's acceptance and influencing factors of advance care planning across different life cycles. METHODS: A cross-sectional study in mainland China was conducted from June 20 to August 31, 2022. We used multi-stage sampling strategy to recruit participators. A stepwise linear regression analysis was used to examine the influencing factors in different life cycles

Clinical approaches to hypersomnolence complaints in psychiatry: A narrative review.

BACKGROUND: Hypersomnolence complaints are common in psychiatric disorders and carry substantial functional burden, yet they remain under-recognized and frequently under-investigated. Clinically, hypersomnolence spans three partially overlapping dimensions: excessive daytime sleepiness, excessive total sleep time/need for sleep, and sleep (wake) inertia. OBJECTIVE: To provide a pragmatic, clinician-oriented framework for evaluating and managing hypersomnolence complaints in psychiatry, integrating sleep medicine concepts with psychiatric comorbidity and real-world care pathways. METHODS: Nar

COVID-19 and Lung Cancer Interactions: A Literature Review.

This review aims to discuss the apparent reduction in pulmonary cancer incidence in the general population during and shortly after the COVID-19 pandemic from a biological and pathophysiological mechanistic point of view. While the epidemiological evidence points to a disruption in the early- and mid-stage diagnostic process, which causes a shift to late-stage lung cancer discovery with no impact on its actual prevalence, an alternative hypothesis based on the intersection of viral and cancer biology could have a real effect on lung carcinogenesis as an independent phenomenon. By weaving toget

Barriers to treatment of infectious and other somatic comorbidity in drug users.

BACKGROUND: Intensive use of illicit drugs in general and drug injecting in particular are associated with higher somatic co-morbidity, especially of infectious aetiology. The treatment participation of drug users is complicated by a number of barriers. MATERIAL AND METHODS: At the end of 2013, a cross-sectional questionnaire study was conducted on a non-random sample of 240 drug users (188 men and 52 women) recruited in low-threshold drug services in Prague based on their willingness to participate in the study. Drug injecting sometimes in their lifetime was reported by 237 respondents (98.8

Cancer Care Delivery and Financial Barriers Among Patients in Lebanon: A Descriptive Multicenter Cross-Sectional Study.

Background Cancer care in Lebanon is provided within a fragmented health system affected by economic instability, incomplete insurance coverage, and variable access to diagnostic and treatment services. These factors may influence the timely initiation of treatment, continuity of care, and the delivery of evidence-based oncology care. This study aimed to characterize access to cancer care in Lebanon by evaluating treatment delays, socioeconomic burdens, health coverage, treatment supply sources, and concordance with National Comprehensive Cancer Network (NCCN) recommendations among adults rece

Access to voluntary counseling and testing for HIV in developing countries.

The counseling that precedes and follows testing of subjects for HIV has become, quite unexpectedly, a focal point for assessment of the ethical propriety, availability, and appropriateness of health services during the AIDS epidemic. It can be anticipated that in the worst affected regions, Voluntary Confidential Counseling and Testing (VCCT) will be an integral component of "...access to comprehensive, essential, quality health care" which is WHO's goal of "Health for All" in the next century. The role, purpose, location, and methods of VCCT, which were reviewed at the previous Global Strate
