Prevention of medication overuse and medication overuse headache in patients with migraine: a randomized, controlled, parallel, allocation-blinded, multicenter, prospective trial using a mobile software application.

BACKGROUND: Overall, 55% of the German population suffers from primary episodic headaches according to recent studies. Inadequate management of headache disorders is a significant medical problem. The prevalence of medication overuse headache (MOH) is about 1% with an estimated number of 800,000 people in Germany. Medication overuse (MO) and MOH are usually managed through a complex process of medication withdrawal and initiating of prophylaxis. However, patients who were successfully treated for MO or MOH have a high relapse rate in the following 2 years. Previously, continued monitoring of s

Corticosteroids in withdrawal therapy following medication-overuse headache: a systematic review.

INTRODUCTION: Medication-overuse headache (MOH) affects 1-2% of the population and imposes substantial burden. Corticosteroids are often used as bridge therapy during withdrawal, although their effectiveness remains uncertain. This study aims to evaluate the effectiveness of corticosteroids as a withdrawal strategy in MOH. METHODS: A systematic review with meta-analysis was conducted in accordance with PRISMA guidelines. The protocol was registered in PROSPERO (ID: 1161824). Searches were performed in PubMed, MEDLINE, EMBASE, the Cochrane Library, Web of Science, SciELO, and LILACS through Ju

Systemic Inflammation and Oxidative Stress Markers in Patients With Medication Overuse Headache.

BACKGROUND: Recent scientific evidence has suggested that inflammation and oxidative stress may play a significant role in the pathogenesis of medication overuse headache (MOH). This study aims to investigate the differences in peripheral markers of systemic inflammation and oxidative stress, such as the monocyte to high-density lipoprotein (HDL) ratio (MHR), and their correlation with clinical symptoms in MOH patients, episodic migraine patients, and healthy controls. METHODS: This cross-sectional study recruited MOH patients along with age- and sex-matched controls. Differences in indicator

An altered reward system characterizes chronic migraine with medication overuse headache.

BACKGROUND: Medication overuse headache shares several characteristics with substance use disorders. However, key features of substance use disorders such as increased impulsivity and alterations in reward processing remain little explored in medication overuse headache. METHODS: Temporal discounting and impulsive decision making behavior and the associated brain mechanisms were assessed in 26 chronic migraine patients with medication overuse headache and in 28 healthy controls. Regions-of-interest analyses were first performed for task-related regions, namely the ventral striatum and the ven

Involvement of Plasma Melatonin in Medication-Overuse Headache: A Cross-Sectional Study.

OBJECTIVES: Patients with medication-overuse headache (MOH) are often complicated with anxiety, depression, and sleep disorders and are associated with dependence behavior and substance abuse. Melatonin has physiological properties including analgesia, regulation of circadian rhythms, soporific, and antidepressant and affects drug preference and addiction. This study aimed to investigate the role of melatonin in MOH compared with episodic migraine (EM) and healthy controls and to verify the relationship between plasma melatonin levels and psychiatric symptoms. METHODS: Thirty patients affecte

Traumatic Experiences, Stressful Events, and Alexithymia in Chronic Migraine With Medication Overuse.

Background: Many factors are involved in the prognosis and outcome of Chronic Migraine and Medication Overuse Headache (CM+MOH), and their understanding is a topic of interest. It is well known that CM+MOH patients experience increased psychiatric comorbidity, such as anxiety, depression, or personality disorders. Other psychological factors still need to be explored. The present study is aimed to evaluate whether early life traumatic experiences, stressful life events, and alexithymia can be associated with CM+MOH. Methods: Three hundred and thirty-one individuals were recruited for this stud

Factors associated to chronic migraine with medication overuse: A cross-sectional study.

BACKGROUND AND AIM: Factors implicated in the evolution of episodic migraine into chronic migraine are largely elusive. Medication overuse is considered to be one of the main determinants, but other possible clinical and psychological factors can play a role. The aim of this study is to identify factors that are associated with chronic migraine with medication overuse. METHOD: We enrolled consecutive migraine patients, subdividing them in two groups: Subjects with a long history of episodic migraine and subjects with chronic migraine and medication overuse. We then compared their clinical and

Anxiety associated with asthma exacerbations and overuse of medication: the role of cultural competency.

Toshi, a 14-year-old Japanese boy, had uncontrolled asthma after relocating from Japan with his family 1 year ago. In Japan, he was diagnosed with moderate, persistent asthma, which was controlled with salmeterol and albuterol on an as needed basis. Since moving to the United States, Toshi complained of frequent dyspnea.Initially, he was seen by a Japanese physician who prescribed 200 mg of fluticasone 3 times a day and albuterol nebulization as needed. When Toshi came to the Pediatric Primary Care Clinic, he reported using his nebulizer up to 25 times daily. A physical examination revealed a

Pathophysiology of medication overuse headache: insights and hypotheses from preclinical studies.

INTRODUCTION: Medication overuse headache (MOH) is a clinical concern in the management of migraine headache. MOH arises from the frequent use of medications used for the treatment of a primary headache. Medications that can cause MOH include opioid analgesics as well as formulations designed for the treatment of migraine, such as triptans, ergot alkaloids, or drug combinations that include caffeine and barbiturates. LITERATURE REVIEW: Gathering evidence indicates that migraine patients are more susceptible to development of MOH, and that prolonged use of these medications increases the progn

Elimination of menstrual-related migraine beneficially impacts chronification and medication overuse.

OBJECTIVES: This study seeks to determine whether menstrual-related migraine (MRM) has a discrete, attributable impact on migraine chronicity and medication overuse. BACKGROUND: Menstrual-related migraine can be a disabling headache on its own; but when seen in headache clinics, it is often enmeshed in the setting of chronic migraine (CM) and medication overuse headache (MOH). Whereas nonspecific migraine preventives bestow their benefit uniformly, hormonal preventives (HPs)--when they are successful--address a discrete hormonal mechanism. They confer no known benefit to migraines that are no

Iron concentration in the ventral pallidum of chronic migraine with medication-overuse headache.

BACKGROUND: Chronic migraine (CM) imposes a significant disease burden and frequently co-occurs with medication-overuse headache (MOH). MOH is a secondary headache disorder occurring especially in individuals with primary migraine and associated with excessive analgesic use. Impaired iron homeostasis may be related to the medication overuse behavior, however, its' pathophysiology is complex and incompletely understood. This study examines regional iron deposition in the subcortical nuclei in two comparisons: (1) controls versus chronic migraine patients; and (2) CM patients with medication-ove
