Assessment of Differences in Inpatient Rehabilitation Services for Length of Stay and Health Outcomes Between US Medicare Advantage and Traditional Medicare Beneficiaries.

IMPORTANCE: Enrollment in Medicare Advantage (MA) has been increasing and has reached one-third of total Medicare enrollment. Because of data limitations, direct comparison of inpatient rehabilitation services between MA and traditional Medicare (TM) beneficiaries has been very scarce. Subgroups of elderly individuals admitted to inpatient rehabilitation facilities (IRFs) may experience different care outcomes by insurance types. OBJECTIVE: To measure the differences in length of stay and health outcomes of inpatient rehabilitation services between TM and MA beneficiaries in the US. DESIGN,

Impact of the change of copay policy in Medicare Part D on zoster vaccine uptake among Medicare beneficiaries in a managed care organization.

BACKGROUND: Kaiser Permanente Southern California (KPSC) adopted the Medicare Part D Tier-6 with zero patient copay for zoster vaccination in 2012. We assessed the impact of the implementation on zoster vaccination rate (GSK study identifier: HO-13-14,182). METHODS: Zoster vaccination rate was examined among an open cohort of ≥65-year-old Medicare Part D beneficiaries during 01/01/2008-06/30/2014, compared to ≥65-year-old commercial health plan members and 60-64-year-old members. The demographics, vaccination records, and insurance and benefit type were confirmed through KPSC electronic medic

Assessing 1-Year Comorbidity Prevalence and Its Survival Implications in Medicare Beneficiaries Diagnosed with Cancer: Insights from a New SEER-Medicare Resource.

BACKGROUND: Almost half of Medicare beneficiaries diagnosed with cancer from 1992 to 2005 had at least one comorbid condition. Conditions affect a range of domains from clinical decision-making to quality of life, which are important to consider when conducting cancer research. We introduce a new Surveillance, Epidemiology, and End Results (SEER)-Medicare resource to facilitate using claims data for patients with cancer. METHODS: We use the SEER-Medicare resource to estimate prevalence of comorbidities, 5-year survival rate by cancer site, stage, age and comorbidity severity, and prevalence o

Disparities in characteristics in accessing public Australian sexual health services between Medicare-eligible and Medicare-ineligible men who have sex with men.

OBJECTIVES: Accessible health services are a key element of effective human immunodeficiency virus (HIV) and sexually transmitted infection (STI) control. This study aimed to examine whether there were any differences in accessing sexual health services between Medicare-eligible and Medicare-ineligible men who have sex with men (MSM) in Melbourne, Australia. METHODS: We conducted a retrospective, cross-sectional study of MSM attending Melbourne Sexual Health Centre between 2016 and 2019. Demographic characteristics, sexual practices, HIV testing practices and STI diagnoses were compared betwe

The Rise of Medicare Advantage is Impacting the Fidelity of Traditional Medicare Claims Data: Implications for Reporting of Long-Term Total Knee Arthroplasty Survivorship.

BACKGROUND: Traditional Medicare (TM) claims data are widely used by researchers and registries to report survivorship following total knee arthroplasty (TKA). The purpose of the present study was to investigate whether the mass exodus of patients from TM to Medicare Advantage (MA) has compromised the fidelity of TM data. METHODS: We identified 11,717 Medicare-eligible patients (15,282 knees) who had undergone primary TKA from 2000 to 2020 at a single institution. Insurance type was analyzed, and 84% of TKAs were covered by TM. The rates of survivorship free from revision or reoperation were

Classification of Emergency Diagnosis-Sensitive Conditions Among Medicare Beneficiaries.

IMPORTANCE: Diagnostic accuracy is a key driver of emergency department (ED) quality, yet little consensus exists on which conditions to target. OBJECTIVE: To develop a list of emergency diagnosis-sensitive conditions (EDSCs) for measuring diagnostic error in the ED. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cross-sectional study examined a national sample of claims for fee-for-service Medicare beneficiaries 65 years or older with nonelective hospitalizations from January 1, 2022, to December 31, 2024. EXPOSURE: Potential diagnostic errors in the ED. MAIN OUTCOMES AND MEASURES:

Emergency diagnosis of multiple myeloma: a SEER-Medicare study.

PURPOSE: Non-specific presenting symptoms make multiple myeloma (MM) one of the most difficult hematologic cancers to diagnose, increasing the risk of late diagnosis and emergency presentation (EP). We examined the frequency of EP in MM, estimated associations between EP and patient-level factors, and MM symptom-related healthcare utilization in the year before diagnosis. METHODS: We studied Medicare beneficiaries age ≥ 66 years diagnosed with MM between 2008 and 2017. Emergency presentation (EP) was defined as diagnosis preceded by a recent ED visit, sub-classified as: inpatients (EP-IP) for

Oncology Organization and Oncologist Networks Under Medicare Advantage Plans.

IMPORTANCE: More than half of Medicare beneficiaries are now enrolled in Medicare Advantage (MA) plans. These plans offer lower premiums and additional benefits compared with Traditional Medicare (TM) but commonly restrict organization and clinician networks, potentially limiting access to specialists, including oncologists. Little is known about the use of oncology networks in MA plans. OBJECTIVE: To evaluate effective oncology network breadth under MA plans based on realized health care utilization. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used linked 2016-2019 Surveil

State Medicaid Programs Face Increased Spending On Medicare Premiums.

Medicaid covers Medicare premiums and out-of-pocket expenses for low-income older adults, who make up a growing segment of the US population. Using administrative data, we characterized the change in the number of beneficiaries dually enrolled in Medicare and Medicaid and corresponding changes in Medicaid spending on Medicare Part A and Part B premiums during the period 2013-24. The number of dually enrolled beneficiaries increased by 27 percent, while Medicaid spending on Medicare premiums nearly doubled, primarily as a result of growing Part B premium spending. We estimated that at least hal

Chemodenervation Use for Overactive Bladder in Female Medicare Beneficiaries.

IMPORTANCE: Only a fraction of women with overactive bladder (OAB) receive effective treatment for this condition; inequities in access to OAB care must be understood. OBJECTIVE: The objective of this study was to explore features associated with intradetrusor chemodenervation treatment for OAB among female Medicare beneficiaries from different racial and ethnic groups. STUDY DESIGN: This was a retrospective cohort study of females ≥65 years old from the Center for Medicare & Medicaid Services' 5% Limited Data Set with a diagnosis of OAB between 2012 and 2020. Perinatal Risk and Outcomes Among Individuals Insured by Medicare.

OBJECTIVE: Pregnant Medicare beneficiaries are typically individuals with chronic conditions causing disability or end-stage organ disease, which may increase obstetric risk. Few studies have examined this population. We compared medical comorbidities and perinatal outcomes between Medicare-insured and non-Medicare-insured obstetric patients. STUDY DESIGN: We performed a cross-sectional analysis of the 2019 Healthcare Cost and Utilization Project National Inpatient Sample, including singleton deliveries (Major Diagnostic Category 14). Using discharge weights, strata, and hospital clusters, we

Medicare Advantage Beneficiary Enrollment Decisions Following New Complex Conditions.

IMPORTANCE: Prior research has demonstrated higher rates of disenrollment from Medicare Advantage (MA) among people with greater health care needs. Yet, less is known about how disenrollment responds to changes in medical complexity, whether disenrollment varies by the degree of complexity, and how these patterns differ by state Medigap protections and MA plan characteristics. OBJECTIVE: To examine changes in MA plan disenrollment following the development of new complex medical conditions and assess heterogeneity by number of conditions, state Medigap policy, and plan characteristics. Intersectional HIV disparities among transgender Medicare beneficiaries of color.

We compared the prevalence of HIV across race and ethnicity groups for transgender and gender-diverse (trans) and cisgender Medicare beneficiaries using quantitative intersectional analysis of Medicare claims data from 2011 to 2020. We found that approximately 61% of the excess HIV prevalence among trans Asian and Pacific Islander beneficiaries, 62% among Hispanic beneficiaries, and 78% among Black beneficiaries is associated with the intersection of racialized and gendered experience.

Opioid consumption after lower limb amputation in Medicare beneficiaries.

BACKGROUND: Despite targeted national quality improvement initiatives, opioid prescription and use patterns after lower limb amputation are not well studied. OBJECTIVE: To determine risk factors associated with extended opioid use and rates of high-risk opioid prescribing after lower limb amputation. DESIGN AND SETTING: A retrospective population-based study was performed by querying a 20% national sample of Medicare claims of patients undergoing lower limb amputation from 2009 to 2018. Patients were excluded if they did not discharge home or had subsequent surgery 180 days after discharge.

Characterizing Guillain-Barré Risk Factors Among Traditional Medicare Beneficiaries.

Guillain-Barré Syndrome (GBS) is a rare and potentially life-threatening autoimmune disorder affecting the peripheral nerves. We sought to identify demographic and clinical characteristics associated with GBS onset. In this retrospective case-control study, we used national 2005 to 2020 fee-for-service Medicare claims data to identify beneficiaries with incident GBS using a chart-validated algorithm (N = 16 280), matched 1:1 with non-GBS controls (N = 16 280) by age, sex, race/ethnicity, number of preventive care visits, and year of diagnosis. We then used 2 separate modified Poisson regressio
