Hallux valgus and metatarsus primus varus treated by bunionectomy and proximal metatarsal osteotomy.

Sixteen feet with hallux valgus and metatarsus primus varus have been treated by excision of the exostosis of the first metatarsal head, advancement of the medial capsule and the abductor hallucis, release of the lateral capsule and adductor tendon, and open-wedge osteotomy at the base of the first metatarsal inserting a bone graft. During a three-year period ther have been no nonunions of the osteotomy, and satisfactory maintenance of the corrections has occurred. This method is preferred over distal metatarsal osteotomy.

Rare Proximal Diaphyseal Stress Fractures of the Fifth Metatarsal Associated With Metatarsus Adductus.

Before the report of English surgeon Robert Jones, who sustained a fracture to his fifth metatarsal while dancing around a tent pole, metatarsal fractures were thought to be the result of direct trauma to the foot. The mechanism of metatarsal fractures, in particular, those involving the fifth metatarsal, is now well understood. Patients with an adducted alignment of their forefoot can overload the fifth metatarsal base, putting them at an increased risk of fractures of this bone. Studies have reported that 2 distinct types of proximal diaphyseal or junctional fractures of the fifth metatarsal

Radiographic Recurrence of Deformity After Hallux Valgus Surgery in Patients With Metatarsus Adductus.

BACKGROUND: Metatarsus adductus (MA) is a congenital condition that may lead to the development of hallux valgus (HV). The associated anatomic deformities may lead to recurrence of the HV in patients with MA. The goals of the study were to identify radiographic rates of recurrence of HV following surgery for HV in patients with MA. METHODS: Between 2002 and 2013, 587 patients who underwent HV surgery were retrospectively identified. The radiographic parameters recorded included the hallux valgus angle (HVA), the intermetatarsal angle (IMA), and the metatarsus adductus angle (MAA) obtained fro

Surgical treatment of metatarsus adductus using a combined soft tissue and osseous approach.

Children between 6 and 8 years of age with metatarsus adductus deformity have been considered to be in the gray zone for surgical correction. Should osseous or soft tissue procedures be implemented to reduce the adducted attitude of the metatarsals? The literature clearly describes one or the other and the intermediary chondrotomy procedure described by Johnson. The use of a combined approach has not been presented. The present authors offer a brief review of metatarsus adductus, and two case reports using a modified, combined approach. Two 7-year-old girls presented with bilateral resistant m

A Systematic Approach to the Surgical Correction of Combined Hallux Valgus and Metatarsus Adductus Deformities.

The presence of metatarsus adductus (MTA) adds complexity to the diagnosis and treatment of hallux valgus (HV). Identification and careful analysis of these combined deformities is of paramount importance. The inability to completely correct HV and an increased incidence of recurrence has been established when MTA deformity is present. We present an option for correction of the combined deformities with multiplanar angular correction arthrodesis of the first, second, and third tarsometatarsal (TMT) joints.

Prevalence of Metatarsus Adductus in Symptomatic Hallux Valgus and Its Influence on Functional Outcome.

BACKGROUND: Metatarsus adductus (MA) increases the risk of developing symptomatic hallux valgus (HV). This study aimed to determine the prevalence of MA in patients with symptomatic HV and to evaluate how it affected the functional outcome after scarf osteotomy. METHODS: Between January 2007 and June 2012, a total of 206 patients who underwent scarf osteotomy for symptomatic HV at a tertiary hospital were included. The metatarsus adductus angle (MAA) was determined using the Modified Sgarlato method, and these patients were categorized into 2 groups: MA (MAA > 20 degrees); and Control (MAA ≤

Hallux valgus correction in a patient with metatarsus adductus with multiple distal oblique osteotomies.

Metatarsus adductus is a structural foot deformity that is rarely associated with hallux valgus deformity. Surgical treatment is challenging, and multiple osteotomies are often required to correct both deformities. However, surgical impact must be considered, especially in elderly patients. We present a clinical case of a 76-year-old woman affected by hallux valgus and metatarsus adductus deformity. Multiple distal oblique osteotomies were performed on the first, second, and third metatarsals, coupled with Z-lengthening of the extensor digitorum longus tendons.

The treatment of rigid metatarsus adductovarus with the use of a new hinged adjustable shoe orthosis.

A retrospective review of the early results of treatment of metatarsus adductovarus with a new hinged adjustable shoe orthosis, the Bebax shoe, is presented. The results in 120 cases (210 feet) are included in the study. Criteria for severity of the initial deformity were developed, and all cases of true clubfoot and those cases with prior treatment were excluded. The average age at institution of treatment was 4.5 months, and treatment time averaged 2.7 months (range 1-7 months). Overall, 96% of the cases were completely resolved with the use of the orthotic alone, 3% required additional cast

Fatty degeneration of osteocytes of the fourth metatarsus in patients with idiopathic necrosis of the femoral head.

(1) Bone biopsies from the femoral heads and the fourth metatarsal head were obtained in the patients with idiopathic necrosis of the femoral head and with rheumatoid arthritis. (2) 71.4% of the patient with idiopathic necrosis of the femoral head revealed fatty degeneration of osteocytes in either the femur and/or metatarsus. (3) 57% of those patients demonstrated hyperlipidemia. (4) Metatarsal biopsies obtained from the patients with rheumatoid arthritis disclosed no fatty degeneration of osteocytes except in one patient treated with steroids.

Plantarflexory base wedge osteotomy in the treatment of functional and structural metatarsus primus elevatus.

Plantarflexory base wedge osteotomy has proven to be a viable, rewarding treatment, where first metatarsal phalangeal joint pathology, as a result of metatarsus primus elevatus, is predicted or in its earliest forms. Although this topic was first addressed by Lambrinudi in 1938, it has received little notice in the literature, and its relevance is probably underestimated. In fact, the diagnosis of metatarsus primus elevatus with associated advancing degenerative joint disease is probably being missed in a significant number of patients. The early signs of this condition are often disregarded e

Correlation Between Hallux Valgus Severity and the Prevalence of Metatarsus Adductus in Hallux Valgus.

INTRODUCTION: Hallux valgus (HV) is a common foot deformity, with metatarsus adductus (MA) identified as a potential predisposing factor. MA has been shown to negatively affect surgical outcomes for HV, particularly in severe cases. This study aims to clarify the prevalence of MA in the HV population using different metatarsus adductus angle (MAA) measurement methods and assess whether MAAs are influenced by HV severity. MATERIALS AND METHODS: This retrospective study included 294 feet from 147 participants. Patients were classified into non-HV (normal) and HV subgroups, with HV severity grad

Early-onset metastatic fibroblastic osteosarcoma of the metatarsus in a young cat: a case report.

BACKGROUND: Osteosarcoma is the most common primary bone tumor in cats, yet it is typically diagnosed in older animals and rarely metastasizes. Among its histological variants, the fibroblastic subtype-defined by spindle-shaped osteogenic cells that generate osteoid-is infrequently reported in young cats. CASE PRESENTATION: A one-year-old intact female domestic shorthair (DSH) cat was referred to the Veterinary Teaching Hospital of Ferdowsi University of Mashhad with a one-month history of progressive lameness and a firm swelling on the right hindlimb. Radiographs revealed an aggressive osteo

The Syndesmosis Procedure Correction of Hallux Valgus Feet Associated With the Metatarsus Adductus Deformity.

Metatarsus adductus and hallux valgus are common foot deformities. Corrective surgery of hallux valgus feet with metatarsus adductus deformity can be challenging and experience a high deformity recurrence rate. The purpose of this study was to demonstrate if the syndesmosis procedure can correct such feet satisfactorily without osteotomies and arthrodesis. 75 hallux valgus feet in 45 patients with a Sgarlato's metatarsal adductus angle ≥15° were studied after having undergone the syndesmosis procedure for an average of 20.22 months. Their average preoperative intermetatarsal angle of 12.56° wa

The influence of hallux valgus and flatfoot deformity on metatarsus primus elevatus: A radiographic study.

BACKGROUND: Metatarsus primus elevatus (MPE), a dorsal elevation of the first metatarsal in relation to the lesser metatarsals on lateral-view radiographs, is an indicator of hallux rigidus. The angle between the articular surfaces of the base of the first metatarsal and the anterior part of the medial cuneiform (M1C1A) reflects the sagittal instability of the first tarsometatarsal (TMT) joint. MPE may also indicate instability of the first metatarsal. The purpose of this study was to identify the influence of hallux valgus (HV) and flatfoot (FF) deformities on measurements obtained from first

A bizarre parosteal osteochondromatous proliferation (Nora's lesion) of metatarsus, a histopathological and etiological puzzlement.

Nora's lesion (bizarre parosteal osteochondromatous proliferation) is a rare, benign lesion that is composed of differing amounts of cartilage, bone, and spindle cells and an unusual form of calcified cartilage so-called "blue bone". Whether it represents a reactive proliferation or a neoplastic lesion still remains controversial. We present a 24-year-old woman having a 4.5cm two lobulated mass attached to second metatarsus of her left foot. The lesion was hyperintense on magnetic resonance imaging and accompanied by an extensive soft tissue and bone marrow edema. In resection specimen, smalle
