Vitamin B12 (s-cobalamin) and methylmalonic acid (s-MMA) are often interpreted together to diagnose cobalamin deficiency, in context with patient symptomatology, which is many cases is unspecific. Today, clinicians assess test results in relation to the univariate reference limits or decision limits. As s-MMA depends on renal function (glomerular filtration rate, GFR), interpretation can be complicated. To ease the interpretation of the two measurements, we propose three new tools: First, we developed a new formula for adjusting s-MMA to an eGFR of 100 mL/min/1.73 m2 (s-MMA100). The formula wa

Cobalamin deficiency is relatively common, but the great majority of cases in epidemiologic surveys have subclinical cobalamin deficiency (SCCD), not classical clinical deficiency. Because SCCD has no known clinical expression, its diagnosis depends solely on biochemical biomarkers, whose optimal application becomes crucial yet remains unsettled. This review critically examines the current diagnostic concepts, tools, and interpretations. Their exploration begins with understanding that SCCD differs from clinical deficiency not just in degree of deficiency but in fundamental pathophysiology, ca

OBJECTIVE: Cobalamin (B(12)) deficiency has been reported in infants born to mothers with low cobalamin intake. Early diagnosis of vitamin B(12) deficiency in infants is critical for the prevention of neurobehavioral disorders. We investigated the relationship between serum vitamin B(12) level in newborns and in their healthy mothers who consumed an omnivorous diet. Anthropometry was studied longitudinally to assess the growth velocity of the infants. Urinary methylmalonic acid (MMA) excretion of 6-month old infants was compared retrospectively as the biomarker correlated with the initial seru

BACKGROUND: An analysis of data from the National Health and Nutrition Examination Survey indicated that in older adults exposed to folic acid fortification, the combination of low serum vitamin B-12 and elevated folate is associated with higher concentrations of homocysteine and methylmalonic acid and higher odds ratios for cognitive impairment and anemia than the combination of low vitamin B-12 and nonelevated folate. These findings await confirmation in other populations.
OBJECTIVE: The purpose was to compare metabolic indicators of vitamin B-12 status, cognitive function, and depressive s

Diabetic kidney disease (DKD) is a prevalent complication of diabetes. Methylmalonic acid (MMA) reflects mitochondrial dysfunction. Oxidative balance score (OBS) assesses the pro-oxidant/anti-oxidant propensity of lifestyle. This study explored the associations of OBS and MMA with the risk of DKD. In this cross-sectional study, diabetes patients aged ≥ 20 years were identified from National Health and Nutrition Examination Survey (NHANES) 2011-2014. Dialysis patients, those without serum and urine creatinine/albumin/education/serum vitamin B12/MMA/OBS data were excluded. The weighted logistic

OBJECTIVE: Serum methylmalonic acid (MMA) arises from impaired metabolism of methylmalonyl-CoA and is exacerbated in chronic kidney disease (CKD) due to reduced renal clearance and nutritional deficiencies. The quality of diet may influence MMA levels by affecting vitamin B12 intake and mitigating inflammatory dietary components. This study aims to explore the relationship between the Healthy Eating Index-2015 (HEI-2015) and the prevalence of high MMA among low poverty/income ratio (PIR) patients with CKD.
METHOD: We conducted a cross-sectional study of participants aged ≥20 years using the d

AIMS: Metformin, the first-line treatment for Type 2 diabetes mellitus (T2DM), may be neuroprotective but also lowers serum vitamin B12 by ~19%, raising concern that B12 depletion could offset this benefit. We used modern exploratory causal mediation methods in a nationally representative sample to test whether vitamin B12 and methylmalonic acid (MMA) mediate the metformin-cognition association.
MATERIALS AND METHODS: We analysed NHANES 2011-2014 data from 587 adults aged ≥ 60 years with T2DM (408 metformin users, 179 users of other non-insulin antidiabetics), using an active-comparator desig

BACKGROUND: Diagnosing low vitamin B-12 status is important to reduce megaloblastic anemia and neurological disorders. The lack of a highly specific and sensitive clinical marker and questions about appropriate cutoffs hamper early detection.
OBJECTIVES: We evaluated the predictive power of 3 conventional markers for inadequate vitamin B-12 status [total cobalamin (B-12), methylmalonic acid (MMA), and total homocysteine (tHcy)] using the combined vitamin B-12 indicator, 3cB-12 (≤-0.5), as a reference. Additionally, we derived optimum cutoffs for each conventional marker and compared the preva

Plasma vitamin B-12 is the most commonly used biomarker of vitamin B-12 status, but the predictive value for low vitamin B-12 status is poor. The urinary methylmalonic acid (uMMA) concentration has potential as a functional biomarker of vitamin B-12 status, but the response to supplemental vitamin B-12 is uncertain. A study was conducted to investigate the responsiveness of uMMA to supplemental vitamin B-12 in comparison with other biomarkers of vitamin B-12 status [plasma vitamin B-12, serum holotranscobalamin (holoTC), plasma MMA] in elderly people with moderately poor vitamin B-12 status. Propionic acid significantly inhibited 14CO2 production from [1-14C] palmitate at a concentration of 10 muM in control fibroblasts and 100 muM in methylmalonic fibroblasts. This inhibition was similar to that produced by 4-pentenoic acid. Methylmalonic acid also inhibited 14CO2 production from [1-14C] palmitate, but only at a concentration of 1 mM in control cells and 5 mM in methylmalonic cells. Propionic acid (5 mM) also inhibited ureagenesis in rat liver slices when ammonia was the substrate but not with aspartate and citrulline as substrates. Propionic acid had no direct effect on either c

Elderly stroke survivors are at risk of malnutrition and long-term cognitive impairment. Vitamin B-related metabolites, folate and methylmalonic acid, have been implicated in cognitive function. We conducted a study exploring the relationship between blood folate, methylmalonic acid and post-stroke cognitive impairment. This is a cross sectional study of elderly Swedish patients (n=149) 20 months post-stroke, assessed using the Mini Mental State Examination, serum blood levels of methylmalonic acid and red blood cell levels of folate. Linear modeling indicated that low levels of blood folate a

Methylmalonic aciduria is a rare disorder caused by an inborn error of organic acid metabolism. Current treatment options are limited and generally focus on disease management. We aimed to investigate the use of fetal progenitor cells to treat this disorder using a mouse model with an intermediate form of methylmalonic aciduria. Fetal liver cells were isolated from healthy fetuses at embryonic day 15-17 and intravenously transplanted into sub-lethally irradiated mice. Liver donor cell engraftment was determined by PCR. Disease correction was monitored by urine and blood methylmalonic acid conc

Two siblings with atypical methylmalonic aciduria and progressive encephalopathy are reported. Initial symptoms were failure to thrive and growth retardation from the first year of life, progressing to severe mental retardation, microcephaly, dystonia, spasticity and cataracts. The amount of methylmalonic acid excreted in the urine was substantially lower than in classical methylmalonic acidemia and was not reduced by vitamin B12 therapy. The activity of methylmalonyl-CoA mutase and the overall assay of propionic acid metabolism in cultured fibroblasts were normal. The primary defect in this p

Methylmalonic acidemia is a rare autosomal recessive metabolic disease. However, because of the atypical clinical symptoms, the type of late-onset methylmalonic academia is often misdiagnosed. Especially when the blood vitamin B12 and folic acid levels are normal, it is not easy to think of this disease. Herein we report a 9-year-old girl who developed normally on a relatively balanced diet before 7 years of age. However, she presented with fatigue and attention deficit when she followed a vegetarian diet. Laboratory examination showed moderate macrocytic anemia, high levels of homocysteine, h

The effects of branched-chain amino acid metabolites on granulocyte-macrophage progenitor cell proliferation in marrow culture are reported. Isovalerate and propionate profoundly suppress granulopoiesis at both 3.2 and 6.4 mM concentrations, whereas methylmalonate and other metabolites suppress to a lesser degree. The parent branched-chain amino acids leucine, isoleucine, and valine do not suppress in vitro granulopoiesis at similar concentrations. Because the concentrations of the organic acids tested fall within the pathophysiologic ranges observed in patients with isovaleric, propionic, and
