Interpreting tissue stiffening with lung tumorigenesis by imaging architectural resembling of extracellular matrix components.

Lung cancer leads to a series of physiological abnormalities. The remodeling of extracellular matrix (especially elastin and collagen fibers) has been drawing increasing attention as it is suggested to be a hallmark of tumorigenesis. However, the interaction between these crucial matrix components, together with their relationship to mechanical changes, remains poorly understood. Here, we develop a quantitative multiphoton microscopy system to elucidate the relationship between tissue stiffening and elastin-collagen interplay in lung cancer. Based on label-free images of both fibers, we establ

MEMS-based near-infrared spectroscopy with AI for real-time breast cancer margin assessment.

BACKGROUND: Achieving clear surgical margins is essential for successful breast-conserving surgery and reducing re-excision rates. Conventional intraoperative approaches such as Frozen Section Analysis and Imprint Cytology are accurate but time-consuming and resource-dependent, underscoring the need for rapid, label-free, and objective diagnostic alternatives. OBJECTIVE: This study presents a clinically oriented framework that integrates diffuse reflectance near-infrared (DR-NIR) spectroscopy with machine-learning (ML) and deep-learning (DL) algorithms for real-time breast-cancer diagnosis an

Concurrent validity of instrumented insoles measuring gait and balance metrics in Parkinson's disease.

Several commercially available instrumented insole systems have been examined for validity and repeatability, but very few studies have focused on validation of the Moticon OpenGo sensor insoles in measuring gait and balance parameters in a clinical population. Given the paucity of studies examining the validity of these novel technologies in PD, there were two main goals of this research: (1) assess the concurrent validity of the Moticon OpenGo sensor insoles for gait and balance assessment in people with PD using a pressure-sensitive electronic walkway (Protokinetics Zeno™ walkway) as a refe

Atypia of undetermined significance and ThyroSeq v3-positive call rates as quality control metrics for cytology laboratory performance.

BACKGROUND: The Bethesda System for Reporting Thyroid Cytopathology (TBSRTC) recommends an upper limit of 10% for atypia of undetermined significance (AUS). Recent data suggest that this category might be overused when the rate of cases with molecular positive results is low. As a quality metric, the AUS and positive call rates for this facility's cytology laboratory and each cytopathologist (CP) were calculated. METHODS: A retrospective analysis of all thyroid cytology cases in a 4.5-year period was performed. Cases were stratified by TBSRTC, and molecular testing results were collected for

Performance comparison of quantitative metrics for analysis of in vivo Cherenkov imaging incident detection during radiotherapy.

OBJECTIVES: Examine the responses of multiple image similarity metrics to detect patient positioning errors in radiotherapy observed through Cherenkov imaging, which may be used to optimize automated incident detection. METHODS: An anthropomorphic phantom mimicking patient vasculature, a biological marker seen in Cherenkov images, was simulated for a breast radiotherapy treatment. The phantom was systematically shifted in each translational direction, and Cherenkov images were captured during treatment delivery at each step. The responses of mutual information (MI) and the γ passing rate (%GP

Quantitative dynamic wearable motion-based metric compared to patient-reported outcomes as indicators of functional recovery after lumbar fusion surgery.

BACKGROUND: Low back pain is a debilitating condition with poor patient outcomes despite the use of a wide variety of diagnostic and treatment modalities. A lack of objective metrics to support clinical decision-making may be a reason for these poor outcomes. This study aimed to compare patient recovery following lumbar fusion surgery using an objective motion-based metric (functional performance) and subjective patient-reported outcomes for pain, disability and kinesophobia. METHODS: A prospective observational study was conducted on 121 patients that received a lumbar fusion surgery. A wear

Fiber-specific white matter reductions in Parkinson hallucinations and visual dysfunction.

OBJECTIVE: To investigate the microstructural and macrostructural white matter changes that accompany visual hallucinations and low visual performance in Parkinson disease, a risk factor for Parkinson dementia. METHODS: We performed fixel-based analysis, a novel technique that provides metrics of specific fiber-bundle populations within a voxel (or fixel). Diffusion MRI data were acquired from patients with Parkinson disease (n = 105, of whom 34 were low visual performers and 19 were hallucinators) and age-matched controls (n = 35). We used whole-brain fixel-based analysis to compare microstr

System Design for Optimizing Drug Infusions Using Cardiovascular Space Mapping for Acute Heart Failure.

Acute heart failure is caused by various factors and requires multiple drug therapies to remedy underlying causes. Due to the complexity of pharmacologic effects of cardiovascular agents, few studies have theoretically addressed the multidrug optimization problem. This paper proposes a drug infusion system for acute heart failure that controls cardiovascular performance metrics (cardiac output, left atrial pressure, and mean arterial pressure) within desired ranges as dictated by the cardiovascular parameters (systemic vascular resistance, cardiac contractility, heart rate, and stressed blood

A New Fractional Particle Swarm Optimization with Entropy Diversity Based Velocity for Reactive Power Planning.

Optimal Reactive Power Dispatch (ORPD) is the vital concern of network operators in the planning and management of electrical systems to reduce the real and reactive losses of the transmission and distribution system in order to augment the overall efficiency of the electrical network. The principle objective of the ORPD problem is to explore the best setting of decision variables such as rating of the shunt capacitors, output voltage of the generators and tap setting of the transformers in order to diminish the line loss, and improve the voltage profile index (VPI) and operating cost minimiza

Biomedical Publishing Metrics as a Barrier to Knowledge Dissemination.

The Journal Impact Factor (JIF) is an outdated biomedical metric that hinders knowledge dissemination. Relying on citation counts creates a skewed, easily manipulated system with a temporal lag that ignores global equity. Frontline clinicians consume research to improve care but rarely generate citations, meaning the JIF cannot capture real-world clinical utility. To ensure rankings reflect true value, publishers must adopt a multi-dimensional matrix prioritizing usage-based Altmetrics, such as verified downloads and views, to instantly measure global clinical application.

Evaluation of Activated Clotting Time Response to Unfractionated Heparin in Vascular Surgery: An Observational Study.

OBJECTIVES: Activated clotting time (ACT) is widely used to monitor intraoperative anticoagulation with unfractionated heparin (UFH) during vascular surgery, but the most informative ACT-derived metric and the optimal approach for weight-based UFH dosing remain uncertain. The recent introduction of new point-of-care ACT analyzers has raised questions regarding the consistency of ACT response. This study aimed to assess the relationship between UFH dose and ACT response and to identify the most informative ACT-derived metric and the most appropriate body weight metric for dose normalization. Quality measurement in the shunt treatment of hydrocephalus: analysis and risk adjustment of the Revision Quotient.

UNLABELLED: OBJECT.: The Revision Quotient (RQ) has been defined as the ratio of the number of CSF shunt revisions to the number of new shunt insertions for a particular neurosurgical practice in a unit of time. The RQ has been proposed as a quality measure in the treatment of childhood hydrocephalus. The authors examined the construct validity of the RQ and explored the feasibility of risk stratification under this metric. METHODS: The Kids' Inpatient Database for 1997, 2000, 2003, 2006, and 2009 was queried for admissions with diagnostic codes for hydrocephalus and procedural codes for CSF

The intraoperative Surgical Apgar Score predicts postdischarge complications after colon and rectal resection.

BACKGROUND: We previously developed an intraoperative 10-point Surgical Apgar Score-based on blood loss, lowest heart rate, and lowest mean arterial pressure-to predict major complications after colorectal resection. However, because complications often arise after uncomplicated hospitalizations, we sought to evaluate whether this intraoperative metric would predict postdischarge complications after colectomy. METHODS: We linked our institution's National Surgical Quality Improvement Program database with an Anesthesia Intraoperative Management System for all colorectal resections over 4 year
