Advances in ultrasound-targeted microbubble-mediated gene therapy for liver fibrosis.

Hepatic fibrosis develops as a wound-healing scar in response to acute and chronic liver inflammation and can lead to cirrhosis in patients with chronic hepatitis B and C. The condition arises due to increased synthesis and reduced degradation of extracellular matrix (ECM) and is a common pathological sequela of chronic liver disease. Excessive deposition of ECM in the liver causes liver dysfunction, ascites, and eventually upper gastrointestinal bleeding as well as a series of complications. However, fibrosis can be reversed before developing into cirrhosis and has thus been the subject of ex

Demonstration of intrahepatic accumulated microbubble on ultrasound represents the grade of hepatic fibrosis.

OBJECTIVES: To examine the feasibility of perflubutane-based ultrasound for grading hepatic fibrosis. METHODS: This prospective study included 202 subjects; main study (controls:33, F0-1:35, F2:26, F3:23, cirrhosis:29) and subsequent study (controls:16, F0-1:7, F2:20, F3:7, cirrhosis:6). Diagnostic abilities for assessing fibrosis grade were compared between contrast findings and FIB4 (age × AST/[platelet count × ALT(0.5)]). RESULTS: High-power emission produced an intrahepatic band-like structure, and the three-layer appearance was less frequent and monolayer appearance was more frequent in

Radiofrequency ablation of liver tumors: the role of microbubble ultrasound contrast agents.

Radiofrequency ablation (RFA) is currently indicated for the treatment of primary and metastatic hepatic malignancies. Real-time ultrasound (US) is generally used during the procedure to guide electrode placement, but for evaluating the results of treatment, contrast-enhanced computed tomography and magnetic resonance imaging have traditionally been considered more effective. This view has changed, however, with the recent development of contrast-enhanced ultrasound (CEUS) (eg, using sulfur hexafluoride microbubbles), which can provide valuable information on the effects of RFA more rapidly an

Stable microbubble test on tracheal aspirate for the diagnosis of respiratory distress syndrome.

BACKGROUND: Exogenous surfactant should be used as early as possible in the presence of respiratory distress syndrome (RDS), but diagnosis may only become clear late in the course of the disease. The stable microbubble test (SMT) in the tracheal aspirates could help in the decision to give early surfactant to preterm babies with respiratory distress. OBJECTIVES: The objective of this study was to evaluate the accuracy of the SMT on tracheal aspirate for the diagnosis of RDS in newborns requiring mechanical ventilation. METHODS: The test was performed on specimens obtained from 74 infants req

Microbubble Track-Based Functional Ultrasound Localization Microscopy in Awake Mice.

Functional neuroimaging with ultrafast ultrasound is an emerging neuroimaging tool for studying neural activities in the rodent brain. Existing methods, however, are challenged by the compromise between functional imaging sensitivity (i.e., sensitivity in detecting neural responses) and spatial resolution. For example, functional ultrasound (fUS) uses native red blood cells (RBCs) as imaging targets, which offers high functional imaging sensitivity but limited spatial resolution that is confined by the diffraction limit of ultrasound. On the other hand, functional ultrasound localization micro

Microbubble-enhanced ultrasound to demonstrate urethral transection in a case of penile fracture.

A 47-year-old man attended the emergency department following trauma during sexual intercourse after which he developed penile swelling and haematuria several hours later. A penile fracture was suspected but given the slightly atypical history, ultrasound was performed to look for a fracture. Given the history of haematuria, both a standard Doppler ultrasound and a microbubble-enhanced retrograde ultrasound urethrogram were performed. The Doppler confirmed the suspected diagnosis of penile fracture, and microbubble urethrogram demonstrated a urethral injury. This facilitated prompt surgical tr

Improvement of Tumor Hypoperfusion and Hypoxia via Low-intensity Ultrasound-stimulated Microbubbles Combined with Alprostadil.

RATIONALE AND OBJECTIVES: Hypoperfusion and related hypoxia are critical factors that contribute to therapeutic resistance in solid tumors. Ultrasound-stimulated microbubble (USMB) has been approved to enhance tumor perfusion, albeit with limited efficacy. This study was aimed to investigate whether combining USMB with alprostadil, a vasodilatory agent, could further improve tumor perfusion and alleviate hypoxia, thereby enhancing drug delivery. MATERIALS AND METHODS: Sixty-nine rabbits bearing VX2 tumors were included in this study. USMB treatment was conducted using a modified diagnostic ul

Enhancing subharmonic response by controlling initial state of monodisperse microbubbles via a multi-gas core.

The subharmonic response of microbubbles exposed to ultrasound has shown significant promise for nonlinear imaging and non-invasive blood pressure estimation. Elucidating the underlying mechanisms of subharmonic response of microbubbles is critical for advancing subharmonic-based ultrasound applications. Using a flow-focusing microfluidic device, we controllably generated monodisperse microbubbles (MDMBs) with lipid-packed shells and varying CO2 volume fractions (0%, 50%, and 80%) in multi-gas (CO2 and C3F8) cores, while maintaining consistent initial surface tension and shell viscoelasticity

Ultrasound-responsive oxygen-loaded microbubbles induce vascular normalization and immune modulation in pancreatic ductal adenocarcinoma.

Vascular normalization (VN) has been proposed as a strategy to restore tumor vascular function and promote antitumor immunity under anti-angiogenic conditions. However, conventional anti-angiogenic agents often cause excessive vessel pruning, which exacerbates subsequent tumor hypoxia and thereby limits the effectiveness of immunotherapy. This study evaluated whether oxygen-loaded microbubbles (OMBs) combined with focused ultrasound could induce tumor VN and activate antitumor immunity in a "cold" tumor model. Tumor perfusion and intratumoral oxygen tension were monitored following OMB treatme

Suspended Microbubbles With Ultrasound Visualization and Sustained Drug Release Properties for Enhancing Intraperitoneal Perfusion Therapy.

Peritoneal carcinomatosis (PC), a multifocal diffuse lesion from peritoneal metastasis, is commonly treated with intraperitoneal perfusion therapy. This treatment infuses drugs directly into the peritoneal cavity to enable direct contact with and killing of tumor cells. However, its efficacy is limited by rapid drug metabolism and short effective duration. To address this limitation, researchers have explored various drug delivery systems but the limited efficacy of local scaffold treatments has hindered tumor eradication. In response, this study developed a drug delivery system based on suspe

Comparative effects of focused ultrasound and microbubbles on healthy and tumor bearing rat spinal cord.

This study examined the differential sensitivity of intramedullary spinal cord tumors (IMSCTs) and healthy tissue to focused ultrasound (FUS) and microbubbles in a rat model of IMSCT. F98 glioma cells were injected into spinal cords of F344 rats. FUS (580 kHz, 10 ms bursts, 1 Hz pulse repetition frequency, 40 s) was delivered to tumor and adjacent healthy tissue at varying pressures (0-1.2 MPa) following intravenous injection of microbubbles (1.00 ± 0.85 µm; 2.4 × 107 microbubbles/100 g). Tissues were collected 24 h post-treatment for histological analysis. Healthy tissue exhibited pressure-de

Ultrasound-Mediated Biomimetic Microbubbles Effectively Reverse LSECs Capillarization and Exert Antiplatelet Therapy in Liver Fibrosis.

Liver fibrosis, characterized by excessive tissue remodeling as a response to chronic liver injury, is accompanied by capillarization of liver sinusoidal endothelial cells (LSECs) and activated hepatic stellate cells (HSCs). Simvastatin (Sim) can modulate endothelial function by increasing endothelial nitric oxide synthase (eNOS)-dependent nitric oxide (NO) release, thereby reversing capillarization and attenuating liver fibrosis. However, monotherapy often demonstrates limited therapeutic effectiveness given the complex pathophysiology of liver fibrosis. Herein, a type of multifunctional lipo

Tumor perfusion enhancement by microbubbles ultrasonic cavitation reduces tumor glycolysis metabolism and alleviate tumor acidosis.

The tumor microenvironment is increasingly acknowledged as a critical contributor to cancer progression, mediating genetic and epigenetic alterations. Beyond diverse cellular interactions from the microenvironment, physicochemical factors such as tumor acidosis also significantly affect cancer dynamics. Recent research has highlighted that tumor acidosis facilitates invasion, immune escape, metastasis, and resistance to therapies. Thus, noninvasive measurement of tumor acidity and the development of targeted interventions represent promising strategies in oncology. Techniques like contrast-enh

Systemically injected oxygen within rapidly dissolving microbubbles improves the outcomes of severe hypoxaemia in swine.

Acute respiratory failure can cause profound hypoxaemia that leads to organ injury or death within minutes. When conventional interventions are ineffective, the intravenous administration of oxygen can rescue patients from severe hypoxaemia, but at the risk of microvascular obstruction and of toxicity of the carrier material. Here we describe polymeric microbubbles as carriers of high volumes of oxygen (350-500 ml of oxygen per litre of foam) that are stable in storage yet quickly dissolve following intravenous injection, reverting to their soluble and excretable molecular constituents. In swi

Ultrasound-mediated multifunctional magnetic microbubbles for drug delivery of celastrol in VX2 liver transplant tumors.

Celastrol (CST) has positive pharmacological effects on various cancers, but clinical application is limited because of poor water solubility and systemic toxicity. Ferric oxide (Fe3O4) has a large specific surface area and can be functionalized by inorganic modification to form complex magnetic drug delivery systems. Herein, Fe3O4 was surface-modified with citric acid and polyethylene glycol (PEG) (via) the Mitsunobu reaction and then covalently bound to CST. Finally, magnetic microbubbles (MMBs) containing perfluoropropane (C3F8) and Fe3O4-PEG2K-CST particles were constructed with poly(lacti

Effect of lipid shell composition in DSPG-based microbubbles on blood flow imaging with ultrasonography.

Diagnostic ultrasound is non-invasive and provides real-time imaging. Microbubbles (MBs) are ultrasound contrast agents used to observe small blood flow, such as tumor tissue. However, MBs have short blood flow imaging time. This study developed lipid-based microbubbles (LMBs) with longer blood flow imaging time by focusing on their shell composition. Liposome research reported that addition 1,2-distearoyl-sn-glycero-3-phosphoglycerol (DSPG) to the lipid composition enhances liposome membrane stability. Therefore, we introduced DSPG at different ratios into the LMBs lipid shell. Results showed

Potential stagnation in the splanchnic hemodynamics demonstrated by the dynamic microbubbles in chronic liver disease.

BACKGROUND AND AIM: Impaired splanchnic hemodynamics are well-documented phenomena in cirrhosis. However, comprehensive hemodynamic features from the superior mesenteric artery (SMA) to the superior mesenteric vein (SMV) via intestinal capillaries have not been studied. The aim was to examine splanchnic hemodynamics and their relationship with clinical presentations. METHODS: Contrast-enhanced ultrasound was performed for both the SMA and SMV under fasting conditions and postprandially following ingestion of a liquid diet. The microbubble traveling time (MTT) was determined as the difference

Ultrasound and microbubbles: their generation, detection and potential utilization in tissue and organ therapy--experimental.

Ultrasound-induced cavitation in tissue and organs has been well recognized and documented. Generally, this phenomenon has been seen as something to be avoided except in cases such as lithotripsy, where its production is considered an essential part of the treatment process or as a desirable contrast media in some areas of visualization enhancement. This article covers three areas in which the phenomenon has been observed, and shows how the effect can or may be therapeutically beneficial. Studies in the pig show that implanted human gallstones and the gallbladder itself can be eliminated in a

Hepatic focal nodular hyperplasia: specific findings at dynamic contrast-enhanced US with carbon dioxide microbubbles.

Dynamic contrast material-enhanced ultrasonography (US) with intraarterial infusion of carbon dioxide microbubbles was performed for four cases of histologically proved focal nodular hyperplasia (FNH) in four patients and for 167 cases of various hepatic nodules in 144 patients. No complications due to dynamic US were observed in any of the 148 patients. All FNH nodules were less than 3 cm in diameter. Consistent specific findings of FNH were not obtained with US, computed tomography, magnetic resonance imaging, radiocolloid scanning, or angiography in the four cases of FNH. In contrast, the c

Effects of Pressure Cycling, Concentration, and Excitation Amplitude on Ambient Pressure Sensitivity of Perfluorobutane Microbubbles.

OBJECTIVE: The high ambient pressure sensitivity of the subharmonic response from contrast microbubbles makes it a promising means for noninvasive diagnosis of vascular pressures, i.e., subharmonic aided pressure estimation (SHAPE). Here, we investigated the effects of concentration and multiple pressure cycles on the subharmonic response of Sonazoid, a commercially available microbubble with a perfluorobutane gas core. METHODS: We performed a pressure sensitivity study at 3 MHz excitation frequency, 100-700 kPa peak negative pressures (PNP), and ambient overpressures of 10-20 kPa (75-150 mm
