Assessing Surgical Capacity in Guam: Current Strengths and Future Goals.

INTRODUCTION: Guam, the largest U.S. territory in Micronesia, plays a central role in surgical care for the local indigenous community and surrounding Pacific Island nations, yet remains underrepresented in surgical systems research. METHODS: We conducted a mixed-methods study. Quantitative data were collected on operating volume, personnel, infrastructure, and surgical services at Guam Memorial Hospital (GMH) and Guam Regional Medical City (GRMC). Semi-structured interviews with hospital leadership and surgical providers captured qualitative insights on strengths, challenges, and future plan

The mysterious Zika virus: Adding to the tropical flavivirus mayhem.

Until now, known as the demure cousin of dengue virus (DENV) inhabiting Africa, Zika virus (ZIKV) has reinvented itself to cause explosive epidemics captivating the Western hemisphere. The outbreak causing potential for ZIKV was realized when it made its way from Africa to Yap Island Micronesia in 2007, and in French Polynesia in 2013. From there, it moved on to Brazil in 2015. Now ZIKV has infected people in more than 33 countries in Central and South America and the Caribbean. Moreover the epidemiological and subsequent virological association with microcephaly cases in Brazil has prompted t

Diagnosis and evaluation of causative factors for the presence of endemic treponemal disease in a Japanese sub-tropical island population from the Tokugawa period.

We present a description and differential diagnosis of pathological lesions observed on skeletal elements found during surface surveys of the Nagabaka site on Miyako-jima Island, Japan. The Nagabaka site served as a bone depository during the Early Modern period (c. AD 1600-1870). We evaluated remains via macroscopic inspection to classify infectious lesions according to criteria in Weston (2008). We also obtained CT scans of three bone elements for more extensive lesion analysis and carried out a differential diagnosis utilizing paleopathological literature. Subsequent investigation yielded c

The Ethics of Discharging Asylum Seekers to Harm: A Case From Australia.

In February 2016 a twelve-month-old asylum seeker, who came to be know as Baby Asha, was transferred from Nauru and hospitalized in Brisbane. This case came to public attention after Doctors refused to discharge Asha as she would have been returned to detention on Nauru. What in other circumstances would have been considered routine clinical care, quickly turned into an act of civil disobedience. This paper will discuss the ethical aspects of this case, along with its implications for clinicians and the broader healthcare community.

An assessment of recycled spectacles donated to a developing country.

BACKGROUND: A rapid assessment methodology was used to determine the suitability of donated recycled spectacles for the Pacific nation of Tuvalu. METHODS: Spectacles (102) from a donated cache were examined. Those with astigmatism of > 1.00 D, anisometropia of > 0.50 D, significant central lens scratches, broken frame, poor cosmetic fit and comfort, or unsuitable frame appearance were excluded. The refractive errors (> or = +/-0.50 D distance or near) of 320 Tuvaluans were determined. RESULTS: Of the spectacles, 62.7% had > 1.00 DC and/or > 0.50 D anisometropia, 30% were broken and/or scratc

Successful treatment of ciguatera fish poisoning with intravenous mannitol.

Twenty-four patients with acute ciguatera fish poisoning were treated with intravenous mannitol, and each patient's condition improved dramatically. All exhibited marked lessening of neurologic and muscular dysfunction within minutes of the administration of mannitol. Gastrointestinal symptoms disappeared more slowly. Two patients in coma and one in shock responded within minutes, with full recovery after infusion. Although these observations were empiric and uncontrolled and the mechanism of action of mannitol in this disease is unclear, mannitol should be considered for initial use in patien

Zika Virus: the Latest Newcomer.

Since the beginning of this century, humanity has been facing a new emerging, or re-emerging, virus threat almost every year: West Nile, Influenza A, avian flu, dengue, Chikungunya, SARS, MERS, Ebola, and now Zika, the latest newcomer. Zika virus (ZIKV), a flavivirus transmitted by Aedes mosquitoes, was identified in 1947 in a sentinel monkey in Uganda, and later on in humans in Nigeria. The virus was mainly confined to the African continent until it was detected in south-east Asia the 1980's, then in the Micronesia in 2007 and, more recently in the Americas in 2014, where it has displayed an

Twelve HLA-DR6-related DRB1 alleles and associated DR, DQ haplotypes in traditional Australians and other populations of Asia-Oceania.

The relative distributions of 12 HLA-DR6-related HLA-DRB1 alleles in indigenous populations of Australia, Melanesia, Polynesia, Micronesia, and northern and southern China have been determined by analysis of nucleotide sequence polymorphisms in 364 examples of HLA-DR6 positive chromosomes. Oligonucleotide hybridizations of polymerase chain reaction products of HLA-DQA1, DQB1, DRB1 and DRB3 genes generated 24 HLA-DR6-related haplotypes. The study aimed to determine the regional distribution of DR,DQ haplotypes associated with three novel HLA-DR6 alleles, namely DRB1*1408, 1409, and 1410, known

Epidermolysis Bullosa Acquisita: A Case Report.

BACKGROUND Epidermolysis bullosa acquisita is a rare, subepithelial bullous disorder, which is distinguished from other autoimmune blistering diseases by the production of antibodies against type VII collagen. CASE REPORT Here, we describe the case of a 79-year-old male resident of the Northern Mariana Islands who presented to the clinic with multiple blistering skin lesions. CONCLUSIONS The primary focus of treatment is to prevent disease progression and serious complications of scarring (including blindness and respiratory obstruction) by avoiding physical trauma and suppressing the immune s

Transuranics in bone of deceased former residents of Rongelap Atoll, Marshall Islands.

Rongelap Atoll received intensive fallout from the 1 March 1954 Bravo thermonuclear test 105 miles upwind at Bikini. Fearful of their continued exposure to radiation, the residents of Rongelap Atoll went into voluntary exile in 1985. Transuranic soil concentrations on Rongelap Island are about 2-3 orders of magnitude greater than the average for the Northern hemisphere; the three dominating transuranics are 239,240Pu and 241Am. Only conflicting information has been available about the extent of transuranic uptake by the Rongelap community. As part of the Rongelap Resettlement Project, the comm

Seroepidemiological characterization of a syphilis epidemic in the Republic of the Marshall Islands, formerly a yaws endemic area.

The annual numbers of reported cases of syphilis in the Republic of the Marshall Islands (RMI) increased from none in 1983 to more than 600 in 1989, suggesting a large outbreak of syphilis. Much of the increase resulted from expanded serological screening. The apparent outbreak of syphilis, therefore, may have been partly the result of increased surveillance or, since the RMI was formerly a yaws endemic area, possibly due to a resurgence of yaws. To address this problem and better characterize the epidemic, we analysed results from a 1989/90 Ministry of Health Services mass serological screeni

Shigellosis in the Marshall Islands: epidemiologic aspects of an outbreak.

An outbreak of diarrheal illness caused by Shigella flexneri 1b and associated with 11 deaths occurred in the Marshall Islands during June and July 1977. A house-to-house survey on Majuro indicated an attack rate of 6.2%. Neither the survey nor a case-control study uncovered a common source of exposure on Majuro, and it is believed that transmission was mainly person-to-person. Socioeconomic factors, type of water supply, distance to municipal water supply, and type of sanitary facility could not be related to the occurrence of illness, but a poor sanitary rating was associated with increased

Zika Virus Mosquito Vectors: Competence, Biology, and Vector Control.

Zika virus (ZIKV) (Flaviviridae, Flavivirus) has become one of the most medically important mosquito-borne viruses because of its ability to cause microcephaly in utero and Guillain-Barré syndrome in adults. This virus emerged from its sylvatic cycle in Africa to cause an outbreak in Yap, Federated States of Micronesia in 2007, French Polynesia in 2014, and most recently South America in 2015. The rapid expansion of ZIKV in the Americas largely has been due to the biology and behavior of its vector, Aedes aegypti. Other arboviruses transmitted by Ae. aegypti include the 2 flaviviruses dengue v

One year after the Zika virus outbreak in Brazil: from hypotheses to evidence.

Zika virusis an arbovirus of the Flaviviridae family with two major strains, an Asian and an African strain. The main vectors involved in the transmission of Zika virus are the Aedes aegypti and Aedes albopictus mosquitoes. Despite its identification, discovered in 1947 in the Zika forest in Uganda, only isolated and sporadic occurrences of human infection were reported within a largely asymptomatic proportion of individuals. The first reported outbreak occurred in 2007 in the Yap Island, which belongs to the Federated States of Micronesia in the Pacific Ocean, and in French Polynesia, where h

Carotenoid-rich bananas: a potential food source for alleviating vitamin A deficiency.

This review article points out that bananas are an important food for many people in the world. Thus, banana cultivars rich in provitamin A carotenoids may offer a potential food source for alleviating vitamin A deficiency, particularly in developing countries. Many factors are associated with the presently known food sources of vitamin A that limit their effectiveness in improving vitamin A status. Acceptable carotenoid-rich banana cultivars have been identified in Micronesia, and some carotenoid-rich bananas have been identified elsewhere. Bananas are an ideal food for young children and fam

Residents' Management of, and Attitudes Towards, the Dog Population on Two Mariana Islands.

This study reports the level of care dogs received from their caretakers and the attitudes of caretakers and non-caretakers toward dogs on the islands of Guam and Saipan. Surveys were collected from residents of Guam and Saipan (274 and 275, respectively) and analyzed using chi-squared tests. Dogs were the most popular companion animal (Guam 66.4%; Saipan 72% ownership) with a strong preference toward pedigree, male puppies (Guam 45.4%; Saipan 62.4%). The more preferred dogs were housed inside and fed dog food, while the less preferred local mixed-breed dogs were free-roaming and fed table scr

A clinical and cytological study of vitamin A deficiency in Kiribati.

Vitamin A status was assessed clinically and by conjunctival impression cytology in a selected group under seven years of age in the Republic of Kiribati. A total of 230 children were studied. Of 185 with readable histology, 14.6% were clinically and histologically abnormal; 19.5% were clinically abnormal but histologically normal; and 15.7% were clinically normal and histologically abnormal. The remainder (50.3%) were clinically and histologically normal. There was a significant correlation between clinical disease and histology (P < 0.001). The prevalence of the disease in males was 45.8% an

The effect of hyperoxic environment on lung function and myopisation in an extended closed-circuit rebreather diving expedition.

INTRODUCTION: Repetitive closed-circuit rebreather (CCR) dives subject the human body to a hyperoxic environment. Prolonged oxygen exposure can lead to lung abnormalities and myopic shift. The aim of this study was to analyse the effects of a 31-day period of repeated CCR dives on divers' lung function and visual acuity. METHODS: The CCR divers recruited for the study were participating in a research project in Chuuk Lagoon, Micronesia. Weekly measurements included lung function using spirometry and visual acuity with a Snellen chart. Oxygen exposure was calculated using multiple models. The

A new species of the feather star Capillaster within C. multiradiatus species complex (Crinoidea, Comatulidae) in Palau and the Great Barrier Reef.

The feather star Capillaster AH Clark, 1909 (Crinoidea, Comatulidae) is a common feather star on coral reefs of the entire Indo-West Pacific region ranging from the Red Sea in the west to reefs of the central Pacific. Extensive studies of crinoids from the 1970s to 2000s by SCUBA diving encountered Capillaster, identified as C. multiradiatus (L., 1758) on reefs in the Palau Islands, Micronesia, and around Lizard Island (northern Great Barrier Reef [GBR], central GBR reefs (Rib Reef, John Brewer Reef, Davies Reef), reefs at Heron Island (southern GBR), and other sites within the Coral Triangle
