PTSD and obesity in U.S. military veterans: Prevalence, health burden, and suicidality.

Post-traumatic stress disorder (PTSD) and obesity are prevalent among U.S. military veterans, though less is known about the mental and physical health burden and suicidality of co-occurring PTSD and obesity in this population. A nationally representative sample of the U.S. veterans was used to assess PTSD and obesity prevalence, co-occurrence and relationships with mental and physical health measures. A total of 16.4% of veterans screened positive for current PTSD, 32.7% for obesity, and 5.8% for co-occurring PTSD and obesity. Relative to obesity-only veterans, veterans with co-occurring PTSD

Behavioral Health Service Use by Military Children During Afghanistan and Iraq Wars.

Medical claims were analyzed from 2810 military children who visited a civilian emergency department (ED) or hospital from 2000 to 2014 with behavioral health as the primary diagnosis and TRICARE as the primary/secondary payer. Visit prevalence was estimated annually and categorized: 2000-2002 (pre-deployment), 2003-2008 (first post-deployment), 2009-2014 (second post-deployment). Age was categorized: preschoolers (0-4 years), school-aged (5-11 years), adolescents (12-17 years). During Afghanistan and Iraq wars, 2562 military children received 4607 behavioral health visits. Behavioral correlates of self-reported health status in US active duty military.

Trends and relationships between health behaviors and conditions in US active duty military and Coast Guard personnel are understudied. Self-reported data from the 2011 and 2015 Department of Defense Health Related Behaviors Surveys were analyzed to estimate associations between seven behaviors (reasons for not exercising; moderate, vigorous, and strength training exercise; alcohol intake; sleep; and smoking) and five health conditions (high blood pressure [BP], blood sugar [BG], cholesterol [CH]; overweight/obesity [OW]; or obesity [OB]). In 33,531 respondents, 14.8%, 1.7%, 13.6%, 65.5%, and

Military Sexual Trauma and Sexual Health in Women Veterans: A Systematic Review.

INTRODUCTION: Sexual trauma during military service is prevalent among women veterans and is associated with multiple negative physical and mental health sequelae. The high prevalence of military sexual trauma (MST), sexual harassment and assault during military service, has prompted the Veterans Health Administration to enact several policies to address the detrimental health impacts of this experience. MST also negatively impacts veterans' sexual health, yet the field lacks a systematic review of the relationship between MST and sexual health among women veterans. AIM: To systematically rev

Drivers of Variation in the Rate of Radiotherapy Following Lumpectomy in the Military Health System.

INTRODUCTION: Radiation therapy (RT) following lumpectomy has been shown to improve survival. However, little is known about drivers of RT within the Military Health System (MHS). We examined how factors previously shown to affect RT rates after lumpectomy in the civilian population and factors exclusive to service members such as rank, service branch, and MHS catchment area are associated with undergoing RT within 6 months after lumpectomy. MATERIALS AND METHODS: We conducted a retrospective cohort study using the 2007-2019 MHS Data Repository (MDR) to obtain data from 9681 service members o

The impact of military occupational combustion smoke inhalation on neuroinflammation and brain health.

Airborne combustion emissions from military burn pits, wildfires, and urban/industrial sources are increasingly recognized as a component of the neurotoxic exposome, with potential consequences extending beyond cardiopulmonary disease to brain health. These aerosols comprise heterogeneous mixtures of fine and ultrafine particulate matter (PM₂.₅/PM₀.₁), polycyclic aromatic hydrocarbons, volatile organic compounds, metals, and reactive gases whose composition varies with fuel type, combustion efficiency, and atmospheric aging. Evidence from experimental models, epidemiology, and exposed human co

A Review of Force Health Protection Aspects of Lyme Disease in the U.S. Military.

INTRODUCTION: Lyme disease (LD) is an underrated threat to the military that negatively impacts mission readiness. Lyme disease has traditionally been thought to only be a risk in an operational context, where training or deployments are frequently conducted in heavily wooded environments. However, this view diminishes risks posed by many off-duty outdoor recreational activities. Furthermore, although the Army introduced a permethrin factory-treated Army Combat Uniform in 2012, permethrin retention and subsequent protection have been shown to decrease significantly after 3 months of wear. Thus

Systematic Review of the Military Career Impact of Mental Health Evaluation and Treatment.

INTRODUCTION: Military leaders are concerned that active duty members' fear of career impact deters mental health (MH) treatment-seeking. To coalesce research on the actual and perceived consequences of MH treatment on service members' careers, this systematic review of literature on the U.S. Military since 2000 has been investigating the following three research questions: (1) is the manner in which U.S. active duty military members seek MH treatment associated with career-affecting recommendations from providers? (2) Does MH treatment-seeking in U.S. active duty military members impact milit

Deployment of personnel to military operations: impact on mental health and social functioning.

UNLABELLED: This Campbell systematic review examines the effects of deployment on mental health. The review summarizes evidence from 185 studies. All studies used observational data to quantify the effect of deployment. This review includes studies that evaluate the effects of deployment on mental health. A total of 185 studies were identified. However, only 40 of these were assessed to be of sufficient methodological quality to be included in the final analysis. The studies spanned the period from 1993 to 2017 and were mostly carried out in the USA, UK and Australia. The studies all had some

Unintended Pregnancy in the Military Health Care System: Who is Really at Risk?

Unintended pregnancy is a global issue and one that is reportedly to be higher in the military population. We sought to assess rates of unintended pregnancy among the active duty population in comparison to other military health care beneficiaries. Using a validated six-question survey, patients aged 18 to 42 were surveyed in five different clinics at three major tertiary hospitals from December 2013 to December 2014. Individual survey questions were scored 0, 1, or 2 and a total score was tabulated. A total score of 0 to 3 indicated unintended pregnancy, 4 to 9 indicated ambivalence toward pr

Military chemical warfare agent human subjects testing: part 2--long-term health effects among participants of U.S. military chemical warfare agent testing.

Military chemical warfare agent testing from World War I to 1975 produced thousands of veterans with concerns about how their participation affected their health. A companion article describes the history of these experiments, and how the lack of clinical data hampers evaluation of long-term health consequences. Conversely, much information is available about specific agents tested and their long-term health effects in other populations, which may be invaluable for helping clinicians respond effectively to the health care and other needs of affected veterans. The following review describes tes

Biomedical research on health and performance of military women: accomplishments of the Defense Women's Health Research Program (DWHRP).

In 1994, Congress provided dollar 40 M for biomedical research on issues of importance for military women. This supported 104 intramural and 30 extramural studies and launched an era of research to narrow the knowledge gap on protection and enhancement of health and performance of military women. Projects addressed issues specific to female physiology (e.g., gynecological health in the field, maternal malaria), problems with higher prevalence for women (e.g., marginal iron deficiency, stress fracture), and issues of drug and materiel safety that had only been extrapolated from studies of men (

Staying Relevant in the Digital Age: Exploring the Evolving Frontier of Telehealth for Mental Health in the Military Health System and Veterans Health Administration.

PURPOSE OF REVIEW: This review examines recent evidence for the effectiveness of telehealth in treating Post-Traumatic Stress Disorder (PTSD), Major Depressive Disorder (MDD), alcohol use disorder, and insomnia in military veterans and active-duty service members (ADSMs). RECENT FINDINGS: Recent randomized controlled trials and prospective cohort studies provide strong evidence that synchronous video-teleconference based therapy is effective for PTSD, MDD, and insomnia in this population. There is growing evidence for the effectiveness of internet-based self-guided therapy, particularly when

Expanding Case Management Surge Capabilities of the Military Health System: A Scoping Review.

INTRODUCTION: The Military Health System (MHS) provides essential case management services for ill and injured service members. Yet the MHS's capability to expand its current case management services in response to medical surge, as would be expected during a large-scale combat operation (LSCO), remains underexplored. MATERIALS AND METHODS: This study uses a scoping literature review to identify existing and required MHS case management capabilities necessary for such a surge event. Data were analyzed using the DOTmLPF-P framework (Doctrine, Organization, Training, Materiel, Leadership & Educ
