The Effect of Storage Conditions of Different Temperatures on the Stability of Nutrients in Pasteurized Donated Human Milk.

BACKGROUND: Donated human milk (DHM) is vital for nutrition of preterm and critically ill infants when the mother's own milk is unavailable. For safety reasons, DHM is routinely pasteurized and stored frozen. The impact of storage duration and temperature on the stability of its nutritional values has not been sufficiently investigated. This study aimed to evaluate the stability of macronutrients and energy content in pasteurized DHM stored at -30°C and -80°C over a 12-month period. METHODS: Pasteurized DHM samples from ten single-donor pools were divided into aliquots and stored at -30°C and

Quantification of illicit and prescription drug residues in pasteurized donor human milk: a comprehensive analytical approach.

OBJECTIVE: To assess the risk of infant exposure to illicit and prescription drug residues, from infant-ready, pooled, pasteurized donor human milk (PDHM) provided by healthy, screened, voluntary non-remunerated donors to a not-for-profit milk bank. STUDY DESIGN: A total of 150 pooled samples, representing 742 donor contributions, were analyzed for 15 illicit and prescription drug analytes using liquid chromatography-tandem mass spectrometry (LC-MS/MS). Detected concentrations were evaluated for clinical significance. RESULT: All 150 PDHM samples tested negative for residues of marijuana, am

Understanding modifiable barriers to human milk donation in the United Kingdom.

BACKGROUND: When premature infants cannot receive their own mother's milk, donor human milk (DHM) is the first-line recommended option, with growing demand for DHM use outside of neonatal units. To meet the potential need, we need to consider whether DHM supply can increase. This study aimed to explore the reasons that prevent women who wish to donate their milk in the United Kingdom from doing so to understand which barriers may be modifiable. METHODS: Women who wanted to donate their milk but did not do so completed an online survey. Open and closed questions examined the response they rece

Impact of fortifiers on donor milk nutrient composition: An experimental study.

BACKGROUND: Preterm infants frequently require human milk fortification to meet their unique nutrients requirements. How commercial fortifiers change essential macronutrients and micronutrients in donor human milk has not been well-studied. METHODS: Our sampling frame included milk from approved United States milk bank donors (n = 400), measured for macronutrients, vitamins, and minerals, and used to produce 1000 pools of donor milk. Fortification ratios were computed to describe the extent that fortifiers changed donor milk composition (<100% indicates decreased; >100% indicates increased).

Breast milk jaundice in premature infants.

In randomised study of 186 preterm infants those fed on maternal or banked breast milk had a significantly higher peak bilirubin concentration and a more prolonged jaundice than infants fed an artificial preterm formula and were over four times more likely to achieve plasma bilirubin values above 200 mumol/l (11.7 mg/100 ml). This dietary effect was seen even in a high risk subgroup of sick ventilated infants below 1500 g who were receiving restricted enteral intakes. We suggest that breast milk jaundice in preterm infants may increase clinical intervention. Our findings are discussed in the l

In Vivo Assessment of Antioxidant Potential of Human Milk Treated by Holder Pasteurization or High Hydrostatic Pressure Processing: A Preliminary Study on Intestinal and Hepatic Markers in Adult Mice.

Preterm infants are highly susceptible to oxidative stress due to an imbalance between endogenous oxidant and antioxidant systems. In addition, these newborns are frequently fed with donor milk (DM) treated by Holder pasteurization (HoP) at 62.5 °C for 30 min, which is known to alter numerous heat-sensitive factors, including some antioxidants. High hydrostatic pressure (HHP) processing was recently proposed as an innovative method for the treatment of DM. The present study aimed to measure the redox balance of HoP- and HHP-DM and to study, in vivo, the effects of HoP- and HHP-DM on the gut an

Changes in growth and metabolism in very low birthweight infants fed with fortified breast milk.

Human milk is often inadequate nutritionally for preterm infants. We investigated the effect of adding a commercially prepared milk fortifier to human (maternal or bank) milk and measured changes in lower leg length velocity (LLLvel) using knemometry, weight gain and biochemical indices of nutrition. Babies were allocated to one of three feed groups, in a semi-randomized fashion, to receive human milk alone (group I), fortified human milk (group II) or a preterm formula (group III). The birthweights (median and R) and birth gestations (median and R) of the three groups were as follows: group I

Sex differences in preterm nutrition and growth: the evidence from human milk associated studies.

Sexual dimorphism of the fetus manifests itself even during pregnancy. Preterm births are more common in pregnancies with male fetuses. Intrauterine and postnatal growth nomograms are sex-specific. The human milk composition in term infants appears to be sex-specific. Early nutrition has sex-specific effects and neurodevelopmental outcomes. A large same-sex twin study suggests that a mother's own milk (MOM) provides sex-specific growth advantages probably related to the calibration of a mother's milk based on her newborn's sex. Formula composition does not vary with infant sex, which may be on

Lactation after infant death: an analysis of Australian healthcare agencies' online health information.

Lactation is a potent signifier of maternal love and care commonly associated with early motherhood and infant survival. It is common, however, for bereaved mothers who have recently undergone miscarriage, stillbirth or infant death to produce breastmilk. Drawing on a critical feminist lens that seeks to understand how maternal subjectivities and lactation norms are constructed through public and reproductive health information, this article tests whether lactation management options after stillbirth and infant death are comprehensively covered in Australia's health organisations' online infor

Pasteurized Donor Human Milk in Healthy Infants.

PURPOSE: Limited research exists examining the use of pasteurized donor human milk (PDHM) in healthy and non-critically ill infants. To inform care for families using PDHM, we present three case exemplars of the experiences of families who used PDHM supplementation in the birth hospital for their healthy infants. STUDY DESIGN: Participants were recruited through word of mouth and social media and interviewed via Zoom using a semi-structured interview guide. Three interviews were conducted in April of 2025. METHODS: Qualitative interviews were compared, and characteristics, similarities, and

Assisted Nursing: A Case Study of an Infant With a Complete Unilateral Cleft Lip and Palate.

BACKGROUND: This case presents a mother's feeding experience of an infant born with a complete unilateral cleft lip and palate from birth until both were repaired. She fed him directly at the breast with assistance, controlling the delivery of the mother's own milk. The infant received only his mother's own milk, no bottles, and the excess expressed milk was donated to a milk bank. MAIN ISSUE: The Cleft Team supported the mother's wish to breastfeed but expressed realism that cleft lip and palate infants could not create suction. Success had never been observed in the unit. Instead, the team

Composition of Breast Milk in Women with Obesity.

Background/Objectives: Among US breastfeeding women, those with obesity have significantly increased breast milk fat and caloric content from foremilk to hindmilk, with a 4-fold increase in fat content from the first to last milk sample. In view of different dietary norms and nutritional standards, we sought to evaluate the relationship between maternal BMI with breast milk fat and calorie content in women from Brazil, a low-middle-income country. Methods: Women who delivered singleton-term neonates were recruited from the Ana Abrao Breastfeeding Center (AABC) and Human Milk Bank at the Federa

Breastfeeding Mothers' and Nurses' Perceptions of Breast Milk Sharing and Donation for Research in Bhutan: A Qualitative Study.

INTRODUCTION: Breast milk is globally recognized as the optimal source of infant nutrition due to its immunological and neurodevelopmental benefits. In low-and middle-income countries like Bhutan, limited availability of donor milk and absence of milk banking infrastructure hinder neonatal nutrition. While breast milk sharing for feeding has been explored elsewhere, little is known about maternal and nursing perspectives of milk donation for research use within Bhutanese sociocultural context. OBJECTIVE: To explore breastfeeding mothers' perceptions of breast milk sharing and donation for bot

Use of donor human milk in nonhospitalized infants: An infant growth study.

When mother's own milk (MOM) is unavailable or insufficient, donor human milk (DHM) is recommended as the next best alternative for low birthweight infants. DHM use for healthy, term infants is increasing, but evidence for growth and tolerability is limited. This retrospective study evaluated growth in term infants in the community who received DHM from a UK milk bank. Mothers of infants receiving DHM between 2017 and 2019 were contacted (n = 49), and 31 (63.2%) agreed to participate. Fourteen infants received DHM as a supplement to other feeds (MOM and/or infant formula) and 17 were exclusive

Meeting the nutritional needs of the low-birth-weight infant.

Delivering adequate amounts of nutrients to premature infants at all times is challenging because the infant's immature gastrointestinal tract is initially unable to accept feedings, necessitating the use of parenteral nutrition. In the past, inadequate amounts of nutrients have commonly been given to premature infants because the administration of nutrients was thought to be hazardous. Inadequate nutrient intakes have resulted in widespread postnatal growth restriction. Now that it is known that postnatal growth restriction is associated with poor neurocognitive development, efforts are made
