The surgical care practitioner: a feasible alternative. Results of a prospective 4-year audit at St Mary's Hospital Trust, London.

INTRODUCTION: Surgical care practitioners (SCPs) are an expanding group of professionals, drawn from nursing and the allied health professions. Amongst other functions, SCPs can provide a range of surgical procedures including a 'minor surgical' service. The aim of this study was to audit the volume and outcomes related to the SCP service at St Mary's since its inception. PATIENTS AND METHODS: All prospectively collected data regarding SCP-managed patients between 2001 and 2005 were retrospectively audited. Volume, case mix, waiting times, complications and patient satisfaction were recorded

National Survey on Antithrombotic Management in Minor Hand Surgery.

PURPOSE: Temporarily withholding antithrombotic medications for surgical procedures can increase thromboembolic risk. Many patients undergoing minor hand surgeries take these medications, requiring careful perioperative management by surgeons. Although general guidelines exist for periprocedural antithrombotic management, guideline adherence among surgeons performing minor hand procedures is unknown. This study aimed to assess current pre-procedural antithrombotic management strategies employed by hand surgeons for patients undergoing minor hand surgeries, specifically focusing on how closely

Procedural Sedation in Minor Procedure Rooms for Pediatric Myringotomy and Tympanostomy: A Quality Improvement Initiative.

OBJECTIVE: Lengthy wait times for elective surgery is a widespread health care system conundrum that may increase patient distress and jeopardize health outcomes. The primary aim of this quality improvement project was to reduce the surgical wait time in patients undergoing tympanostomy tube insertion. METHODS: As of January 2018, our tertiary care institution implemented a novel protocol whereby healthy children may undergo tympanostomy tube insertion in a minor procedure room under ketamine sedation administered by pediatric emergency physicians to address lack of both physical and anesthes

Observations upon techniques for reconstruction of the urethral meatus, the hypospadiac glans deformity and the penile urethra.

Many minimal defects of the terminal urethra may be left untreated; others may be considerably improved by a simple one-stage extension to a terminal position by one of the many one-stage "flap-strip" procedures. Patients with a marked ventriflexed glans are much better treated by the "kippered-glans" procedure; undertaken as a two-stage procedure, the functional and the cosmetic results of this procedure are extremely good and reliable. The results of one-stage procedures reflect the experience and the judgement of the surgeon but they tend to be less perfect and furthermore there is always a

Comprehensive complication index for NOTES procedures: results from a randomized controlled trial and comparison to published NOTES complication data.

OBJECTIVE: This investigation uses the comprehensive complication index (CCI) to compare complications after natural orifice transluminal endoscopic surgery (NOTES) procedures. BACKGROUND: NOTES procedures are developed to miniaturize surgical trauma. NOTES publications inconsistently report complications. The CCI improves reporting of complications. METHODS: The CCI is calculated using complication data from a single center, double blind, randomized controlled trial comparing transvaginal [transvaginal cholecystectomy (TVC), N = 41] and conventional laparoscopic cholecystectomy (CLC, N = 51

Recombinant factor VIIa to prevent surgical bleeding in factor XI deficiency.

Factor XI (FXI) deficiency is associated with bleeding after invasive procedures. Risks of human plasma-derived FXI replacement products include transfusion transmitted infection, thrombosis and fluid overload. This study was designed to test the hypothesis that recombinant factor VIIa (rFVIIa) is an effective haemostatic agent in patients with FXI deficiency undergoing surgery. Fourteen FXI deficient patients [five severely deficient (FXI:C <20 U dL(-1)) and nine partially deficient (FXI:C 20-70 U dL(-1)] received rFVIIa to prevent surgical bleeding during five major, four minor and six denta

Surprise met in minor ops.

The case of a woman who was initially diagnosed as having a sebaceous cyst but who died from metastatic lung carcinoma 2 weeks after her initial 'minor op' procedure. Minor surgical procedures are increasingly being performed by specialist nurses and by general practitioners in primary care. However, such non-experts may lack the resources to cope with surprises such as was found in this case. It is, therefore, imperative that adequate training and support is provided as the model of care changes.

Tumescent technique in digits: a subcutaneous single-injection digital block.

BACKGROUND: A modified subcutaneous single-injection approach to achieve digital block using a tumescent technique is described. METHOD: A convenient sample of patients requiring digital anesthesia for minor surgical procedures on the fingers or thumb in the emergency and plastic departments were enrolled into the study. Digital nerve block was performed by injecting 1% lidocaine into the volar subcutaneous space at the proximal digit to create a firm, turgid feel to the tissue, the so-called tumescent state. The volume of anesthetic was based on the size of the digit. All nerve blocks were p

Why is urological laparoscopy minimally invasive?

OBJECTIVES: Laparoscopic procedures have been developed and established with the view that a similar operative effect can be achieved with less traumatization, especially as far as systemic stress response is concerned. We report a prospective, controlled, nonrandomized animal and patient study to determine the systemic response to laparoscopic and open surgical procedures. METHODS: In the animal study, 26 female pigs underwent either a laparoscopic bilateral varix ligation followed by bilateral nephrectomy (group I), sole introduction of trocars (group II) or sole establishment of an open su
