The Economic Model of Retinopathy of Prematurity (EcROP) Screening and Treatment: Mexico and the United States.

PURPOSE: To describe an economic (Ec) model for estimating the impact of screening and treatment for retinopathy of prematurity (ROP). DESIGN: EcROP is a cost-effectiveness, cost-utility, and cost-benefit analysis. METHODS: We surveyed caregivers of 52 children at schools for the blind or pediatric eye clinics in Atlanta, Georgia and 43 in Mexico City. A decision analytic model with sensitivity analysis determined the incremental cost-effectiveness (primary outcome) and incremental monetary benefit (secondary outcome) of an ideal (100% screening) national ROP program as compared to estimates

Modelling the economic value of cross- and sustained-protection in vaccines against cervical cancer.

OBJECTIVE: Two human papillomavirus (HPV) vaccines are on the market. Based on expected differences in sustained- and cross-protection between the two vaccines, their long-term economic value is modelled and compared for France, Ireland and Italy. METHODS: A Markov cohort model reproducing the natural history of HPV infections, screening and vaccination, is adapted to country-specific data. Two hypothetical HPV vaccines (VA and VB) are compared. At baseline VA provides lifetime protection against HPV-16 and 10-year protection against HPV-18 before waning. VB is the same as VA with a 10-year p

Family economic pressure and adolescent suicidal ideation: application of the family stress model.

This study used a sample of 501 families from the Mississippi Delta region to examine the feasibility of the Family Stress Model for understanding adolescent suicidal ideation. The results indicated that family economic pressure was related to parental depressive symptoms, which, in turn, was related to parental hostile behavior and physical abuse. These parental behaviors were related to adolescent self-esteem and depressive symptoms, which, in turn, were related to adolescent suicidal ideation. Therefore, family economic pressure and parental depressive symptoms and behaviors were indirectly

Technical-economic modelling of integrated water management: wastewater reuse in a French island.

An integrated technical-economic model is used to address water management issues in the French island of Noirmoutier. The model simulates potable water production and supply, potable and non potable water demand and consumption, wastewater collection, treatment and disposal, water storage, transportation and reuse. A variety of water management scenarios is assessed through technical, economic and environmental evaluation. The scenarios include wastewater reclamation and reuse for agricultural and landscape irrigation as well as domestic non potable application, desalination of seawater and b

A review of economic evaluation models for cardiac resynchronization therapy with implantable cardioverter defibrillators in patients with heart failure.

OBJECTIVES: Cardiac resynchronization therapy with a biventricular pacemaker (CRT-P) is an effective treatment for dyssynchronous heart failure (DHF). Adding an implantable cardioverter defibrillator (CRT-D) may further reduce the risk of sudden cardiac death (SCD). However, if the majority of patients do not require shock therapy, the cost-effectiveness ratio of CRT-D compared to CRT-P may be high. The objective of this study was to systematically review decision models evaluating the cost-effectiveness of CRT-D for patients with DHF, compare the structure and inputs of these models and ident

Economic models to estimate changes in herd sustainability and structure associated with clinical cure failure of metritis in dairy cattle.

This study aimed to evaluate the economic and environmental impacts of clinical cure failure of metritis in dairy herds using a dynamic Markov chain model. Metritis incidence was set at 25% of postpartum cows, with 3 scenarios simulated over a 10-yr horizon: base (no metritis), cured (all metritis cases cured), and not cured (no cases cured). Herd size was constrained to 900 to 1,000 milking cows, and all metritis cases were treated with ceftiofur crystalline-free acid. Outputs from year 10 (steady-state herd) were used for comparison. Total herd population was greatest in the not cured scenar

A Comparative Study of Forward Osmosis Systems: An Integrated Experimental, Modeling, and Techno-Economic Analysis.

Draw solution chemistry is a design parameter in forward osmosis (FO), governing hydraulic performance, organic rejection, fouling dynamics, and process economics, yet these dimensions are rarely evaluated together, particularly under wastewater feed conditions. This study addresses that gap through an integrated experimental, modeling, and techno-economic framework using three draw solutions (NaCl, MgSO4, and sodium acetate), each prepared at an initial osmotic pressure of 30 bar, and two feed solutions (deionized water (DI) and real municipal wastewater (WW)). Water flux (J w), reverse salt

A Hemophilia Joint Health Score-Based Model for the Economic Evaluation of Hemophilia A Prophylaxis Interventions.

BACKGROUND AND OBJECTIVE: Hemophilia A is a costly, lifelong illness with multiple prophylaxis options. Previously, these options were assessed using a Peterson score-based model to simulate joint damage over time. This study built a model for the economic evaluation of hemophilia A with less socioeconomic selection bias utilizing the hemophilia joint health score (HJHS). METHODS: A mechanistically defined HJHS-based state-transition microsimulation model was implemented for the cost-utility analysis conducted over a lifetime horizon from a Canadian provincial Ministry of Health perspective,

Economic evaluation of diagnostic tests for Thai patients with tuberculosis: A dynamic transmission model approach.

Conventional tuberculosis (TB) diagnosis is time-consuming, while newer molecular assays such as Xpert MTB/RIF and loop-mediated amplification test for TB (TB-LAMP) provide faster results but at a higher cost compared to sputum smear microscopy (SSM) with culture and drug susceptibility testing (DST) in Thailand. This study assessed the cost-utility of TB diagnostic algorithms as either initial or add-on tests from a societal perspective for TB diagnosis in the general Thai population. A dynamic transmission model was employed to evaluate five TB diagnostic algorithms over a 15-year period. Co

Empirical Testing of Alternative Search Methods to Retrieve Utility Values for Health Economic Modelling.

OBJECTIVES: The objective of this study is to compare different information retrieval methods that can be used to identify utility inputs for health economic models. METHODS: The usual practice of using systematic review methods was compared with two alternatives (iterative searching and rapid review), using a health technology assessment (HTA) case study in ulcerative colitis (UC). We analysed whether there were differences in the utility values identified when using the alternative search methods. Success was evaluated in terms of time, burden and relevance of identified information. The id

Epidemiological and health economic implications of symptom propagation in respiratory pathogens: A mathematical modelling investigation.

BACKGROUND: Respiratory pathogens inflict a substantial burden on public health and the economy. Although the severity of symptoms caused by these pathogens can vary from asymptomatic to fatal, the factors that determine symptom severity are not fully understood. Correlations in symptoms between infector-infectee pairs, for which evidence is accumulating, can generate large-scale clusters of severe infections that could be devastating to those most at risk, whilst also conceivably leading to chains of mild or asymptomatic infections that generate widespread immunity with minimal cost to public
