Mononeuropathy multiplex in the course of idiopathic thrombocytopenic purpura. A case report.

Bcl-2 family proteins reorganize mitochondrial membranes during apoptosis, to form pores and rearrange cristae. In vitro and in vivo analysis integrated with human genetics reveals a novel homeostatic mitochondrial function for Bcl-2 family protein Bid. Loss of full-length Bid results in apoptosis-independent, irregular cristae with decreased respiration. Bid-/- mice display stress-induced myocardial dysfunction and damage. A gene-based approach applied to a biobank, validated in two independent GWAS studies, reveals that decreased genetically determined BID expression associates with myocardi

Acute multiple mononeuropathy after accidental exposure to oven microwaves.

A patient accidentally exposed to oven microwaves developed an acute and severe dystrophy and sympathetic reflex of the right hand, a peripheral neuropathy of the digits of the left hand, and a sensory injury of the trigeminal nerve in the right side of the face. The physiopathological involvement in the injury of the peripheral nerve and the failure of analgesia with carbamazepine are emphasized. In this causalgia, relief from pain and modification of the autonomic disorders can be achieved by a sympathetic block.

Diabetic oculomotor mononeuropathy: involvement of pupillomotor fibres with slow resolution.

The third and sixth cranial nerves are most frequently affected in diabetic extraocular mononeuropathies. In diabetic oculomotor involvement the pupil is undisturbed. Of the 21 patients of diabetic oculomotor paralysis 4 patients (19 percent) had dilated unreactive pupil suggestive of pupillomotor involvement. No other cause could be attributed to oculomotor paralysis. These patients showed complete but slow resolution as compared to those without pupil involvement.

Mononeuropathy multiplex as an uncommon presentation of intravascular lymphoma: A case report.

KEY CLINICAL MESSAGE: Although intravascular lymphoma rarely presents with peripheral neuropathy, learning about this presentation can lead to timely diagnosis and improved prognosis in patients with intravascular lymphoma. ABSTRACT: A 64-year-old man presented with asymmetric paresthesia and subsequent weakness of his feet and a 10 kg weight loss over 40 days. Electrodiagnostic studies revealed distal axonal sensory-motor polyneuropathy with ongoing axonal loss. A peroneal nerve biopsy showed intravascular proliferation of CD-20 positive lymphocytes, which suggested intravascular large B-cel

[Mononeuropathy of dorsal interosseous nerve: compression by return radial arteries].

UNLABELLED: Among many causes of compression neuropathies involving the radial nerve, compression of the posterior interosseous nerve (PIN) by the returning radial arteries is rare. OBJECTIVE: To describe the clinical instrumental characteristics of cases of mononeuropathy caused by compression of the return radial arteries. MATERIAL AND METHODS: Two observations with neuropathy of the deep branch of the radial nerve, manifested by weakness in the corresponding muscles, inconsistent pain syndrome with previous stereotypic (recurring) physical activity are presented. RESULTS: According to th

Nerve biopsy in Indian patients with mononeuropathy multiplex of undetermined etiology.

INTRODUCTION: A diagnosis of mononeuropathy multiplex (MM) requires detailed evaluation to determine etiology. We performed nerve biopsy on patients with MM in whom the etiology could not be established via other investigations. METHODS: Sixty-eight patients with MM seen between January 2013 and June 2014 underwent detailed diagnostic evaluation. Those in whom the investigations failed to establish an etiology underwent nerve biopsy. RESULTS: A diagnosis of leprosy was confirmed in 14 patients and was highly probable in 17 others. Eleven patients had vasculitic neuropathy, and in 1 patient t

Guanosine prevents thermal hyperalgesia in a rat model of peripheral mononeuropathy.

UNLABELLED: It is well known that adenine-based purines exert multiple effects on pain transmission. Less attention has been given, however, to the antinociceptive effects of guanine-based purines. The aim of this study was to investigate the effects of intraperitoneal administration of guanosine on a rat model of peripheral mononeuropathy. Additionally, investigation of the mechanism of action of guanosine, its general toxicity and measurements of central nervous system purine levels were performed. Rats received an intraperitoneal administration of vehicle (0.1 mM NaOH) or guanosine (up to 1

Prevalence of carpal tunnel syndrome and median mononeuropathy among dentists.

BACKGROUND: The authors undertook a study to determine the prevalence in dentists of abnormal sensory nerve conduction and/or symptoms of carpal tunnel syndrome, or CTS, the most common nerve entrapment syndrome. METHODS: In a cross-sectional study, dentists (n = 1,079) were screened during the American Dental Association's Annual Health Screening Program in 1997 and 1998 by means of standard electrodiagnostic measures in the dominant hand and a self-reported symptom questionnaire. The authors diagnosed a median mononeuropathy from a 0.5- or 0.8-millisecond, or ms, prolongation of the median

Mononeuropathy multiplex in sickle cell disease: a complication in need of recognition.

Peripheral nervous system involvement is rare in sickle cell disease. A review of the literature has uncovered only a few reports of nerve deficit in association with sickle cell disease, and often a clear relationship between the neuropathy and the anemia is absent. Mononeuropathy resulting from peripheral nerve infarction, as a complication of sickle vaso-occlusive crisis, seemingly is uncommon and, to our knowledge, has been reported only once. We report two patients who developed acute mononeuropathy multiplex in the setting of sickle cell pain crisis. The clinical and electrodiagnostic fi

Mononeuropathy of the medial branch of the deep peroneal nerve in a scuba diver.

Peripheral mononeuropathies occur only rarely in association with decompression illness. The sites previously reported to be affected are areas of potential entrapment in which a peripheral nerve traverses a confined area. In these instances, the pathophysiology has been presumed to be mechanical pressure in an enclosed space by a gas bubble. A rare case is now presented of a peripheral mononeuropathy of the medial branch of the deep peroneal nerve in a scuba diver following surfacing from a 195 foot dive. This case differs from prior reports of mononeuropathy in association with decompression

A case-control and nerve biopsy study of CREST multiple mononeuropathy.

From 536 patients with the CREST syndrome (calcinosis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, and telangiectasis), seven were identified as having peripheral neuropathy not attributable to another cause. Peripheral neuropathy developed 0 to 25 years after their first symptoms of scleroderma. Unexplained neuropathy in CREST patients (seven patients) was more frequent than in control subjects (two patients) matched for age, sex, time of evaluation, and geographic referral region. Multiple mononeuropathy occurred significantly more frequently in the CREST group (six patients

Isolated femoral mononeuropathy to the vastus lateralis: EMG and MRI findings.

A 16-year-old female track athlete experienced sudden onset of right anterolateral thigh pain, initially thought to be cramping. After 2 months of continued postexercise pain she sought medical evaluation. A 3-cm thigh circumference discrepancy was noted on physical exam. She was referred for electromyography (EMG) and magnetic resonance imaging (MRI) studies. Femoral and screening nerve conduction studies were normal. Needle EMG revealed acute neuropathic changes confined to the right vastus lateralis only. These findings were confirmed by MRI, including short T1 inversion recovery (STIR) seq

Cryoneurolysis as a Neuroregenerative Intervention for Chronic Painful Mononeuropathies: A Four-Patient Case Series and Discussion.

Posttraumatic painful mononeuropathy involves a permanent structural change in the affected nerve. Clinical and animal models have shown that exposure of nerves to specific cold temperatures for distinct time periods can induce: cessation of signal transduction, Wallerian degeneration, and predictable axonal regeneration. It is hypothesized that these processes may return a structurally altered painful nerve to a non-pathologic or normal state. We present four patients in which percutaneous cryoneurolysis was employed with the intent of inducing regeneration and repair. Primary outcomes includ

Pediatric radial mononeuropathies: a clinical and electromyographic study of sixteen children with review of the literature.

Sixteen pediatric radial mononeuropathies were seen among 2077 electromyograms performed in the electromyography laboratory at The Children's Hospital, Boston, during 16.5 years, 1979-1995. Eight (50%) of these radial neuropathies, including 2 in newborns with apparent prenatal onset, were atraumatic, primarily related to compression in 6 and entrapment in 2. The other 8 (50%) were traumatic related to fractures or lacerations. Electromyography documented the radial neuropathy to be localized to the proximal main radial nerve trunk in 2 (13%), distal main radial nerve trunk in 9 (56%), and pos

A Case Report of Systemic Lupus Erythematosus Presenting as Isolated Mononeuropathy Multiplex.

Mononeuropathy multiplex is a pattern of progressive sensory and motor deficits in the distribution of two or more peripheral nerves. The differential for mononeuropathy multiplex includes vasculitis, autoimmune disorders, infectious diseases, sarcoidosis, amyloidosis, cryoglobulinemia, and paraneoplastic disease. We present a case of a 42-year-old woman with hypothyroidism who presented with 1 week of ascending numbness and weakness, persistent fevers, and 3 months of constant burning pain in both feet. Her neurologic exam was notable for asymmetric weakness, loss of sensation, and diminished

Ulnar nerve mononeuropathy in a patient with Hansen's disease: Clinical and radiological features.

Leprosy is a chronic infection caused by bacteria called Mycobacterium leprae. This is a prevalent disease in low-income countries, and it has not been completely eradicated. We present the case of a 29-year-old man with a previous diagnosis of Hansen's disease, who consulted for pain in the left elbow and wrist, associated with fever with preserved mobility and pain on palpation. Diagnosis of mononeuropathy of the ulnar nerve was made by MRI of the elbow and forearm. This entity corresponds to an infrequent complication of this infection with few cases reported in the literature. Our added va
