A case of radiation recall mucositis associated with docetaxel.

BACKGROUND: Radiation recall reactions, in particular dermatitis, are well documented in the literature. However, radiation recall mucositis is a rare clinical phenomenon. METHODS: We report a case of a 45-year-old man diagnosed with squamous cell carcinoma of the base of tongue. He was treated with surgery followed by chemotherapy and radiation therapy. Several months after completing treatment, he had a recurrence develop outside of the previously irradiated field. He was offered radiation therapy concurrent with docetaxel as salvage therapy. RESULTS: During salvage therapy, acute recall m

Anaerobic bacteremia in a neutropenic patient with oral mucositis.

An increasing number of anaerobic bloodstream infections in neutropenic cancer patients have been reported in the last decade. The type of anaerobes isolated from most of these patients suggests an oral source of infection. We describe a case of anaerobic bacteremia in a neutropenic patient with oral mucositis that highlights the importance of considering these organisms when selecting empiric prophylactic or therapeutic antimicrobial regimens, especially in the setting of periodontal disease or oral mucositis.

New frontiers in the management of chemotherapy-induced mucositis.

About one-third of patients undergoing chemotherapy treatment suffer oral mucositis, an inflammatory-like change of the oral mucosa. Severe pseudomembranous/ulcerative mucositis can lead to secondary infection of lesions, sepsis and even cessation of treatment. Patients receiving curative head-neck irradiation are most susceptible and children undergoing chemotherapy are three times more likely to be affected. Mucositis is a costly side-effect of cancer therapy due to the extra time patients spend in hospital and currently there is no consistently effective treatment. Experimental studies with

Treatment-induced mucositis: an old problem with new remedies.

Mucositis may be a painful, debilitating, dose-limiting side-effect of both chemotherapy and radiotherapy for which there is no widely accepted prophylaxis or effective treatment. The basis of management is pain relief, prevention of dehydration and adequate nutrition. When tested vigorously, most antiseptic mouthwashes and anti-ulcer agents are ineffective. Simple mechanical cleansing by saline is the most effective traditional measure. A variety of new agents are effective. Granulocyte macrophage colony-stimulating factor (GM-CSF) and granulocyte colony-stimulating factor (G-CSF) act outwith

An animal model for mucositis induced by cancer chemotherapy.

Mucositis induced by chemotherapy is a painful and often dose-limiting side effect of cancer therapy. Furthermore, loss of the integrity of the oral epithelium often provides a microbial portal of entry and leads to sepsis. The present study describes the first animal model for chemotherapy-induced mucositis. The combination of three intraperitoneal injections of 5-fluorouracil at 5-day intervals and superficial mechanical mucosal irritation resulted in clinical breakdown of the oral mucosa characterized by ulcerative mucositis in Golden Syrian hamsters. Both clinical and histologic evaluation

Propolis allergy: a cause of oral mucositis with ulceration.

Propolis has been used since ancient times in folk medicine for its alleged beneficial effects. It is a potent sensitizer and is well recognized as a cause of occupational allergic eczematous contact dermatitis in apiarists. Recently, there has been an increase in allergic eczematous contact dermatitis because of nonoccupational exposure to propolis in "natural products" and biocosmetics. This report describes a patient who had acute oral mucositis with ulceration as a result of using propolis-containing lozenges.

Ketamine Analgesia for Refractory Mucositis Pain (KARM): A prospective, double-blind, randomised, phase 2 study of ketamine for oral and pharyngeal mucositis pain.

BACKGROUND: Oral and pharyngeal mucositis (OM) pain is often severe, distressing, and effective analgesic management remains challenging, particularly when pain is inadequately controlled with opioids. Ketamine, a potent NMDA-receptor antagonist, has shown promise in open-label studies. This study evaluated the efficacy of ketamine versus an active placebo for treatment-related OM pain, alongside examining its pharmacokinetics, adverse effects, and the influence of CYP2B6 polymorphisms. PATIENTS AND METHODS: Adults with OM and an average Brief Pain Inventory pain score ≥4/10 were randomised t

5-Fluorouracil-induced intestinal mucositis: Pathogenesis and potential therapeutic interventions.

Intestinal mucositis is one of the most common toxicities of 5-fluorouracil (5-FU)-based chemotherapy, occurring in up to 80% of treated patients. Clinically, it presents as abdominal pain, watery or bloody diarrhea, malabsorption and body-weight loss, often forcing chemotherapy dose reduction or discontinuation, thereby aggravating the psychological burden of patients, prolonging hospital stay and potentially increasing mortality. Histologically, 5-FU-induced intestinal mucositis (FIM) is characterized by villus atrophy, crypt apoptosis, disruption of epithelial barrier and pronounced inflamm

Mycoplasma pneumoniae-Induced Rash and Mucositis: How Can We Help?

Mycoplasma pneumoniae-induced rash and mucositis (MIRM) is a rare clinical pathology distinct from erythema multiforme major and Stevens-Johnson syndrome. We present a case of a patient who developed severe oral mucositis following an M. pneumoniae respiratory tract infection. We aim to raise awareness of MIRM, its varying presentations, and its management, particularly in light of the limited scientific evidence and the variability in treatment strategies depending on disease severity.

Mycoplasma pneumoniae-Induced Rash and Mucositis (MIRM): A Case Report.

This case report describes a 27-year-old military serviceman in Saudi Arabia who developed severe mucocutaneous lesions - oral and genital ulcers, conjunctivitis, and eruptive rash - following an upper respiratory infection. Initially, his clinical presentation raised suspicion of Behçet's disease; however, further evaluation and investigation favored Mycoplasma pneumoniae-induced rash and mucositis (MIRM) as a final diagnosis. His management included antibiotics and systemic corticosteroids, leading to marked clinical improvement. This case report highlights the diagnostic challenges of MIRM,

Bacillus clausii Attenuates 5-Fluorouracil-Induced Intestinal Mucositis in Mice.

5-Fluorouracil (5-FU) is an antimetabolite widely prescribed in cancer treatments, but its use in highly proliferative tissues can cause significant problems such as mucositis. Bacillus clausii is a probiotic commonly used for protection against acute diarrhea, gastrointestinal dysbiosis and inflammatory bowel diseases. We investigated the effect of B. clausii on 5-FU intestinal mucositis in mice. B. clausii was administered concomitantly with 5-FU on the first day and alone for the other two days. After the third day of 5-FU (450 mg/kg, ip), the animals were euthanized. Ileum samples were rem

A Therapy-Terminating Event: Programmed Death-1 Inhibitor-Induced Mucositis.

Oral mucositis is a painful inflammatory response that can lead to infection, cachexia, and therapy termination. This immune-related adverse event (IRAE) has been well documented within the newly developing field of immunotherapy. This case series presents three patients, aged 73 to 81 years, who were undergoing treatment with programmed death-1 (PD-1) immunotherapy for cancer; each patient developed grade III mucositis, one after the fourth cycle and two after the seventh. All three patients had no prior history of oral pathology, yet each patient reported ulcerated and inflamed oral mucosa t

Losartan improves intestinal mucositis induced by 5-fluorouracil in mice.

Intestinal mucositis (IM) is a common side effect of 5-fluorouracil (5-FU)-based chemotherapy, which negatively impacts therapeutic outcomes and delays subsequent cycles of chemotherapy resulting in dose reductions and treatment discontinuation. In search of new pharmacological alternatives that minimize your symptoms, this work set out to study the effect of losartan (LOS), a receptor type I (AT1) angiotensin II antagonist, on intestinal mucositis induced by 5-FU. Intestinal mucositis was induced by a single intraperitoneal administration of 5-FU (450 mg/kg) in Swiss mice. Losartan (5, 25 or

LiCl Promotes Recovery of Radiation-Induced Oral Mucositis and Dysgeusia.

Oral mucositis and taste dysfunction are frequently complained by patients with head and neck cancer receiving radiotherapy, challenging the clinical outcome of cancer treatment. Recent studies have indicated the protective role of Wnt/β-catenin signaling in radiation-induced oral mucositis (RIOM) and its pivotal role in the development and self-renewal of taste buds. The current study hypothesizes that lithium chloride (LiCl), a potent activator of the Wnt/β-catenin signaling pathway, can promote the postirradiation restoration of oral mucosa integrity and taste function. To validate this hyp

Understanding oral mucositis and the principles of effective mouth care.

Mucositis involves an inflammation of the oral or gastrointestinal mucosa and is a common complication of cancer treatment. Mucositis is usually associated with high-dose chemotherapy, radiotherapy and, more recently, molecular targeted therapy. Oral mucositis refers specifically to inflammation and ulceration that occurs in the mouth. If not treated promptly, it can potentially lead to life-threatening infection requiring hospitalisation and intravenous antibiotics. This article discusses the causes and nursing management of oral mucositis, with an emphasis on self-care and optimal oral hygie
