Chronic musculoskeletal pain in children: part II. Rheumatic causes.

Primary care physicians should have a working knowledge of rheumatic diseases of childhood that manifest primarily as musculoskeletal pain. Children with juvenile rheumatoid arthritis can present with painless joint inflammation and may have normal results on rheumatologic tests. Significant morbidity may result from associated painless uveitis, and children with juvenile rheumatoid arthritis should be screened by an ophthalmologist. The spondyloarthropathies (including juvenile ankylosing spondylitis and reactive arthritis) often cause enthesitis, and patients typically have positive results

Sleep patterns in female adolescents with chronic musculoskeletal pain.

This study examined sleep patterns in female adolescents with chronic musculoskeletal pain. Twenty-six participants with chronic musculoskeletal pain completed questionnaires during their clinic visit, and three 24-Hour Sleep Patterns Interviews during the following 2 weeks. Compared to normative data (Acebo & Carskadon, 2002), adolescents with chronic pain reported similar total sleep time (TST) and bedtimes. However, study participants reported significantly longer sleep onset latency, more night wakings, a later morning wake time, and more symptoms of daytime sleepiness. Pain improved after

Physical exercise and musculoskeletal pain among forest industry workers.

The study investigated the relations between physical exercise and musculoskeletal pain among forest industry workers. We studied a population of 3312 Finnish forest industry workers, who replied to a questionnaire survey in 1994 (response rate 77%). The outcome variables in this cross-sectional study were the number of days with pain in the low back, neck, shoulder and knee during the preceding 12 months. Multivariable logistic regression models were used in statistical analyses. Active walkers had more sciatic pain, active volleyball players had more shoulder pain and those who practiced tre

Regional musculoskeletal pain. The elbow, forearm, wrist and hand.

Two main groups of soft-tissue disorders are identified: specific soft tissue syndromes and a non-specific disorder as yet not fully characterized. Specific soft tissue syndromes occur in joint, muscle or nerve and are associated with characteristic symptoms and physical signs. These include epicondylitis at the elbow, tendon disorders at the wrist and nerve entrapments such as carpal tunnel syndrome. The non-specific soft tissue syndrome is primarily described by symptoms of pain but may include muscular symptoms (such as weakness and cramp) or nerve symptoms (such as numbness, pins and needl

Relief of acute musculoskeletal pain using transcutaneous electrical neurostimulation.

In the emergency department of Dover Air Force Base Hospital, a primary care facility for active duty, retired, and dependent patients, transcutaneous electrical neurostimulation was employed for six months as initial treatment for musculoskeletal pain in 34 patients with acute symptoms. Pain relief ranging from 0 to 100% was achieved in a variety of acute pain states. Only one patient did not obtain pain relief. Although some patients did not receive permanent relief from pain, a significant number of patients did benefit from this new modality.

Comparison of the effect of psychosocial skills training on acute and chronic musculoskeletal pain intensity: The effectiveness of early intervention in the reduction of acute musculoskeletal pain.

BACKGROUND: Pain, as a psychological experience, is caused by complex interactions among sensory-discriminative, motivational-affective, and cognitive-evaluative dimensions. The psychosocial approach is one of the important approaches in managing musculoskeletal pain in patients. Therefore, this study aimed to determine and compare the effects of psychosocial skills training on pain intensity in patients with acute and chronic musculoskeletal pain. METHODS: In this quasi-experimental study, 64 patients with acute and chronic musculoskeletal pain were selected using convenience sampling. Both

Neuroimmune Phenotyping as the Next Frontier in Chronic Pain Medicine for Musculoskeletal Back Pain.

BACKGROUND: Chronic low back pain encompasses heterogeneous mechanical, neuropathic, nociplastic, and myofascial presentations that may reflect distinct neuroimmune mechanisms. We synthesized evidence linking these phenotypes to measurable biological features and clinically relevant treatment strategies. METHODS: We conducted a structured literature search of MEDLINE, Scopus, Web of Science, and the Cochrane Central Register of Controlled Trials for studies published from January 2005 through January 2025. Systematic reviews, randomized controlled trials, and mechanistic studies addressing ne

Distinguishing Pain and No Pain in Musicians Through Machine Learning Analysis of Musculoskeletal Data.

Musculoskeletal disorders are common among professional musicians and often linked to altered movement patterns. This study examined whether a combined Principal Component Analysis-Linear Discriminant Analysis (PCA-LDA) framework can identify interpretable motion features associated with shoulder-neck pain during violin performance. Kinematic data from 26 violinists (11 with pain, 15 pain-free) were recorded with a 10-camera motion capture system during playing a standardized scale. PCA was applied to 41 upper-body waveforms to extract dominant movement components, which served as inputs for L

Virtual reality for the management of musculoskeletal pain: an umbrella review.

BACKGROUND: Musculoskeletal pain (MSK) is a condition that affects multiple parts of the musculoskeletal system, including limbs, neck, and back, leading to deterioration in both mental and physical health and overall quality of life. Despite the available treatments, they are not considered effective enough to eradicate pain symptoms, thereby requiring new methods as a substitute. This review comprehensively summarizes virtual reality (VR) technology as an adjunct or an alternative treatment for MSK pain and aims to explore the most suitable conditions and settings of VR. METHODS: Pubmed, Sc

Virtual reality as a treatment for chronic musculoskeletal pain syndromes.

Chronic musculoskeletal pain syndromes, including fibromyalgia, are often resistant to conventional medications and invasive therapies. Central hypersensitization, neurotransmitter dysregulation, and autonomic nervous system abnormalities are key pathomechanisms, frequently resulting in widespread pain and a variety of psychosomatic symptoms. Virtual Reality (VR) applications have demonstrated effectiveness in reducing pain, both during and after interventions, and in chronic conditions such as fibromyalgia and back pain. The proposed mechanisms behind VR's effectiveness include distraction an

Effects of stretching intervention on musculoskeletal pain in dental professionals.

OBJECTIVE: This study aimed to quantitatively confirm the effects of dental specialists' work and stretching on musculoskeletal pain. METHODS: The pain pressure threshold was divided into five parts (neck, shoulder, trunk, lower back, and hand/arm) of the upper body and measured at 15 muscle trigger points. The pain pressure threshold before and after work was measured, and 30 min of stretching and rest were stipulated as an intervention. RESULTS: The pain pressure thresholds reduced significantly in all muscles after work (P < .05). In the stretching group, the pain pressure thresholds incr

Aberrant plasticity in musculoskeletal pain: a failure of homeostatic control?

Aberrant synaptic plasticity is hypothesised to underpin chronic pain. Yet, synaptic plasticity regulated by homeostatic mechanisms have received limited attention in pain. We investigated homeostatic plasticity in the human primary motor cortex (M1) of 21 healthy individuals in response to experimentally induced muscle pain for several days. Experimental pain was induced by injecting nerve growth factor into the muscle belly of the right extensor carpi radialis brevis muscle. Pain and disability were monitored until day 21. Homeostatic plasticity was induced on day 0, 2, 4, 6, and 14 in the l

Sonographic assessment of musculoskeletal causes of calf pain and swelling.

Calf pain or swelling is a common presentation to the emergency department. The differential diagnoses are wide. Deep vein thrombosis (DVT) is often the first diagnosis to be excluded given its potentially fatal complications. Musculoskeletal causes of calf pain or swelling such as Baker's cyst, muscle or tendon tear, soft tissue infection, and inflammation are not uncommon and can often be confidently diagnosed with ultrasonography (US). Familiarity with these conditions and the sonographic findings would be useful in making timely and correct diagnosis.

The phenotypic and genetic signatures of common musculoskeletal pain conditions.

Musculoskeletal pain conditions, such as fibromyalgia and low back pain, tend to coexist in affected individuals and are characterized by a report of pain greater than expected based on the results of a standard physical evaluation. The pathophysiology of these conditions is largely unknown, we lack biological markers for accurate diagnosis, and conventional therapeutics have limited effectiveness. Growing evidence suggests that chronic pain conditions are associated with both physical and psychological triggers, which initiate pain amplification and psychological distress; thus, susceptibilit
