Open bite and atypical swallowing: orthodontic treatment, speech therapy or both? A literature review.

AIM: The cause-effect relationship between anterior open bite and atypical swallowing, two frequently associated conditions, is currently not completely understood. These conditions are often accompanied by speech disorders and represent a problem for both young patients and untreated adult patients. Treatment of these complex cases may be orthodontic, logopedic therapy or both. The purpose of this review is to compare the various types of treatment to determine their effectiveness in improving skeletal condition, normalisation of muscle activity, and temporal stability. METHODS: The present

Preliminary Assessment of an Ambulatory Device Dedicated to Upper Airway Muscle Training in Patients With Sleep Apnea: Proof-of-Concept Study.

BACKGROUND: Obstructive sleep apnea/hypopnea syndrome (OSAHS) is a prevalent condition affecting a substantial portion of the global population, with its prevalence increasing over the past 2 decades. OSAHS is characterized by recurrent upper airway (UA) closure during sleep, leading to significant impacts on quality of life and heightened cardiovascular and metabolic morbidity. Despite continuous positive airway pressure (CPAP) being the gold standard treatment, patient adherence remains suboptimal due to various factors, such as discomfort, side effects, and treatment unacceptability. [Simultaneous rehabilitation of the tongue and breathing: a physiological necessity].

INTRODUCTION: Pr. Delaire has understood since 2005 that functional therapy shouldn't try to rehabilitate a praxis but instead focus on the whole created by different oro-facial functions to ensure the creation of a global dynamic balance. That's why rehabilitation of the right tongue posture is a necessary condition for automation as nasal breathing; in the same meaning that rehabilitation of nasal breathing is a necessary condition for automation of the right tongue posture. Fortunately, nature can help us if we respect and follow physiological processes that take into account key points of

Muscular Assessment in Patients With Severe Obstructive Sleep Apnea Syndrome: Protocol for a Case-Control Study.

BACKGROUND: Myofunctional therapy is currently a reasonable therapeutic option to treat obstructive sleep apnea-hypopnea syndrome (OSAHS). This therapy is based on performing regular exercises of the upper airway muscles to increase their tone and prevent their collapse. Over the past decade, there has been an increasing number of publications in this area; however, to our knowledge, there are no studies focused on patients who can most benefit from this therapy. OBJECTIVE: This protocol describes a case-control clinical trial aimed at determining the muscular features of patients recently di

Atypical swallowing: a review.

AIM: Atypical swallowing is a myofunctional problem consisting of an altered tongue position during the act of swallowing. High incidence in population, multifactorial etiology and the recurring connection with the presence of malocclusions made it a topic of strong interest and discussion in science. The purpose of this review is to illustrate the current orientation on the topic of atypical swallowing, trying in particular to answer two questions: 1) what kind of connection is there between atypical swallowing and malocclusion; 2) what kind of therapy should be used to solve it. METHODS: Th

Giant Sublingual, Submental, and Lingual Dermoid Cyst Restricting Tongue Movement Undiagnosed for Several Years.

(1) Background: Dermoid cysts occurring in the sublingual space are uncommon, typically manifesting as painless, gradually enlarging masses, usually not exceeding 3 cm in diameter. These cysts can resemble various conditions due to their clinical presentation, with a relatively low occurrence rate in the oral cavity, accounting for about 1.6% of all dermoid cysts. (2) Methods: We present the case of a 17-year-old female with a giant dermoid cyst involving the submental, sublingual, and lingual areas, undiagnosed for several years. Diagnosis was achieved through MRI and fine-needle aspiration,

Comparative Assessment of Orofacial Myofunction, Strength, and Quality of Life in Children With and Without Anterior Open Bite: The OMBIT Study.

PURPOSE: Anterior open bite (AOB) is a multifactorial malocclusion frequently associated with nonnutritive sucking habits (NNSHs) and orofacial dysfunctions. However, little is known about the specific orofacial strength and endurance profile in affected children. This study evaluated myofunctional history, orofacial myofunctional status, orofacial strength, and oral health-related quality of life (OHRQoL) in children in the mixed dentition stage with AOB, compared to controls without AOB. METHOD: Sixteen children with AOB in the mixed dentition stage (Mage = 8.2 years) and 16 age- and gender

Development and Validation of an Extraoral Condyle Locating Device and Its Effect on Low-Level Laser Therapy in Patients With Skeletal Class II Malocclusion: Protocol for a 2-Phase Randomized Controlled Trial.

BACKGROUND: Precise localization of the mandibular condyle is essential for extraoral therapeutic procedures such as low-level laser therapy (LLLT) in patients with skeletal class II malocclusion undergoing myofunctional appliance therapy. Conventional localization methods rely on palpation and visual estimation, which are operator dependent and may compromise reproducibility and treatment outcomes. To address this limitation, a spectacle-mounted extraoral condyle locating device (EOCLD) has been developed to facilitate accurate and consistent condylar targeting. OBJECTIVE: This study aims to

Cephalometrics and oral myofunctional impairment.

It is essential that the practitioner determine the factors that are etiologically operative in oral myofunctional therapy in order to establish effective methods of intervention. Of these methods, cephalometrics and facial analysis are especially valuable in revealing and differentiating open bite syndrome. They are discussed here along with procedures for their use.

Sensory/motor therapy for the treatment of oral dyskinesia. A new approach to the treatment of oromyofunctional disorders with the use of tactile cuing handheld exercisers.

Persons with oral myofunctional disorders suffer from multiple risk factors for impairments. A craniofacial malformation may be responsible for both behavioral as well as structural modifications. This includes the effect of appearance on self concept and behavior (Case, 1988). Structural modifications as well as negative oral habits include: an open mouth resting posture; mouth breathing; low forward tongue carriage; forward articulatory tongue placement (linguadental/lingua alveolar); abnormal lip, tongue and facial muscle movements during swallowing; reduced upper lip length and mobility; o

Effect of a 1-week intense myofunctional training on obstructive sleep apnoea in children with Down syndrome.

BACKGROUND: Obstructive sleep apnoea (OSA) is common in children with Down syndrome (DS), yet difficult to treat. As muscular hypotonia of the upper airway may cause OSA and is also common in DS, we tested whether intense myofunctional therapy improves OSA in children with DS. PATIENTS AND METHODS: Forty-two children underwent cardiorespiratory sleep studies immediately before and after a 1-week intensive training camp consisting of three daily 45 min sessions of myofunctional exercises according to Padovan. Primary outcome was the mixed-obstructive-apnoea/hypopnoea index (MOAHI), secondary o

A Multidisciplinary Approach to Swallowing Rehabilitation in Patients with Forward Head Posture.

(1) Background and Objectives: The forward head posture (FHP) is characterized by increased extensions of upper cervical vertebrae and flexion of the lower cervical vertebrae and upper thoracic regions, associated with muscle shortening. The compressive loading on the tissues in the cervical spine negatively impacts suprahyoid and infrahyoid muscles and generates increased tension of the masticatory muscles. The tongue has relations with the suprahyoid and the infrahyoid muscles. The pattern of swallowing evolves gradually from birth to the age of four. If this developmental transition does no

Nonsurgical treatment: swallowing rehabilitation.

This article provides an overview of nonsurgical and nonpharmacologic treatments for oropharyngeal dysphagia. The speech-language pathologist (SLP) is the primary member of the swallowing management team who will provide this type of dysphagia management. The primary focus of the SLP for dysphagia management is first to eliminate or reduce aspiration risk, as well as to improve or restore swallowing function. Ultimately, the management plan will depend on the physiologic underpinnings of the disorder and patient variables such as cognition, motivation, and ability to attend therapy sessions or
