Endoscopic transcanal myringoplasty for anterior tympanic membrane perforation.

Anterior tympanic membrane perforations constitute a surgical challenge because of the anatomy of the anterior wall of the external auditory canal and the specific physical properties of the anterior tympanic membrane. The author reports an endoscopic transcanal myringoplasty technique with anterior fixation of the graft onto the anterior wall of the external auditory canal, avoiding canaloplasty in the majority of cases, detachment of the annulus in the anterior angle and medialization of the graft in the middle ear.

Pediatric partial gelfoam myringoplasty with ventilation tube placement.

OBJECTIVE: For children with a history of persistent Eustachian tube dysfunction (ETD) or otitis media with effusion presenting with recurring tympanic membrane (TM) perforation, surgeons must often balance the treatment goals of correcting the perforation and maintaining ventilation to the middle ear to prevent future perforation formation. A partial gelfoam myringoplasty with ventilation tube placement is a previously unreported procedural option for addressing these goals. The objective of this study is to describe the partial myringoplasty technique and report preliminary outcome data for

Hearing loss following myringoplasty - implications for informed consent.

BACKGROUND: There are many reports of operations performed to successfully close ear drum perforations. Hearing deterioration after myringoplasty is not a widely published topic. This paper presents an audit of this complication. METHODS: A six-year retrospective analysis of a series of myringoplasty operations was performed using electronic patient records. Patients with post-operative hearing loss were identified and those with hearing loss greater than 10 dB were further scrutinised. RESULTS: Out of 187 patients who underwent myringoplasty procedures, 44 (23.53 per cent) experienced a red

Equine versus bovine pericardium in transmeatal underlay myringoplasty.

OBJECTIVES: Many different grafting materials have been proposed in myringoplasty. The aim of this study was to evaluate the results obtained in transmeatal underlay myringoplasty using bovine and equine pericardium. The results were compared with those obtained by using autologous temporalis fascia. METHODS: The study group consisted of 52 patients with tympanic perforation. Twenty-nine patients were randomly selected for treatment with bovine pericardium and 23 for equine pericardium. A group of 14 patients was treated with autologous temporalis fascia. RESULTS: Closure of the perforation

Homograft myringoplasty-controlled button technique: a preliminary report.

The successful outcome of homograft myringoplasty in small and medium perforation of the tympanic membrane was only 44.12 per cent. A better result was obtained in the small perforations (59.10%) compared with the medium ones (16.77%). We conclude that the procedure should be appropriate for repairing the incomplete healing of the tympanic membrane following standard tympanoplasty and removal of the tympanostomy tube. Furthermore this procedure is simple, safe, cost-effective and can be done in the out-patient department.

Sometimes less is more: experience with endoscopic myringoplasty.

A retrospective review of the outcomes of patients who underwent endoscopic myringoplasties in our institution was conducted. The aim was to highlight our results with this procedure. The database of patient records was manually checked, and the patients who had undergone Endoscopic Myringoplasties were identified, and their demographics, admitting notes, operating notes, and discharge summaries were reviewed. Graft failure was considered if the patient had a perforation in the graft during the outpatient follow-up. The information was compiled, and basic statistics were derived. A total of 31

Myringoplasty in children: retrospective analysis of 60 cases.

Myringoplasty or type 1 tympanoplasty aims the restoration of the anatomic integrity of the tympanic membrane; it's a very common surgery in otology. The objective was to evaluate the anatomic and functional results of this surgery in children using the retro-auricular approach. Sixty young patients with diagnosis of simple tympanic perforation were evaluated; these patients underwent myringoplasty by a retro-auricular approach (underlay technique) between November 2010 and May 2013. It's a retrospective evaluation of the anatomic and functional results of theses myringoplasties. Mean age at s

[Myringoplasty: onlay vs. underlay. Review of 460 cases].

A retrospective review was made of 460 myringoplasties performed in our department from 1984 to 1990. In 80%, the perforation closed successfully. There were no differences in site, size, cause, or previous operations. Results were statistically better with the onlay technique than with the underlay technique, particularly in large perforations. Poorer results were obtained in younger patients, a difference that almost reached statistical significance. Hearing improved in 64% of patients with closed perforations and hearing deteriorated in 29% (2.1% complete hearing loss).

Effectiveness of fibronectin in myringoplasty using temporal fascia.

A study of myringoplasty with temporal fascia homograft employing fibronectin was conducted in 14 ears in which the same myringoplasty procedure without fibronectin (1-5 times) had failed to afford closure of tympanic membrane perforation. Closure was obtained in eight ears. These results suggest that fibronectin is effective in myringoplasty using treated temporal fascia homograft.

Myringoplasty: long-term results in adults and children.

This report presents the long-term results 3 years after primary myringoplasties performed with formolized fascia temporalis autografts. The overall closure rate was 87.7% of cases, with improvement in hearing in 67.2% of cases. The best results were achieved with total perforations, and not with partial posterior perforations, contrary to popular opinion. The anterior perforations were the most difficult to close. Anterior angle blunting was avoided by careful technique. Although the results of myringoplasty are clearly better in adults than in children, there is not a determined "frontier ag

Myringoplasty: hearing gain in relation to perforation site.

One hundred perforations of the tympanic membrane with successful myringoplasties have been reviewed. A partially reversible impairment of bone conduction was noted, being more obvious in posterior and subtotal perforations. It was also shown that the site of perforation affects the degree of hearing loss and the degree of subsequent improvement after myringoplasty; marginal and malleolar perforations had a greater hearing loss and less post-operative hearing improvement than central and non-malleolar perforations. It was also shown that posterior perforations had a greater hearing loss than a
