The role of surgical therapy in the management of premature ejaculation: a narrative review.

BACKGROUND AND OBJECTIVE: Premature ejaculation (PE) is a common sexual disorder among male adults and negatively impacts a man's sexual life. Currently, the mainstay treatment of PE is still medical therapy which has drawbacks among patients as a consequence of side effects. Despite the new definitions, the evolution of medical therapy, and the consensus for the management of PE, it remains challenging to treat for many clinicians especially when medical treatment fails. However, the International Society for Sexual Medicine (ISSM) and the American Urological Association (AUA) guidelines igno

Narrative review of current and emerging pharmacological therapies for nonalcoholic steatohepatitis.

Nonalcoholic steatohepatitis (NASH) is the most common cause of chronic liver disease today, and it has now emerged as the leading etiology of end-stage liver disease requiring liver transplantation. It is a progressive form of non-alcoholic fatty liver disease which can not only progress to cirrhosis of liver and hepatocellular carcinoma (HCC), but is associated with increased cardiovascular risks too. Despite all the advances in the understanding of the risk factors and the pathogenetic pathways involved in the pathogenesis and progression of NASH, an effective therapy for NASH has not been

A narrative review on antimicrobial therapy in septic shock: updates and controversies.

PURPOSE OF REVIEW: Antibiotics are an essential treatment for septic shock. This review provides an overview of the key issues in antimicrobial therapy for septic shock. We include a summary of available evidence with an emphasis on data published in the last few years. RECENT FINDINGS: We examine apparently contradictory data supporting the importance of minimizing time to antimicrobial therapy in sepsis, discuss approaches to choosing appropriate antibiotics, and review the importance and challenges presented by antimicrobial dosing. Lastly, we evaluate the evolving concepts of de-escalatio

Physical therapies for the conservative treatment of the trigger finger: a narrative review.

Trigger finger (TF) disorder is a sudden release or locking of a finger during flexion or extension. Treatments for this disease are conservative and surgical, including NSAIDs, hand splints, corticosteroid injections, physical therapies and percutaneous or open surgery. However, the effectiveness about the optimal treatment of TF is still in lack of evidence. The aim of this study is to investigate the effectiveness of physical therapies as conservative treatment for trigger finger. A comprehensive literature search of the MEDLINE (via PubMed), Cochrane Library Databases and PEDro databases h

Electroconvulsive therapy (ECT) in bipolar disorder: A narrative review of literature.

In many countries including India electroconvulsive therapy (ECT) is frequently used to treat different phases of bipolar disorder. The response to ECT is impressive in mania, depression and in mixed affective states. Preliminary evidence also suggests benefit from maintenance ECT in bipolar disorder. However, most of the literature on efficacy and adverse effects comes from case series, retrospective reports and open trials - controlled trials have been few and far between. Official guidelines recommend the use of ECT only when there is a dire emergency or when all other options have been exh

Narrative therapy: a psychotherapeutic approach in the treatment of adolescents with Asperger's disorder.

TOPIC: The conundrum of how to carry out psychotherapy with an adolescent whose primary currency of information exchange is visual as opposed to linguistic, and who is unable to develop a unified base of knowledge and hence generalize concepts, has confronted many mental health nurses in their encounters with individuals with Asperger's disorder. At the currently estimated escalating rate of occurrence of 1 to every 100 individuals having an autism spectrum disorder, and the estimate that 80% of these people have average or above intelligence, it makes sense that Asperger's disorder is frequen

Narrative therapy group intervention for nurses' moral resilience: A feasibility study.

BackgroundEthically complex care exposes nurses to persistent moral distress, harming well-being, integrity, and care quality. Moral resilience offers a targeted response, but intervention evidence is scarce. Because narrative therapy enables meaning-making and identity re-authoring, it may cultivate moral agency and integrity.AimTo develop and refine a narrative therapy-based group intervention for nurses' moral resilience and to examine its feasibility and acceptability in a pilot study, with exploratory assessment of outcome trends.DesignA preliminary group intervention program aimed at sup

Fluid therapy in adults having non-cardiac surgery: A narrative review.

BACKGROUND: Adequate intraoperative fluid therapy is essential, as both uncorrected fluid loss and excessive fluid administration are associated with increased complications. However, current practice varies widely. In this narrative review, we examine current concepts of intraoperative fluid therapy in adults having non-cardiac surgery, focusing on fluid type, volume of fluid, and fluid administration strategy. RESULTS: Balanced crystalloids, compared to unbalanced crystalloids, more closely resemble the body's natural electrolyte composition. However, moderate intraoperative volumes of 0.9%

Exploring medical gas therapy in hemorrhagic stroke treatment: A narrative review.

Hemorrhagic stroke (HS) is a neurological disorder caused by the rupture of cerebral blood vessels, resulting in blood seeping into the brain parenchyma and causing varying degrees of neurological impairment, including intracerebral hemorrhage (ICH) and subarachnoid hemorrhage (SAH). Current treatment methods mainly include hematoma evacuation surgery and conservative treatment. However, these methods have limited efficacy in enhancing neurological function and prognosis. The current challenge in treating HS lies in inhibiting the occurrence and progression of secondary brain damage after blee

Narrative therapy vs. cognitive-behavioral therapy for moderate depression: empirical evidence from a controlled clinical trial.

BACKGROUND: Systematic studies of the efficacy of Narrative Therapy (NT) for depression are sparse. OBJECTIVE: To evaluate the efficacy of individual NT for moderate depression in adults compared to Cognitive-Behavioral Therapy (CBT). METHOD: Sixty-three depressed clients were assigned to either NT or CBT. The Beck Depression Inventory-II (BDI-II) and Outcome Questionnaire-45.2 (OQ-45.2) were used as outcome measures. RESULTS: We found a significant symptomatic reduction in both treatments. Group differences favoring CBT were found on the BDI-II, but not on the OQ-45.2. CONCLUSIONS: Pre- t

Otoferlin Gene Therapy: Narrative Review for Ear and Hearing Special Issue: Gene Therapy for Hearing Loss.

In the last decade, gene therapy has shown promise in treating many genetic disorders. Hereditary hearing impairment represents one of the next promising frontiers. Many initial development efforts have focused on hearing impairment caused by mutations in the otoferlin gene (OTOF), as these mutations cause profound deafness in the setting of preserved auditory outer hair cells, which often remain for many years after birth. Animal models have shown that the target cells for gene delivery, the inner hair cells, are also present and viable, and they reside in a structurally intact organ that is

Beyond traditional therapies: a narrative overview of antimicrobial photodynamic therapy in combating bacterial and viral infections.

The increasing prevalence of multidrug-resistant bacteria and emerging viruses urgently requires new antimicrobial approaches. Photodynamic therapy (PDT) combining light, photosensitizers, and molecular oxygen generates reactive oxygen species (ROS) that disrupt microbial cells, including in biofilms. In this review, we present recent clinical and preclinical evidence for PDT's effectiveness against both Gram-positive and Gram-negative bacteria, oral/dental pathogens, and viral structures. We highlight applications such as periodontal therapy, root canal disinfection, denture stomatitis, and s

Antibiotic Therapy in the Critically Ill with Acute Renal Failure and Renal Replacement Therapy: A Narrative Review.

The outcome for critically ill patients is burdened by a double mortality rate and a longer hospital stay in the case of sepsis or septic shock. The adequate use of antibiotics may impact on the outcome since they may affect the pharmacokinetics (Pk) and pharmacodynamics (Pd) of antibiotics in such patients. Acute renal failure (ARF) occurs in about 50% of septic patients, and the consequent need for continuous renal replacement therapy (CRRT) makes the renal elimination rate of most antibiotics highly variable. Antibiotics doses should be reduced in patients experiencing ARF, in accordance wi

Advances in Gene Therapy for Age-Related Macular Degeneration: A Narrative Review.

Age-related macular degeneration (AMD) is the most common cause of blindness and vision impairment in individuals over 60 years of age in the United States (US). Despite this, current treatment options have limitations related to drug efficacy and durability. Gene therapy provides a potential solution by providing a more durable and longer- acting treatment option that can decrease treatment burden and improve long-term visual outcomes. This review presents the current treatment approaches, routes of administration, and vectors being investigated for gene therapy delivery in AMD. It also provi

Chimeric antigen receptor (CAR) T-cell therapy in dermatology: A narrative review.

Chimeric antigen receptor (CAR) T-cells are autologous T-cells genetically engineered to express an antigen receptor that can recognise and kill cells expressing that target antigen. Originally developed for refractory or relapsed B-cell and plasma cell malignancies, CAR T-cell therapy is now being explored as a promising treatment for B-cell-mediated autoimmune diseases. CAR-T cells produce 'deep' B-cell depletion and cause 'resetting' of the immune system, thereby achieving long-lasting remissions and potentially even 'cure', addressing some major limitations of current immunosuppressive and

Palliative Electroconvulsive Therapy in Serious Illness: A Narrative Review and Conceptual Framework.

CONTEXT: Severe depression, catatonia, psychosis, and agitation can become major sources of suffering in serious illness, impairing communication, decision-making, and quality of life (QOL). Although electroconvulsive therapy (ECT) may offer rapid relief of these syndromes, its role as a palliative intervention remains poorly defined. OBJECTIVES: To review the literature relevant to palliative electroconvulsive therapy (PECT), clarify how the term has been conceptualized, and examine the clinical contexts in which ECT may serve palliative goals in serious illness care. METHODS: We conducted

Narrative processes in schema therapy: Theoretical background, clinical methods, and case illustrations.

OBJECTIVE: To develop a mechanism-oriented framework that specifies how narrative processes operate within schema therapy, detailing a core sequence of change (activation of emotionally salient autobiographical memories, shift in narrative position, emergence of corrective internal voices, and reconsolidation of meaning) through which narrative-based interventions support identity-level change in individual and group settings. METHODS: This paper adopts a theory-building approach combining a narrative review with an interpretive qualitative analysis of clinical case illustrations from schema
