The role of nasal obstruction in obstructive sleep apnea syndrome.

In summary, there is much evidence in the literature that nasal obstruction causes obstructive sleep apnea, usually of mild to moderate degree, in many individuals. In addition, it is a contributory factor in individuals with severe obstructive sleep apnea. This appears to occur because of its contribution to upper airway resistance which directly or indirectly results in a reduction in intraluminal pressure during inspiration and collapse. Nasal obstruction via its influence on the so-called nasal-pulmonary reflex, may also result in alveolar hypoventilation. In patients who have symptoms con

Evaluating Severity and Trends of Nasal Obstruction Following Mohs Reconstruction of Nasal Subunits.

BACKGROUND: Although long-term complications are uncommon following Mohs micrographic surgery (MMS), some studies suggest postoperative nasal airway obstruction (NAO) is underreported. We aimed to evaluate severity and impacting factors for postoperative NAO. METHODS: This is a survey study and retrospective review. The Nasal Obstruction Symptom Evaluation (NOSE) scale was used for clinically significant changes in pre- and postoperative nasal breathing secondary to factors related to reconstruction after MMS. Subset analysis was performed. RESULTS: The survey response rate was 51.4%. There

Congenital nasal pyriform aperture stenosis: a rare cause of neonatal nasal airway obstruction.

Congenital nasal pyriform aperture stenosis (CNPAS) is a rare cause of nasal airway obstruction that clinically mimics choanal atresia in a neonate. The differentiation between the two is very important as the management of the two conditions is different. Timely recognition is important to prevent fatal outcome. CNPAS may present as an isolated condition or with associated craniofacial anomalies. Despite typical findings of CNPAS being present on cross-sectional imaging, this condition is commonly overlooked, probably because of a lack of familiarity with the normal morphological features of

The Role of Nasal Endoscopy in the Preoperative Evaluation of Nasal Airway Obstruction.

OBJECTIVE: To examine the prevalence and nature of nasal endoscopic findings in patients referred for structural nasal obstruction, and analyze how such findings influence the preoperative evaluation or operative plan. STUDY DESIGN: Cross-sectional study. SETTING: University-based academic otolaryngology practice. METHODS: Nasal endoscopy was performed by a single surgeon and the exam findings were documented. Patient demographics, variables in the patient history, Nasal Obstruction Symptom Evaluation scores, and an Ease-of-Breathing Likert Scale were tested for associations with findings o

Measuring Nasal Airway Resistance to Personalize Surgery for Nasal Obstruction in OSA Patients.

Objective: This study aimed to measure nasal airway resistance (NAR) in obstructive sleep apnea (OSA) patients with nasal obstruction using active anterior rhinomanometry (AAR) and to evaluate whether NAR can predict the indication to include septoplasty as an additional procedure alongside drug-induced sleep endoscopy (DISE) and inferior turbinoplasty. Methods: We performed a retrospective observational study in OSA patients with nasal obstruction. According to nasal endoscopy and CT findings, patients were planned for either DISE with inferior turbinoplasty alone or DISE with inferior turbin

Comparison of different nasal septal areas between patients with and without nasal obstruction.

OBJECTIVES: We intended to investigate the deviation of septal swell body, perpendicular plate, septal spur and vomer in patients with and without nasal obstruction. METHODS: We compared the deviation of these septal areas in computed tomography scans of patients scheduled for nasal surgical procedures (cases) and of patients without clinically relevant nasal obstruction (controls). RESULTS: Septal swell body was similarly deviated between 56 cases (median value: 6.5 mm) and 56 controls (6.4 mm; p > 0.2). Septal spur was more deviated in cases (5.6 mm) than in controls (4.7 mm; p < 0.001). T

Nasal airway obstruction in patients with cleft lip nasal deformity: A systematic review.

BACKGROUND: Cleft lip nasal deformity (CLND)-associated nasal airway obstruction (CL-NAO) may be inadequately characterized, with its functional implications subsequently underappreciated and neglected. The purpose of this systematic review is to (1) summarize the available assessment results in CL-NAO, (2) evaluate the reliability of current assessment tools, and (3) identify ongoing gaps and inconsistencies for future study. METHODS: A systematic search of the MEDLINE, EMBASE, and Scopus databases was performed for articles studying CL-NAO. Articles focusing on noncleft populations or surgi

A comparison of an absorbable nasal implant versus functional rhinoplasty for nasal obstruction.

PURPOSE: An absorbable nasal implant for the treatment lateral nasal wall collapse was approved for use in patients with nasal obstruction. It remains to be seen whether this treatment is equivalent to open techniques for the treatment of nasal valve incompetence from collapsibility. MATERIALS AND METHODS: Two groups were analyzed for the study. One group had surgery which included the implant, septoplasty, and inferior turbinate submucous reduction and the other group had a variety of functional rhinoplasty techniques for lateral wall insufficiency in addition to septoplasty and inferior tur

Relationship between nasal septum morphology and nasal obstruction symptom severity: computed tomography study.

INTRODUCTION: The impact of the nasal septum morphology on the severity of obstruction symptoms has not been fully explored. OBJECTIVE: This study aimed to investigate whether the morphology of the deviated nasal septum assessed by computed tomography may explain nasal obstruction severity. METHODS: The study included 386 patients who were referred to the computed tomography examination of the paranasal sinuses. Patient selection criteria were the absence of facial anomalies, facial trauma, nasal surgery, and sinonasal tumors. Computed tomography images were used to estimate deviated nasal s

The spontaneous course of nasal obstruction in patients with normal nasal airway resistance.

Sixty-seven patients, candidates for septoplasty in the years 1976-1978 because of nasal obstruction and deviated nasal septum, but not having surgery because their nasal airway resistance was normal, were reviewed in 1982 and again in 1989. Between 1982 and 1989, 6% of the patients became free of their respective nasal complaints, and 18% of their nasal obstruction, the corresponding overall figures for the period between 1976 and 1978, and 1989, being 20 and 36%. In fact, the majority of the patients were suffering from vasomotor rhinitis and because in so many cases nasal obstruction tends

Development of the Nasal Obstruction Disability Index (NODI).

PURPOSE: The aim of this study was to develop a new scale that assesses the subjective severity of nasal obstruction from functional and psychosocial perspectives, independent of etiology. METHODS: A 27-question prototype scale evaluating nasal obstruction from a functional and psychosocial perspective was created by three expert otolaryngologists. This scale was applied to 203 patients who presented to our clinic with nasal obstruction and to a control group consisting of 135 people without nasal obstruction, and the significance of the scale was tested. Validity, reliability, and factor ana

Newborn nasal obstruction: Rare anatomical causes to consider.

Neonatal nasal obstruction, though rare, can be life-threatening due to the obligate nasal breathing pattern in newborns. Even partial obstruction may compromise ventilation and feeding. This study aims to describe rare anatomical causes of neonatal nasal obstruction, highlighting their clinical presentation, diagnostic evaluation, and management. A retrospective review was conducted of four neonates with respiratory distress who were diagnosed with congenital nasal malformations through endoscopic examination and craniofacial imaging. The identified cases included bilateral choanal atresia in

[Nasal obstruction: Odontogenic cysts in 4 brachycephalic dogs].

OBJECTIVE: Odontogenic cysts may be of developmental or inflammatory origin. They are frequently observed in brachycephalic dogs. Due to their expansive growth, cysts in the maxilla may extend into the nasal cavity, obstruct the nasal airway and cause nasal discharge. Epithelial cysts may lead to a comparable clinical picture. A new endonasal endoscopic intervention for the removal of these cysts is described. MATERIAL AND METHODS: Four brachycephalic dogs (Boxer, Chihuahua, French bulldog, Pug) with unilateral cysts obstructing the nasal cavity were included in the study. The animals underwe

Position paper on nasal obstruction: evaluation and treatment.

Nasal obstruction (NO) is defined as the subjective perception of discomfort or difficulty in the passage of air through the nostrils. It is a common reason for consultation in primary and specialized care and may affect up to 30%-40% of the population. It affects quality of life (especially sleep) and lowers work efficiency. The aim of this document is to agree on how to treat NO, establish a methodology for evaluating and diagnosing it, and define an individualized approach to its treatment. NO can be unilateral or bilateral, intermittent or persistent and may be caused by local or systemic

Craniofacial Deviations in the Children With Nasal Obstruction.

Nasal obstruction mainly caused by adenoid hypertrophy in children affects the craniofacial growth and development process, and the craniofacial deviations and/or differences reported in the children are very similar to those in the adults with obstructive sleep apnea syndrome (OSAS). The authors aimed to look for relationships of the linear craniofacial dimensions in the children suffering from nasal obstruction with age, degree of clinical nasal obstruction score (CNOS), and relative size of the adenoid mass within the nasopharynx in their study.Fifty-five children suffering from nasal obstr
