Harm reduction in the hospital: An overdose prevention site (OPS) at a Canadian hospital.

BACKGROUND: Substance use management in hospitals can be challenging. In response, a Canadian hospital opened an overdose prevention site (OPS) where community members and hospital inpatients can inject pre-obtained illicit drugs under supervision. This study aims to: (1) describe program utilization patterns; (2) characterize OPS visits; and (3) evaluate overdose events and related outcomes. METHODS: A retrospective chart review was completed at one hospital in Vancouver, Canada. All community members and hospital inpatients who visited the OPS between May 2018 and July 2019 were included. C

A wound care and immunization needs assessment for participants of a mobile syringe services program in Austin, TX.

BACKGROUND: People who inject drugs (PWID) are subject to injection-related harm, including skin and soft tissue infections. Syringe services programs (SSPs) provide sterile syringes, disposal of used syringes, and other supportive services. Given their contact and credibility with PWID, SSPs could facilitate triage and treatment of wounds and access to immunizations for communicable diseases. OBJECTIVE: This work aimed to assess wound care and immunization needs among participants accessing mobile SSP services in Austin, TX. METHODS: A 21-item mixed-methods survey was created to assess freq

Implementation and first-year operating costs of an academic medical center-based syringe services program.

BACKGROUND: Syringe services programs (SSPs) remain highly effective, cost-saving interventions for the prevention of blood-borne infections among people who inject drugs. However, there have been restrictions regarding financial resources allocated to these programs, particularly in the US South. This study aimed to provide cost data regarding the implementation and first-year operations of an academic-based SSP utilizing fixed and mobile strategies, including the integration of onsite wound care. METHODS: We conducted a micro-costing study that retrospectively collected detailed resource ut

Analytical findings in used syringes from a syringe exchange program.

BACKGROUND: Since it's first implementatation in 1984, Syringe Exchange Programs (SEP) are a critical component of harm reduction interventions among people who inject drugs.. The aim of this work was to use a scientific analytical approach to obtain drug use information through the analysis of the content of used syringes. METHODS: 357 syringes were collected in New York City and submitted to qualitative analysis. Screening analysis was performed by gas chromatography mass spectrometry (GC-MS) and confirmatory analysis by liquid chromatography quadrupole time of flight mass spectrometry (LC-

The dynamics of the human immunodeficiency virus epidemics in the south of Brazil: increasing role of injection drug users.

Changes in human immunodeficiency virus (HIV) infection among injection drug users (IDUs) are provoking a shift in the pattern of the HIV/AIDS epidemic in some regions of Brazil. IDUs living in 2 cities (Porto Alegre and Itajaí) in southern Brazil were contacted, interviewed, and tested for HIV. In 1998, 187 IDUs were surveyed and, in 2000, 352 IDUs. In Porto Alegre, HIV seroprevalence, age, and duration of injection history increased significantly over time. In contrast, a trend toward decline was observed in Itajaí. Homelessness, joblessness, and prison incarceration, as well as lifetime nee

Harm reduction actions as a health care construction process at a public health center in Rosario, Argentina.

Even faced with serious numbers of human immunodeficiency virus (HIV) and other infections among injection drug users (IDUs), Argentina lacks large-scale harm reduction programs at national, state, and local levels. We describe the strong relationship of IDUs with the HIV/acquired immunodeficiency syndrome epidemic in Rosario. We discuss a 2-year-long ongoing experience of drug-related harm reduction actions developed at a public health center, which began as a response to a spontaneous demand for needles and syringes and became a permanent and sustainable set of health actions for a group of

Risky behaviors for HIV infection among male incarcerated injection drug users in Taiwan: a case-control study.

Taiwan has experiencing an outbreak of HIV infection among injection drug users (IDUs) since 2003. A case-control study was conducted to determine the behavioral characteristics associated with HIV infection among IDUs. The study identified 120 inmates with HIV infection and 121 matched HIV-negative controls from prisons. Data were collected by face-to-face interviews. The study found 48.1% of IDUs shared needles, less than a half of IDUs reported sexual intercourse before incarceration and only 19.7% used condoms. The multivariate logistic regression analysis, adjusted for years of heroin inj

The effect of syringe exchange use on high-risk injection drug users: a cohort study.

OBJECTIVE: To determine whether syringe exchange program use is associated with cessation of syringe sharing among high-risk injection drug users. DESIGN AND METHODS: Between 1992 and 1996, street-recruited injection drug users were interviewed and received HIV testing and counseling semi-annually, as part of a dynamic cohort study. We examined a cohort of 340 high-risk injection drug users for whom two observations, 6-months apart, were available and who reported syringe sharing at the first interview. Multivariate logistic regression analysis was performed to determine the relationship betw

Time for tailored harm reduction focus on new initiates to injecting drug use.

The risk of drug overdose and disease acquisition is highly elevated for those recently initiated into injecting drug use. Despite this, there are few harm reduction interventions developed specifically for new initiates. Further, international technical documents have historically provided little guidance on how to appropriately meet the needs of what is an inexperienced and difficult to engage sub-population. However, the recently updated operational guide for Needle and Syringe Programmes for People Who Inject Drugs from the World Health Organization (WHO), stresses that reaching new initia

Optimal provision of opioid agonist therapy (OAT) and needle and syringe programmes (NSP): a realist evaluation.

BACKGROUND: Understanding the factors that hinder or support engagement with Opioid Agonist Therapy (OAT) and Needle and Syringe Provision (NSP) is essential for increasing retention and reducing drug-related harm. This study comprised a multimethod realist evaluation to develop a theory of the factors influencing service provision, focusing on access, engagement, retention, and successful exit (clients outcome met) from services. DESIGN: Phase 1 involved an online survey of UK drug and alcohol service commissioners, focus groups with interest holders, and a qualitative systematic review to b

Understanding drug-related harms as risk-amplifying loops among people who inject drugs in Sweden.

BACKGROUND: Reducing risks and harms among people who inject drugs by, for example, Needle and Syringe exchange Programs (NSP) may be impeded in practice by, for example, policy restrictions, particularly in settings like Sweden where a zero-tolerance drug strategy prevails. In practice, risks and harms are produced through an interplay of multiple mutually reinforcing factors. Moreover, risk management strategies may constitute risks per se and generate new risks, potentially intensifying overall harm. This study aimed to increase our understanding of how such risks are generated in the lives

The public health impacts of supervised injection sites in Canada: Moving beyond social acceptability and impacts on crime.

Canada has been a pioneer in adopting a harm reduction approach to address risks associated with drug use for people who inject drugs. Today, Canada is home to 39 supervised injection sites spread throughout the country. The scientific literature demonstrates, unequivocally, that these sites have numerous health benefits for people who inject drugs, namely by decreasing risks of blood-borne diseases, overdose, and mortality. Yet, a lack of clear guidelines on optimal locations for the implementation of such sites and NIMBYISM ("Not In My Back Yard") have been stumbling blocks for planned and o

Monitoring for fentanyl within Australian supervised injecting facilities: Findings from feasibility testing of novel methods and collaborative workshops.

BACKGROUND: Australia is yet to see widespread fentanyl-contaminated heroin, despite the established presence of fentanyl in other countries. International mortality trends alongside a local cluster of fentanyl-related deaths prompted interest in developing methods to monitor for fentanyl and other potentially harmful novel psychoactive substances (NPS) in Australia. METHODS: We tested novel methods to monitor for fentanyl and other NPS. From 2017-2021, clients from supervised injecting facilities (SIFs) in Melbourne and Sydney, Australia, contributed urine screens (UDS) with BTNX Rapid Respo

A snapshot of injecting drug consumption from the analysis of used syringes within the Medically Supervised Injecting Centre in Sydney, Australia.

INTRODUCTION: The administration of illicit drugs by injection is associated with considerable harm, including an increased risk of overdose. The chemical analysis of used syringes can enhance knowledge on injecting drug consumption beyond traditional data sources (self-report surveys). This additional information may be useful during significant global events like the COVID-19 pandemic. This study aimed to examine a snapshot of the drugs injected at the Medically Supervised Injecting Centre (MSIC) in Sydney, Australia, in 2019-2020. METHODS: Used syringes were collected from MSIC across thre

Modeling the impact of interventions during an outbreak of HIV infection among people who inject drugs in 2012-2013 in Athens, Greece.

BACKGROUND: A large HIV outbreak in People Who Inject Drugs (PWID) occurred in Athens, Greece in 2011-2013. In response, opioid substitution treatment (OST) and needle and syringe programs (NSP) were scaled-up and a seek-test-treat program was introduced in mid-2012. We aim to assess the impact of these interventions. METHODS: A mathematical model of HIV transmission among PWID was calibrated to data available over time (2009-2013) on HIV prevalence, NSP/antiretroviral treatment (ART) coverage and high-risk injection. A combined interventions scenario, including decrease in high-risk injectio
