Papillary renal neoplasm with reverse polarity: A case report.

Papillary renal neoplasm with reverse polarity is a rare, newly identified subtype of papillary renal cell carcinoma believed to originate from the distal renal tubules. It is characterized by distinct histomorphological features and an associated immunophenotype. Contrast-enhanced scans usually do not exhibit a rapid wash-in and wash-out enhancement pattern, and this tumor is generally associated with a favorable overall prognosis. This study reports a case of papillary renal neoplasm with reverse polarity in a male patient in his 50 s who was admitted with recurrent left lumbar pain and disc

Uncommon presentation of a giant psoas muscle lipoma: a case report and brief literature review.

Giant retroperitoneal lipomas, particularly within the psoas muscle, are a rare condition. We herein present one such case of a 45-year-old Italian man and a literature review. There are only two case reports published in the literature, thus posing challenges for the appropriate diagnosis and treatment. Our patient was admitted to the emergency department with colicky abdominal pain. Computerized tomography (CT) with contrast enhancement revealed kidney stones and a 19.5×13.6×18 cm mass of adipose tissue with septa located in the right retroperitoneum, in close continuity with the right psoas

Adipose tissue dysfunction in cancer cachexia.

Cancer cachexia is a complex disorder that is driven by inflammation and metabolic imbalances, resulting in extreme weight loss. Adipose tissue, a main player in cancer cachexia, is an essential metabolic and secretory organ consisting of both white adipose tissue (WAT) and brown adipose tissue. Its secretory products, including adipokines and cytokines, affect a wide variety of central and peripheral organs, such as the skeletal muscle, brain, pancreas, and liver. Therefore, a combination of metabolic alterations, and systemic inflammation dysregulation of both anti-inflammatory and proinflam

Contemporary management of lipoblastoma.

PURPOSE: Lipoblastoma is a rare, benign, adipose tissue tumor. We report the largest single institution experience managing these uncommon neoplasms. METHODS: We retrospectively reviewed 32 cases of lipoblastoma entered in the pathology database at our institution between January 1991 and August 2005. We conducted a comprehensive literature review of lipoblastoma and summarized the results of the largest series published. RESULTS: Most patients presented with an enlarging, palpable, firm, nontender, mobile mass. The male-to-female ratio was 1.9:1. The anatomical distribution was trunk (n = 1

PLAG1-Driven Lipoblastomatosis: Diagnostic Insights from a Case Report and Literature Review.

Lipoblastomatosis is a rare benign adipocytic tumour of infancy and early childhood, characterized by rapid growth. Diagnosis is challenging, owing to tumour heterogeneity and overlap with other pediatric soft-tissue neoplasms. PLAG1 rearrangement is the defining molecular hallmark, with an expanding list of fusion partners. We report a case of a 1-year-old child with a firm, nonmobile mass in the right thigh, which on CECT was deep-seated and heterogeneously enhanced. Gross evaluation of the excision specimen demonstrated an ill-circumscribed soft tissue mass. Histopathologically, the tumour

The 'split fat sign' revisited.

This review article revisits the 'split fat sign', reviewing its aetiology and prevalence in benign and malignant soft tissue tumours in both the intermuscular and intramuscular locations. The term 'split fat sign' was first used in 1999, referring to the presence of a rim of fat surrounding neurogenic neoplasms in the intermuscular space on magnetic resonance imaging (MRI). The presence of fat in the intermuscular plane is a normal finding and therefore the 'split fat sign' can be seen in both benign and malignant intermuscular lesions. The term 'split fat sign' has also been used to refer to

Fatty masses of the abdomen and pelvis and their complications.

There are numerous common and rare macroscopic fat-containing masses found in the abdomen and pelvis. These include benign masses, such as lipoleiomyoma, ovarian teratoma, mesenteric teratoma, and lipoma, as well as malignant masses, including liposarcoma and malignant transformation of benign entities. Any mass may become symptomatic due to the development of a complication which may range from ovarian torsion to intussusception to hemorrhage. Imaging plays a vital role in diagnosis and treatment planning when confronted with a symptomatic fat-containing mass.

Understanding the molecular basis of anorexia and tissue wasting in cancer cachexia.

Cancer cachexia syndrome is a major cause of morbidity and mortality in cancer patients in the advanced stage. It is a devastating disorder characterized by nutritional impairment, weakness, and wasting, and it affects treatment success and quality of life. Two major symptoms of cancer cachexia are anorexia and weight loss. Weight loss in cachexia is not reversed through increased food intake, suggesting that anorexia and weight loss in cancer patients are regulated by independent molecular mechanisms. Although the wasting phenotype mostly occurs in skeletal muscle and adipose tissue, other or

Macroscopic Fat in Adrenocortical Carcinoma: A Systematic Review.

OBJECTIVE. The objective of this systematic review was to determine the number and quality of reports of adrenocortical carcinoma (ACC) containing macroscopic fat; this information may inform guidelines for diagnosis and management of ACC. MATERIALS AND METHODS. A comprehensive search of databases of published studies was performed. Two reviewers independently selected original research, case series, or case reports of ACC with macroscopic fat on imaging and extracted data. Risk of bias was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) tool. RESULTS. Three c

Nevus psiloliparus: a distinct fatty tissue nevus.

BACKGROUND: Encephalocraniocutaneous lipomatosis is usually associated with a peculiar type of fatty tissue nevus which represents a smoothly surfaced and hairless lesion involving the scalp. This disorder has so far not been recognized as a cutaneous entity. OBJECTIVE: The purpose of this article is to describe the characteristic features of this nevus and to give it a name. METHOD: From the study of two cases and from a review of the literature we delineate the clinical and histopathological criteria of this disorder for which we propose the term 'nevus psiloliparus'. This name is derived

Sacral parasite conjoined twin.

BACKGROUND: A variant of conjoined twins is one in which one twin is incomplete. CASE: A female infant was born vaginally at 40 weeks' gestation to a healthy primipara. No important abnormalities were noted during prenatal examinations. The infant was fully developed in all external aspects except for a parasitic body conjoined with her sacrococcygeal region. Separated by operation 2 weeks after birth, the parasite contained lower limbs, adipose tissue, muscles, and a bowel sac. Over 4 years of observation, no abnormalities have been found since the operation. CONCLUSION: Obstetricians shoul

Lipoblastomatosis combined with intestinal neuronal dysplasia.

We report the case of a male infant with multifocal lipoblastomatosis and intestinal pseudo-obstruction for whom the pathologic findings of rectal biopsy were consistent with intestinal neuronal dysplasia. Lipoblastomatosis is a benign neoplasm of human fetal white adipose tissue that mainly occurs in infants and children. Intestinal neuronal dysplasia is a disorder that clinically mimics Hirschsprung's disease and that is pathologically characterized by parasympathetic hyperganglionosis of the enteric nervous system. To our knowledge, the combination of lipoblastomatosis and intestinal neuron

[Hybrid laparo-endoscopic access for giant fibrovascular esophageal polyp: a case report].

Fibrovascular polyp is a rare non-epithelial esophageal tumor arising from submucosal layer and consisting of connective and adipose tissue, as well large number of vessels. Large tumors can cause dysphagia, vomiting, chest pain, shortness of breath and/or asthma, while giant neoplasms are potentially life threatening. Despite active introduction of minimally invasive treatment of patients with non-epithelial gastrointestinal tumors, there are still difficulties in surgical treatment of fibrovascular polyps. The patient with a giant fibrovascular esophageal polyp presented with cough, discomfo

[Adrenal myelolipoma, review of the literature in Mexico apropos of two cases].

BACKGROUND: Adrenal myelolipomas (ML) are rare benign neoplasms compound of adipose and myeloid tissue. Clinically they are usually asymptomatic, being diagnosed generally by incident. In Mexico, there are only 32 published cases of ML, these occur between 37 and 65 years, with the male-female ratio being 1:1.1, clinically they present with abdominal or lumbar pain, open surgery being the main surgical approach (89%). CLINICAL CASE: We made a literature review of ML in Mexico and present two clinical cases: a 67-year-old man in followup for diverticular disease and a 40-year-old woman with pa

The morphologic spectrum of uterine PEC-cell associated tumors in a patient with tuberous sclerosis.

Perivascular epithelioid cell (PEC) neoplasms are an unusual group of mesenchymal tumors thought to arise from the PEC, which characteristically expresses melanocytic markers with common coexpression of muscle markers. They show a wide morphologic spectrum and have been described in multiple anatomical locations, including the uterus. We report a 59-year-old woman with tuberous sclerosis, initially diagnosed with a uterine malignant mixed müllerian tumor, in whom the hysterectomy specimen also showed 3 other incidental tumors: a subserosal angiomyolipoma consisting of blood vessels of varying

Myxoid Dermatofibrosarcoma Protuberans of the Vulva: Case Report of a Rare Variant in an Unusual Location, With Unusual Morphologic and Immunohistochemical Features.

Dermatofibrosarcoma protuberans (DFSP) is a low-to-intermediate grade infiltrative dermal neoplasm with a predilection for the trunk and extremities. DFSP in the vulvar region is extremely rare, with fewer than 50 cases reported to date in the literature. The histologic diagnosis of this neoplasm is facilitated by the characteristic storiform pattern of spindle cells with infiltration into the subcutaneous fat in a "honeycomb" pattern. However, morphologic variants including the very rare myxoid DFSP have been recognized that pose significant diagnostic difficulties, especially when they occur

Myolipoma of soft tissue.

Nine cases of a previously undescribed benign soft tissue tumor are reported. They were composed of variable amounts of benign smooth muscle and mature adipose tissue. Patient ages ranged from 28 to 73 years. One was located in the subcutaneous adipose tissue, one in the rectus sheath of the anterior abdominal wall, two within the abdominal cavity and attached to the abdominal wall, two in the inguinal region, and three in the retroperitoneum. Sizes varied between 3.5 and 26 cm and averaged 16 cm in greatest dimension. Two of the retroperitoneal tumors were incidental findings during other ope

Mixed epithelial and stromal tumor of the kidney with prominent peritumoral adipose tissue: a case report radiologically mimicking angiomyolipoma with epithelial cysts.

Mixed epithelial and stromal tumor of the kidney (MEST) is a rare renal neoplasm with biphasic differentiation comprising epithelial and stromal components. It typically appears as a mixed cystic-solid mass on imaging, and the prognosis is favorable in the vast majority of cases. We report a case of MEST with a prominent fat component on imaging: a 54-year-old woman was admitted with a 1-month history of left flank distension and pain. Contrast-enhanced computed tomography (CT) showed a mixed cystic-solid mass at the lower pole of the left kidney, with a prominent low-attenuation fat-density a

CircAMOTL1 promotes adipose lipolysis and browning in cancer cachexia through miR-211-5p-mediated TET2 activation.

Cancer cachexia is characterized by profound adipose tissue loss and metabolic remodeling, yet the regulatory mechanisms driving adipocyte dysfunction remain incompletely understood. Through whole-transcriptome sequencing of human subcutaneous adipose tissue, we identified circAMOTL1 as one of the most significantly upregulated circRNAs in patients with cachexia. circAMOTL1 expression positively correlated with weightloss severity and demonstrated strong diagnostic performance for cachexia. Functional studies revealed that circAMOTL1 promotes lipolysis and white adipose browning in adipocytes
