Nerve conduction studies for carpal tunnel syndrome: essential prelude to surgery or unnecessary luxury?

Although carpal tunnel syndrome is a relatively trivial condition, controversy surrounds the use of nerve conduction studies, and whether they are essential to make the diagnosis, or as a prelude to surgery. This is partly due to the lack of a generally agreed definition of the condition, and failure to recognize that the patient's first priority is rapid relief of symptoms. If nerve conduction studies do not contribute to achieving that aim it would be better not to do them. Supporters of routine preoperative nerve conduction studies ignore their shortcomings, which include lack of standardiz

Nerve conduction studies show no exclusive ulnar or median innervation of the ring finger.

In 2047 hands of 1260 patients referred for an analysis of their brachialgia, distal antidromic sensory conduction was investigated in the median and ulnar nerves. No median sensory nerve action potentials (SNAPs) were obtained from the ring finger (D4) in 257 hands, while ulnar D4 SNAPs were lacking in 38 hands. An obvious lesion of the relevant nerve could be demonstrated in each of these cases. In all remaining cases, in which D4 SNAPs were obtained with both median and ulnar stimulation, it could be excluded that they were due to co-stimulation. These findings are not in agreement with the

Neurogenic impotence: diagnostic value of nerve conduction studies, bulbocavernosus reflex, and heart rate variability.

In a group of 10 neurogenic impotent patients the diagnostic sensitivities of peripheral nerve conduction, bulbocavernosus reflex latency, and heart rate variability have been compared. The bulbocavernosus reflex latency appears as the least affected parameter, while nerve conduction and heart rate variability-measured during hyperventilation and expressed as the percentage of the RR interval variation-show similar diagnostic yields.

Nerve Conduction Studies in Carpal Tunnel Syndrome: Application of Quantitative Methods of Decision Making.

BACKGROUND: Carpal tunnel syndrome (CTS) is a common compressive neuropathy that is diagnosed primarily through clinical symptoms and electrodiagnostic testing. However, the lack of a gold standard and the varied accuracy of nerve conduction studies (NCS) present challenges in diagnosis and treatment decisions. OBJECTIVES: This study aimed to evaluate the efficacy of different measurements of NCS in patients with CTS, analyze the postoperative changes in NCS, and identify factors associated with surgical outcomes. MATERIALS AND METHODS: A total of 565 patients (1077 hands) presenting with CT

Influence of Preoperative Nerve Conduction Studies on the Outcome of Carpal Tunnel Release Surgery.

Nerve conduction study (NCS) is the most common investigative tool used in diagnosing and determining the severity of carpal tunnel syndrome (CTS). We aimed to evaluate the relationship between preoperative NCS and the outcome of patients who underwent carpal tunnel release (CTR). Medical records of patients who underwent CTR from January 2018 to December 2023 were reviewed retrospectively. Demographic, clinical characteristics and NCS reports were extracted. CTS severity was graded according to the Canterbury criteria. The outcome was defined as improved or not improved based on subjective sy

Patients Prefer Ultrasound to Nerve Conduction Studies for the Diagnosis of Carpal Tunnel Syndrome.

BACKGROUND: The net promoter score (NPS) allows analysis of patient satisfaction and preference between treatment and/or diagnostic testing. Electrodiagnostic testing (EDX) and ultrasound (US) are commonly used diagnostic tests for carpal tunnel syndrome. Although EDX is reliable for diagnosing carpal tunnel syndrome (CTS), it can be uncomfortable and inconvenient for patients. We aimed to determine whether patients preferred US or EDX studies for the diagnosis of CTS, using the NPS. METHODS: Seventy-five patients presenting to the clinic for evaluation of CTS complaints who had EDX were pros

Ultrasound guidance may have advantages over landmark-based guidance for some nerve conduction studies.

BACKGROUND: Precise placement of stimulating and recording electrodes is vital when performing nerve conduction studies (NCSs). In this study, we aimed to determine whether ultrasonography (US) was more precise in localizing the superficial radial nerve (SRN), dorsal ulnar cutaneous nerve (DUCN), ulnar nerve (UN) crossing the cubital tunnel, and radial nerve (RN) crossing the spiral groove (SG) compared to conventional techniques. METHODS: Thirty healthy young subjects (15 male) were recruited. Each subject underwent both landmark-based and US-guided NCS. Onset latencies and amplitudes of com

Comparison between nerve conduction studies and current perception threshold test in carpal tunnel syndrome.

AIM OF STUDY: Nerve conduction studies (NCS) only test large myelinated A(alpha) or A(beta) nerve fibers, whereas the current perception threshold (CPT) test has been suggested to evaluate a wide range of nerve fibers (A(beta), A(delta) and C). This study was undertaken to compare CPT and the standard NCS test by Bland's severity scale with the patient-based measurement of symptoms and functional status of the hand by Boston CTS questionnaire assessment. PATIENTS AND METHODS: We performed NCS and CPT on 31 patients (mean age 54.6+/-11.7 years; 31-79 years) with clinical diagnosis of CTS. NCS

Conduction studies along the accessory nerve and follow-up of patients with trapezius palsy.

The accessory nerve was stimulated at the posterior triangle of the neck and responses were evoked simultaneously from the upper, middle and lower part of the trapezius muscle. Sixteen patients were investigated, 10 with trapezius palsy following surgical procedures at the posterior cervical triangle, three with a history suggestive of neuralgic amyotrophy and three of unknown origin. On the unaffected side the latency increases 0.16 ms per 10 mm increase in conduction distance corresponding to a conduction velocity of 63 m/s. Evidence is presented that the upper, middle and lower part of the

Serial nerve conduction studies in carpal tunnel syndrome secondary to rheumatoid arthritis: preliminary study.

Twenty patients with definite rheumatoid arthritis fulfilling the criteria of the American Rheumatism Association were examined for clinical and electrodiagnostic evidence of carpal tunnel syndrome. Of the 40 wrists studied, 8 (20%) had evidence of carpal tunnel syndrome. All wrists were followed up with serial electrodiagnostic, clinical, and laboratory studies at intervals of 4 months over a period of a year. All patients were seen by a rheumatologist and were put on effective anti-inflammatory drug therapy. At each visit serum drug level and sedimentation rate were estimated. Two of the wri

Performance standards of the nerve conduction study technologist.

The primary performance standard of the nerve conduction study technologist is to perform nerve conduction studies (NCS) on patients with trauma or diseases of the peripheral nervous system. However, to be in compliance with various healthcare/medical accreditation agencies, such as Occupational Safety and Health Administration (OSHA), Centers for Disease Control and Prevention (CDC), or The Joint Commission [formerly known as the Joint Commission on Accreditation of Healthcare Organizations (JCAHO)], there are many other important performance objectives that must be met as well. Neuromuscular ultrasound and nerve conduction studies as complementary tools for screening of diabetic peripheral neuropathy.

Diabetic Polyneuropathy (DPN) is a common debilitating complication of Diabetes Mellitus (DM), causing pain which may progress to numbness and motor involvement. DPN may lead to diabetic foot ulcers and possible amputation. Thus, DPN affects the patients' quality of life and poses a financial burden if not managed early. Nerve conduction studies (NCS) have been used as an objective method for diagnosis of DPN. Neuromuscular Ultrasound (NMUS) is a more comfortable tool used to complement NCS in diagnosis of many diseases. The study aimed to investigate NMUS as a possible screening tool for DPN.
