[Epilepsy and Autonomic Nervous System].

Epileptic activity that involves the central autonomic system, including the insular lobe, medial prefrontal cortex, amygdala, hypothalamus, periaqueductal gray, parabrachial complex, nucleus tractus solitarius, and ventrolateral medulla results in seizures with various autonomic manifestations. Some autonomic manifestations suggest localization and lateralization of epileptic foci. The autonomic nervous system modulates cerebral activity under physiological and pathological conditions. Vagus nerve stimulation (VNS) has attracted much attention for treatment of various neurological and psychia

Radiation and the nervous system.

Radiation therapy is widely used for benign and malignant brain tumours as it is effective and well tolerated. However, damage to the surrounding healthy nervous system tissue leads to a variety of complications both in the short term and long term, ranging from mild and self-limiting to irreversible and fatal. Radiation neurotoxicity is due to a combination of early inflammation and oligodendroglial damage followed later by brain tissue necrosis, white matter damage, accelerated vascular disease and the development of secondary tumours. This article explains the basic principles of radiation

Toxocariasis of the Nervous System.

PURPOSE: Toxocariasis is a helminthozoonosis caused by the infection of a human host by the larva of Toxocara spp., predominately involving Toxocara canis and Toxocara cati, which are common nematodes in dogs and cats, respectively. Human transmission occurs through contact with animals or by consumption of food contaminated with parasite's eggs. The purpose of this article is to review the current knowledge regarding human neurotoxocariasis. METHODS: We conducted a systematic review of the existing literature concerning toxocariasis of the nervous system. RESULTS: Clinical spectrum of human

Familial Nervous System Tumor Syndromes.

PURPOSE OF REVIEW: Although sporadic primary neoplasms account for the majority of nervous system tumors, familial nervous system tumor syndromes are important and clinically relevant conditions for the neurologist to understand. This article reviews common inherited nervous system tumor syndromes including neurofibromatosis type 1, neurofibromatosis type 2, schwannomatosis, tuberous sclerosis complex, and von Hippel-Lindau syndrome. The epidemiology, genetics, approach to diagnosis, neurologic and nonneurologic manifestations, and management options are reviewed. RECENT FINDINGS: Awareness o

Primary Central Nervous System Lymphoma.

Primary central nervous system lymphoma (PCNSL) is an aggressive form of non-Hodgkin lymphoma restricted to the central nervous system. Stereotactic biopsy is the gold-standard for diagnosis of PCNSL. Extent of disease evaluation for newly diagnosed PCNSL patients includes brain imaging, eye examination, cerebrospinal fluid assessment, body imaging, and bone marrow biopsy. Methotrexate-based chemotherapy is the standard induction for PCNSL patients. Optimal consolidation therapy for PCNSL has not been defined, with several options feasible, including chemotherapy, high-dose chemotherapy, and a

Primary central nervous system lymphoma.

Primary central nervous system lymphoma (PCNSL) is a rare and aggressive extranodal non-Hodgkin lymphoma (NHL), confined to the brain, eyes, spinal cord or leptomeninges without systemic involvement. Overall prognosis, diagnosis and management of PCNSL differ from other types of NHL. Prompt diagnosis and initiation of treatment are vital to improving clinical outcomes. PCNSL is responsive to radiation therapy, however whole-brain radiotherapy (WBRT) inadequately controls the disease when used alone and its delayed neurotoxicity causes neurocognitive impairment, especially in elderly patients.

[Peripheral nervous system immunological disorder].

Autoimmune diseases of the peripheral nervous system are common in pediatrics. Guillain-Barré syndrome, juvenile myasthenia gravis, and juvenile dermatomyositis are the most important. Their common pathogenesis involves the action of specific autoantibodies which are frequently triggered by viral or bacterial infection. Acute inflammatory demyelinating polyneuropathy is the most frequent pathological feature. There is also a motor axonal form. Both have a progressive ascending clinical course. The specific treatment is immunoglobulin 2 g/kg. Juvenile myasthenia gravis is expressed by ocular si

Infections of the Nervous System.

Microorganisms can affect the entire neuraxis, producing a variety of neurologic complications that frequently entail prolonged hospitalizations and complicated treatment regimens. The spread of pathogens to new regions and the reemergence of opportunistic organisms in immunocompromised patients pose increasing challenges to health care professionals. Because rapid diagnosis and treatment may prevent long-term neurologic sequelae, providers should approach these diseases with a structured, neuroanatomic framework, incorporating a thorough history, examination, laboratory analysis, and neuroima

Primary central nervous system lymphoma.

Primary central nervous system lymphoma (PCNSL) is an extranodal non-Hodgkin lymphoma (NHL) confined to the brain, leptomeninges, eyes, or spinal cord. The majority of PCNSL cases occur in the immunocompetent host, the focus of this review. The prognosis of PCNSL is inferior to that of other NHL subtypes including other organ-specific subtypes of extranodal NHL. The 5- and 10-year survival proportions for PCNSL are 29.3% and 21.6%, respectively. The diagnosis and management of PCNSL differs from that of other primary brain cancers and NHL in other parts of the body.

Breathing and the nervous system.

Breathing requires complex interactions of the central and peripheral nervous systems with the respiratory system. It involves cortical (volitional) as well as subcortical (automatic) output. Cortical output is mainly through the corticospinal tract, whereas the brainstem sends signals via the reticulospinal tract. Groups of nuclei in the brainstem (pneumotaxic center, dorsal and ventral respiratory group), situated in the pons and medulla, function as rhythm generators. Some of these nuclei have intrinsic pacemaker activity; however, their output is affected extensively by various chemical (t

Leukemia and the nervous system.

Leukemia affects the central and peripheral nervous system. Neurologic complications are a consequence of direct leukemic infiltration, as occurs with leukemic meningitis, and due to complications of either antileukemic treatment (thrombocytopenic or disseminated intravascular coagulation-related intracranial hemorrhage, steroid myopathy, vinca alkaloid peripheral neuropathy) or immune compromise (Herpes zoster shingles or Aspergillus meningitis).

Central nervous system Lyme disease.

Nervous system infection with Borrelia burgdorferi frequently causes meningitis and rarely causes encephalomyelitis. Altered cognitive function also can occur in the absence of central nervous system infection. Recently developed serodiagnostic tools, such as the C6 assay, and appropriate use of Western blotting promise to improve diagnostic accuracy. Treatment trials have demonstrated the efficacy of relatively brief courses of oral antimicrobial agents, even in peripheral nervous system infection and meningitis. Several well-performed studies have clearly shown that prolonged antimicrobial t

[Hemangioblastomas of central nervous system].

Hemangioblastomas of the central nervous system are benign, richly blood-supplied tumors that may be encountered in its any part, more commonly in the cerebellar hemispheres. Supratentorially, they occur rarely. In the literature there are reports on less than 100 cases of tumors at this site. The histogenesis of hemangioblastomas has not been clearly ascertained so far. Magnetic resonance imaging is the only valid diagnostic study of hemangioblastomas. Hemangioblastomas may be presented as sporadic forms and a manifestation of the Hippel-Lindau syndrome. In the latter case, hemangioblastomas

Primary central nervous system lymphomas.

Primary central nervous system lymphoma (PCNSL) is widely regarded as one of the primary brain tumors most amenable to treatment. Although whole brain radiotherapy was the cornerstone of therapy for decades, recent work clearly indicates that chemotherapy has become the primary focus of treatment for this disease. The initial treatment of PCNSL for all patients, including the elderly, should be chemotherapy using a high-dose methotrexate-based regimen. Although cranial irradiation has often been combined with methotrexate, the unacceptably high incidence of late neurotoxicity, particularly in
