Neurocognition as a biomarker in the rare autonomic disorders of CCHS and ROHHAD.

PURPOSE: Congenital central hypoventilation syndrome (CCHS) and rapid-onset obesity with hypothalamic dysfunction, hypoventilation, and autonomic dysregulation (ROHHAD) are rare disorders of autonomic regulation with risk for disrupted neurocognitive development. Our aim is to summarize research on neurocognitive outcomes in these conditions, advance understanding of how to best support these individuals throughout development, and facilitate future research. METHODS: We conducted a narrative review of literature on neurocognitive outcomes in CCHS and ROHHAD, supplemented with previously unpu

Neurocognitive deficits in obsessive-compulsive disorder: A selective review.

Obsessive-compulsive disorder (OCD) is a debilitating mental illness characterized by an early onset and chronic course. Evidence from several lines of research suggests significant neuropsychological deficits in patients with OCD; executive dysfunction and nonverbal memory deficits have been reported consistently in OCD. These deficits persist despite controlling potential confounders such as comorbidity, severity of illness, and medications. Neuropsychological impairments are independent of illness severity, thus suggesting that the neuropsychological deficits are trait markers of the diseas

Neurocognitive risk markers in pediatric obsessive-compulsive disorder.

BACKGROUND: Obsessive-compulsive disorder (OCD) has complex genetic underpinnings, particularly in its early-onset form, which places siblings at a 10-fold increased risk of developing the disorder. Examination for neurocognitive markers preceding pediatric OCD onset has not been conducted, although markers have been identified in adult OCD. This study compared neurocognition across groups of OCD-affected youth (n = 87), unaffected siblings of those with early-onset OCD (n = 67), and healthy controls (HC; n = 79). METHODS: A total of 233 participants aged 6-18 years old completed standardized

Neurocognitive disorder in hypertensive patients. Heart-Brain Study.

The relation between hypertension and cognitive impairment is an undisputable fact. The aims of this study were to determine the prevalence of cognitive impairment in hypertensive patients, to identify the most affected cognitive domain, and to observe the association with different parameters of hypertension and other vascular risk factors. A multicentre study was carried out, and 1281 hypertensive patients of both genders and ≥21 years of age were included. Data on the following parameters were obtained: cognitive status (Minimal Cognitive Examination), behavioural status (Hospital Anxiety a

Neurocognitive correlates of treatment response in children with Tourette's Disorder.

This paper examined neurocognitive functioning and its relationship to behavior treatment response among youth with Tourette's Disorder (TD) in a large randomized controlled trial. Participants diagnosed with TD completed a brief neurocognitive battery assessing inhibitory functions, working memory, and habit learning pre- and post-treatment with behavior therapy (CBIT, Comprehensive Behavioral Intervention for Tics) or psychoeducation plus supportive therapy (PST). At baseline, youth with tics and Attention Deficit Hyperactivity Disorder (ADHD) exhibited some evidence of impaired working memo

Social anxiety disorder: a critical overview of neurocognitive research.

Social anxiety is a common disorder characterized by a persistent and excessive fear of one or more social or performance situations. Behavioral inhibition is one of the early indicators of social anxiety, which later in life may advance into a certain personality structure (low extraversion and high neuroticism) and the development of maladaptive cognitive biases. While there are several effective psycho- and pharmacotherapy options, a large number of patients benefit insufficiently from these therapies. Brain and neuroendocrine research can help uncover both the biological basis of social an

Endothelial function, folate pharmacogenomics, and neurocognition in psychotic disorders.

Cardiovascular disease (CVD) is a well-described complication of schizophrenia, however, mechanisms connecting CVD with other facets of psychotic disorders, such as neurocognition, are not understood. The current study examined folate metabolism as a potential mechanism of CVD and neurocognitive deficits by: 1) using endothelial dysfunction as a biomarker of CVD, and 2) comparing enzymes associated with neurocognition, CVD, and critical to folate metabolism, methylenetetrahydrofolate reductase (MTHFR) and catechol-o-methyl transferase (COMT). Endothelial function was assessed in 147 participan

Prevalence of neurocognitive disorders and depression in a Brazilian HIV population.

INTRODUCTION: Combined antiretroviral therapy has enabled human immunodeficiency virus (HIV) carriers to live longer. This increased life expectancy is associated with the occurrence of degenerative diseases, including HIV-associated neurocognitive disorders (HAND), which are diagnosed via a complex neuropsychological assessment. The International HIV Dementia Scale (IHDS) is a screening instrument validated in Brazil for use in the absence of neuropsychological evaluation. HIV patients are frequently diagnosed with depression. We aimed to determine the prevalence of neurocognitive impairment

Neurocognitive functioning in patients recently diagnosed with bipolar disorder.

OBJECTIVES: Cognitive dysfunction in bipolar disorder (BD) is well established in the literature; however, there are few studies of neurocognition in patients early in the course of the illness. In this study we compare neurocognitive function in a cohort of first-contact mania patients with a healthy control group matched for age, gender, and education. METHODS: Patients with a first manic episode (FM) (n = 34) or previous untreated manic episodes (PM) (n = 21) were neuropsychologically tested following their first treated manic episode. A total of 110 matched healthy control comparison subj

Obsessive beliefs and neurocognitive flexibility in obsessive-compulsive disorder.

A substantial proportion of individuals with obsessive-compulsive disorder (OCD) do not endorse the dysfunctional beliefs proposed by cognitive models of OCD to be important in the onset and maintenance of symptoms. Previous research has attempted to characterize Low and High obsessive beliefs groups in terms of cognitive and symptom correlates to distil potential etiological differences in these subgroups of OCD patients. The current study sought to further examine potential neurocognitive differences between obsessive beliefs subgroups. Performance on the Wisconsin Card Sorting Test (WCST) w

Astrocytic GABAergic signaling in perioperative neurocognitive disorders: mechanisms and intervention strategies.

Perioperative neurocognitive disorders (PND) are a major burden for older surgical patients, yet little is known about the cellular events that set them in motion, and no therapy currently changes their course. Anesthesia and amnesia after etomidate, sevoflurane, or isoflurane depend mainly on gamma-aminobutyric acid type A receptors (GABA(A)Rs), and an increasing body of work shows that astrocytes also respond to these drugs. This review examines an astrocyte-centered GABAergic cascade that may link anesthetic exposure to PND and grades the strength of the evidence at each node. When anesthet

Advances in electroacupuncture for perioperative neurocognitive disorders: mechanisms and clinical evidence.

Perioperative neurocognitive disorders (PND), including postoperative delirium, delayed neurocognitive recovery, and postoperative cognitive dysfunction, are common complications in older surgical patients and are associated with impaired recovery, reduced quality of life, and increased postoperative morbidity. Current management remains largely supportive and preventive, and effective targeted therapies are still lacking. Electroacupuncture (EA), as a minimally invasive neuromodulatory intervention, has attracted increasing attention because of its potential multi-target regulatory effects. T

The Shoulder-Tapping Sign: A Highly Specific Indicator of Neurocognitive Disorders.

BACKGROUND: Perseveration, an inappropriate continuation of a response, is a well-established clinical sign of neurological dysfunction. The shoulder-tapping sign (STS) is a newly described and easily identified motor perseveration. OBJECTIVE: Investigate the diagnostic utility of the STS for identifying neurocognitive disorders in patients referred for neuropsychological evaluation. METHOD: We administered a five-step verbal command from the Commands subtest of the Boston Diagnostic Aphasia Examination ("Tap each shoulder twice with two fingers keeping your eyes shut") to 400 consecutive pa

The psychopathology of mood disorders: implications for identifying neurocognitive intervention targets.

BACKGROUND: Neurofeedback and neuromodulation treatments are of increasing clinical interest, but their neurocognitive targets are poorly understood. METHODS: In this review, we will use Jaspers' phenomenological psychopathology combined with modern network analysis to identify neurocognitive treatment targets by focussing on distinctive and necessary symptoms of mood disorders as well as their subsyndromal and prognostic variations. RESULTS: We discuss the early descriptions of Kraepelin's mixed affective states and suggest a model of four mood states (depressed, anxious, irritable, and ela

Information and Counseling Gaps on Perioperative Neurocognitive Disorders Among Older Adults.

BACKGROUND: Perioperative neurocognitive disorders (PNDs) are among the most common complications in older adults after surgery. Awareness of PNDs is important as they often can be prevented, and early recognition can improve recovery. We sought to understand what older adults know about PNDs, the information provided, and the counseling provided to guide recovery when PND symptoms occur. We hypothesized that older adults rarely receive information regarding PND before surgery or counseling after experiencing symptoms. METHODS: We conducted a mixed methods study by (1) employing a survey to b

Esketamine and neurocognitive disorders in adult surgical patients: a meta-analysis.

BACKGROUND: Prior meta-analyses have established the potential of intravenous ketamine in safeguarding against neurocognitive impairment, but the efficacy of intravenous esketamine for the prevention of perioperative neurocognitive disorders (PND) remains uncertain. The primary aim of this meta-analysis was to conduct a comprehensive evaluation of the effects of esketamine on PND in adult surgical patients undergoing general anesthesia. METHODS: We searched several electronic databases and clinical trial registries to find relevant trials. Randomized controlled trials of perioperative use of

Microglial HIV-1 Expression: Role in HIV-1 Associated Neurocognitive Disorders.

The persistence of HIV-1 viral reservoirs in the brain, despite treatment with combination antiretroviral therapy (cART), remains a critical roadblock for the development of a novel cure strategy for HIV-1. To enhance our understanding of viral reservoirs, two complementary studies were conducted to (1) evaluate the HIV-1 mRNA distribution pattern and major cell type expressing HIV-1 mRNA in the HIV-1 transgenic (Tg) rat, and (2) validate our findings by developing and critically testing a novel biological system to model active HIV-1 infection in the rat. First, a restricted, region-specific

Impulsivity traits and neurocognitive mechanisms conferring vulnerability to substance use disorders.

Impulsivity - the tendency to act without sufficient consideration of potential consequences in pursuit of short-term rewards - is a vulnerability marker for substance use disorders (SUD). Since impulsivity is a multifaceted construct, which encompasses trait-related characteristics and neurocognitive mechanisms, it is important to ascertain which of these aspects are significant contributors to SUD susceptibility. In this review, we discuss how different trait facets, cognitive processes and neuroimaging indices underpinning impulsivity contribute to the vulnerability to SUD. We reviewed stud

Pharmacological agents to treat severe neuropsychiatric symptoms in major neurocognitive disorders.

BACKGROUND: Behavioral and psychological symptoms of dementia (BPSD) are challenging to recognize and treat effectively. Despite very effective nonpharmacologic strategies, there are cases where severe BPSD may require biological intervention. The purpose of this review was to clarify safe and evidence-based use of pharmacological agents based on available clinical trial evidence. METHODS: A structured search strategy was used in PubMed, Embase, and PsycInfo to obtain the most relevant data regarding treatment of BPSD in patients with any-cause dementia. RESULTS: We screened 1,442 English-la
