Puberty and plexiform neurofibroma tumor growth in patients with neurofibromatosis type I.

OBJECTIVE: To assess the relationship between pubertal progression and change in plexiform neurofibroma (PN) burden over time in pediatric and young adult patients with neurofibromatosis type 1 and PNs. STUDY DESIGN: Analyses accounted for sex, age, race, and chemotherapy. Forty-one patients with neurofibromatosis type 1 (15 female and 26 male patients) were studied at the National Institutes of Health. Tanner stage, testosterone, progesterone, estradiol, insulin-like growth factor -1, luteinizing hormone, and follicle-stimulating hormone were assessed. Tumor volume was measured using magneti

Plexiform neurofibroma genesis: questions of Nf1 gene dose and hyperactive mast cells.

PURPOSE OF REVIEW: Tumorigenic cells can co-opt normal functions of nonmalignant hematopoietic cells, promoting tumor progression. Recent mouse and human studies indicate that mast cells underpin inflammation in the plexiform neurofibroma microenvironment of neurofibromatosis type 1. In this model, Nf1 homozygous-deficient Schwann cells recruit hyperactive mast cells, promoting tumorigenesis. Here, we discuss the importance of Nf1 gene dosage, delineate hematopoietic contributions to the plexiform neurofibroma microenvironment, and highlight applications to human treatment. Successful utilization of topical trametinib for neurofibromatosis type I-associated plexiform neurofibroma.

An 11-year-old female with neurofibromatosis type 1 (NF-1) and history of optic glioma presented with a progressive cutaneous plexiform neurofibroma of the breast. The lesion was treated with topical application of a mitogen-activated protein kinase inhibitor, trametinib, resulting in stable, non-progression cutaneous plexiform neurofibroma for greater than 2 years. This case demonstrates the potential application of topical trametinib for NF1-associated superficial cutaneous plexiform neurofibroma without the toxicities associated with systemic treatment.

Neurofibromatosis Type 1 with a Giant Diffuse Plexiform Neurofibroma Invading the Liver.

Plexiform neurofibromas (PNs) occur in approximately 50% of patients with neurofibromatosis type 1 (NF1). PNs are rare in the abdominal cavity and especially rare in hepatobiliary lesions. A 31-year-old man with NF1 had a tumor extending along the celiac artery, superior mesenteric artery, and intrahepatic portal vein. We diagnosed him with diffuse PN based on liver tumor biopsy findings and the tumor form. Because the tumor had invaded along the intrahepatic portal vein, surgical resection was deemed difficult, and the patient was followed up with imaging studies. The patient remained asympto

Management of an expansile orbital mass: Plexiform neurofibroma decompression by orbitozygomatic approach.

Neurofibromatosis types 1 (NF-1) and 2 are significant entities to otolaryngologists because they frequently involve head and neck skin, brain, skull base, orbits, and surrounding neural and vascular structures. We present the case of a 52-year-old male with progressive, marked unilateral proptosis due to a multilobulated orbital mass, secondary to biopsy-proven plexiform neurofibroma (PN). Acute worsening of proptosis leading to corneal abrasion, diplopia, and pain required debulking surgery, for which an orbitozygomatic approach was utilized. Genetic testing for NF-1 revealed no mutation. Th

Isolated Plexiform Neurofibroma at the Liver Hilum in a Pediatric Patient: A Case Report.

Neurofibromatosis type 1 (NF1) is a common autosomal dominant genetic disorder, presenting with hallmark benign tumors including cutaneous and plexiform neurofibromas. Plexiform-type neurofibromas most commonly present in peripheral tissue nerves underlying the dorsal trunk, but rarely present in visceral organs such as the liver. This case presents a 12-year-old male patient with a history of NF1 who presented to the emergency department with non-specific abdominal pain. Hepatic sonography showed an infiltrating, nodular enhancement along the porta hepatis. A diagnosis of plexiform neurofibro

Surgical management of a plexiform neurofibroma in neurofibromatosis type 1: The haemostatic challenge.

BACKGROUND: Plexiform neurofibromas (PNFs) are benign tumours associated with neurofibromatosis type 1 (NF1) that may cause significant functional and cosmetic impairment. Surgical management is challenging because of diffuse infiltration and marked vascularity, increasing the risk of severe intraoperative bleeding. CASE REPORT: A 70-year-old woman with NF1 presented with a progressively enlarging mass of the left flank causing contour deformity with functional impairment. Previous resection 20 years earlier had been complicated by major haemorrhage requiring transfusion of red blood cells. M

Radical Resection of a Giant Facial Plexiform Neurofibroma in a Resource-Limited Setting.

Giant facial plexiform neurofibromas (PNFs) in neurofibromatosis type 1 are surgically challenging, particularly in low- and middle-income countries where patients may present late and repeated staged procedures may be impractical. We report a 20-year-old woman from rural Zimbabwe with a massive recurrent left facial PNF after three prior debulking procedures. She underwent single-stage radical excision of involved skin, mucosa, parotid tissue, and facial nerve branches, followed by immediate anterolateral thigh free flap reconstruction and static facial suspension. Two units of packed red blo

Selumetinib in adults with NF1 and inoperable plexiform neurofibroma: a phase 2 trial.

The MEK inhibitor selumetinib induces objective responses and provides clinical benefit in children with neurofibromatosis type 1 (NF1) and inoperable plexiform neurofibromas (PNs). To evaluate whether similar outcomes were possible in adult patients, in whom PN growth is generally slower than in pediatric patients, we conducted an open-label phase 2 study of selumetinib in adults with NF1 PNs. The study was designed to evaluate objective response rate (primary objective), tumor volumetric responses, patient-reported outcomes and pharmacodynamic effects in PN biopsies. The objective response r

Selumetinib use as targeted therapy for plexiform neurofibroma: a comprehensive review of the literature.

Neurofibromatosis type 1 is inherited in an autosomal dominant fashion. The approximate prevalence of neurofibromatosis type 1 is 1:2500 to 1:3500 regardless of ethnic group. Around 50% of neurofibromatosis type 1 patients develop plexiform neurofibroma, a lesion that has a tendency for malignant transformation. Understanding the pathophysiology of plexiform neurofibromatosis and the implication of neurofibromin signal transduction has led to the development and use of targeted therapy on this pathway. In this article, we comprehensively reviewed the recently published literature on the use of

A National Modified Delphi Consensus on the Referral and Management of NF1 Plexiform Neurofibroma.

BACKGROUND AND OBJECTIVES: Neurofibromatosis type 1 (NF1) is a genetic disorder. Up to 50% of NF1 patients develop plexiform neurofibromas (PN). Despite revisions in diagnostic standards, there remains a lack of consensus on referral, treatment, monitoring and transition processes for NF1-PN. The study aimed to establish a Canada-wide consensus on the best practice for referral and management of patients with NF1-PN to help generate guidance where evidence on the long-term use of MEK inhibitors is lacking. METHODS: The study used a modified Delphi method. The steering committee (SC) identifie

A Retrospective Study of Diffuse Plexiform Neurofibroma With Surgical Intervention in Neurofibromatosis 1.

Diffuse plexiform neurofibroma (DPN) is a subtype of plexiform neurofibroma that primarily develops on a large pigmented macule. These tumors often exhibit substantial growth and carry a risk of malignant transformation. Consequently, DPN can significantly impact the quality of life and prognosis in patients with neurofibromatosis 1, underscoring the critical need for effective therapeutic options. MEK inhibitors, such as Mirdametinib and Selumetinib, have recently become available for symptomatic, unresectable pediatric PN. However, surgical resection remains the only viable treatment for adu

Safety and Efficacy of Mek Inhibitors in the Treatment of Plexiform Neurofibromas: A Retrospective Study.

INTRODUCTION: Plexiform neurofibromas (PN) represent the main cause of morbidity in patients affected by Neurofibromatosis Type 1 (NF1). Until recently, surgery has been the main treatment option in these patients, but it is burdened with a low efficacy rate and a high incidence of side effects as well as recurrence. In recent years, MEK inhibitors (MEKi) such as selumetinib and trametinib have shown great promise. METHODS: We retrospectively describe a single center cohort of NF1 patients affected by PN1 and treated with MEKi since 2019 to 2021. Patients recruited in the study were affected

Isolated, Nonsyndromic Mucocutaneous Plexiform Neurofibromas: A Systematic Review of the Clinicopathologic Features.

Plexiform neurofibromas are benign neural tumors observed in association with neurofibromatosis. Isolated lesions exist. We conducted a systematic review of the published literature indexed in the PubMed/Medline database using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Thirty-five studies describing isolated plexiform neurofibromas were included detailing 41 tumors. Isolated lesions occur in all age groups, in both sexes and in all races. Cutaneous and mucosal lesions were reported. Asymptomatic, slowly enlarging masses were the most common clinical pres

Trametinib for orbital plexiform neurofibromas in young children with neurofibromatosis type 1.

INTRODUCTION: Plexiform neurofibromas (PNF) in neurofibromatosis type 1 (NF1) are usually diagnosed in childhood and can grow rapidly during this period. In 10% of patients, PNF involve the orbital-periorbital area and may cause visual problems including glaucoma, visual loss from amblyopia (deprivational, strabismic, or refractive), optic nerve compression, or keratopathy. Ptosis, proptosis, and facial disfigurement lead to social problems and decreased self-esteem. Complete surgical removal involves significant risks and mutilation, and regrowth after debulking is not uncommon. Inhibitors of

Giant Plexiform Neurofibroma of the Perineum and Pelvic Cavity Manifesting as Segmental Neurofibromatosis.

Segmental neurofibromatosis (SN) is rare form of neurofibromatosis characterized that cutaneous or neural changes are limited to one region of the body. SN present neurofibroma and less frequently, café au lait macules (CALMs) on usually unilateral or rarely bilateral of the body region. SN seems to have fewer systemic complications than neurofibromatosis type I or II, except patients with plexiform neurofibromas (PNFs). PNFs are rare benign peripheral nerve sheath tumors which arise from single or multiple nerves. PNFs can easily become aggressive growth particularly during puberty or pregnan

Distribution of diffuse plexiform neurofibroma on the body surface in patients with neurofibromatosis 1.

Neurofibromatosis 1 (NF1) is a genetic disease characterized by cutaneous, neurological and osseous abnormalities. Approximately 20% of patients develop plexiform neurofibroma (PN), resulting in impaired quality of life. To evaluate distribution of diffuse PN on the body surface, a retrospective study was conducted for 354 patients with NF1 from 2007 to 2018 in Japan. We investigated a total of 40 patients with clinically apparent superficial diffuse PN. In the cases evaluated, 57.4% of the diffuse PN were located on the trunk, 19.2% the head and neck, 12.8% the lower limbs and 10.6% the upper
