Delirium: a neurologist's view--the neurology of agitation and overactivity.

Delirium is a term used variously to characterize a change in behavior. Neurologists most often use the term to describe a patient who has acutely developed a hyperactive agitated state. In many patients, agitation and overactivity are explained by toxic and metabolic factors and infections. Lesions, especially strokes, in some brain regions have been reported to cause sudden agitation and a hyperactive state, often with an increased amount of speech output, the topics of which flit from one subject to another. Strokes and other lesions that involve the temporal lobes, fusiform and lingual gyr

Treatment of Parkinson's disease: a survey of patients and neurologists.

BACKGROUND: The treatment of Parkinson's disease (PD) is complex and highly individual. The choice between available treatment options depends on clinical characteristics such as the patient's age, disease severity and presence of comorbidities, lifestyle characteristics and preferences, costs of different medications and awareness and perception of available treatment options, and education of the treating physician. The impact of PD treatment regimens on patients' health-related quality of life (QOL) is also an important healthcare feature. The objective of the present study was to assess tr

Questionnaire on neuropathic pain: a European neurologist survey.

A survey was conducted among European neurologists to obtain information on their interest in, knowledge on, and practice in the treatment of neuropathic pain. A simple questionnaire with 11 closed and one open question was distributed at national meetings, via e-mail lists of national Neurological Societies, and through the EFNS website. Data were collected from 745 neurologists in 11 countries. The majority of the respondents stated to be familiar with neuropathic pain and considered it part of their specialty. The ratio between peripheral and central neuropathic pains was 10:1, with diabeti

Secondary prevention of stroke and the expanding role of the neurologist.

Stroke is the leading cause of adult disability and dependency in western society. Following stroke, the risk of myocardial infarction (MI) is increased by a factor of around 2-3 compared with baseline. Indeed, after the first 30 days, stroke survivors are more likely to die from a cardiac event than from a cerebrovascular event. In patients with atherothrombotic stroke, preventing subsequent manifestations of the underlying disease is therefore an important therapeutic goal. A number of options have been shown to reduce the risk of stroke. Aggressively controlling stroke risk factors, such as

Temporomandibular disorders: what the neurologist needs to know.

Temporomandibular disorders are a complex group of conditions which are common causes of orofacial pain and are frequently associated with headaches. Because a wide variety of diseases can ultimately cause symptoms that result in dysfunction of the masticatory system, it is important for the clinician to establish an accurate diagnosis and rule out any serious neurologic pathology. Serious extra-articular conditions often can simulate the nonspecific symptoms of a temporomandibular joint disorder. A careful history and clinical examination of the head, neck, and cranial nerves are important in

A neurologist's reflections on boxing. V. Conclude remarks.

Clinical and morphological publications have shown convincingly, that participation in boxing leads to a severe permanent brain damage. The extent of the brain damage is correlated to the number of bouts fought, which correspondents in a certain way how many blows against his head a boxer received and to his weight class. The intensity of a boxing blow of a heavyweight is much more severe than those achieved by boxers of lighter weight classes. The permanent brain damage in a boxer, the amateur and the professional boxer, manifests itself in several clinical syndromes in which the pyramidal, t

Personality disorders: what the neurologist should know.

Personality traits and disorders are frequently encountered in outpatient neurology. This can significantly influence the clinical consultation and the patient's neurological presentation and management. We offer practical guidance for neurologists to understand personality-related presentations better, along with clinical pointers to distinguish lifelong traits from changes secondary to neurological disease and advice on managing structured consultations with empathy. Recognising this interplay between personality and neurological care will support neurologists to deliver targeted, compassion

[Drug-induced Parkinsonism as Viewed from Neurologist].

Drug-induced Parkinsonism (DIP) is a common iatrogenic movement disorder. Its clinical manifestations cannot be distinguished from those of idiopathic Parkinson's disease. Prior exposure to dopamine receptor-blocking agents (DRBA) is required for diagnosis. This article reviews the epidemiology, pathophysiology, clinical features, ancillary testing, and treatment of DIP. Clinicians must always suspect DIP when a patient develops subacute Parkinsonism while taking DRBA. Ancillary testing such as DaT scans is often helpful in identifying prodromal Parkinson's disease. When DIP develops, steps sh

An Autobiographical Report of Postherpetic Abdominal Pseudohernia in a Neurologist.

Herpes zoster is a dermatological disorder characterized by painful vesicles in the affected dermatome. While sensory symptoms are common, motor complications such as postherpetic pseudohernia, a rare condition involving abdominal muscle paresis, can occur. This report describes a neurologist in his mid-30s who developed herpes zoster followed by postherpetic abdominal pseudohernia, which resolved within three months. Pseudohernia associated with herpes zoster is a rare complication that may be misdiagnosed as an abdominal wall hernia. Stress and overwork may contribute to the onset of herpes

The Neurologist's Guide to Drug Pricing in the United States.

As newer, innovative neurology drugs enter the US health care system, neurologists should consider the cost of these treatments in addition to their efficacy, safety, and tolerability. To do so thoughtfully requires an understanding of how prescription drugs are priced in the United States. The process of drug pricing is linked to the distribution supply chain and the many stakeholders involved. Stakeholders include pharmaceutical manufacturers; wholesalers; pharmacies; pharmacy benefit managers; payers, including health insurers; hospital systems; neurologists and other clinicians; and patien

Auto brewery syndrome from the perspective of the neurologist.

BACKGROUND AND PURPOSE: Auto-brewery syndrome (ABS) is a rare condition that causes the digestive system to produce intoxicating amounts of alcohol due to abnormal growth of the gut microbiota. Medicolegal inferences of ABS have two distinct edges. First, malingering in drunk-driving and abusing the syndrome as a factitious disorder may occur. Second, patients suffering from the syndrome may face medicolegal and social problems. Moreover, chronic exposure to alcohol due to undiagnosed ABS might result in cognitive and behavioral disturbances. Here, we present a patient with recurrent encephalo

The patient with epilepsy and medicolegal aspects: a view for the neurologist.

Patients with epilepsy face innumerable obstacles in daily life, related to work, permission to drive and interpersonal relationships, which require medical guidance. This paper reports a literature review based on scientific articles and civil and traffic system, as a way to resolve doubts about medical obligations in the patient's permission to drive and work. An employment agreement requires the contractor to guarantee safety conditions as well as requiring the patient, at the pre-employment medical examination, to let the physician know previous medical conditions, including epilepsy. Eye signs for the neurologist in the Intensive Care Unit.

INTRODUCTION: The eyes are a window to the brain' is a maxim that holds true especially in the intensive care setting. Recognising specific eye signs aids rapid decision-making regarding diagnosis or prognosis. Eye signs play a pivotal role in intensive care for the neurologist. STATE OF THE ART: Eye signs have long been considered the best clinical clue for assessment of a comatose patient. In critically ill patients, the recognition of brainstem involvement hinges primarily on eye signs. The ability to recognise and interpret these signs goes a long way towards ensuring proper care of neuro

Obstetric anaesthesia: what a neurologist needs to know.

Neurologists are often consulted to see women postpartum who are having difficulties involving the lower extremities; weakness, numbness and pain. Many of these women have received labour analgesia. Often, there is limited understanding by the neurologist of how these procedures are performed, why a neuraxial technique is chosen and their potential complications. This case-based review will explain the differences in the neuraxial procedures: epidural, spinal and combined spinal epidural; their advantages and disadvantages, why one technique might be chosen over another, contraindications as w

Atypical parkinsonian syndromes: a general neurologist's perspective.

The differential diagnosis of atypical parkinsonian syndromes is challenging. These severe and often rapidly progressive neurodegenerative disorders are clinically heterogeneous and show significant phenotypic overlap. Here, clinical, imaging, neuropathological and genetic features of multiple system atrophy, progressive supranuclear palsy, corticobasal degeneration and frontotemporal lobar degeneration (FTLD) are reviewed. The terms corticobasal degeneration and FTLD refer to pathologically confirmed cases of corticobasal syndrome and frontotemporal dementia (FTD). Frontotemporal lobar degene
