[Vecuronium-induced neuromuscular blockade in two patients with hyperparathyroidism and a patient with hypoparathyroidism].

Vecuronium-induced neuromuscular blockade was evaluated in two patients with primary hyperparathyroidism and in a patient with hypoparathyroidism. A 39 year old male with typical primary hyperparathyroidism was scheduled for surgical removal of the parathyroid adenoma. Serum levels of calcium and ionized calcium were 15.0 mg.dl-1 and 1.95 mmol.l-1, respectively. A 44 year old female suffering from primary hyperparathyroidism was also scheduled for surgical removal of the adenoma. Serum levels of calcium and ionized calcium were 12.5 mg.dl-1 and 1.51 mmol.l-1, respectively. A 63 year old male,

The influence of atropine dose on recovery from vecuronium-induced neuromuscular blockade.

To determine whether the dose of atropine affects the rate of neostigmine-induced recovery from vecuronium-induced neuromuscular blockade, the authors monitored isometric adductor pollicis mechanical activity in 36 anesthetized (thiopental, fentanyl, nitrous oxide) adult patients (ASA physical status 1 or 2). Once surgery was completed and twitch height had spontaneously regained 25% of its initial value, the patients were randomly allocated into three groups (A10, A15, A20; n = 12 in each group) according to the dose of atropine (10, 15, or 20 micrograms/kg) that was mixed with 40 micrograms/

Effects of residual concentrations of isoflurane on the reversal of vecuronium-induced neuromuscular blockade.

Thirty-six anesthetized patients (ASA physical status 1 or 2) undergoing elective surgery were monitored (isometric adductor pollicis mechanical activity) to detect the effects of discontinuing isoflurane anesthesia upon the reversal of vecuronium-induced neuromuscular blockade. Neuromuscular blockade was produced by vecuronium 100 micrograms/kg and additional doses of 20 micrograms/kg until completion of surgery. The patients were randomly divided into three groups: in the control group (n = 12), only fentanyl/N2O was given; in the "isostable" group (n = 12), isoflurane at an end-tidal concen

The Prevalence of Adverse Events After Prehospital Neuromuscular Blockade-Assisted Airway Management.

OBJECTIVES: Neuromuscular blockade-assisted airway management is associated with harmful physiologic adverse events, such as hypoxemia, hypotension, bradycardia, and cardiac arrest. The national scope of these events in the prehospital setting is unknown. We sought to estimate the prevalence of adverse events associated with prehospital neuromuscular blockade-assisted airway management in the United States. METHODS: We conducted an observational, cross-sectional study of the 2023 National Emergency Medical Services Information System Public-Release Research Dataset. We included all adult 9-1-

Use of sugammadex after neostigmine incomplete reversal of rocuronium-induced neuromuscular blockade.

BACKGROUND AND OBJECTIVES: Neuromuscular blockers (NMB) have been used for more than half of a century in anesthesia and have always been a challenge for anesthesiologists. Until recently, the reversal of nondepolarizing neuromuscular blockers had only one option: the use of anticholinesterase agents. However, in some situations, such as deep neuromuscular blockade after high doses of relaxant, the use of anticholinesterase agents does not allow adequate reversal of neuromuscular blockade. Recently, sugammadex, a gamma-cyclodextrin, proved to be highly effective for reversal of NMB induced by

Sugammadex reversal of rocuronium-induced neuromuscular blockade in two types of neuromuscular disorders: Myotonic dystrophy and spinal muscular atrophy.

Neuromuscular disorders like myotonic dystrophy (dystrophia myotonica or Steinert's disease) and spinal muscular atrophy are associated with perioperative complications related to muscle weakness. These patients have an increased sensitivity to non-depolarising neuromuscular blocking agents, which can lead to postoperative residual curarization (PORC) and its associated respiratory complications. Adequate reversal of neuromuscular blockade is essential to prevent this. Sugammadex is the first selective relaxant binding agent and it reverses rocuronium- and vecuronium-induced neuromuscular bloc

Paediatric Acute Respiratory Distress Syndrome Neuromuscular Blockade study (PAN-study): a phase IV randomised controlled trial of early neuromuscular blockade in moderate-to-severe paediatric acute respiratory distress syndrome.

BACKGROUND: Paediatric acute respiratory distress syndrome (PARDS) is a manifestation of severe, life-threatening lung injury necessitating mechanical ventilation with mortality rates ranging up to 40-50%. Neuromuscular blockade agents (NMBAs) may be considered to prevent patient self-inflicted lung injury in PARDS patients, but two trials in adults with severe ARDS yielded conflicting results. To date, randomised controlled trials (RCT) examining the effectiveness and efficacy of NMBAs for PARDS are lacking. We hypothesise that using NMBAs for 48 h in paediatric patients younger than 5 years

Improving Neuromuscular Monitoring and Reducing Residual Neuromuscular Blockade With E-Learning: Protocol for the Multicenter Interrupted Time Series INVERT Study.

BACKGROUND: Muscle relaxants facilitate endotracheal intubation under general anesthesia and improve surgical conditions. Residual neuromuscular blockade occurs when the patient is still partially paralyzed when awakened after surgery. The condition is associated with subjective discomfort and an increased risk of respiratory complications. Use of an objective neuromuscular monitoring device may prevent residual block. Despite this, many anesthetists refrain from using the device. Efforts to increase the use of objective monitoring are time consuming and require the presence of expert personne

Axillary nerve palsy incidence after total shoulder arthroplasty with and without neuromuscular blockade.

BACKGROUND: Axillary nerve palsy is a rare but significant complication of shoulder arthroplasty. The axillary nerve lies just beneath the inferior glenoid, increasing susceptibility to traction and compression injures during surgery. Neuromuscular blocking agents are commonly used to facilitate exposure and dissection but may reduce the surgeon's ability to perceive excessive traction or tension on the nerve. The aims of this study are twofold, to (1) determine whether intraoperative paralytic use is associated with post-operative axillary nerve palsy and (2) assess its relationship with oper

Neuromuscular Blockade and Reversal Agent Practice Variability in the US Inpatient Surgical Settings.

INTRODUCTION: The management of neuromuscular blockade (NMB) has evolved over time and remains a critical component of general anesthesia. However, NMB use varies by patient and procedural characteristics, clinical practices, protocols, and drug access. National utilization patterns are unknown. We describe changes in NMB and NMB reversal agent administration in surgical inpatients since the US introduction of sugammadex in December 2015. METHODS: In a retrospective observational study of inpatients involving NMB with rocuronium or vecuronium in the Premier Healthcare Database, we estimate as

Water-soluble acylhydrazone macrocycles as potent reversal agents for cisatracurium-induced neuromuscular blockade.

Cisatracurium is a widely used non-depolarizing neuromuscular blocking agent during anesthesia; however, residual neuromuscular blockade post-surgery can lead to severe respiratory complications, highlighting the urgent need for effective reversal agents. In this study, we designed and synthesized a highly water-soluble acylhydrazone macrocycle featuring an electron-rich cavity as an efficient supramolecular reversal agent for cisatracurium. The macrocycle binds cisatracurium through electrostatic and hydrophobic interactions, exhibiting a binding constant of up to 105 M-1. Cytotoxicity assays

Clinical Use of Endotracheal Intubation Without Neuromuscular Blockade: The Current Stage of Knowledge.

Endotracheal intubation is a fundamental component of modern airway management in elective and emergency settings. Neuromuscular blocking agents (NMBAs) are commonly used to optimize intubating conditions by facilitating jaw relaxation, improving vocal cord visualization, and reducing airway trauma. However, their administration can be contraindicated or undesirable in specific situations, such as procedures requiring intraoperative neuromonitoring, short-duration surgeries, patients with neuromuscular disorders, or critically ill individuals at risk of prolonged blockade or adverse reactions.

A Practical Dosing Algorithm for Deep Neuromuscular Blockade during Total Intravenous Anesthesia: ROCURITHM.

BACKGROUND: The number of trials investigating the effects of deep neuromuscular blockade (NMB) on surgical conditions and patient outcomes is steadily increasing. Consensus on which surgical procedures benefit from deep NMB (a posttetanic count [PTC] of 1 to 2) and how to implement it has not been reached. The European Society of Anaesthesiology and Intensive Care does not advise routine application but recommends use of deep NMB to improve surgical conditions on indication. This study investigates the optimal dosing strategy to reach and maintain adequate deep NMB during total intravenous an
