Stem cell transplantation for neuroprotection in stroke.

Stem cell-based therapies for stroke have expanded substantially over the last decade. The diversity of embryonic and adult tissue sources provides researchers with the ability to harvest an ample supply of stem cells. However, the optimal conditions of stem cell use are still being determined. Along this line of the need for optimization studies, we discuss studies that demonstrate effective dose, timing, and route of stem cells. We recognize that stem cell derivations also provide uniquely individual difficulties and limitations in their therapeutic applications. This review will outline the

Traumatic brain injury: oxidative stress and neuroprotection.

SIGNIFICANCE: A vast amount of circumstantial evidence implicates high energy oxidants and oxidative stress as mediators of secondary damage associated with traumatic brain injury. The excessive production of reactive oxygen species due to excitotoxicity and exhaustion of the endogenous antioxidant system induces peroxidation of cellular and vascular structures, protein oxidation, cleavage of DNA, and inhibition of the mitochondrial electron transport chain. RECENT ADVANCES: Different integrated responses exist in the brain to detect oxidative stress, which is controlled by several genes term

Neuroprotection in glaucoma--delusion, reality or hope?

The global definition of glaucoma considers it as an optic neuropathy with multifactor etiology, which affects the optic nerve head (ONH), provoking visual field loss and permanent impairment of visual function. Bearing in mind the fact that the exact pathogenic mechanism is still not completely established, glaucoma treatment strategies so far are based upon the identification of glaucoma risk factors. Among them, only elevated intraocular pressure (IOP) could undergo therapeutic treatment. However, in spite of the adequate therapy and IOP lowering, very often the disease is still progressing

Serotonin and neuroprotection in functional bowel disorders.

The 5-HT(4) partial agonist tegaserod is effective in the treatment of chronic constipation and constipation predominant irritable bowel syndrome. 5-HT(4) receptors are located on presynaptic terminals in the enteric nervous system. Stimulation of 5-HT(4) receptors enhances the release of acetylcholine and calcitonin gene related peptide from stimulated nerve terminals. This action strengthens neurotransmission in prokinetic pathways, enhancing gastrointestinal motility. The knockout of 5-HT(4) receptors in mice not only slows gastrointestinal activity but also, after 1 month of age, increases

Molecular and cellular immune mediators of neuroprotection.

Our view of the immune privileged status of the brain has dramatically changed during the past two decades. Even though systemic immune stimuli have the ability to activate different populations of neurons, cells of monocytic lineage also have access to the neuronal tissue and populate it as microglia. Although such a phenomenon is limited in intact brains, it is greatly increased during neurodegenerative processes associated with innate immunity and the release of pro-inflammatory molecules by either resident microglia or those derived from the bone marrow stem cells. The role of these events

Neuroprotection in experimental stroke with targeted neurotrophins.

More than 30 neurotrophins have been identified, and many of them have neuroprotective effects in brain ischemia or injury. However, all the clinical trials with several neurotrophins for the treatment of acute ischemic stroke or neurodegenerative diseases have failed so far, primarily because of their poor blood-brain barrier (BBB) permeability. This article is an overview of recent progress in the research focused on BBB targeted neurotrophins using a chimeric peptide approach, in which antitransferrin receptor antibody was used as a BBB delivery vector, and neurotrophin peptide was conjugat

Lipid signaling: sleep, synaptic plasticity, and neuroprotection.

Increasing evidence indicates that bioactive lipids participate in the regulation of synaptic function and dysfunction. We have demonstrated that signaling mediated by platelet-activating factor (PAF) and cyclooxygenase (COX)-2-synthesized PGE2 is involved in synaptic plasticity, memory, and neuronal protection [Clark GD, Happel LT, Zorumski CF, Bazan NG. What can adenosine neuromodulation do for neuroprotection?

The possibility of repairing brain lesions is a crucial issue. Knowing how regeneration occurs allows novel concepts in the process of protecting the nervous system, in other words to induce and to develop neuroprotection. Brain insults cause irreversible tissue damage by at least three mechanisms: First, through consequences of mechanical disruption of neurons or their projections; secondly, through biochemical or metabolic changes that are initiated by the insult; and finally, through inflammatory reactions or gliotic changes. The cellular elements and the chemical neuro-mediators involved i

Perinatal brain injury: from pathogenesis to neuroprotection.

Brain injury secondary to hypoxic-ischemic disease is the predominant form of all brain injury encountered in the perinatal period. The focus of this article is the most recent research developments in this field and especially those developments that should lead to the most profound effects on interventions in the first years of the new millennium. Neuronal injury is the predominant form of cellular injury in the term infant. The principal mechanisms leading to neuronal death after hypoxia-ischemia/reperfusion are initiated by energy depletion, accumulation of extracellular glutamate, and act

[Neuroprotection in neuroanesthesia: current practices in Germany].

UNLABELLED: This survey collected and analyzed data on the current practice of clinical neuroprotection in neuroanesthesia in Germany. METHODS: The data were collected by a questionnaire sent to departments of anesthesiology in Germany in 1997 which provided care for neurosurgical patients on a routine basis, and which were registered members of the German Society of Anesthesiology and Intensive Care Medicine (DGAI). Since the questions concerning "neuroprotective therapy" were linked to a general survey on clinical neuroanesthesia performed by the scientific neuroanesthesia working group of

Glaucomatous neurodegeneration and the concept of neuroprotection.

Glaucomatous neurodegeneration is a progressive pathological affair morphologically characterized by RGC death resulting in functional visual deterioration. Although a variety of primary insults ranging from mechanical trauma to ischemia to genetic susceptibility can initiate disease onset, apparently loss of trophic support and excitotoxin release remain the common themes responsible for neuronal damage and eventual cell death by apoptosis. The spread of this damage is perpetuated by secondary degeneration that allows directly injured neurons to release noxious and degenerative substances int

Vitamin E and other antioxidants in neuroprotection.

Several pathological conditions are believed to be causally related to the generation of reactive oxygen species and free radicals including various neurodegenerative disorders. In the histopathology of Alzheimer's disease (AD) many signs of oxidative reactions can be found building the basis of the oxidative stress hypothesis of AD. One major player in the generation of an overall oxidative microenvironment for the nerve cells is the amyloid beta protein (A beta) of the senile plaques in brain areas affected in AD. A beta can be neurotoxic and this toxicity is mediated by peroxides and by the

Cocaine dependence. A clinical syndrome requiring neuroprotection.

Cocaine use has increased the frequency of medical complications among younger individuals. Neurological and neurovascular complications include strokes, seizures, transient ischemic attacks, and headaches. Subclinical deficits in cerebral perfusion and EEG have been noted in this population. Although these subclinical deficits may be an indication of increased risk of medical complications, the prophalactic treatment of cocaine abusers with neuroprotective agents has not yet been advocated. Blood flow of the anterior and medial cerebral arteries was measured by transcranial Doppler sonography

Prophylactic pharmacologic neuroprotection against focal cerebral ischemia.

The crucial importance of the earliest possible treatment of ischemic stroke has been stressed. This leads to the concept of prophylactic neuroprotection: long-term administration of neuroprotective agents to people at risk for stroke, so that the effects of focal ischemia are countered from the moment of onset of vascular occlusion. We at NYU (and researchers at several other centers) have been testing possible neuroprotective agents in the cardiopulmonary bypass setting. We believe that by such means we can rapidly and economically screen agents and dose schedules for efficacy. We believe th

Quinones as Neuroprotective Agents.

Quinones can in principle be viewed as a double-edged sword in the treatment of neurodegenerative diseases, since they are often cytoprotective but can also be cytotoxic due to covalent and redox modification of biomolecules. Nevertheless, low doses of moderately electrophilic quinones are generally cytoprotective, mainly due to their ability to activate the Keap1/Nrf2 pathway and thus induce the expression of detoxifying enzymes. Some natural quinones have relevant roles in important physiological processes. One of them is coenzyme Q10, which takes part in the oxidative phosphorylation proces

Neuroprotective Role of Phytochemicals.

Neurodegenerative diseases are normally distinguished as disorders with loss of neurons. Various compounds are being tested to treat neurodegenerative diseases (NDs) but they possess solitary symptomatic advantages with numerous side effects. Accumulative studies have been conducted to validate the benefit of phytochemicals to treat neurodegenerative diseases including Alzheimer's disease (AD) and Parkinson's disease (PD). In this present review we explored the potential efficacy of phytochemicals such as epigallocatechin-3-galate, berberin, curcumin, resveratrol, quercetin and limonoids again

Neuroprotective Strategies in Glaucoma.

BACKGROUND: Glaucoma is characterized as a neuropathic disease that causes progressive degeneration of retinal ganglion cells (RGCs) in the retina, resulting in irreversible loss of vision. All conventional treatments for glaucoma are focused on reducing intraocular pressure (IOP) in the anterior chamber of the eye. However, these treatments alone are insufficient to halt the progression of the disease. As a result, neuroprotective strategies have been developed that prevent retinal neuron loss and disease progression. METHODS: The goal of this review is to summarize and discuss neuroprotecti

Neuroprotective therapies for glaucoma.

Glaucoma is the second leading cause for blindness worldwide. It is mainly caused by glaucomatous optic neuropathy (GON) characterized by retinal ganglion cell loss, which leads to visual field defect and blindness. Up to now, the main purpose of antiglaucomatous therapies has been to lower intraocular pressure (IOP) through surgeries and medications. However, it has been found that progressive GON is still present in some patients with effective IOP decrease. Therefore, risk factors other than IOP elevation, like neurotrophin deprivation and excitotoxicity, contribute to progressive GON. Nove
