Effect of risk-stratified, protocol-based perioperative chemoprophylaxis on nosocomial infection rates in a series of 31 927 consecutive neurosurgical procedures (1994-2006).

OBJECTIVE: Although the use of prophylactic antibiotics has been shown to significantly decrease the incidence of meningitis after neurosurgery, its effect on extra-neurosurgical-site infections has not been documented. The authors explore the effect of risk-stratified, protocol-based perioperative antibiotic prophylaxis on nosocomial infections in an audit of 31 927 consecutive routine and emergency neurosurgical procedures. METHODS: Infection rates were objectively quantified by bacteriological positivity on culture of cerebrospinal fluid (CSF), blood, urine, wound swab, and tracheal aspira

Development and validation of a perioperative risk prediction model for pressure ulcers in neurosurgical procedures: a machine learning approach with protocol compliance metrics.

BACKGROUND: This study aimed to develop and validate a nomogram for predicting pressure ulcer (PU) incidence in neurosurgical patients to enhance postoperative risk management. METHODS: A retrospective analysis of 1,020 patients across four tertiary centers (2005-2025) evaluated 20 variables. Propensity score matching (PSM) addressed confounding, while LASSO regression and machine learning identified predictors. Model performance was assessed via AUC-ROC, C-index, and decision curve analysis. RESULTS: Eight independent predictors of PU were identified: diabetes duration, BMI, albumin, prealb

Protean infectious types and frequent association with neurosurgical procedures in adult Serratia marcescens CNS infections: report of two cases and review of the literature.

Serratia marcescens is a rare pathogen of adult central nervous system (CNS) infection. We report on the clinical features and therapeutic outcomes of two adult patients with such infections. The clinical characteristics of 13 other reported adult cases are also included for analysis. The 15 cases were nine males and six females, aged 19-83 years, in whom, underlying post-neurosurgical states and ear operation were noted in 93% (14/15). Fever and conscious disturbance were the most common clinical manifestations of these 15 cases, followed by hydrocephalus, seizures, and wound infections. The

Effect of an additional neurosurgical resident on procedure length, operating room time, estimated blood loss, and post-operative length-of-stay.

Introduction: Neurosurgical residency training is costly, with expenses largely borne by the academic institutions that train residents. One expense is increased operative duration, which leads to poorer patient outcomes. Although other studies have assessed the effect of one resident assisting, none have investigated two residents; thus, we sought to investigate if two residents versus one scrubbed-in impacted operative time, estimated blood loss (EBL), and length-of-stay (LOS).Methods: In this retrospective review of patients who underwent a neurosurgical procedure involving one or two resid

Intraoperative radiotherapy after neurosurgical resection of brain metastases as institutional standard treatment- update of the oncological outcome form a single center cohort after 117 procedures.

PURPOSE: Stereotactic radiotherapy (SRT) is the predominant method for the irradiation of resection cavities after resection of brain metastases (BM). Intraoperative radiotherapy (IORT) with 50 kV x-rays is an alternative way to irradiate the resection cavity focally. We have already reported the outcome of our first 40 IORT patients treated until 2020. Since then, IORT has become the predominant cavity treatment in our center due to patients´ choice. METHODS: We retrospectively analyzed the outcomes of all patients who underwent resection of BM and IORT between 2013 and August 2023 at Augsbu

Effect of phenylephrine versus ephedrine on the incidence of postoperative delirium in elderly patients undergoing non-cardiac and non-neurosurgical procedures: protocol for a multicentre, double-blind, randomized, controlled trial.

BACKGROUND: Whether ephedrine, when used to maintain intraoperative blood pressure, is superior to phenylephrine in reducing postoperative delirium remains uncertain. This study aims to determine whether the intraoperative management of hypotension with ephedrine, compared with phenylephrine, reduces the incidence of postoperative delirium in elderly patients undergoing major non-cardiac, non-neurosurgical surgery. METHODS: This multicentre, randomized, double-blind, controlled trial will enrol 1,084 elderly patients scheduled for elective non-cardiac and non-neurosurgical major surgeries und

Emergency Intraoperative Ultrasound for the Neurosurgical Trainee.

The use of intraoperative ultrasound in emergency cranial neurosurgical procedures is not well described. It may improve surgical outcomes and is useful when other neuro-navigation systems are not readily available. We provide a practical guide for neurosurgical trainees to utilize ultrasound for various emergency cranial neurosurgical procedures, including lesion localization, insertion of an external ventricular drain, and shunt revision surgery. Intraoperative ultrasound is a useful modality for urgent neurosurgical procedures.

Expanding neurosurgical care in Northwest Tanzania: the early experience of an initiative to teach neurosurgery at Bugando Medical Centre.

OBJECTIVE: Tanzania suffers from a severe lack of health practitioners trained in neurosurgical procedures. To address this problem, we have implemented an initiative, modeled after the experience of other pioneers in international neurosurgery, to establish cost-effective and sustainable neurosurgical care by teaching fundamental neurosurgical skills to local surgeons. In this report we describe our early experience in Northwest Tanzania and discuss the potential for this training model to improve neurosurgical care to a region in need. METHODS: Between September 2009 and October 2010, three

Red blood cell transfusion in neurosurgery.

BACKGROUND: The necessity of red blood cell (RBC) transfusions in neurosurgical procedures is under debate. Although detailed recommendations exist for many other surgical disciplines, there are very limited data on the probability of transfusions during neurosurgical procedures. METHODS: Three-thousand and twenty-six consecutive adult patients undergoing neurosurgical procedures at Saarland University Hospital from December 2006 to June 2008 were retrospectively analyzed for administration of RBCs. The patients were grouped into 11 main diagnostic categories for analysis. The transfusion pro

Clipping of Intracranial Aneurysms by Neurosurgical Trainees Is Safe and Effective: A Statewide Retrospective Cohort of 614 Consecutive Cases in Queensland, Australia.

BACKGROUND AND OBJECTIVES: Craniotomy and clipping of intracranial aneurysms remain some of the most technically challenging procedures in neurosurgical practice. There are no contemporary published data to describe the safety and efficacy of these procedures when performed by neurosurgical trainees. This study aims to report the outcomes of intracranial aneurysm clipping procedures performed by neurosurgical trainees. METHODS: All consecutive patients undergoing microsurgical clipping of intracranial aneurysms at public hospitals in Queensland in the period from 1/1/2018 to 1/1/2024 were ide

Transient Diabetes Insipidus With Pituitary Dysfunction Following Abdominal Surgery: A Rare Postoperative Endocrine Complication.

Diabetes insipidus (DI) developing postoperatively is a recognized complication following neurosurgical procedures involving the hypothalamic-pituitary region. However, its occurrence after non-neurosurgical operations is exceedingly rare. We report a case of polyuria associated with hypogonadotropic hypogonadism, closely resembling central DI, following major abdominal surgery. Diagnosing DI after non-neurosurgical procedures is challenging, as polyuria is often attributed to more common causes, such as osmotic diuresis or fluid overload. Awareness of this rare but significant postoperative c

Safety and efficacy of electrocautery scalpel utilization for skin opening in neurosurgery.

Diathermy is used widely in neurosurgical procedures, mainly for subcutaneous and deeper layers dissection. The use on scalp and skin of other body parts has been precluded by the fear of scar formation and wound dehiscence. One-hundred-and-seventy-seven skin incisions for neurosurgical procedures have been performed using the micro-needle electrocautery scalpel (MES) and the steel scalpel. The elements in this study included: electrocautery generator unit, cutting mode, power, waveform, size and shape of the MES, depth of the incision, the speed of the electrode movement through tissue, amoun
