Treatments of choice for bullous pemphigoid.

Bullous pemphigoid (BP) is the most frequently occurring autoimmune blistering disease in Europe and North America. Although it is primarily a disease of the elderly, children and young adults can also develop it. The aim of treatment is to suppress the clinical signs and symptoms of BP without over-treating the patient, because BP tends to spontaneously remit in most patients within approximately 5 years. Mild or localized disease may respond to super-potent topical corticosteroids alone or in combination with tetracyclines with or without niacinamide. More severe or generalized disease usual

Linear IgA bullous dermatosis associated with ulcerative proctitis: treatment challenge.

Linear IgA bullous dermatosis is a rare bullous disease in children and adults that can be associated with autoimmune conditions, malignancies, infections, or medication exposure. The definitive diagnosis relies on the biopsy. A 58-year-old man presented to our clinic with a pruritic vesicular and bullous eruption. Histology showed the classic findings of a subepidermal blister with neutrophilic infiltrate and linear IgA deposition along the dermal-epidermal junction. Upon further evaluation, he was diagnosed with ulcerative proctitis. His therapy was complicated owing to side effects and lack

Management of Periocular Granuloma Annulare Using Topical Dapsone.

Granuloma annulare is a disease characterized by granulomatous inflammation of the dermis. Localized granuloma annulare may resolve spontaneously, while generalized granuloma annulare may persist for decades. The authors present the case of a 41-year-old Hispanic man with a two-week history of periocular granuloma annulare. Due to previously reported success in the use of systemic dapsone for the treatment of granuloma annulare, and the periocular proximity of the patient's lesion, topical dapsone was used for treatment. Various additional therapies for the management of granuloma annulare hav

Dermatomyositis in a family of Working Kelpies.

Eight members of a family of Working Kelpies were presented with signs compatible with dermatomyositis. Alopecia, crusts, ulcerations of the skin, depigmentation of nasal planum and lips, onychodystrophy and atrophy of the masticatory muscles were present with varying degree. Histopathology of the skin, but not from muscles was performed in three dogs and confirmed the clinical diagnosis. Different immunomodulating drugs (steroids, cyclosporine, mycophenolate mofetil, pentoxifylline, doxycyline/niacinamid, omega-3 fatty acids and vitamin E) were used with variable success. Dermatomyositis is a

Pigmentation effects of blue light irradiation on skin and how to protect against them.

BACKGROUND: Visible light, in particular blue light, has been identified as an additional contributor to cutaneous photoageing. However, clinical studies demonstrating the clear effect of blue light on photoageing are still scarce, and so far, most studies have focused on broad-spectrum visible light. Although there is evidence for increased skin pigmentation, the underlying mechanisms of photoageing in vivo are still unclear. Furthermore, there is still a need for active ingredients to significantly protect against blue light-induced hyperpigmentation in vivo. Our study had two aims: to detec

Natural ingredients for darker skin types: growing options for hyperpigmentation.

Dyschromia is one of the most common dermatological concerns in patients with darker skin.1 Disorders of hyperpigmentation, including postinflammatory hyperpigmentation, melasma, solar lentigines, and miscellaneous causes of facial hyperpigmentation, are the most frequently treated dyschromias and can have a considerable psychosocial impact. Given the high prevalence of hyperpigmentation and the considerable demand for an even complexion, newer treatment options for hyperpigmentation are of growing interest among consumers, manufacturers, and dermatologists. Blinded, controlled studies demonst

[Drug-induced pellagra].

In industrialized countries pellagra may nowadays be induced by inappropriate diet schedules, alcohol abuse, endogenous disorders of metabolism, and also intake or abuse of drugs. In particular, INH, hydantoine, and other anticonvulsive drugs may induce pellagra by causing disorders in vitamin B metabolism. On this pathway disorders of tryptophan metabolism may also occur. We report here on a woman who developed pellagra after long-term abuse of analgetics and psychopharmaca obviously due to disorders in gastrointestinal resorption and liver metabolism. Phenobarbital, salicylic amide, phenacet

Chronic Diarrhea and Alcoholism: Unravelling the Connection to Pellagra.

Pellagra is a deadly systemic disease caused by niacin deficiency or a disorder of its metabolism. It typically presents as a classic triad of diarrhea, dermatitis, and dementia. It is increasingly seen among high-risk populations such as those with poor socioeconomic status, immunocompromised states, chronic alcoholism, malabsorption disorders, and populations heavily reliant on a corn-based diet. We report a case of a 39-year-old man presenting with watery diarrhea and hyperpigmentation over the sun-exposed areas of the face and arms, with a history of alcohol use disorder. Differential diag

Telogen Effluvium: A Review of the Literature.

Telogen effluvium is one of the most common causes of alopecia. It is a scalp disorder characterized by excessive shedding of hair. Several factors such as drugs, trauma, and emotional and physiological stress can lead to the development of telogen effluvium. Multiple alterations in the hair cycle have been proposed as the underlying mechanism. Telogen effluvium can present as acute or chronic hair fall with symptoms such as trichodynia. Diagnostic tests that can be used include hair wash test, trichogram, phototrichogram, and scalp biopsy. In the treatment of telogen effluvium, it is essentia

The mechanisms of action of nicotinamide and zinc in inflammatory skin disease.

Nicotinamide (niacinamide), a physiologically active form of niacin (nicotinic acid), in combination with zinc is being assessed in clinical studies for the treatment of inflammatory skin diseases such as acne vulgaris and bullous pemphigoid. The basis for these investigations is the variety of potential mechanisms of action of nicotinamide and zinc, including an anti-inflammatory effect via inhibition of leukocyte chemotaxis, lysosomal enzyme release, lymphocytic transformation, mast cell degranulation, bacteriostatic effect against Propionibacterium acnes, inhibition of vasoactive amines, pr

A subepidermal bullous eruption associated with IgG autoantibodies to a 200 kd dermal antigen: the first case report from the United States.

We describe an 81-year-old white man in whom a subepidermal bullous eruption developed that clinically resembled bullous pemphigoid. The eruption promptly responded to oral tetracycline and niacinamide and topical clobetasol. Histologic examination of perilesional skin revealed neutrophilic infiltration with formation of papillary microabscesses and subepidermal cleavage. Direct immunofluorescence showed linear deposition of IgG and C3 along the basement membrane zone. By indirect immunofluorescence, circulating IgG autoantibodies bound exclusively to the dermal side of salt-split normal human

Congenital hydrocephalus mimicking Dandy-Walker syndrome induced by 6-aminonicotinamide injection in pregnant rat.

Fetal hydrocephalus was induced by single intraperitoneal injection of 8 mg/kg 6-aminonicotinamide (6-AN), a niacinamide antagonist, in Sprague-Dawley rat on day 13 of gestation. Materials for histological examination were obtained by uterotomy 1, 2, 4, and 8 days after injection, and untreated fetuses of the same ages were used as controls. Macrocephalus was clear at day 17 (4 days after injection), when cerebral dysgenesis was suggested by bromodeoxyuridine immunohistochemical study. The entire ventricular system was dilated, including the aqueduct and foramen of Monro, and hypoplasia of the

Diabetes mellitus following rodenticide ingestion in man.

Ketotic, insulin-requiring diabetes mellitus and a severe peripheral neuropathy developed in a previously healthy 25-year-old man several days after he attempted suicide with rat poison containing N-3-pyridylmethyl N'-p-nitrophenyl urea. Study of islet-cell function ten months after ingestion showed a reduced disappearance rate of intravenous glucose and depressed C-peptide response to intravenous glucose when compared with a normal control but no impairment of glucagon release after intravenous arginine stimulation. Nerve conduction studies demonstrated severe sensory and mild motor neuropath

Photochemotherapy of generalized granuloma annulare.

Various forms of treatment for generalized granuloma annulare have been employed with little success. The results of treatment with topical and intralesional corticosteroids, as well as systemic therapy with corticosteroids, salicylates, aspirin, niacinamide, and chloroquine, have been generally disappointing. We describe five patients with generalized granuloma annulare of several years' duration who were treated with oral psoralen plus ultraviolet A irradiation. Lesions were present on the extremities, buttocks, and trunk in the form of macules, papules, and plaques. One patient also had per

Evidence-Based Topical Therapy for Facial Scars in Diverse Skin Types.

Facial trauma repair frequently results in scarring that can significantly impact psychological well-being, particularly in highly visible areas, such as the face. While topical agents provide a non-invasive means of optimizing scar outcomes, clinical guidance regarding their use remains limited, especially for diverse skin types. This narrative review synthesizes findings from PubMed, Google Scholar, and Embase, prioritizing randomized controlled trials, meta-analyses, and expert consensus. Evidence supports the use of silicone gel, corticosteroids, retinoids, vitamin C, niacinamide, and onio

Systemic Treatment of Cutaneous Adverse Events After Immune Checkpoint Inhibitor Therapy: A Review.

As treatment with immune checkpoint inhibitors (CPIs) for cancer increases, so has the incidence of immune-related cutaneous adverse events (irCAEs). These toxicities can significantly impact quality of life and may be dose-limiting. Current guidelines for irCAEs offer only corticosteroids or CPI discontinuation. Evidence supports biologic immunomodulatory therapies when corticosteroids fail or need avoidance. A review of literature from 2010 to 2020 yielded 45 articles, resulting in 185 irCAEs, including bullous pemphigoid-like eruption (n = 55), psoriasis/psoriasiform dermatitis (n = 41), an

The Role and Benefits of Dermocosmetics in Acne Management in Japan.

In Japan, as in other countries around the world, acne vulgaris is a common disease and a frequent reason for patients to consult dermatologists. For optimal management of acne, it is important to understand how available products to support skin health can be used both with and without prescription products. Dermocosmetics can be defined as skincare agents with dermatologically active ingredients that directly support or care for the symptoms of various skin conditions (distinct from vehicle effects). There are products with active ingredients-including familiar ones such as niacinamide, reti

Nutritional deficiency in an intestine-on-a-chip recapitulates injury hallmarks associated with environmental enteric dysfunction.

Environmental enteric dysfunction (EED)-a chronic inflammatory condition of the intestine-is characterized by villus blunting, compromised intestinal barrier function and reduced nutrient absorption. Here we show that essential genotypic and phenotypic features of EED-associated intestinal injury can be reconstituted in a human intestine-on-a-chip lined by organoid-derived intestinal epithelial cells from patients with EED and cultured in nutrient-deficient medium lacking niacinamide and tryptophan. Exposure of the organ chip to such nutritional deficiencies resulted in congruent changes in si

Sublingual reactive histiocytosis in a dog.

Lingual lesions are relatively uncommon in dogs and are mostly represented by neoplasms and glossitis secondary to trauma or infections. Reactive histiocytosis is an uncommon, poorly understood, reactive disorder characterized by proliferation of activated, interstitial, dendritic, antigen-presenting cells associated with lymphocytes and neutrophils with a specific angiocentric orientation and occasional angioinvasion and angiodestruction. Clinically, the disease has a waxing and waning behavior with possible response to treatment and regression, or progression to multiple lesions and internal
