Nocardia asteroides mediastinitis complicating coronary artery bypass surgery.

A 63 years diabetic male was admitted with mediatinitis and sternal dehiscence. Nocardia asteroides sensu stricto Type VI was isolated from the mediastinal tissue and fluid during debridement. Prompt surgical intervention and treatment with ofloxacin both intravenously and later orally led to the cure.

Nocardia asteroides sclerokeratitis in a contact lens wearer.

PURPOSE: To report a case of sclerokeratitis caused by Nocardia asteroides in a soft contact lens wearer. CASE REPORT: A 65-year-old male presented with a 2-month history of a corneal ulcer in the left eye. He wore two weekly disposable soft contact lenses on an extended basis. He revealed his history of gardening before the onset of symptoms. On examination, his best-corrected visual acuity was 20/30 in the right eye and 20/400 in the left eye. In the left eye, there was conjunctival injection. His cornea showed multiple patchy infiltrates, with a feathery border that was raised and involved

Primary cutaneous Nocardia asteroides infection after heart transplantation.

Skin diseases due to Nocardia are rare in Europe and usually occur as the secondary localization of a pulmonary lesion in immunocompromised patients. We describe a cutaneous pyoderma and cold abscess due to Nocardia asteroides infection in a cardiac transplant recipient. An insect bite represents the wound of this soil saprophyte belonging to the actinomycetes. Treatment by sulfone and ampicillin-clavulanic acid allowed the regression of lesions without extracutaneous dissemination.

Treatment of Nocardia asteroides keratitis with polyhexamethylene biguanide.

PURPOSE: Polyhexamethylene biguanide (PHMB) is a polymeric biguanide disinfectant that has been used previously in the treatment of Acanthamoeba keratitis. The authors report the first case of Nocardia asteroides keratitis treated successfully with PHMB. Further studies documenting the efficacy of PHMB against N. asteroides in vitro also are presented. METHODS: A soft contact lens wearer presented with an infectious keratitis. Cultures from the cornea grew N. asteroides. The infection resolved with the treatment with PHMB 0.02%. Serial dilutions of PHMB were performed against N. asteroides in

Chronic Nocardia asteroides endophthalmitis after extracapsular cataract extraction.

A 65-year-old man underwent an uncomplicated extracapsular cataract extraction with placement of a posterior chamber intraocular lens. The patient developed delayed-onset chronic endophthalmitis caused by Nocardia asteroides. This was confirmed on multiple intraocular cultures, as well as histopathologic examination of the lens capsular bag. The patient was treated with periocular, intravitreal, and systemic antibiotic treatment, combined with intraocular lens removal, total capsulectomy, and three vitrectomies in an attempt to eradicate the organism and calm the eye. The patient went on to de

Nocardia asteroides peritonitis during continuous ambulatory peritoneal dialysis.

This paper reports a case of peritonitis by Nocardia asteroides during continuous ambulatory peritoneal dialysis in a man who had systemic lupus erythematous and chronic renal failure. Diagnosis was established by microscopic examination (Gram and Kinyoun) and culture of centrifuged dialysis fluid and the patient was treated with Trimethoprim-Sulfamethoxazole by intraperitoneal route.

Mediastinitis due to Nocardia asteroides after cardiac transplantation.

Nocardia infection classically occurs in immuno-compromized patients. Only a few cases of mediastinal infection due to this pathogen have been described in the literature. We report a patient who developed mediastinitis due to Nocardia asteroides after cardiac transplantation. The treatment was surgical debridement, dressing, sugaring and antibiotic therapy. The emergence of a severe acute renal failure possibly induced by drug interaction between Cyclosporin, cyclines and aminoglycosides, led us to modify the antimicrobial treatment. The intravenous use of Imipenem 2 g per day and Ciprofloxac

[Isolation of Nocardia asteroides using hemoculture (clinical report)].

The authors describe the finding of Nocardia asteroides from a haemoculture in a patient with right-sided pneumonia and several predisposing factors (TB, chronic bronchitis, anamnestic hepatitis, chronic hepatopathy, anaemia, ethylism and a neoplasm revealed in the final stage). Early bacteriological diagnosis and aimed antimicrobial therapy led to radical improvement of the very dramatic clinical condition. The authors discuss the possible higher incidence of the infectious agent, which previously was detected only very rarely.

Experience with Nocardia asteroides in renal transplant recipients.

Six cases of Nocardia asteroides infection in renal transplant recipients are described. Respiratory symptoms predominated, complicated in one case by cerebral abscess. General dissemination did not occur in any patient. Environmental testing failed to show source(s) of the organism. Nocardiosis patients should be isolated to prevent airborne spread to renal transplant recipients, who are prone to infection and from whom specimens for culture should be incubated for at least 7 days to exclude Nocardia spp.

Immunobiology of germfree mice infected with Nocardia asteroides.

Nocardia asteroides GUH-2 was administered either intranasally or by intravenous inoculation into (i) conventionally grown, (ii) germfree, and (iii) lipopolysaccharide-treated germfree NIH:S mice. The number of bacteria within the lungs, brain, kidneys, adrenals, liver, spleen, and blood was quantitated at 3, 24, 72, and 168 h after infection. Further, the histological changes that occurred in each of these organs after infection were studied. The data demonstrated that germfree mice were significantly more susceptible to the acute phase of infection caused by N. asteroides than the convention

Virulence of Nocardia asteroides during its growth cycle.

Cells of Nocardia asteroides undergo structural and chemical changes, especially in the cell wall, during growth in brain heart infusion broth. Experiments were devised to determine whether these changes affected the virulence of Nocardia for mice. It took, on the average, 1,380 times the number of colony-forming units at the stationary phase to achieve the same mortality induced by the log-phase cells. Cells in either the lag phase or early stationary phase of growth were intermediate in the numbers of colony forming units required to kill mice. Dry-weight determinations at different stages o

Nocardia asteroides primary cerebral abscess and secondary meningitis.

A rare case of primary cerebral abscess due to Nocardia asteroides, and its surgical removal, are described. Postoperative meningitis caused by the same fungi was succesfully cured with sulphadiazine and cycloserine. The nocardial disease, its neurological complications, and therapy are reviewed.

Nocardia asteroides thenar space infection: a case report.

Nocardia commonly is thought of as producing a chronic pulmonary or granulomatous skin infection, but this report illustrates that it may begin as an acute inflammatory process. In view of the tenosynovitis which followed exploration of the tendon sheath in the patient presented and which required a second operation, it may be better to aspirate the sheath at a distant site to look for pus. Actinomycetes should be suspected and looked for with appropriate methods in any suppurative lesion, especially one not responding to conventional antibiotics.
